1急性缺血性与出血性卒中危险因素的对比研究显示文摘目的通过对1个大样本的医院脑卒中注册资料的分析,观察各种危险因素在脑卒中患者中的发生率,并对脑梗死与脑出血的危险因素,特别是心脏和动脉病变的因素进行比较。方法研究资料来自瑞士洛桑卒中注册的卒中患者共3901例,其中脑梗死3525例,脑出血376例,均预先对各项临床和辅助资料进行编码,输入计算机数据库。所有病例均进行系统的临床和辅助检查,心脏病变经心电图和超声心动图证实;颈内动脉和椎基底动脉的病变采用彩色超声多普勒、磁共振血管成像或常规脑血管造影确诊。结果高血压在脑出血和脑梗死患者中的发生率分别为53.7%和46.7%,两者相比差异有统计学意义(P<0.01)。高血压是本组脑卒中最突出的危险因素,其后依次是吸烟、轻度颈内动脉狭窄(≤50%)、高胆固醇血症、短暂性脑缺血发作、糖尿病以及心脏缺血。多因素分析研究发现,本组卒中患者无论是男性还是女性,吸烟、高胆固醇血症、短暂性脑缺血发作、心房纤颤、器质性心脏疾病以及动脉病变是脑梗死而非脑出血密切相关的危险因素。易患脑出血的惟一显著相关的危险因素是高血压(OR=0.64,P<0.01);心脏和动脉病变患者更容易发生脑梗死,但是,器质性心脏病和轻度颈内动脉狭窄在全体卒中患者中均特别常见。结论缺血性卒中和出血性卒中的危险因素并不一致,心脏和动脉病变是脑梗死而非脑出血的重要的危险因素,某些危险因素还存在性别差异。刘新峰 徐格林 Guy van Melle Julien Bogousslavsky 2005中华神经科杂志2005,38,7:42

3Signet-ring cell carcinoma of the stomach: Impact on prognosis and specific therapeutic challenge显示文摘While the incidence of gastric cancer has decreased worldwide in recent decades,the incidence of signetring cell carcinoma(SRCC) is rising. SRCC has a specific epidemiology and oncogenesis and has two forms: early gastric cancer,which can be resected endoscopically in some cases and which has a better outcome than non-SRCC,and advanced gastric cancer,which is generally thought to have a worse prognosis and lower chemosensitivity than non-SRCC. However,the prognosis of SRCC and its chemosensitivity with specific regimens are still controversial as SRCC is not specifically identified in most studies and its poor prognosis may be due to its more advanced stage. It therefore remains unclear if a specific therapeutic strategy is justified,as the benefit of perioperative chemotherapy and the value of taxanebased chemotherapy are unclear. 5Colorectal cancer and immunity:what we know and perspectives显示文摘Strong evidence supports the concept of immunosurveillance and immunoediting in colorectal cancer.In particular,the density of T CD8+and CD45+lymphocyte infiltration was recently shown to have a better prognostic value than the classic tumor node metastasis classification factor.Other immune subsets,as macrophages,natural killer cells or unconventionnal lymphocytes,seem to play an important role.Induction of regulatory T cells(Tregs)or immunosuppressive molecules such as PD-1 or CTLA-4 and downregulation of antigen-presenting molecules are major escape mechanisms to antitumor immune response.The development of these mechanisms is a major obstacle to the establishment of an effective immune response,but also to the use of immunotherapy.Although im-munotherapy is not yet routinely used in colorectal cancer,we now know that most treatments used(chemotherapy and biotherapy)have immunomodulatory effects,such as induction of immunogenic cell death by chemotherapy,inhibition of immunosuppression by antiangiogenic agents,and antibody-dependent cytotoxicity induced by cetuximab.Finally,many immunotherapy strategies are being developed and tested in phaseⅠtoⅢclinical trials.The most promising strategies are boosting the immune system with cytokines,inhibition of immunoregulatory checkpoints,vaccination with vectorized antigens,and adoptive cell therapy.Comprehension of antitumor immune response and combination of the different approaches of immunotherapy may allow the use of effective immunotherapy for treatment of colorectal cancer in the near future.Simon Pernot Magali Terme Thibault Voron Orianne Colussi Elie Marcheteau Eric Tartour Julien Taieb 2014World Journal of Gastroenterology2014,20,14:19

8Gender-related differences in irritable bowel syndrome: Potential mechanisms of sex hormones显示文摘According to epidemiological studies,twice as many women as men are affected by irritable bowel syndrome(IBS)in western countries,suggesting a role for sex hormones in IBS pathophysiology.Despite growing evidence about the implications of sex hormones in IBS symptom modulation,data on mechanisms by which they influence disease development are sparse.This review aims to determine the state of knowledge about the role of sex hormones in sensorimotor dysfunctions and to address the possible interplay of sex hormones with common risk factors associated with IBS.The scientific bibliography was searched using the following keywords:irritable bowel syndrome,sex,gender,ovarian hormone,estradiol,progesterone,testosterone,symptoms,pain,sensitivity,motility,permeability,stress,immune system,brain activity,spinal,supraspinal,imaging.Ovarian hormones variations along themenstrual cycle affect sensorimotor gastrointestinal function in both healthy and IBS populations.They can modulate pain processing by interacting with neuromodulator systems and the emotional system responsible for visceral pain perception.These hormones can also modulate the susceptibility to stress,which is a pivotal factor in IBS occurrence and symptom severity.For instance,estrogen-dependent hyper-responsiveness to stress can promote immune activation or impairments of gut barrier function.In conclusion,whereas it is important to keep in mind that ovarian hormones cannot be considered as a causal factor of IBS,they arguably modulate IBS onset and symptomatology.However,our understanding of the underlying mechanisms remains limited and studies assessing the link between IBS symptoms and ovarian hormone levels are needed to improve our knowledge of the disease evolution with regard to gender.Further studies assessing the role of male hormones are also needed to understand fully the role of sex hormones in IBS.Finally,investigation of brain-gut interactions is critical to decipher how stress,ovarian hormones,and female brain processing of pain can translate into gut dysfunctions.Mathieu Meleine Julien Matricon 2014World Journal of Gastroenterology2014,20,22:14

10Selective laser melting(SLM)of CX stainless steel:Theoretical calculation,process optimization and strengthening mechanism显示文摘In the present work,selective laser melting(SLM)technology was utilized for manufacturing CX stainless steel samples under a series of laser parameters.The effect of laser linear energy density on the microstructure characteristics,phase distribution,crystallographic orientation and mechanical properties of these CX stainless steel samples were investigated theoretically and experimentally via scanning electron microscope(SEM),X-ray diffraction(XRD),electron backscatter diffraction(EBSD)and transmission electron microscope(TEM).Based on the systematic study,the SLM CX stainless steel sample with best surface roughness(Ra=4.05±1.8μm)and relative density(Rd=99.72%±0.22%)under the optimal linear density(η=245 J/m)can be obtained.SLM CX stainless steel was primarily constituted by a large number of fine martensite(α’phase)structures(i.e.,cell structures,cellular dendrites and blocky grains)and a small quantity of austenite(γphase)structures.The pre ferred crystallographic orientation(i.e.,<111>direction)can be determined in the XZ plane of the SLM CX sample.Furthermore,under the optimal linear energy density,the good combinations with the highest ultimate tensile strength(UTS=1068.0%±5.9%)and the best total elongation(TE=15.70%±0.26%)of the SLM CX sample can be attained.Dislocation strengthening dominates the strengthening mechanism of the SLM CX sample in as-built state.Dongdong Dong Cheng Chang Hao Wang Xingchen Yan Wenyou Ma Min Liu Sihao Deng Julien Gardan Rodolphe Bolot Hanlin Liao 2021Journal of Materials Science & Technology2021,,14:11
11Dysregulation of innate immunity in ulcerative colitis patients who fail anti-tumor necrosis factor therapy显示文摘AIM To study the innate immune function in ulcerative colitis(UC) patients who fail to respond to anti-tumor necrosis factor(TNF) therapy.METHODS Effects of anti-TNF therapy, inflammation and medications on innate immune function were assessed by measuring peripheral blood mononuclear cell(PBMC) cytokine expression from 18 inflammatory bowel disease patients pre- and 3 mo post-anti-TNF therapy. Toll-like receptor(TLR) expression and cytokine production post TLR stimulation was assessed in UC 'responders'(n = 12) and 'non-responders'(n = 12) and compared to healthy controls(n = 12). Erythrocyte sedimentation rate(ESR) and C-reactive protein(CRP) levels were measured in blood to assess disease severity/activity and inflammation. Pro-inflammatory(TNF, IL-1β, IL-6), immuno-regulatory(IL-10), Th1(IL-12, IFNγ) and Th2(IL-9, IL-13, IL-17A) cytokine expression was measured with enzyme-linked immunosorbent assay while TLR cellular composition and intracellular signalling was assessed with FACS.RESULTS Prior to anti-TNF therapy, responders and nonresponders had similar level of disease severity and activity. PBMC's ability to respond to TLR stimulation was not affected by TNF therapy, patient's severity of the disease and inflammation or their medication use. At baseline, non-responders had elevated innate but not adaptive immune responses compared to responders(P < 0.05). Following TLR stimulation, nonresponders had consistently reduced innate cytokine responses to all TLRs compared to healthy controls(P < 0.01) and diminished TNF(P < 0.001) and IL-1β(P < 0.01) production compared to responders. This innate immune dysfunction was associated with reduced number of circulating plasmacytoid dendritic cells(p DCs)(P < 0.01) but increased number of CD4+ regulatory T cells(Tregs)(P = 0.03) as well as intracellular accumulation of IRAK4 in non-responders following TLR-2,-4 and-7 activation(P < 0.001). CONCLUSION Reduced innate immunity in non-responders may explain reduced efficacy to anti-TNF therapy. 12激光选区熔化成形nano-WC/CX钢微观组织及机械性能初探显示文摘为进一步提高激光3D打印CX不锈钢材料的机械性能,首先使用静电自组装方法对CX钢粉末表面进行了纳米WC粉末的修饰,之后对经纳米WC表面修饰的CX钢粉末进行了激光选区熔化成形(Selective laser melting,SLM),对比了添加体积分数5%的WC前后的SLM CX钢试样的机械性能.结合金相显微镜(OM)、扫描电子显微镜(SEM)、能谱分析(EDS)及电子背散射衍射(EBSD)等材料表征手段,对SLM nano-WC/CX钢试样的微观组织结构、元素分布及相应机械性能进行了对比分析.结果表明:体积分数为5%的纳米WC的加入,对SLM nano-WC/CX钢的硬度提升有所帮助,使硬度值从352±8.2 HV_(0.2)提升至361±23.5 HV_(0.2);尽管纳米WC的加入使SLM nano-WC/CX钢的延伸率有所降低,但其最大拉伸强度则由1091.4±6.12 MPa提升到了1109.7±15.26 MPa.总之,初步探索可以发现,通过静电自组装方法,在CX钢粉末表面添加适量的纳米WC颗粒可以有效改善SLM CX钢的最终机械性能.常成 闫星辰 GARDAN Julien DENG Sihao BOLOT Rodolphe LIAO Hanlin 2021材料研究与应用2021,15,4:10

16Chloramination of wastewater effluent: Toxicity and formation of disinfection byproducts显示文摘The reclamation and disinfection of waters impacted by human activities(e.g., wastewater effluent discharges) are of growing interest for various applications but has been associated with the formation of toxic nitrogenous disinfection byproducts(N-DBPs). Monochloramine used as an alternative disinfectant to chlorine can be an additional source of nitrogen in the formation of N-DBPs. Individual toxicity assays have been performed on many DBPs, but few studies have been conducted with complex mixtures such as wastewater effluents. In this work, we compared the cytotoxicity and genotoxicity of wastewater effluent organic matter(Ef OM) before and after chloramination. The toxicity of chloraminated Ef OM was significantly higher than the toxicity of raw Ef OM, and the more hydrophobic fraction(HPO)isolated on XAD-8 resin was more toxic than the fraction isolated on XAD-4 resin.More DBPs were also isolated on the XAD-8 resin. N-DBPs(i.e., haloacetonitriles or haloacetamides) were responsible for the majority of the cytotoxicity estimated from DBP concentrations measured in the XAD-8 and XAD-4 fractions(99.4% and 78.5%, respectively).Measured DBPs accounted for minor proportions of total brominated and chlorinated products, which means that many unknown halogenated compounds were formed and can be responsible for a significant part of the toxicity. 17Water Resource Monitoring Exploiting Sentinel-2 Satellite and Sentinel-2 Satellite Like Time Series;Application in Yangtze River Water Bodies显示文摘As part of the Dragon 4 project,the water extents of Wuchang and Shengjin lakes have been extracted from Sentinel-2 time series,using all exploitable images since the beginning of the acquisitions in 2015.The aim of the study is to assess the capability of the Sentinel-2 constellation and Landsat 8 over the Anhui region,especially the high temporal resolution.A total of 32 dates have been used and 10 Landsat 8 images(Libra)have been added to try to reduce the temporal gaps in the Sentinel-2 acquisitions caused by cloudy conditions.Extractions were done using a SERTIT-ICube automatized routine based on a supervised Maximum Likelihood Classification.These extractions allow to recreate the dynamic of the two lakes and show the drought and wet periods.During the 3 years interval,the surface peaks in July 2016 for both lakes.The lowest level appears at two different dates for each lake;in January 2018 for Wuchang,in February 2017 for Shengjin.Wuchang Lake surface area appears to be more variable than Shengjin Lake,with many local maximum and minimum between the end of 2017 and April 2018.In the case of Wuchang Lake,floating vegetation is a problem for automatic water surface area extraction.The lake is covered by vegetation during long periods of time and the water below can’t be detected by automatic radiometric means.Nevertheless,Sentinel-2 stays a pertinent and powerful tool for hydrological monitoring of lakes confirming the expectation from the remote sensing wetland community before launch.The presence of NIR and SWIR bands induces a strong discrimination between water and other classes,and the systematic acquisitions create dense time series,making analysis more consistent.It makes possible to sensor events occurring over short periods of time.Thanks to this a link can be done between endangered bird species,such as the Siberian Crane and the Lesser White-Fronted Goose and periodically flooded areas.These midterm results illustrated the pertinence and powerful of multi-source optical satellite data for environmental analysis.Julien BRIANT Huber CLAIRE Studer MATHIAS Lei CAO Kunpeng YI Yésou HERV 2020Journal of Geodesy and Geoinformation Science2020,3,4:8

19How to assess the efficacy or failure of targeted therapy:deciding when to stop sorafenib in hepatocellular carcinoma显示文摘Sorafenib is thus far the only systemic treatment for hepatocellular carcinoma(HCC) based on the results of two randomized controlled trials performed in Western and in Eastern countries, despite a poor response rate(from 2% to 3.3%) following conventional evaluation criteria. It is now recognized that the criteria(European Association of the Study of the Liver criteria, modified response evaluation criteria in solid tumors) based on contrast enhanced techniques(computed tomography scan, magnetic resonance imaging) aimed to assess the evolution of the viable part of the tumor(hypervascularized on arterial phase) are of major interest to determine the efficacy of sorafenib and of most antiangiogenic drugs in patients with HCC. The role of alphafetoprotein serum levels remains unclear. In 2016, in accordance with the SHARP and the Asia-Pacific trials, sorafenib must be stopped when tolerance is poor despite dose adaptation or in cases of radiological and symptomatic progression. This approach will be different in cases of available second-line therapy trials. Some recent data(in renal cell carcinoma) revealed that despite progression in patients who received sorafenib, this drug can still decrease tumor progression compared to drug cessation. Then, before deciding to continue sorafenib post-progression or shift to another drug, knowing other parameters of post-progression survival(Child-Pugh class, Barcelona Clinic Liver Cancer, alphafetoprotein, post-progression patterns in particular, the development of extrahepatic metastases and of portal vein thrombosis) will be of major importance.Jean-Luc Raoul Xavier Adhoute Marine Gilabert Julien Edeline 2016World Journal of Hepatology2016,8,35:8
