| 1 | 中国妇女妊娠前后单纯服用叶酸对神经管畸形的预防效果显示文摘目的 评价妇女在妊娠前后服用单纯 40 0 μg叶酸增补剂对胎 /婴儿神经管畸形 (NTDs)的预防效果。方法 1993~ 1995年在中国北方的NTDs高发地区和南方的NTDs低发地区妇女增补叶酸的推广项目中 ,共募集从孕前或孕后任何时间开始服药的妇女 130 142名 ,未服药的妇女 1176 89名 ;设计的服药方法从婚检时开始到孕满 3个月为止 ,每天服用单纯叶酸片 40 0 μg ;最后对妇女的分娩结局进行监测并进行预防效果的对比评价研究。结果 服药组妇女生育的胎婴儿中共发现 10 2例NTDs ,对照组胎婴儿中共发现 173例NTDs。末次月经前募集的未服药妇女在孕 2 0周以后分娩的胎婴中NTDs发生率 ,北方为 4 8‰ (16 / 3 318) ,南方为 1 0‰ (2 8/ 2 82 6 5 ) ,而妊娠前后服药妇女组则分别为1 0‰ (13/ 13 0 12 )和 0 6‰ (34/ 5 86 38)。与末次月经前募集的未服药妇女组相比 ,北方服药妇女NTDs发生率明显降低 ,其中依从性大于 80 %的服药组妇女预防率达 85 % (95 %CI为 6 2 %~ 94% ) ,南方地区服叶酸的预防率为 41% (95 %CI为 3 %~ 6 4% )。结论 妇女在妊娠前后每天服用单纯叶酸 40 0 | 李竹 RobertJ Berry 李松 J David Erickson 王红 Cynthia A Moore 赵平 Joseph Mulinare 洪世欣 Lee-Yang C Wong 呼和牧人 Jacqueline Gindler 郝玲 Adolfo Correa 朱慧萍 Elaine Gunter | 2000 | 中华医学杂志2000，80，7： | 185 |
| 2 | Rheumatic manifestations of inflammatory bowel disease显示文摘This article reviews the literature concerning rheu-matic manifestations of inflammatory bowel disease (IBD),including common immune-mediated pathways,frequency,clinical course and therapy. Musculoskel-etal complications are frequent and well-recognized manifestations in IBD,and affect up to 33% of pa-tients with IBD. The strong link between the bowel and the osteo-articular system is suggested by many clinical and experimental observations,notably in HLA-B27 transgenic rats. The autoimmune pathogenic mechanisms shared by IBD and spondyloarthropathies include genetic susceptibility to abnormal antigen pre-sentation,aberrant recognition of self,the presence of autoantibodies against specific antigens shared by the colon and other extra-colonic tissues,and increased intestinal permeability. The response against microor-ganisms may have an important role through molecular mimicry and other mechanisms. Rheumatic mani-festations of IBD have been divided into peripheral arthritis,and axial involvement,including sacroiliitis,with or without spondylitis,similar to idiopathic anky-losing spondylitis. Other periarticular features can oc-cur,including enthesopathy,tendonitis,clubbing,peri-ostitis,and granulomatous lesions of joints and bones.Osteoporosis and osteomalacia secondary to IBD and iatrogenic complications can also occur. The manage-ment of the rheumatic manifestations of IBD consists of physical therapy in combination with local injection of corticosteroids and nonsteroidal anti-inflammatory drugs; caution is in order however,because of their possible harmful effects on intestinal integrity,perme-ability,and even on gut inflammation. Sulfasalazine,methotrexate,azathioprine,cyclosporine and lefluno-mide should be used for selected indications. | 3 | Functions of NKG2D in CD8^(+) T cells: an opportunity for immunotherapy显示文摘Natural killer group 2 member D(NKG2D)is a type II transmembrane receptor.NKG2D is present on NK cells in both mice and humans,whereas it is constitutively expressed on CD8+T cells in humans but only expressed upon T-cell activation in mice.NKG2D is a promiscuous receptor that recognizes stress-induced surface ligands.In NK cells,NKG2D signaling is sufficient to unleash the killing response;in CD8+T cells,this requires concurrent activation of the T-cell receptor(TCR).In this case,the function of NKG2D is to authenticate the recognition of a stressed target and enhance TCR signaling.CD28 has been established as an archetype provider of costimulation during T-cell priming.It has become apparent,however,that signals from other costimulatory receptors,such as NKG2D,are required for optimal T-cell function outside the priming phase.This review will focus on the similarities and differences between NKG2D and CD28;less well-described characteristics of NKG2D,such as the potential role of NKG2D in CD8+T-cell memory formation,cancer immunity and autoimmunity;and the opportunities for targeting NKG2D in immunotherapy. | Kushal Prajapati Cynthia Perez Lourdes Beatriz Plaza Rojas Brianna Burke Jose A Guevara-Patino | 2018 | Cellular & Molecular Immunology2018，15，5： | 7 |
| 4 | Endoscopic retrograde cholangiography for pediatric choledocholithiasis: Assessing the need for endoscopic intervention显示文摘AIM:To assess pediatric patients for choledocholithiasis.We applied current adult guidelines to identify predictivefactors in children.METHODS:A single-center retrospective analysis was performed at a tertiary children's hospital.We evaluated 44 consecutive pediatric patients who underwent endoscopic retrograde cholangiography(ERCP) for suspected choledocholithiasis.Patients were stratified into those with common bile duct stones(CBDS) at ERCP vs those that did not using the American Society of Gastrointestinal Endoscopy(ASGE) guidelines(Very Strong and Strong criteria) for suspected CBDS.RESULTS:CBDS were identified in 84% at the time of ERCP.Abdominal ultrasound identified CBDS in 36% of patients.Conjugated bilirubin ≥ 0.5 mg/d L was an independent risk factor for CBDS(P = 0.003).The Very Strong(59.5%) and Strong(48.6%) ASGE criteria identified the majority of patients(P = 0.0001).A modified score using conjugated bilirubin had a higher sensitivity(81.2% vs 59.5%) and more likely to identify a stone than the standard criteria,odds ratio of 25.7 compared to 8.8.Alanine aminotransferase and gamma-glutamyl transferase values identified significant differences in a subset of patients with odds ratio of 4.1 and 3.25,respectively.CONCLUSION:Current adult guidelines identified the majority of pediatric patients with CBDS,but specific pediatric guidelines may improve detection,thus decreasing risks and unnecessary procedures. | Douglas S Fishman Bruno P Chumpitazi Isaac Raijman Cynthia Man-Wai Tsai E O’Brian Smith Mark V Mazziotti Mark A Gilger | 2016 | World Journal of Gastrointestinal Endoscopy2016，8，11： | 6 |

| 11 | Thyroid lymphoma: A case report and literature review显示文摘Thyroid lymphoma is an unusual pathology. Different subtypes of lymphoma can present as primary thyroid lymphoma. This review illustrates via imaging, findings and treatment the need for accurate diagnosis and timely treatment. Patients and methods: patient's chart, pathological findings and radiological images were reviewed in a retrospective analysis. Over several days, this 80 years old woman developed airway obstruction and rapid enlargement of her thyroid secondary to diffuse large B-cell lymphoma. She rapidly responded to her oncological protocol. Primary thyroid lymphoma is a rare disease. It is an important diagnosis to consider in patients presenting with rapidly enlarging neck masses. | 18 | Immunological treatment of liver tumors显示文摘Although multiple options for the treatment of liver tumors have often been described in the past, including liver resection, radiofrequency ablation with or without hepatic pump insertion, laparoscopic liver resection and the use of chemotherapy, the potential of immunotherapy and gene manipulation is still largely unexplored.Immunological therapy by gene manipulation is based on the interaction between virus-based gene delivery systems and dendritic cells. Using viruses as vectors, it is possible to transduce dendritic cells with genes encoding tumor-associated antigens, thus inducing strong humoral and cellular immunity against the antigens themselves.Both chemotherapy and radiation therapy have the disadvantage of destroying healthy cells, thus causing severe side-effects. We need more precisely targeted therapies capable of killing cancer cells while sparing healthy cells.
