| 1 | 全科医学中胸痛患者的管理:一项访谈研究显示文摘背景在全科医学中胸痛的评估是具有挑战性的。全科医生在处理胸痛时经常感到不确定性,新型诊断工具的作用尚不明确。因此,本研究致力于了解:全科医生是否有经验能够感知胸痛的发生和表现形式的变化;全科医生如何处理不确定性;全科医生对新型诊断工具的想法、要求和疑虑。方法针对6个主要课题:有经验的感知胸痛表现形式的变化、对胸痛患者的管理、处理不确定性、全科医生转诊胸痛患者的方法、全科医生对不必要转诊的看法、全科医生对改善胸痛患者管理的建议进行半结构化面对面访谈。结果 145名比利时和荷兰全科医生受邀参加访谈,其中27人(比利时15人,荷兰12人)接受了采访。数据达到饱和度。胸痛患者中,发生急性冠脉综合征的患者人数减少,而非典型胸痛患者增加,这致使全科医生诊断胸痛的不确定性增加。全科医生尤其依靠自己的判断,并希望新型诊断工具能够具有更多价值。结论全科医学中胸痛的发病率未降低,而胸痛的表现形式却在变化。将无急性冠脉综合征的胸痛患者进行转诊,全科医生感到安心,因为转诊更保险。不确定性被视为全科医生职业中无可避免的一部分,我们需谨慎对待新型诊断工具。 | BIESEMANS L CLEEF L E WILLEMSEN R T A 本刊编辑部 | 2018 | 中国全科医学2018，21，20： | 2 |
