1Prevalence of obesity and diabetes in patients with schizophrenia显示文摘AIM To compare the prevalence of diabetes in patients with schizophrenia treated at a community mental health center with controls in the same metropolitan area and to examine the effect of antipsychotic exposure on diabetes prevalence in schizophrenia patients. METHODS The study was a comprehensive chart review of psychiatric notes of patients with schizophrenia and schizoaffective disorder treated at a psychosis program in a community mental health center. Data collected included psychiatric diagnoses, diabetes mellitus diagnosis, medications, allergies, primary care status, height, weight, body mass index(BMI), substance use and mental status exam. Local population data was downloaded from the Centers for Disease Control Behavioral Risk Factor Surveillance System. Statistical methods used were χ~2 test, Student's t test, general linear model procedure and binary logistic regression analysis.RESULTS The study sample included 326 patients with schizophrenia and 1899 subjects in the population control group. Demographic data showed control group was on average 7.6 years older(P = 0.000), more Caucasians(78.7% vs 38.3%, P = 0.000), and lower percentage of males(40.7% vs 58.3%, P = 0.000). Patients with schizophrenia had a higher average BMI than the subjects in the population control(32.11, SD = 7.72 vs 27.62, SD = 5.93, P = 0.000).Patients with schizophrenia had a significantly higher percentage of obesity(58.5% vs 27%, P = 0.000) than the population group. The patients with schizophrenia also had a much higher rate of diabetes compared to population control(23.9% vs 12.2%, P = 0.000). After controlling for age sex, and race, having schizophrenia was still associated with increased risk for both obesity(OR = 3.25, P = 0.000) and diabetes(OR = 2.42, P = 0.000). The increased risk for diabetes remained even after controlling for obesity(OR = 1.82, P = 0.001). There was no difference in the distribution of antipsychotic dosage, second generation antipsychotic use or multiple antipsychotic use within different BMI categories or with diabetes status in the schizophrenia group. CONCLUSION This study demonstrates the high prevalence of obesity and diabetes in schizophrenia patients and indicates that antipsychotics may not be the only contributor to this risk.Aniyizhai Annamalai Urska Kosir Cenk Tek 2017World Journal of Diabetes2017,8,8:6
2Management of refractory ascites in cirrhosis:Are we out of date?显示文摘Cirrhosis is a major cause of morbidity and mortality worldwide with liver transplantations as it only possible cure.In the face of a significant organ shortage many patients die waiting.A major complication of cirrhosis is the development of portal hypertension and ascites.The management of ascites has barely evolved over the last hundred years and includes only a few milestones in our treatment approach,but has overall significantly improved patient morbidity and survival.Our mainstay to ascites management includes changes in diet,diuretics,shunt procedures,and large volume paracentesis.The understanding of the pathophysiology of cirrhosis and portal hypertension has significantly improved in the last couple of decades but the changes in ascites management have not seemed to mirror this newer knowledge.We herein review the history of ascites management and discuss some its current limitations.Alagappan Annamalai Lauren Wisdom Megan Herada Mazen Nourredin Walid Ayoub Vinay Sundaram Andrew Klein Nicholas Nissen 2016World Journal of Hepatology2016,8,28:4
3p-Coumaric acid ameliorates ethanol-induced kidney injury by inhibiting inflammatory cytokine production and NF-κB signaling in rats显示文摘Objective: To examine the effects of p-coumaric acid on ethanol-induced kidney injury in Swiss Wistar rats.Methods: Ethanol(25% v/v) was used to induce nephrotoxicity in rats. p-Coumaric acid was orally administered at 50, 100, or 200 mg/kg body weight. The levels of oxidative parameters were determined; pro-inflammatory biomarkers were analyzed by Western blotting and apoptotic protein was analyzed by immunohistochemistry. Results: Ethanol treated rats showed decreased levels of antioxidants and aberrant production of pro-inflammatory cytokines(IL-6, IL1β, TNF-α), NF-κB activation and imbalance of proand anti-apoptotic proteins(Bcl-2, Bax, caspase 3). Meanwhile, p-coumaric acid restored antioxidant levels and decreased the levels of inflammatory cytokines, NF-κB, and proapoptotic proteins and increased Bcl-2 expression. 5Screening and antibacterial efficacy of selected Indian medicinal plants显示文摘Objective: To evaluate the antibacterial efficacy of five Indian medicinal plants such as Acalypha indica L.(A. indica), Aerva lanata(L.) Juss. ex Schult.(A. lanata), Clerodendrum inerme(L.) Gaertn., Pergularia daemia(Forsk.) Chiov. and Solanum surattense Burm. f. against opportunistic bacterial pathogens isolated from HIV infected patients for the potential phytoconstituents in plant extracts.Methods: The opportunistic bacterial pathogens such as Escherichia coli(E. coli),Pseudomonas aeruginosa, Salmonella typhi and Serratia marcescens from Gramnegative group and Staphylococcus aureus from Gram-positive group were isolated from HIV infected patients. The antibacterial efficacy of ethanolic extracts of selected medicinal plants was carried out by disc diffusion method. The potential phytoconstituents of medicinal plant extracts were identified by gas chromatography and mass spectrometry(GC–MS) analysis.Results: Among the five medicinal plants tested, A. indica and A. lanata showed the significant antibacterial activity. A. indica showed potential activity against Staphylococcus aureus and E. coli. A. lanata significantly exhibited antibacterial activity against E. coli, Salmonella typhi and Pseudomonas aeruginosa. A total of 19 phytoconstituents were identified in the ethanolic extract of A. indica and A. lanata by GC–MS analysis respectively.Conclusions: The results of the present investigation revealed that A. indica and A. lanata, possessed significant antibacterial activity when compared with the other plant extracts tested.
