1Bone Regeneration Based on Tissue Engineering Conceptions – A 21st Century Perspective显示文摘The role of Bone Tissue Engineering in the field of Regenerative Medicine has been the topic of substantial research over the past two decades.Technological advances have improved orthopaedic implants and surgical techniques for bone reconstruction.However,improvements in surgical techniques to reconstruct bone have been limited by the paucity of autologous materials available and donor site morbidity.Recent advances in the development of biomaterials have provided attractive alternatives to bone grafting expanding the surgical options for restoring the form and function of injured bone.Specifically,novel bioactive(second generation)biomaterials have been developed that are characterised by controlled action and reaction to the host tissue environment,whilst exhibiting controlled chemical breakdown and resorption with an ultimate replacement by regenerating tissue.Future generations of biomaterials(third generation)are designed to be not only osteoconductive but also osteoinductive,i.e.to stimulate regeneration of host tissues by combining tissue engineering and in situ tissue regeneration methods with a focus on novel applications.These techniques will lead to novel possibilities for tissue regeneration and repair.At present,tissue engineered constructs that may find future use as bone grafts for complex skeletal defects,whether from post-traumatic,degenerative,neoplastic or congenital/developmental'origin'require osseous reconstruction to ensure structural and functional integrity.Engineering functional bone using combinations of cells,scaffolds and bioactive factors is a promising strategy and a particular feature for future development in the area of hybrid materials which are able to exhibit suitable biomimetic and mechanical properties.This review will discuss the state of the art in this field and what we can expect from future generations of bone regeneration concepts.Jan Henkel Maria A.Woodruff Devakara R.Epari Roland Steck Vaida Glatt Ian C.Dickinson Peter F.M.Choong Michael A.Schuetz Dietmar W.Hutmacher 2013Bone Research2013,1,3:33
2Antioxidants,inflammation and cardiovascular disease显示文摘Multiple factors are involved in the etiology of cardiovascular disease(CVD). Pathological changes occur in a variety of cell types long before symptoms become apparent and diagnosis is made. Dysregulation of physiological functions are associated with the activation of immune cells,leading to local and finally systemic inflammation that is characterized by production of high levels of reactive oxygen species(ROS). Patients suffering from inflammatory diseases often present with diminished levels of antioxidants either due to insufficient dietary intake or,and even more likely,due to increased demand in situations of overwhelming ROS production by activated immune effector cells like macrophages. Antioxidants are suggested to beneficially interfere with diseases-related oxidative stress,however the interplay of endogenous and exogenous antioxidants with the overall redox system is complex. Moreover,molecular mechanisms underlying oxidative stress in CVD are not fully elucidated. Metabolic dybalances are suggested to play a major role in disease onset and progression. Several central signalingpathways involved in the regulation of immunological,metabolic and endothelial function are regulated in a redox-sensitive manner. During cellular immune response,interferon γ-dependent pathways are activated such as tryptophan breakdown by the enzyme indoleamine 2,3-dioxygenase(IDO) in monocyte-derived macrophages,fibroblasts,endothelial and epithelial cells. Neopterin,a marker of oxidative stress and immune activation is produced by GTP-cyclohydrolase Ⅰ in macrophages and dendritic cells. Nitric oxide synthase(NOS) is induced in several cell types to generate nitric oxide(NO). NO,despite its low reactivity,is a potent antioxidant involved in the regulation of the vasomotor tone and of immunomodulatory signaling pathways. NO inhibits the expression and function of IDO. Function of NOS requires the cofactor tetrahydrobiopterin(BH4),which is produced in humans primarily by fibroblasts and endothelial cells. Highly toxic peroxynitrite(ONOO-) is formed solely in the presence of superoxide anion(O2-). Neopterin and kynurenine to tryptophan ratio(Kyn/Trp),as an estimate of IDO enzyme activity,are robust markers of immune activation in vitro and in vivo. Both these diagnostic parameters are able to predict cardiovascular and overall mortality in patients at risk. Likewise,a significant association exists between increase of neopterin concentrations and Kyn/Trp ratio values and the lowering of plasma levels of vitamin-C,-E and-B. Vitamin-B deficiency is usually accompanied by increased plasma homoycsteine. 5Sirolimus inhibits growth of human hepatoma cells alone or combined with tacrolimus, while tacrolimus promotes cell growth显示文摘AIM: Standard immunosuppression after organ transplantation stimulates tumor growth. Sirolimus has a strong antiproliferative and a tumor inhibiting effect. The purpose is to assess the effect on tumor growth of the immunosuppressive compounds sirolimus and tacrolimus alone and in combination on cells of human hepatocellular carcinoma.METHODS: We used the human cell lines SK-Hep 1 and Hep 3B derived from hepatocellular carcinoma. Proliferation analyses after treatment with sirolimus, tacrolimus, or the combination of both were performed. FACS analyses were done to reveal cell cycle changes and apoptotic cell death. The expression of apoptosis-related proteins was estimated by Western blots.RESULTS: Sirolimus alone or combined with tacrolimus inhibited the growth of both cell lines after 5 d by up to 35% in SK-Hep 1 cells, and by up to 68% in Hep 3B cells at 25 ng/mL. Tacrolimus alone stimulated the growth by 12% after 5 ng/mL and by 25% after 25 ng/mL in Hep 3B cells. We found an increase of apoptotic Hep 3B cells from 6 to 16%, and a G1-arrest in SK-Hep 1 cells with an increase of cells from 61 to 82%, when sirolimus and tacrolimus were combined. Bcl-2 was down-regulated in Hep 3B, but not in SK-Hep 1 cells after combined treatment.CONCLUSION: Sirolimus appears to inhibit the growth of hepatocellular carcinoma cells alone and in combination with tacrolimus. 8BRAIN INJURY BIOMECHANICS IN REAL WORLD VEHICLE ACCIDENT USING MATHEMATICAL MODELS显示文摘This paper aims at investigating brain injury mechanisms and predicting head injuries in real world accidents. For this purpose, a 3D human head finite element model (HBM-head) was developed based on head-brain anatomy. The HBM head model was validated with two experimental tests. Then the head finite element(FE) model and a multi-body system (MBS) model were used to carry out reconstructions of real world vehicle-pedestrian accidents and brain injuries. The MBS models were used for calculating the head impact conditions in vehicle impacts. The HBM-head model was used for calculating the injury related physical parameters, such as intracranial pressure, stress, and strain. The calculated intracranial pressure and strain distribution were correlated w/th the injury outcomes observed from accidents. It is shown that this model can predict the intracranial biomechanical response and calculate the injury related physical parameters. 9Matched-pair analysis of postoperative morbidity and mortality for pancreaticogastrostomy and pancreaticojejunostomy using mattress sutures in soft pancreatic tissue remnants显示文摘BACKGROUND:After pancreaticoduodenectomy,the incidence of postoperative pancreatic fistula remains high,especially in patients with 'soft' pancreatic tissue remnants.No 'gold standard' surgical technique for pancreaticoenteric anastomosis has been established.This study aimed to compare the postoperative morbidity and mortality of pancreaticogastrostomy and pancreaticojejunostomy for 'soft' pancreatic tissue remnants using modified mattress sutures.METHODS:Seventy-five patients who had undergone pancreaticogastrostomy and 75 who had undergone pancreaticojejunostomy after pancreaticoduodenectomy between 2002 and 2008 were retrospectively compared using matched-pair analysis.A modified mattress suture technique was used for the pancreaticoenteric anastomosis.Patients with an underlying 'hard' pancreatic tissue remnant,as in chronic pancreatitis,were excluded.Both groups were homogeneous for age,gender,and underlying disease.Postoperative morbidity,mortality,and preoperative and operative data were analyzed.RESULTS:There were no significant differences between the groups for the incidence of postoperative pancreatic fistula (10.7% in both).Postoperative morbidity and mortality,median operation time,median length of hospital stay,intraoperative blood loss,and the amount of intraoperatively transfused erythrocyte concentrates also did not significantly differ between the groups.Patient age >65 years (P=0.017),operation time >350minutes (P=0.001),and intraoperative transfusion of erythrocyte concentrates (P=0.038) were identified as risk factors for postoperative morbidity.CONCLUSIONS:Our results showed no significant differences between the groups in the pancreaticogastrostomy and pancreaticojejunostomy anastomosis techniques using mattress sutures for 'soft' pancreatic tissue remnants.In our experience,the mattress sutures are safe and simple to use,and pancreaticogastrostomy in particular is feasible and easy to learn,with good endoscopic accessibility to the anastomosis region.However,the location of the anastomosis and the surgical technique need to be individually evaluated to further reduce the incidence of postoperative pancreatic fistula.Fritz Klein Marcus Bahra Matthias Glanemann Wladimir Faber Peter Warnick Andreas Andreou Safak Gül Dietmar Jacob 2012Hepatobiliary & Pancreatic Diseases International2012,11,1:9
10Subsidence history and forming mechanism of anomalous tectonic subsidence in the Bozhong depression, Bohaiwan basin显示文摘The Bozhong depression of the Bohaiwan basin belongs to a family of extensional basins in East China, but is quite different from other parts of the basin. The Cenozoic subsidence of the depression is controlled by a combination of lithospheric thinning and polycyclic strike-slip movements. Three episodic rifts have been identified, i.e. Paleocence-early Eocene, middle-late Eocene and Oligocene age. The depression underwent syn-rift and post-rift stages, but two episodic dextral movement events of the strike-slip faults modify the subsidence of the Bozhong depression since the Oligocene. The early dextral movement of the Tan-Lu fault associated with crustal extension resulted in accelerated subsidence during the time of deposition of the Dongying Formation with a maximum thickness of 4000 m. A late reactivation of dextral movement of the Tan-Lu fault began in late Miocene (about 12 Ma), which resulted in the intense subsidence of Minghuazhen Formation and Quaternary. In addition, dynamic mantle convection-driven topography also accelerated the post-rift anomalous subsidence since the Miocene (24.6 Ma). Our results indicate that the primary control on rapid subsidence both during the rift and post-rift stages in the Bozhong depression originates from a combination of multiple episodic crustal extension and polycyclic dextral movements of strike-slip faults, and dynamic topography.MüLLER R. 13微血管减压术治疗原发性三叉神经痛显示文摘目的提出微血管减压术是治疗原发性三叉神经痛的最佳手术方式。方法报告了12例原发性三叉神经痛患者行微血管减压术,其中11例发现三叉神经入脑干区动脉压迫神经根,将压迫血管与神经根分离,在其间植入纤维蛋白海绵或聚四氟乙烯片维持分离状态,术后除1例疼痛减轻外,其余疼痛完全缓解,另1例术中未发现责任血管,但在神经根远侧段发现有粘连,将粘连松解,使神经根游离。结果所有患者术后疼痛减轻或缓解,所有病例无并发症。结论微血管减压术治疗原发性三叉神经痛是去除病因的手术方式,对已行微血管减压术后复发的患者再次行微血管减压术亦可获得满意疗效,作者还认为,神经内镜辅助微血管减压术顺应了当今微侵袭外科的趋势,将有望被更多地用来治疗原发性三叉神经痛。黄元奎 Dietmar Stolke 2006中华神经外科疾病研究杂志2006,5,1:7
14经颅修补及颅底重建治疗外伤性脑脊液鼻漏(附2例报告)显示文摘目的探讨经颅修补及颅底重建手术在治疗外伤性脑脊液鼻漏中的有效性和价值。方法分析2例典型外伤性脑脊液鼻漏病人的手术治疗,均经影像学检查和鼻分泌物Beta-2转铁蛋白实验确诊。其中1例存在右侧筛板骨折,部分塌陷;另1例存在额窦内壁及框顶部分缺损。均行经颅修补及颅底重建手术,术中用略大于漏口的颅骨膜片修补漏口,再用肌肉筋膜覆盖其上,涂以生物胶,颅底及额窦内壁用相应大小的钛网重建并固定,以恢复颅底及颅腔正常解剖结构。结果病人脑脊液鼻漏停止,术后未发生感染和脑膜膨出,随访1个月未见复发。结论对于合并存在较大颅底缺损的外伤性脑脊液鼻漏病人,经颅修补及颅底重建是彻底治愈脑脊液鼻漏的有效手术方式。黄元奎 金保山 陈涛 Dietmar Stolke 2005中国微侵袭神经外科杂志2005,10,3:7

17Single-port versus multi-port cholecystectomy for patients with acute cholecystitis: a retrospective comparative analysis显示文摘BACKGROUND: Trans-umbilical single-port laparoscopic cholecystectomy for chronic gallbladder disease is becoming increasingly accepted worldwide. But so far, no reports exist about the challenging single-port surgery for acute cholecystitis. The objective of this study was to describe our experience with single-port cholecystectomy in comparison to the conventional laparoscopic technique. METHODS: Between August 2008 and March 2010, 73 patients with symptomatic gallbladder disease and histopathological signs of acute cholecystitis underwent laparoscopic cholecystec- tomy at our institution. Thirty-six patients were operated on with the single-port technique (SP group) and the data were compared with a control group of 37 patients who were treated with the multi-port technique (MP group). RESULTS: The mean age in the SP group was 61.5 (range 21-81) years and in the MP group was 60 (range 21-94) (P=0.712). Gender, ASA status and BMI were not significantly different. The number of white blood cells was different before [SP: 9.2 (range 2.8-78.4); MP: 13.2 (range 4.4-28.6); P=0.001] and after the operation [SP: 7.8 (range 3.5-184.8); MP: 11.1 (range 5-20.8); P=0.002]. Mean operating time was 88 (range 34-174) minutes in the SP group vs 94 (range 39-209) minutes in the MP group (P=0.147). Four patients (5%) required conversion to an open procedure (SP: 1; MP: 3; P=0.320). During the follow- up period of 332 (range 29-570) days in the SP group and 428 (range 111-619) days in the MP group (P=0.044), eleven (15%) patients developed postoperative complications (P=0.745) and two patients in the SP group required reoperation (P=0.154). CONCLUSIONS: Trans-umbilical single-port cholecystectomy for beginning acute cholecystitis is feasible and the complicationrate is comparable with the standard multi-port operation. 18The fungal UmSrt1 and maize ZmSUT1 sucrose transporters battle for plant sugar resources显示文摘The biotrophic fungus Ustilago maydis causes corn smut disease, inducing tumor formation in its host Zea mays. Upon infection, the fungal hyphae invaginate the plasma membrane of infected maize cells, establishing an interface where pathogen and host are separated only by their plasma membranes. At this interface the fungal and maize sucrose transporters, UmSrt1 and ZmSUT1, compete for extracellular sucrose in the corn smut/maize pathosystem. Here we biophysically characterized ZmSUT1 and UmSrt1 in Xenopus oocytes with respect to their voltage-,pH-and substrate-dependence and determined affinities toward protons and sucrose. In contrast to ZmSUT1,UmSrt1 has a high affinity for sucrose and is relatively pH-and voltage-independent. Using these quantitative parameters, we developed a mathematical model to simulate the competition for extracellular sucrose at the contact zone between the fungus and the host plant. This approach revealed that UmSrt1 exploits the apoplastic sucrose resource, which forces the plant transporter into a sucrose export mode providing the fungus with sugar from the phloem. Importantly, the high sucrose concentration in the phloem appeared disadvantageous for the ZmSUT1, preventing sucrose recovery from the apoplastic space in the fungus/plant interface.Anke Wittek Ingo Dreyer Khaled A.S.
