| 1 | Rho/ROCK pathway and neural regeneration: a potential therapeutic target for central nervous system and optic nerve damage显示文摘· Rho-associated kinase (ROCK) is a serine/threonine kinase and one of the major downstream effectors of the small GTPase RhoA. The Rho/ROCK pathway is closely related to the pathogenesis of several central nervous system (CNS) disorders, and involved in many aspects of neuronal functions including neurite outgrowth and retraction. In the adult CNS, the damaged neuron regeneration is very difficult due to the presence of myelin-associated axon growth inhibitors such as Nogo, myelin-associated glycoprotein (MAG) and oligodendrocyte-myelin glycoprotein (Omgp), etc. The effects of these axon growth inhibitors are reversed by blocking the Rho/ROCK pathway in vitro, and the inhibition of Rho/ROCK pathway can promote axon regeneration and functional recovery in the injured CNS in vivo. In addition, the therapeutic effects of the Rho/ROCK inhibitors have also been demonstrated in some animal models and the Rho/ROCK pathway becomes an attractive target for the development of drugs for treating CNS disorders. | 2 | Epidemiology, genetics and treatments for myopia显示文摘· Myopia is a significant public health problem and its prevalence is increasing over time and genetic factors in disease development are important. The prevalence and incidence of myopia within sampled population often varies with age, country, sex, race, ethnicity, occupation, environment, and other factors. Myopia growth is under a combination of genes and their products in time and space to complete the coordination role of the guidance. Myopia-related genes include about 70 genetic loci to which primary myopias have been mapped, although the number is constantly increasing and depends to some extent on definition. Of these, several are associated with additional abnormalities, mostly as part of developmental syndromes. These tend to result from mutations in genes encoding transcriptional activators, and most of these have been identified by sequencing candidate genes in patients with developmental anomalies. Currently, collagen alpha-1 chain of type I(COL1A1), collagen alpha-1 chain of type II(COL2A1), actin, alpha, cardiac muscle 1(ACTC1), paired box gene 6(PAX6) and NIPBL (nipped-B homolog), and so on have been mapped. Myopia is most commonly treated with spectacles or glasses. The most common surgical procedure performed to correct myopia is laser keratomileusis (LASIK). | 3 | Multiple methods of surgical treatment combined with primary IOL implantation on traumatic lens subluxation/dislocation in patients with secondary glaucoma显示文摘AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary intraocular lens(IOL)implantation.METHODS:Non-comparativeretrospectiveobservational case series.Participants:30 cases(30 eyes)of lens subluxation/dislocation in patients with secondary glaucoma were investigated which accepted the surgical treatment by author in the Ophthalmology of Xi'an No.4Hospital from 2007 to 2011.According to the different situations of lens subluxation/dislocation,various surgical procedures were performed such as crystalline lens phacoemulsification,crystalline lens phacoemulsification combined anterior vitrectomy,intracapsular cataract extraction combined anterior vitrectomy,lensectomy combined anterior vitrectomy though peripheral transparent cornea incision,pars plana lensectomy combined pars plana vitrectomy,and intravitreal cavity crystalline lens phacofragmentation combined pars plana vitrectomy.And whether to implement trabeculectomy depended on the different situations of secondary glaucoma.The posterior chamber intraocular lenses(PC-IOLs)were implanted in the capsular-bag or trassclerally sutured in the sulus decided by whether the capsular were present.Main outcome measures:visual acuity,intraocular pressure,the situation of intraocular lens and complications after the operations.RESULTS:The follow-up time was 11-36mo(21.4±7.13).Postoperative visual acuity of all eyes were improved;28 cases maintained IOP below 21 mm Hg;2cases had slightly IOL subluxation,4 cases had slightlytilted lens optical area;1 case had postoperative choroidal detachment;4 cases had postoperative corneal edema more than 1wk,but eventually recovered transparent;2 cases had mild postoperative vitreous hemorrhage,and absorbed 4wk later.There was no postoperative retinal detachment,IOL dislocation,and endophthalmitis.CONCLUSION:To take early treatment of traumatic lens subluxation/dislocation in patients with secondary glaucoma by individual surgical plan based on the different eye conditions would be safe and effective,which can effectively control the intraocular pressure and restore some vision. | Rui Wang Chun-Chao Bi Chun-Ling Lei Wen-Tao Sun Shan-Shan Wang Xiao-Juan Dong | 2014 | International Journal of Ophthalmology(English edition)2014,7,2: | 26 |
| 4 | Foveal avascular zone area and parafoveal vessel density measurements in different stages of diabetic retinopathy by optical coherence tomography angiography显示文摘AIM:To investigate foveal avascular zone(FAZ) and parafoveal vessel densities(PRVD) by means of optical coherence tomography angiography(OCTA) in diabetic patients with or without diabetic retinopathy(DR) and to assess the reproducibility of FAZ and PRVD measurements.METHODS:Sixty diabetic patients(60 eyes) with different stage of DR(graded according to the International Clinical Severity Scale for DR) and 20 healthy subjects underwent FAZ area and PRVD measurements using OCTA by two experienced examiners.FAZ area in all patients was also assessed using fluorescein angiography(FA).RESULTS:In subject with proliferative DR and with moderate-severe non proliferative DR,FAZ area was significantly increased compared to healthy controls(P=0.025 and P=0.050 respectively measured with OCTA and P=0.025 and P=0.048 respectively measured with FA).OCTA showed significantly less inter-observer variability compared to FA.Concordance correlation coefficient(CCC) for FAZ area measurements was 0.829(95%CI:0.736-0.891) P<0.001 with FA and 1.000(95%CI:0.999-1.000) P<0.001 with OCTA.CCC was 0.834(95%CI:0.746-0.893) P<0.001 and 0.890(95%CI:0.828-0.930) P<0.001 for parafoveal superficial and deep vessel density measurements,respectively.CONCLUSION:OCTA shows progressive increase of FAZ area and reduction of PRVD in both superficial and deep plexus at increasing DR severity.FAZ area and PRVD measurements using OCTA are highly reproducible. | Rodolfo Mastropasqua Lisa Toto Alessandra Mastropasqua Raffaella Aloia Chiara De Nicola Peter A Mattei Guido Di Marzio Marta Di Nicola Luca Di Antonio | 2017 | International Journal of Ophthalmology(English edition)2017,10,10: | 26 |
| 5 | Establishment of an untransfected human corneal epithelial cell line and its biocompatibility with denuded amniotic membrane显示文摘AIM: To establish an untransfected human corneal epithelial (HCEP) cell line and characterize its biocompatibility with denuded amniotic membrane (dAM).METHODS: The torn HCEP pieces were primarily cultured in DMEM/F12 media (pH 7.2) supplemented with 20% fetal bovine serum and other necessary factors,yielding an HCEP cell line which was its growth performance,chromosome morphology,tumorigenicity and expression of marker proteins analyzed.In addition,the biocompatibility of HCEP cells with dAM was evaluated through histological and immunocytochemistry analyses and with light,electron and slit-lamp microscopies.RESULTS: HCEP cells proliferated to confluence in 3 weeks,which have been subcultured to passage 160.A continuous untransfected HCEP cell line,designated as utHCEPC01,was established with a population doubling time of 45.42 hours as was determined at passage 100.The cells retained HCEP cell properties as were approved by chromosomal morphology and the expression of keratin 3.They,with no tumorigenicity,formed a multilayer epithelium-like structure on dAMs through proliferation and differentiation during air-liquid interface culture,maintained expression of marker proteins including keratin 3 and integrin β1 and attached tightly to dAMs.The reconstructed HCEP was highly transparent and morphologically and structurally similar to the original.CONCLUSION: An untransfected and non-tumorigenic HCEP cell line was established in this study.The cells maintained expression of marker proteins.The cell line was biocompatible with dAM.It holds the potential of being used for in vitro reconstruction of tissue-engineered HCEP,promising for the treatment of diseases caused by corneal epithelial disorders. | Ting-Jun Fan Bin Xu Jun Zhao Hong-Shou Yang Rui-Xin Wang and Xiu-Zhong Hu | 2011 | International Journal of Ophthalmology(English edition)2011,4,3: | 22 |
| 6 | Differentiation of isolated human umbilical cord mesenchymal stem cells into neural stem cells显示文摘AIM:To investigate whether umbilical cord human mesenchymal stem cell(UC-MSC)was able to differentiate into neural stem cell and neuron in vitro.·METHODS:The umbilical cords were obtained from pregnant women with their written consent and the approval of the Clinic Ethnics Committee.UC-MSC were isolated by adherent culture in the medium contains 20%fetal bovine serum(FBS),then they were maintained in the medium contain 10%FBS and induced to neural cells in neural differentiation medium.We investigated whether UC-MSC was able to differentiate into neural stem cell and neuron in vitro by using flow cytometry,reverse transcriptase-polymerase chain reaction(RT-PCR)and immunofluorescence(IF)analyzes.·RESULTS:A substantial number of UC-MSC was harvested using the tissue explants adherent method at about 2wk.Flow cytometric study revealed that these cells expressed common markers of MSCs,such as CD105(SH2),CD73(SH3)and CD90.After induction of differentiation of neural stem cells,the cells began to form clusters;RT-PCR and IF showed that the neuron specific enolase(NSE)and neurogenic differentiation 1-positive cells reached 87.3%±14.7%and 72.6%±11.8%,respectively.Cells showed neuronal cell differentiation after induced,including neuron-like protrusions,plump cell body,obviously and stronger refraction.RT-PCR and IF analysis showed that microtubule-associated protein 2(MAP2)and nuclear factor-M-positive cells reached 43.1%±10.3%and 69.4%±19.5%,respectively.·CONCLUSION:Human umbilical cord derived MSCscan be cultured and proliferated in vitro and differentiate into neural stem cells,which may be a valuable source for cell therapy of neurodegenerative eye diseases. | Song Chen Wei Zhang Ji-Ming Wang Hong-Tao Duan Jia-Hui Kong Yue-Xin Wang Meng Dong Xue Bi Jian Song | 2016 | International Journal of Ophthalmology(English edition)2016,9,1: | 22 |
| 7 | Inhibition of RhoA/Rho-kinase pathway suppresses the expression of extracellular matrix induced by CTGF or TGF-β in ARPE-19显示文摘AIM:To investigate the role of Rho-associated protein kinase (ROCK) inhibitor, Y27632, in mediating the production of extracellular matrix (ECM) components including fibronectin, matrix metallo-proteinase-2 (MMP-2) and type I collagen as induced by connective tissue growth factor(CTGF) or transforming growth factor-β (TGF-β) in a human retinal pigment epithelial cell line, ARPE-19. METHODS:The effect of Y27632 on the CTGF or TGF-β induced phenotype in ARPE-19 cells was measured with immunocytochemistry as the change in F-actin. ARPE-19 cells were treated with CTGF (1, 10, 100ng/mL)and TGF-β (10ng/mL) in serum free media, and analyzed for fibronectin, laminin, and MMP-2 and type I collagen by RT-qPCR and immunocytochemistry. Cells were also pretreated with an ROCK inhibitor, Y27632, to analyze the signaling contributing to ECM production. ·RESULTS:Treatment of ARPE-19 cells in culture with TGF-β or CTGF induced an ECM change from a cobblestone morphology to a more elongated swirl pattern indicating a mesenchymal phenotype. RT-qPCR analysis and different gene expression analysis demonstrated an upregulation in expression of genes associated with cytoskeletal structure and motility. CTGFor TGF-β significantly increased expression of fibronectin mRNA (P =0.006, P =0.003 respectively), laminin mRNA (P =0.006, P =0.005), MMP-2 mRNA (P =0.006, P =0.001), COL1A1 mRNA (P =0.001, P =0.001), COL1A2 mRNA (P = 0.001, P =0.001). Preincubation of ARPE-19 with Y27632 (10mmol/L) significantly prevented CTGF or TGF-β induced fibronectin (P=0.005, P=0.003 respectively), MMP-2 (P = 0.003, P =0.002), COL1A1 (P =0.006, P =0.003), and COL1A2 (P =0.006, P =0.004) gene expression, but not laminin (P =0.375, P =0.516). CONCLUSION:Our study demonstrated that both TGF-β and CTGF upregulate the expression of ECM components including fibronectin, laminin, MMP-2 and type I collagen by activating the RhoA/ROCK signaling pathway. During this process, ARPE-19 cells were shown to change from an epithelial to a mesenchymal phenotype in vitro. Y27632, a ROCK inhibitor, inhibited the transcription of fibronectin, MMP-2 and type I collagen, but not laminin. The data from our work suggest a role for CTGF as a profibrotic mediator. Inhibiting the RhoA/ROCK pathway represents a potential target to prevent the fibrosis of retinal pigment epithelial (RPE) cells. | 8 | Elevation of serum apelin-13 associated with proliferative diabetic retinopathy in type 2 diabetic patients显示文摘AIM: To compare apelin-13, a ligand of G-proteincoupled receptor which has been shown to be involved in retinal angiogenesis, and vascular endothelial growth factor(VEGF) serum levels in type 2 diabetes mellitus(T2DM) with or without retinopathy, and to investigate the relationship between the serum concentration of apelin-13 and diabetes retinopathy.METHODS: Sixty-nine patients with T2 DM were enrolled.Of the 69 patients, 16 had proliferative diabetic retinopathy(PDR group), 23 had non-PDR(NPDR group)and 30 had no retinopathy(T2DM group). Subjects' information, including demographics, medical history,and use of medications were recorded. Their serum samples were collected for measuring the levels of C-reactive protein(CRP), serum lipid and glycosylated hemoglobin. Apelin-13 and VEGF serum levels were measured by enzyme-linked immunosorbent assay.Kruskal-Wallis test and one-way ANOVA were used to compare the differences among these groups. Chi-square test was used to assess categorical variables.Correlations between variables were investigated by Spearman rho correlation test and stepwise regression analysis. All statistical analyses were performed through SPSS 17.0 software.RESULTS: Sex, age, body mass index(BMI), blood pressure, CRP, hemoglobin A1c(Hb A1c) have no significantly difference in the three groups. Serum level of apelin-13 was significantly elevated in PDR group as compared with T2 DM group(P =0.041). Differences of VEGF serum concentration in the three groups were statistically significant(P =0.007, P =0.007 and P <0.001,respectively). Spearman rho correlation test showed that serum apelin-13 was positively correlated with BMI,serum triglycerides, VEGF, but not with age, duration of diabetes, blood pressure, CRP, Hb A1 c and total-cholesterol. Stepwise regression analysis showed that BMI also significantly associated with serum apelin-13(P =0.002), while VEGF and serum triglycerides were irrelevant. CONCLUSION: This study elucidated a positive association of apelin-13 serum level with PDR, but not with VEGF. | 9 | Posterior scleral reinforcement for the treatment of pathological myopia显示文摘AIM: To investigate the effects of posterior scleral reinforcement(PSR) in the treatment of pathological myopia.·METHODS: The study included 52 eyes in 43 patients with pathological myopia who underwent PSR(PSR group), and 52 eyes in 36 age- and myopia-matched patients who did not undergo such treatment as control group. Axial length, refraction error, best corrected visual acuity(BCVA), and macular scans by optical coherence tomography(OCT) were recorded at baseline, 6mo, 1, 3and 5y after the surgery, and the complications were noted.·RESULTS: There were no statistical differences in axial length, refractive error, or BCVA between the PSR group and the control group at baseline. At the end of the follow-up, the mean axial length was 29.79 ±1.26 mm in the PSR group, which was significantly shorter than that in the control group(30.78±1.30 mm)(P <0.01), and the mean refractive error was-16.86 ±2.53 D in the PSR group, which was significantly lower than that in the control group(-19.18 ±2.12 D)(P <0.01). A statistically significant difference in BCVA was found between the PSR group(0.51 ±0.25 log MAR) and the control group(0.62±0.26 log MAR) at the postoperative 5-year follow-up(P <0.01). Those, however, are not real artificial vitreous due to lack of structure and function like the natural vitreous body. | 11 | Outcomes and possible risk factors associated with axis alignment and rotational stability after implantation of the Toric implantable collamer lens for high myopic astigmatism显示文摘·AIM:To assess the visual outcomes and possible risk factors associated with axis alignment and rotational stability after implantation of Toric implantable collamer lens (TICL) for the correction of high myopic astigmatism. ·METHODS:In this prospective, nonrandomized clinical study, 54 consecutive eyes of 29 patients with high myopic astigmatism received TICL implantation. To evaluate postoperative axis deviation from the intended axis, a digital anterior segment photograph was taken. The ultrasound biomicroscopy(UBM) was used to observe footplate-position. ·RESULTS:After mean follow-up of 8.6 months, mean manifest refractive cylinder (MRC) decreased 79.3% from (-1.88±1.49)D preoperatively to (0.39±0.61)D postoperatively. MRC within 1.00D occurred in 68.5% (37/54) of eyes, whereas 48.1% (26/54) had MRC within 0.50D. Mean manifest refraction spherical equivalent (MRSE) changed from (-12.08±4.22)D preoperatively to (-0.41±0.61)D postoperatively. Uncorrected binocular vision of 20/20 or better occurred in 72.2%(39/54) of patients compared with binocular best-corrected visual acuity (BCVA) of 20/20 or better in 44.4% (24/54) preoperatively. The mean difference between intended and achieved TICL axes was (6.96±8.37)°. Footplates of TICLs were in the ciliary sulcus in 22 eyes (46.3%), below the ciliary sulcus in 32 eyes (53.7%). The angle of TICL rotation had significant correlation with the footplates-position (t =2.127; P =0.045) and the postoperative TICL vaulting (r =-0.516; P =0.000). ·CONCLUSION:The results of our study further support the safety, efficacy and predictability of TICL for the correct high myopic astigmatism. | 12 | Changes of meibomian glands in patients with type 2diabetes mellitus显示文摘AIM: To investigate the morphological changes of meibomian glands in patients with type 2 diabetes mellitus(DM).·METHODS: Of 118 eyes(118 patients) with type 2 DM(DM group) and 100 eyes of 100 control subjects(control group) were enrolled. After completing an ocular surface disease index(OSDI) questionnaire, the non-invasive tear film break-up time(NI-BUT) and the structure of the meibomian glands(MGs, meibography) were assessed by the Keratograph 5M system. Partial or complete loss of MG was scored for each eyelid from grade 0(no loss) to grade 3(lost area was >2/3 of the total MG area), which were also examined by laser scanning confocal microscopy(LSCM). The primary outcomes were meibomian gland acinar unit density(MGAUD),meibomian gland acinar longest diameter(MGALD) and meibomian gland acinar shortest diameter(MGASD).·RESULTS: Compared with control group, the OSDI was significantly higher in DM group(Z =-5.916; P <0.001),while the NI-BUT was significantly lower(Z =-7.765; P <0.001). Keratograph showed that there were more MGs dropout in DM group than that in control group. The meiboscore was significantly higher in DM group compared with control group(Z =-3.937; P <0.001).LSCM revealed that there were cytological alterations of MGs in DM group compared with control group, which included enlargement of MG acinar units and decreased in density of MG acinar units. | 13 | Changes in vitreous VEGF, bFGF and fibrosis in proliferative diabetic retinopathy after intravitreal bevacizumab显示文摘AIM: To evaluate the relationship between intravitreal bevacizumab(IVB) treatment and the levels of vitreous vascular endothelial growth factor(VEGF), basic fibroblast growth factor(b FGF) and vitreous-retina surface fibrosis in patients with proliferative diabetic retinopathy(PDR).·METHODS: This study was a prospective, open-label,controlled, randomized clinical trial. Sixty-eight eyes of PDR patients(n =53) and macular hole patients(n =15)were enrolled in this study. Thirty-four eyes of the PDR patients received IVB before vitrectomy. Twenty-three of the 34 PDR patients received IVB treatment 5d before vitrectomy(subgroup a), and 11 of the 34 PDR patients received IVB treatment greater than 2wk prior to vitrectomy(subgroup b). Nineteen of the PDR patients did not receive IVB treatment at any time prior to vitrectomy. The levels of b FGF and VEGF in vitreous samples were measured using enzyme-linked immunosorbent assay(ELISA) and the degree of vitreoretinal fibrosis was characterized using clinical data and data obtained intra-operatively.·RESULTS: In PDR patients, VEGF and b FGF levels were significantly increased compared to non-PDR(control) subject's eyes(P <0.01). In PDR patients,vitreous VEGF levels were significantly decreased following IVB treatment compared to PDR patients that did not receive IVB treatment(P <0.01). The degree of vitreoretinal fibrosis was significantly increased in subgroup b compared to subgroup a(P <0.05) and to patients that did not receive IVB(P <0.05). Vitreous b FGF levels were significantly greater in subgroup b than subgroup a(P <0.01) or in patients who did not receive IVB treatment(P <0.05). A Spearman's rank correlationtest indicated that higher levels of vitreous b FGF, but not VEGF, correlated with the degree of vitreoretinal fibrosis.·CONCLUSION: We found that b FGF levels increase in PDR patient's vitreous after IVB treatment longer than two weeks prior to vitrectomy and correlated with the degree of fibrosis after IVB treatment. | 14 | Prevalence of diabetic retinopathy, proliferative diabetic retinopathy and non-proliferative diabetic retinopathy in Asian T2DM patients: a systematic review and Metaanalysis显示文摘AIM: To investigate the pooled prevalence of diabetic retinopathy(DR), proliferative DR(PDR) and nonproliferative DR(NPDR) in Asian type 2 diabetes mellitus(T2 DM) patients. METHODS: We performed a systematic search online search using PubMed, EMBASE, Web of Science, the Cochrane Library, and China WeiPu Library to identify eligible studies that reported the prevalence of DR, PDR and NPDR in Asian T2 DM patients. Effect size(ES) with 95% confidence interval(CI) was used to evaluate the prevalence of DR, PDR and NPDR in Asian T2 DM patients, respectively. RESULTS: There were 41 references and 48 995 T2 DM patients involved in this study. The prevalence of DR, PDR, and NPDR was 28%, 6%, and 27% in T2 DM patients, respectively; while the prevalence of PDR and NPDR in DR patients was 17% and 83%, respectively. Subgroup analysis showed that prevalence of DR in T2 DM patients from Singaporean, Indian, South Korean, Malaysian, Asian, and Chinese was 33%, 42%, 16%, 35%, 21% and 25%, respectively. In T2 DM patients with NPDR from Indian, South Korean, Malaysian, Asian, Chinese, higher prevalence was found than that in PDR patients(45% vs 17%, 13% vs 3%, 30% vs 5%, 23% vs 2% and 22% vs 3%), as well as in DR patients(74% vs 26%, 81% vs 19%, 86% vs 14%, 92% vs 8% and 85% vs 15%). The prevalence of PDR in T2 DM from India was higher than patients fromother locations of Asia, and the same results were also observed in NPDR patients. CONCLUSION: In either T2 DM Asian patients or DR patients, NPDR is more common than PDR. | 15 | Comparing the intraoperative complication rate of femtosecond laser-assisted cataract surgery to traditional phacoemulsification显示文摘AIM: To compare the complication rate of femtosecond laser-assisted cataract surgery(FLACS) and traditional phacoemulsification for the first 18 mo of FLACS use at a private surgical center in Hawaii.METHODS: A retrospective chart review was conducted from January 2012 to June 2013. The first 273 consecutive eyes receiving FLACS and 553 eyes receiving traditional phacoemulsification were examined.All surgeries were performed at a single surgical center in Hawaii. The presence of intraoperative complications was used as the main outcome measure. Approval was obtained from the institutional review board of the University of Hawaii.RESULTS: The overall complication rate for FLACS was 1.8%, while that of the traditional procedure was5.8%(P <0.05). | 16 | Molecular hydrogen regulates the expression of miR-9,miR-21 and miR-199 in LPS-activated retinal microglia cells显示文摘AIM: To explore the potential mechanism of molecular hydrogen in the regulation of miRNA expression and signal-modulating activities. ·METHODS: Retinal microglia cells were activated by Lipopolysaccharides (LPS) and then treated with hydrogen -saturated medium or normal medium without hydrogen. qRT -PCR was used to detect the expression difference in miR-9, miR-21 and miR-199 between these two groups. | 17 | Phacoemulsification versus combined phacotrabeculectomy in the treatment of primary angle-closure glaucoma with cataract: a Meta-analysis显示文摘AIM: To compare the efficacy and safety of phacoemulsification(Phaco) against combined phacotrabeculectomy(Phacotrabe) in primary angle-closure glaucoma(PACG) with coexisting cataract.·METHODS: By searching electronically the Pub Med,EMBASE, Scientific Citation Index and Cochrane Library published up from inception to January 2014, all randomized controlled trials that matched the predefined criteria were included. The quality of included trials was evaluated according to the guidelines developed by the cochrane collaboration. And the outcomes estimating efficacy and safety of two different surgical treatments were measured and synthesised by Rev Man 5.0.· RESULTS: Five randomized controlled trials were selected and included in Meta-analysis with a total of468 patients(468 eyes) with both PACG and cataract. We found that Phacotrabe had a greater intraocular pressure(IOP) lowing effect [preoperative IOP: weighted mean difference(WMD)=0.58, 95% confidence intervals(95% CI,-0.53 to 1.69), P =0.31; postoperative IOP: WMD =1.37,95% CI(0.45 to 2.28), P =0.003], a lower number of antiglaucoma medications [risk ratio(RR)=0.05, 95% CI(0.02 to 0.18), P <0.00001] needed postoperatively and less serious damage of optic nerve [RR =0.48, 95% CI(0.21 to 1.07), P =0.07], but a higher risk of complications[odds ratio(OR)=0.04, 95% CI(0.01 to 0.16), P <0.00001]compared with Phaco. The rest studies indicated that there had no significantly difference between the two surgical methods for postoperative best-corrected visual acuity(BCVA) [WMD =-0.05, 95% CI(-0.14 to 0.05), P =0.32] and loss of visual field [RR=1.06, 95% CI(0.61 to1.83), P =0.83].·CONCLUSION: Phaco alone compared with Phacotrabe had a better effect in IOP reduction, whereas the securitydecline. | 18 | Comparison of myopia control between toric and spherical periphery design orthokeratology in myopic children with moderate-to-high corneal astigmatism显示文摘AIM: To compare clinical results between toric and spherical periphery design orthokeratology(ortho-k) in myopic children with moderate-to-high corneal astigmatism. METHODS: This retrospective study enrolled 62 eyes of 62 subjects using toric ortho-k lenses. These subjects were assigned to the toric group. Based on the one-to-one match principle(same age, proximate spherical equivalence and corneal astigmatism), 62 eyes of 62 subjects were enrolled and included in the spherical group. At one-year followup visit, visual acuity, corneal astigmatism, treatment zone decentration, axial elongation and adverse reaction were compared between these two groups. RESULTS: At the one-year visit, corneal astigmatism was significantly lower in the toric group(1.22±0.76 D) than in the spherical group(2.05±0.85 D)(P=0.012). The mean magnitude of the treatment zone decentration was 0.62±0.42 mm in the toric group and 1.07±0.40 mm in the spherical group(P=0.004). Axial elongation was significantly slower in the toric group(0.04±0.13 mm) than in the spherical group(0.09±0.13 mm)(P=0.001). The oneyear axial elongation was significantly correlated with initial age(r=-0.487, P<0.001) and periphery design of ortho-k lens(r=0.315, P<0.001). The incidence of corneal staining was lower in the toric group(8.1%) than in the spherical group(19.4%)(P<0.001). | 19 | The relationship between insulin resistance/β-cell dysfunction and diabetic retinopathy in Chinese patients with type 2 diabetes mellitus: the Desheng Diabetic Eye Study显示文摘AIM: To investigate the relationship between insulin resistance(IR)/β-cell dysfunction and diabetic retinopathy(DR) in Chinese patients with type 2 diabetes mellitus(T2 DM), and to explore further whether there were differences in the relationship among diabetic patients with higher and lower body mass index(BMI).METHODS: Cross-sectional study. A total of 1466 subjects with T2 DM were recruited in a local Desheng Community of urban Beijing from November 2009 to June 2012 for the cohort of Beijing Desheng Diabetic Eye Study. Standardized evaluation was carried out for each participant, including questionnaire, ocular and anthropometric examinations, and laboratory tests. Seven fields 30° color fundus photographs were used for DR grading according to the Early Treatment Diabetic Retinopathy Study protocols. Homeostatis Model Assessment(HOMA) method was employed for IR and β-cell function assessment.RESULTS: After excluding those participants who were treated with insulin(n=352) or had missing data of fasting insulin(n=96), and further excluding those with poor quality of retinal photographs(n=10), a total of 1008 subjects were included for the final analysis, 406(40.3%) were men and 602(59.7%) were women, age ranging from 34 to 86(64.87±8.28)y.Any DR(levels 14 and above) was present in 278(27.6%) subjects. After adjusting for possible covariates, the presenceof any DR did not correlate with HOMA IR [odds ratio(OR) 1.51, 95% confidence interval(CI) 0.87-2.61, P=0.14] or HOMA β-cell(OR 0.71, 95%CI 0.40-1.26, P=0.25). After stratification by BMI, the presence of any DR was associated positively with HOMA IR(OR 2.46, 95%CI: 1.18-5.12, P=0.016), and negatively with HOMA β-cell(OR 0.40, 95%CI: 0.19-0.87, P=0.021) in the group of patients with higher BMI(≥25 kg/m2). In the group of patients with lower BMI(<25 kg/m2), the presence of any DR was not associated with HOMA IR(OR 1.00, 95%CI: 0.43-2.33, P=1.00) or HOMA β-cell(OR 1.41, 95%CI: 0.60-3.32, P=0.43). CONCLUSION: The data suggest that higher IR and lower β-cell function are associated with the presence of DR in the subgroup of diabetic patients with higher BMI. | 20 | Expressions of type I collagen, α2 integrin and β1 integrin in sclera of guinea pig with defocus myopia and inhibitory effects of bFGF on the formation of myopia显示文摘AIM:To investigate the expressions of type I collagen, α2 integrin and β1 integrin in the posterior sclera of guinea pigs with defocus myopia and whether basic fibroblast growth factor (bFGF) injection inhibits the formation and development of myopia by upregulating the expression of type I collagen, α2 integrin and β1 integrin. METHODS:After 14 days of treatment, the refractive state and axial length were measured and the levels of type I collagen, α2 integrin and β1 integrin were assayed in the posterior sclerae of groups of guinea pigs that wore a monocular-7D polymethylmethacrylate (PMMA) lens or had -7D lens wear followed by the peribulbar injection of Phosphate Buffer Solution (PBS) or bFGF. The untreated fellow eye served as a control. Guinea pigs with no treatment served as normal group. ·RESULTS:The results showed that 14 days of monocular defocus increased axial eye length and refraction, while bFGF delivery inhibited them markedly. Further, it was also found that the monocular-7D lens could decrease the levels of type I collagen, α2 integrin and β1 integrin expressions, while, unlike PBS, bFGF increased them significantly in comparison to contralateral control eyes and normal eyes. CONCLUSION:bFGF can prevent the formation anddevelopment of defocus myopia by upregulating the expressions of type I collagen, α2 integrin and β1 integrin.
