| 1 | 世界家庭医生组织(WONCA)研究论文摘要汇编——老年人慢性病共病状况与其参与医疗决策偏好之间关系的调查研究显示文摘目的了解个人参与医疗决策的偏好是实施以患者为中心的医疗服务的基础。本文旨在探究美国老年人参与医疗决策的偏好及其慢性病共病情况是否与不爱参与或被动参与临床决策相关。方法本项为横断面研究。研究对象为随机抽取的2 017例具有全国代表性的样本,其数据来源于国家卫生和老龄化趋势研究。偏好被动参与医疗决策是指,患者自身不参与决策过程,只让医生做临床决策。研究对象的慢性病共病状况从以下3个方面进行探讨:(1)判定是否患有慢性病共病:患有2种或2种以上慢性病共病谱(本研究中此疾病谱共包含13种慢性病)中所包括的慢性病被认定为慢性病共病患者;(2)慢性病共病患者所患慢性病简单分类结果;(3)慢性病共病患者所患慢性病聚类分析结果。在调整年龄、性别、文化程度、英语是否熟练及活动受限程度等因素后,使用多元Logistic回归对老年人慢性病共病状况与其参与医疗决策的偏好之间的关系进行分析。结果大部分老年人喜欢主动参与医疗决策。不过,患有4种或4种以上慢性病的老年人以及患有多聚类慢性病的老年人,较少参与医疗决策。结论全科医师应鼓励患有4种或4种以上慢性病的老年人以及患多聚类慢性病的老年人参与临床决策。同时,全科医师应考虑到不同老年人有不同的参与临床决策的偏好,并且要适应他们的偏好以保证能更好地实施以患者为中心的医疗服务。 | CHI W C WOLFF J CREER R 本刊编辑部 | 2018 | 中国全科医学2018，21，2： | 2 |
