| 1 | Proton pump inhibitor administration delays rebleeding after endoscopic gastric variceal obturation显示文摘AIM:To clarify the efficacy of proton pump inhibitors(PPIs)after endoscopic variceal obturation(EVO)with N-butyl-2-cyanoacrylate.METHODS:A retrospective study was performed on16 liver cirrhosis patients with gastric variceal bleeding that received EVO with injections of N-butyl-2-cyanoacrylate at a single center(Kyung Hee University Hospital at Gangdong)from January 2008 to December 2012.Medical records including patient characteristics and endoscopic findings were reviewed.Treatment results,liver function,serum biochemistry and cirrhosis etiology were compared between patients receiving PPIs and those that did not.Furthermore,the rebleeding interval was compared between patients that received PPI treatment after EVO and those who did not.RESULTS:The patient group included nine males and seven females with a mean age of 61.8±11.7years.Following the EVO procedure,eight of the 12patients that received PPIs and three of the four nonPPI patients experienced rebleeding.There were no differences between the groups in serum biochemistry or patient characteristics.The rebleeding rate was not significantly different between the groups,however,patients receiving PPIs had a significantly longer rebleeding interval compared to non-PPI patients(22.2±11.2mo vs 8.5±5.5 mo;P=0.008).The duration of PPI use was not related to the rebleeding interval.A total of six patients,who had ulcers at the injection site,exhibited a shorter rebleeding interval(16.8±5.9 mo)than patients without ulcers(19.9±3.2 mo),though this difference was not statistically significant.CONCLUSION:PPI therapy can extend the rebleeding interval,and should therefore be considered after EVO treatment for gastric varices. | Won Seok Jang Hyun Phil Shin Joung Ⅱ Lee Kwang Ro Joo Jae Myung Cha Jung Won Jeon Jun Uk Lim | 2014 | World Journal of Gastroenterology2014,20,45: | 17 |
| 2 | Efficacy of cap-assisted colonoscopy according to lesion location and endoscopist training level显示文摘AIM: To evaluate the efficacy of cap-assisted colonoscopy(CAC) for detection of colorectal polyps and adenomas according to the lesion location and endoscopist training level.METHODS: Patients 20 years or older, who underwent their first screening colonoscopy in a single tertiary center from May 2011 to December 2012 were enrolled in this study. All patients underwent either CAC or standard colonoscopy(SC), and all of the procedures were performed by 11 endoscopists(8 trainees and 3 experts). All procedures were performed with highdefinition colonoscopes and narrow band imaging. The eight trainees had experiences of performing 150 to 500 colonoscopies, and the three experts had experiences of performing more than 3000 colonoscopies. A 4-mmlong transparent cap was attached to the end of a colonoscope in the CAC group. We retrospectively evaluated the number of polyps and adenomas, polyp detection rate(PDR), and the number of adenomas and adenoma detection rate(ADR) according to the lesion location and endoscopist training level between CAC and SC. We also evaluated the number of polyps and adenomas according to their size between CAC and SC.RESULTS: Overall, PDR and ADR using CAC were significantly higher than those using SC for both whole colon(48.5% vs 40.7%, P = 0.012; 35.7% vs 28.3%, P = 0.012) and right-side colon(35.3% vs 26.6%, P = 0.002; 27.0% vs 16.9%, P < 0.001). The number of polyps and adenomas per patient using CAC was significantly higher than that using SC for both the whole colon(1.07 ± 1.59 vs 0.82 ± 1.31, P = 0.008; 0.72 ± 1.32 vs 0.50 ± 1.01, P = 0.003) and right-side colon(0.66 ± 1.18 vs 0.41 ± 0.83, P < 0.001; 0.46 ± 0.97 vs 0.25 ± 0.67, P < 0.001). In the trainee group, the PDR and ADR using CAC were significantly higher than those using SC for both the whole colon(46.7% vs 39.7%, P = 0.040; 33.9% vs 26.0%, P =0.012) and right-side colon(34.2% vs 26.5%, P = 0.015; 25.3% vs 15.9%, P = 0.001). | 3 | Recent Progress and Development in Extrusion of Rare Earth Free Mg Alloys: A Review显示文摘Mg and its alloys are the lightest structural metals available and are extremely attractive for applications as lightweight components, particularly in the automobile, electronic, and aerospace industries. The global market for wrought Mg alloys has steadily expanded over the past decade. And numerous studies have been carried out to meet this increasing demand of high-performance Mg alloys. However, Mg extrusion alloys have had a very limited usage so far. To overcome existing industrial challenges, one desirable approach is the development of low-cost rare earth(RE) free Mg extrusion alloys with superior mechanical properties. This review will introduce the recent research highlights in the extrusion of Mg alloys, specifi cally focusing on low-cost RE-free Mg alloy. The results from both the literature and our previous study are summarized and critically reviewed. Several aspects of RE-free Mg extrusion alloys are described in detail:(1) novel alloying designs including Mg–Al-, Mg–Zn-, Mg–Ca-, Mg–Sn-, and Mg–Bi-based alloys,(2) advanced extrusion techniques, and(3) extrusion-related severe plastic deformation(SPD) processing. Accordingly, considering the large gap in mechanical properties between the current RE-free Mg alloys and high-performance aluminum alloys, new alloy design, processing route control, and recommendations for future research on RE-free Mg extrusion alloys are also proposed. | 7 | CFD analysis of sound pressure in tank gun muzzle silencer显示文摘The high pressure waves generated due to muzzle blast flow of tank gun during firing is a critical issue to examine. The impulsive noise from the gun has various negative effects such as damage of human bodies, damage of structures, creating an environmental, social problem and also military problems such as exposure of location of troops. This high pressure impulsive sound, generated during the blast flow, was studied and attenuated. An axisymmetric computational domain was constructed by employing Spalart Allmaras turbulence model. Approximately 90% of pressure and 20 dB of sound level are reduced due to the use of the three baffle silencer at the muzzle end of the gun barrel, in comparison with the tank gun without silencer. Also, the sound pressure level at different points in the ambient region shows the same attenuation in results. | 8 | New risk assessment methodology for coal mine excavated slopes显示文摘This paper presents a new risk assessment methodology for coal mine excavated slopes. This new empirical-statistical slope stability assessment methodology(SSAM) is intended for use by geotechnical engineers at both the design review and operational stages of a mine's life to categorise the risk of an excavated coal mine slope. A likelihood of failure is determined using a new slope stability classification system for excavated coal mine slopes developed using a database of 119 intact and failed case studies sourced from open cut coal mines in Australia. Consequence of failure is based on slope height and stand-off distance at the toe of the excavated slope. Results are presented in a new risk matrix, with slope risk being divided into low, medium and high categories. The SSAM is put forward as a new risk assessment methodology to assess the potential for, and consequence of, excavated coal mine slope failure.Unlike existing classification systems, assumptions about the likely failure mode or mechanism are not required. Instead, the SSAM applies an approach which compares the conditions present within the excavated slope face, with the known past performance of slopes with similar geotechnical and geometrical conditions, to estimate the slope's propensity for failure. The SSAM is novel in that it considers the depositional history of strata in an excavated slope and how this sequence affects slope stability. It is further novel in that it does not require explicit measurements of intact rock, rock mass and/or defect strength to rapidly calculate a slope's likelihood of failure and overall risk. Ratings can be determined entirely from visual observations of the excavated slope face. | 9 | Clear cell papillary renal cell carcinoma: A case report and review of the literature显示文摘Clear cell papillary renal cell carcinoma(ccpRCC) was recently established as a distinct type of epithelial neoplasm by the International Society of Urological Pathology Vancouver Classification of Renal Neoplasia. Here,we report a case of partial nephrectomy for a ccpRCC detected during the routine follow-up of a previously treated liposarcoma in a 70-year-old male patient. The patient was referred to the urology department for a right-sided renal mass(size: 2 cm)detected during routine annual imaging follow-up for a malignant right inguinal fibrous histocytoma and liposarcoma that had been diagnosed 6 and 4 years earlier,respectively,and treated with surgery and adjuvant radiation therapy.Following partial nephrectomy,the renal mass was pathologically diagnosed as ccpRCC,and immunohistochemistry revealed carbonic anhydrase 9(CA9)expression. No recurrences or metastases were detected on follow-up imaging for6 months. | 10 | Endoscopic resection using band ligation for esophageal SMT in less than 10mm显示文摘AIM:To evaluate the safety and feasibility of endoscopic resection using band ligation(EMR-B) for the diagnostic and therapeutic removal of tumors located in the esophageal subepithelial region having originated from the submucosa.METHODS:From May 2009 to September 2014,after medical chart and endoscopic ultrasonography report review,a total of 15 esophageal tumors located in the submucosal layer were resected by EMR-B.Previous symptom,location,pathology,complete resection rate,incidence of complications,incidence of minor complication,size,length of procedures time and follow up months were evaluated.To evaluate local recurrence at the resection site,periodic follow-up endoscopic examination was undertaken in all of the patients.The first endoscopic examination was performed about 6 mo after the endoscopic resection.Thereafter,the endoscopic follow up were scheduled annually.RESULTS:The mean age was 50.3 ± 9.67 years.The mean tumor size was 6.93 ± 3.15 mm and most of the lesions size was between 5-10 mm in diameter(10/15,66.6%).In all patients,endoscopic en bloc resection was achieved.In one patient,the vertical margin was involved.The mean procedural time was 8.86 ± 3.66 min.In all patients,no evidence of severe complications such as perforation or bleeding occurred.Minor complications such as chest pain(2/15,13.3%) and heartburn(3/15,13.3%) were reported but they symptoms were controlled by proton pump inhibitors,ulcermin and/or analgesics.Histologic assessments of the removed specimens revealed 10 granular cell tumors(66.6%),4 leiomyomas(16.6%) and one lipoma(6.6%).No recurrence was observed during the mean follow up period of 45 ± 3.5 mo(range:5-64 mo).CONCLUSION:EMR-B might be considered safe and effective for the diagnosis and treatment of lesions measuring less than 10 mm in diameter. | Joung Boom Hong Cheol Woong Choi Hyung Wook Kim Dae Hwan Kang Su Bum Park Su Jin Kim Dong Jun Kim | 2015 | World Journal of Gastroenterology2015,21,10: | 3 |
| 11 | Nogo receptor expression in microglia/macrophages during experimental autoimmune encephalomyelitis progression显示文摘Myelin-associated inhibitory factors within the central nervous system(CNS) are considered to be one of the main obstacles for axonal regeneration following disease or injury. The nogo receptor 1(NgR1) has been well documented to play a key role in limiting axonal regrowth in the injured and diseased mammalian CNS. However, the role of nogo receptor in immune cell activation during CNS inflammation is yet to be mechanistically elucidated. Microglia/macrophages are immune cells that are regarded as pathogenic contributors to inflammatory demyelinating lesions in multiple sclerosis(MS). In this study, the animal model of MS, experimental autoimmune encephalomyelitis(EAE) was induced in ngr1^(+/+) and ngr1^(–/–) female mice following injection with the myelin oligodendrocyte glycoprotein(MOG_(35–55)) peptide. A fatemap analysis of microglia/macrophages was performed throughout spinal cord sections of EAE-induced mice at clinical scores of 0, 1, 2 and 3, respectively(increasing locomotor disability) from both genotypes, using the CD11 b and Iba1 cell markers. Western immunoblotting using lysates from isolated spinal cord microglia/macrophages, along with immunohistochemistry and flow cytometric analysis, was performed to demonstrate the expression of nogo receptor and its two homologs during EAE progression. Myelin protein engulfment during EAE progression in ngr1^(+/+) and ngr1^(–/–) mice was demonstrated by western immunblotting of lysates from isolated spinal cord microglia/macrophages, detecting levels of Nogo-A and MOG. The numbers of M1 and M2 microglia/macrophage phenotypes present in the spinal cords of EAE-induced ngr1^(+/+) and ngr1^(–/–) mice, were assessed by flow cytometric analysis using CD38 and Erg-2 markers. A significant difference in microglia/macrophage numbers between ngr1^(+/+) and ngr1^(–/–) mice was identified during the progression of the clinical symptoms of EAE, in the white versus gray matter regions of the spinal cord. This difference was unrelated to the expression of Ng R on these macrophage/microglial cells. We have identified that as EAE progresses, the phagocytic activity of microglia/macrophages with myelin debris, in ngr1^(–/–) mice, was enhanced. Moreover, we show a modulation from a predominant M1-pathogenic to the M2-neurotrophic cell phenotype in the ngr1^(–/–) mice during EAE progression. | 14 | Duodenal variceal bleeding after balloon-occluded retrograde transverse obliteration: Treatment with transjugular intrahepatic portosystemic shunt显示文摘We report a case of duodenal varix bleeding as a long term complication of balloon occluded retrograde transvenous obliteration (BRTO), which was successfully treated with a transjugular intrahepatic portosystemic shunt (TIPS). A 57-year-old man was admitted to the emergency room suffering from melena. He had under-gone BRTO to treat gastric varix bleeding 5 mo before admission. Endoscopy and a computed tomography (CT) scan showed complete obliteration of the gastric varix, but the nodular varices in the second portion of the duodenum expanded after BRTO, and spurting blood was seen. TIPS was performed for treatment of duodenal variceal bleeding, because attempts at endoscopic varix ligation were unsuccessful. The post-operative course was uneventful and the patient was discharged without complications. A follow up CT scan obtained 21 mo after TIPS revealed a patent TIPS tract and complete obliteration of duodenal varices, but multinodular hepatocellular carcinoma had developed. | 19 | Enhancing Hydrophilicity of Thick Electrodes for High Energy Density Aqueous Batteries显示文摘Thick electrodes can substantially enhance the overall energy density of batteries.However,insufficient wettability of aqueous electrolytes toward electrodes with conventional hydrophobic binders severely limits utilization of active materials with increasing the thickness of electrodes for aqueous batteries,resulting in battery performance deterioration with a reduced capacity.Here,we demonstrate that controlling the hydrophilicity of the thicker electrodes is critical to enhancing the overall energy density of batteries.Hydrophilic binders are synthesized via a simple sulfonation process of conventional polyvinylidene fluoride binders,considering physicochemical properties such as mechanical properties and adhesion.The introduction of abundant sulfonate groups of binders(i)allows fast and sufficient electrolyte wetting,and(ii)improves ionic conduction in thick electrodes,enabling a significant increase in reversible capacities under various current densities.Further,the sulfonated binder effectively inhibits the dissolution of cathode materials in reactive aqueous electrolytes.Overall,our findings significantly enhance the energy density and contribute to the development of practical zinc-ion batteries. | Jungeun Lee Hyeonsoo Lee Cheol Bak Youngsun Hong Daeha Joung Jeong Beom Ko Yong Min Lee Chanhoon Kim | 2023 | Nano-Micro Letters2023,15,7: | 2 |
| 20 | Attenuated FOLFIRINOX in the salvage treatment of gemcitabine-refractory advanced pancreatic cancer: a phase II study显示文摘Background:Combination therapy with oxaliplatin,irinotecan,fluorouracil,and leucovorin(FOLFIRINOX)chemotherapy drastically improves survival of advanced pancreatic cancer patients.However,the efficacy of FOLFIRINOX as a second-line treatment after gemcitabine failure has not been tested prospectively.We investigated the feasibility and safety of attenuated FOLFIRINOX in patients with gemcitabine-refractory advanced pancreatic cancer.Methods:A multicenter phase II prospective open-label,single-arm study was conducted at 14 hospitals.Patients with histologically proven invasive ductal pancreatic adenocarcinoma,a measurable or evaluable lesion,Eastern Cooperative Oncology Group performance status 0 or 1,adequate organ function,and aged 19 years or older were eligible.Attenuated FOLFIRINOX consisted of oxaliplatin 65 mg/m2,irinotecan 135 mg/m2,and leucovorin 400 mg/m2 injected intravenously on day 1 and 5-fluorouracil 2000 mg/m2 continuously infused intravenously over 46 h on days 1-2,repeated every 2 weeks.The primary endpoint was progression-free survival from the initiation of FOLFIRINOX.Secondary endpoints were the objective response rate,disease control rate,overall survival,safety,and tolerability.We estimated overall survival and progression-free survival using the Kaplan-Meier methods.Results:We enrolled 39 patients from 14 institutions.The objective response rate was 10.3%,while the disease control rate was 64.1%.The 6-month and 1-year overall survival rates were 59.0%and 15.4%,respectively.Median progression-free survival and overall survival were 3.8 months(95%confidence interval[CI]1.5-6.0 months)and 8.5 months(95%CI 5.6-11.4 months),respectively.Grade 3 or 4 adverse events were neutropenia(41.0%),nausea(10.3%),anorexia(10.3%),anemia(7.7%),mucositis(7.7%),pneumonia/pleural effusion(5.1%),and fatigue(5.1%).One treatment-related death attributable to septic shock occurred.Conclusion:Attenuated FOLFIRINOX may be promising as a second-line therapy for gemcitabine-refractory pancre-atic cancer. | Jung Hoon Kim Sang-Cheol Lee Sung Yong Oh Seo-Young Song Namsu Lee Eun Mi Nam Soonil Lee In Gyu Hwang Hyo Rak Lee Kyu Taek Lee Sang-Byung Bae Han Jo Kim Joung Soon Jang Do Hyoung Lim Hyun Woo Lee Seok Yun Kang Jung Hun Kang | 2018 | Cancer Communications2018,38,1: | 2 |
