| 3 | ^(18)F-FDG PET/CT显像在鼻咽癌分期与疗效监测中的临床应用价值显示文摘背景与目的:18F-脱氧葡萄糖(fluorine-18 fluorodeoxyglucose,18F-FDG)PET/CT显像可明显提高肺癌、食管癌等多种肿瘤的诊断、分期与疗效监测的准确性,有助于更准确地制定治疗方案。本研究探讨全身18F-FDG PET/CT显像在鼻咽癌首次分期、再分期及疗效监测中的临床应用价值。方法:回顾性分析澳大利亚Peter MacCallum肿瘤中心2002年2月至2005年12月43例鼻咽癌患者的18F-FDG PET/CT全身扫描报告,根据临床资料、病理结果及临床随访结果,计算18F-FDG PET/CT与传统影像学检查CT、MRI的准确性、特异性、灵敏度、阳性预测值与阴性预测值,并对结果进行比较和分析。结果:18F-FDG PET/CT诊断鼻咽癌总的准确率、敏感性、特异性、阳性预测值与阴性预测值分别为95.3%、100.0%、85.7%、93.8%、100.0%,传统影像学检查CT、MRI分别为65.5%、79.4%、64.7%、81.8%、57.9%。18F-FDGPET/CT诊断结果使2例首次分期、7例再分期患者治疗方案得到改变,并影响1例首次分期和3例再分期患者治疗方案的制定;在疗效监测组中,指导医生修改治疗方案共11例(其中5例为原则性的修改)。18F-FDG PET/CT检测到2例第二原发肿瘤,1例是甲状腺癌,1例是低度恶性胃癌。结论:18F-FDG PET/CT全身显像对鼻咽癌N、M分期与疗效监测的临床作用可能优于CT、MR检查。 | 王国慧 Eddie W. | 5 | Estrogen deficiency reversibly induces telomere shortening in mouse granulosa cells and ovarian aging in vivo显示文摘Estrogen is implicated as playing an important role in aging and tumorigenesis of estrogen responsive tissues;however the mechanisms underlying the mitogenic actions of estrogen are not fully understood.Here we report that estrogen deficiency in mice caused by targeted disruption of the aromatase gene results in a signi-ficant inhibition of telomerase maintenance of telomeres in mouse ovaries in a tissue-specific manner.The inhibition entails a significant shortening of telomeres and compromised proliferation in the follicular granulosa cell compartment of ovary.Gene expression analysis showed decreased levels of proto-oncogene c-Myc and the telomerase catalytic subunit,telomerase reverse transcriptase(TERT),in response to estrogen deficiency.Estrogen replacement therapy led to increases in TERT gene expression,telomerase activity,telomere length and ovarian tissue growth,thereby reinstating ovary development to normal in four weeks.Our data demonstrate for the first time that telomere maintenance is the primary mechanism mediating the mitogenic effect of estrogen on ovarian granulosa cell proliferation by upregulating the genes of c-Myc and TERT in vivo.Estrogen deficiency or over-activity may cause ovarian tissue aging or tumorigenesis,respectively,through estrogen regulation of telomere remodeling. | Sharyn Bayne He Li Margaret E.E.Jones Alex R.Pinto Michelle van Sinderen Ann Drummond Evan R.Simpson Jun-Ping Liu | 2011 | Protein & Cell2011,2,4: | 8 |

| 7 | ^(18)F-FDG PET/CT发现肿瘤患者意外病灶的临床价值显示文摘目的评价^(18)F-脱氧葡萄糖(FDG)PET/CT 全身显像探测肿瘤患者意外病灶的临床价值。方法回顾性分析澳大利亚 Peter MacCallum 肿瘤医院 PET/CT 中心2002年8月~2003年7月1727例肿瘤患者^(18)F-FDG PET/CT 全身显像报告,男980例,女747例,中位年龄63岁,随访37个月(中位时间为27.5个月)。意外病灶指新发现的与原发肿瘤无关的良性病灶或第2原发肿瘤。通过分析病历记录、病理结果、其他影像学检查结果及临床随访,确定病灶最终结果。结果 1727例患者共发现238例(13.8%)有意外病灶,其中213例220个意外病灶具有完整的随访资料。60个病灶怀疑为意外第2原发肿瘤,其中21.7%(13/60)证实为真阳性,70.0%(42/60)为假阳性,8.3%(5/60)至研究结束时未得到证实。38个甲状腺意外良性病灶,其中9个被证实为临床前恶性或恶性病灶,26个良性,3个未能证实。38个纵隔意外良性病灶,其中86.8%(33/38)证实为良性,7.9%(3/38)为原发肿瘤淋巴结侵犯,5.3%(2/38)未能证实其性质。其他部位84个意外良性病灶,94.0%(79/84)证实为真阴性,6.0%(5/84)未能证实。共22例患者被证实存在第2原发肿瘤病灶,占本研究总人群的1.3%。结论 ^(18)F-FDG PET/CT 全身显像是目前探测肿瘤患者意外病灶较好的无创方法,为制定临床治疗方案提供及时准确的依据。 | 王国慧 Eddie W.F. | 8 | Carbon dioxide insufflation during colonoscopy in deeply sedated patients显示文摘AIM:To compare the impact of carbon dioxide(CO2) and air insufflation on patient tolerance/safety in deeply sedated patients undergoing colonoscopy.METHODS:Patients referred for colonoscopy were randomized to receive either CO2 or air insufflation during the procedure.Both the colonoscopist and patient were blinded to the type of gas used.During the procedure,insertion and withdrawal times,caecal intubationrates,total sedation given and capnography readings were recorded.The level of sedation and magnitude of patient discomfort during the procedure was assessed by a nurse using a visual analogue scale(VAS)(0-3).Patients then graded their level of discomfort and abdominal bloating using a similar VAS.Complications during and after the procedure were recorded.RESULTS:A total of 142 patients were randomized with 72 in the air arm and 70 in the CO2 arm.Mean age between the two study groups were similar.Insertion time to the caecum was quicker in the CO2 group at 7.3 min vs 9.9 min with air(P = 0.0083).The average withdrawal times were not significantly different between the two groups.Caecal intubation rates were 94.4% and 100% in the air and CO2 groups respectively(P = 0.012).The level of discomfort assessed by the nurse was 0.69(air) and 0.39(CO2)(P = 0.0155) and by the patient 0.82(air) and 0.46(CO2)(P = 0.0228).The level of abdominal bloating was 0.97(air) and 0.36(CO2)(P = 0.001).Capnography readings trended to be higher in the CO2 group at the commencement,caecal intubation,and conclusion of the procedure,even though this was not significantly different when compared to readings obtained during air insufflation.There were no complications in both arms.CONCLUSION:CO2 insufflation during colonoscopy is more efficacious than air,allowing quicker and better cecal intubation rates.Abdominal discomfort and bloating were significantly less with CO2 insufflation. | Rajvinder Singh Eu Nice Neo Nazree Nordeen Ganesananthan Shanmuganathan Angelie Ashby Sharon Drummond Garry Nind Elizabeth Murphy Andrew Luck Graeme Tucker William Tam | 2012 | World Journal of Gastroenterology2012,18,25: | 3 |

| 10 | It takes more than tau to tangle:using proteomics to determine how phosphorylated tau mediates toxicity in neurodegenerative diseases显示文摘Two of the most common causes of dementia are Alzheimer’s disease(AD)and frontotemporal dementia(FTD).AD is an irreversible,progressive neurodegenerative disorder that is clinically characterized by severe memory loss and behavioral impairment that eventually interferes with everyday function.AD is neuropathologically defined by the presence of extracellularβ-amyloid plaques and intracellular accumulation of neurofibrillary tangles(NFTs)that primarily contain aggregated,hyperphosphorylated tau(pTau).Intriguingly,pTau is also the central protein in multiple subtypes of FTD(e.g.corticobasal degeneration,progressive supranuclear palsy,Pick’s disease).FTD is an umbrella term for a group of neurological conditions that primarily affect the temporal and frontal regions of the brain.Mutations in the tau gene(MAPT)can cause familial FTD,providing further evidence of the integral role of tau in FTD.Physiologically,tau regulates microtubule structure and dynamics,as well as axonal transport through interaction with tubulin.Tau is also involved in neuronal development and synaptogenesis.In AD and FTD,tau becomes hyperphosphorylated and undergoes major conformational changes,causing it to aggregate into the characteristic neuropathological lesions that define AD and FTD.Despite the known involvement of tau in these diseases,exactly how tau mediates toxicity is still unclear. | Geoffrey Pires Eleanor Drummond | 2021 | Neural Regeneration Research2021,16,11: | 2 |
