| 1 | Circulating tumor and cancer stem cells in hepatitis C virusassociated liver disease显示文摘AIM:To assess the role of circulating tumor cells(CTCs)and cancer stem cells(CSCs)in hepatitis C virus(HCV)-associated liver disease.METHODS:Blood and/or tissue samples were obtained from HCV(genotype 4)-associated hepatocellularcarcinoma patients(HCC;n=120),chronic hepatitis C patients(CH;n=30)and 33 normal control subjects(n=33).Serum levels of alpha-fetoprotein(AFP),alkaline phosphatase,and alanine and aspartate aminotransferases were measured.Cytokeratin 19(CK19)monoclonal antibody was used to enumerate CTCs,and CD133 and CD90 were used to enumerate CSCs by flow cytometry.The expression levels of the CSCs markers(CD133 and CD90)as well as telomerase,melanoma antigen encoding gene 1(MAGE1)and MAGE3 were assessed by RTPCR and quantitative real-time polymerase chain reactions.The number of CTCs and/or the expression levels of CK19,CD133,telomerase,MAGE1 and MAGE3 were correlated to the standard clinicopathologic prognostic factors and disease progression.RESULTS:Levels of AFP,alkaline phosphatase and aspartate aminotransferase were significantly different among the HCC,CH and control groups(P<0.001),whereas alanine aminotransferase differed significantly between patient(HCC and CH)and control groups(P<0.001).At the specified cutoff values determine by flow cytometry,CK19(49.8),CD90(400)and CD133(73)were significantly higher in the blood of HCC patients compared to those in the CH and control groups(P<0.001).On the other hand,CD133 at a 69.5 cutoff was significantly higher in the CH compared to the control group(P≤0.001).Telomerase,MAGE1 and MAGE3RNA were expressed in 55.71%,60.00%and 62.86%of the HCC patients,respectively,but were not detected in patients in the CH or control groups,which were statistically significant(Ps<0.001).The expression levels of telomerase,CD90,MAGE3,CD133 and CK19 were all significantly associated with high tumor grade and advanced stage in HCC patients(all P s<0.05).CONCLUSION:CTC counts and AFP,CK19,telomerase,and MAGE1/MAGE3 expression predict disease progression in patients with HCV,whereas telomerase,MAGE3,CD90,CD133 and CK19 are prognostic mark- | Abeer A Bahnassy Abdel-Rahman N Zekri Ahmed El-Bastawisy Amal Fawzy Marwa Shetta Nehal Hussein Dalia Omran Abdallah A S Ahmed Samir S El-Labbody | 2014 | World Journal of Gastroenterology2014,20,48: | 9 |

Chronic HCV is the primary cause of cirrhosis, hepatocellular carcinoma(HCC), and end-stage liver disease. Management of chronic HCV is aimed at preventing cirrhosis, reducing the risk of HCC, and treating extra hepatic complications. New treatments for chronic HCV has been devoted based on direct-acting antivirals, as pegylated interferon(peginterferon) is responsible for many side effects and limits treatment access. | 3 | Progression of diethylnitrosamine-induced hepatic carcinogenesis in carnitine-depleted rats显示文摘AIM:To investigate whether carnitine deficiency is a risk factor during the development of diethylnitrosamine (DENA)-induced hepatic carcinogenesis. METHODS:A total of 60 male Wistar albino rats were divided into six groups with 10 animals in each group.Rats in group 1(control group)received a single intraperitoneal(i.p.)injection of normal saline. Animals in group 2(carnitine-supplemented group) were given L-carnitine(200 mg/kg per day)in drinking water for 8 wk.Animals in group 3(carnitine-depleted group)were given D-carnitine(200 mg/kg per day)and mildronate(200 mg/kg per day)in drinking water for 8 wk.Rats in group 4(DENA group)were injected with a single dose of DENA(200 mg/kg,i.p.)and 2 wk later received a single dose of carbon tetrachloride(2 mL/kg) by gavage as 1:1 dilution in corn oil.Animals in group 5(DENA-carnitine depleted group)received the same treatment as group 3 and group 4.Rats in group 6 (DENA-carnitine supplemented group)received the same treatment as group 2 and group 4.RESULTS:Administration of DENA resulted in a significant increase in alanine transaminase(ALT), gamma-glutamyl transferase(G-GT),alkaline phosphatase(ALP),total bilirubin,thiobarbituric acid reactive substances(TBARS)and total nitrate/ nitrite(NOx)and a significant decrease in reduced glutathione(GSH),glutathione peroxidase(GSHPx), catalase(CAT)and total carnitine content in liver tissues.In the carnitine-depleted rat model,DENA induced a dramatic increase in serum ALT,G-GT,ALP and total bilirubin,as well as a progressive reduction in total carnitine content in liver tissues.Interestingly, L-carnitine supplementation resulted in a complete reversal of the increase in liver enzymes,TBARS and NOx,and a decrease in total carnitine,GSH,GSHPx, and CAT induced by DENA,compared with the control values.Histopathological examination of liver tissues confirmed the biochemical data,where L-carnitine prevented DENA-induced hepatic carcinogenesis while D-carnitine-mildronate aggravated DENA-induced hepatic damage. | 4 | Micro RNAs and clinical implications in hepatocellular carcinoma显示文摘AIM To assess the role of some circulating miRNAs(miR-23a, miR-203, miR338, miR-34, and miR-16) as tumor markers for diagnosis of hepatocellular carcinoma(HCC).METHODS One hundred and seventy-one subjects were enrolled, 57 patients with HCC, 57 patients with liver cirrhosis(LC) and 57 healthy subjects as control group. Severity of liver disease was assessed by Child Pugh score. Tumor staging was done using Okuda staging system. Quantification of Micro RNA(miR-23a, miR-203, miR338, miR-34, and miR-16) was performed.RESULTS All studied miRNA showed significant difference between HCC and cirrhotic patients in comparison tohealthy control. miR-23a showed statistically significant difference between HCC and cirrhotic patients being higher in HCC group than cirrhotic. miR-23a is significantly higher in HCC patients with focal lesion size equal or more than 5cm, patients with multiple focal lesions and Okuda stage Ⅲ. At cutoff value ≥ 210, miR-23a showed accuracy 79.3% to diagnose HCC patients with sensitivity 89.47% and specificity about 64.91%. At cut off level ≥ 200 ng/mL, serum alpha fetoprotein had 73.68% sensitivity, 52.63% specificity, 43.75% PPV, 80% NPV for diagnosis of HCC.CONCLUSION MicroR NA 23a can be used as a screening test for early detection of HCC. | 5 | Cytokine profile in Egyptian hepatitis C virus genotype-4 in relation to liver disease progression显示文摘AIM: To observe the imbalance between T helper cell Th1 and Th2 cytokines in several chronic hepatitis disease at different stages of disease progression.METHODS: We measured the cytokine levels of Th1 (IL-2 and IL-2R), Th2 (IL-10) and the pro-inflammatory cytokines (IL-6 and IL-6R and TNF and TNF-RⅠ and Ⅱ)by the ELISA technique in the sera of 33 hepatocellular carcinoma (HCC) patients and 20 chronic liver disease (CLD) patients. In addition, 20 asymptomatic hepatitis C virus carriers and 20 healthy subjects negative for hepatitis C virus(HCV) markers served as controls.RESULTS: Anti-HCV antibodies were found to be positive in 94% of HCC cases and 75% of CLD cases.On the other hand, HCV viremia was detected using RTPCR in 67% of HCC cases and 65% of CLD cases. HBsAg was positive in 9% of HCC cases and 30% of CLD cases.Also bilharzial-Ab was positive in 55% of HCC cases,65% of CLD cases and in 70% of asymptomatic carriers (ASC). HCC patients had significantly higher values of IL-2R, TNF-RⅡ (P<0.001), and TNF-RⅠ (P>0.05), but lower TNFα (P<0.001) and IL-6 (P = 0.032) in comparison to ASC. But, in comparison to non-cancer controls, HCC patients had higher values of IL-2R, IL-6R, TNF-RⅠ and TNF-RⅡ, but lower TNF-α (P<0.001). CLD patients had higher IL-2R, TNF-RⅠ, and TNF-RⅡ (P<0.001) than ASC. But, in comparison to non-cancer controls, CLD patients had higher values of IL-2R, TNF-RⅠ and TNF-RⅡ, but lower TNF-α (P<0.001). IL-10 was higher (though not significantly) in HCC and CLD patients than in symptomatic carriers and non-cancer controls.CONCLUSION: Liver disease progression from CLD to HCC due to HCV genotype-4 infection is associated with an imbalance between Th1 and Th2 cytokines. | 6 | Vitamin D status and viral response to therapy in hepatitis C infected children显示文摘AIM: To study the frequency of vitamin D deficiency in patients with hepatitis C virus(HCV) infection and to evaluate the role of vitamin D supplementation in improving antiviral therapy.METHODS: Sixty-six children aged from 7-14 years(mean ± SD, 11.17 ± 2.293) diagnosed with HCV infection were matched to 28 healthy controls. Serum levels of 25(OH) D3, calcium, phosphorus, alkaline phosphatase and plasma level of parathormone were measured. Quantitative PCR for HCV was performed Bone density was determined by dual energy X-ray absorptiometry. All cases received conventional therapy, and only 33 patients received vitamin D supplementation.RESULTS: Children with HCV showed significantly increased levels of HCV RNA(P < 0.001), parathormone(P < 0.01) and decreased vitamin D levels(P < 0.05)(33.3% deficient and 43.3% insufficient) compared with controls. Abnormal bone status(Z score-1.98 ± 0.75) was found in ribs, L-spine, pelvis and total body. Cases treated with vitamin D showed significant higher early(P < 0.04) and sustained(P < 0.05) virological response. There was a high frequency of vitamin D deficiency among the Egyptian HCV children, with significant decrease in bone density. The vitamin D level should be assessed before the start of antiviral treatment with the correction of any detected deficiency. | 7 | Effectiveness of hepatitis B virus vaccination program in Egypt:Multicenter national project显示文摘AIM:To assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated children.METHODS:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULTS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. Multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to < 10,10 to < 15 and ≥ 15 years respectively compared to those < 5 years and 1.1 among girls compared to boys with P < 0.01. HBs Ag was positive in 0.11% and breakthrough infection was 0.36% and 0.39% depending on positivity of anti-HBc and DNA detection respectively. The prevalence of HBV infection was significantly higher among children aged ≥ 7 years(0.59%) compared to 0.07% among younger children with odds ratio equal to 8.4(95%CI:1.1-64.2) and P < 0.01.The prevalence was higher among girls(0.48%) than boys(0.29%) with P > 0.05.
