| 2 | Endoscopic submucosal dissection as excisional biopsy for anorectal malignant melanoma:A case report显示文摘BACKGROUND Anorectal malignant melanoma (AMM) is a rare disorder with an extremely poor prognosis. Although there is currently no consensus on the treatment methods for AMM, surgical procedures have been the most common treatment methods used until now. We recently encountered a case of AMM that we diagnosed using endoscopic submucosal dissection (ESD). To our knowledge, this is the first case of ESD for AMM, suggesting that ESD can potentially be a diagnostic and treatment method for AMM. CASE SUMMARY A 77-year-old woman visited our hospital with a chief complaint of anal bleeding and a palpable rectal mass. Colonoscopy revealed a 20-mm protruded lesion in the lower rectum. After obtaining biopsy specimens from the lesion, although a malignant rectal tumor was suspected, a definitive diagnosis was not made. Endoscopic ultrasonography revealed tumor invasion into the submucosal layer but not the muscular layer. Therefore, we performed an excisional biopsy using ESD. Immunohistochemical examination of the ESD-resected specimen revealed tumor cells positive for Human Melanin Black-45, Melan-A, and S-100. Moreover, the tumor cells lacked melanin pigment;thus, a diagnosis of amelanotic AMM was made. Although the AMM had massively invaded the submucosal layer and both lymphatic and venous invasion were present, we closely monitored the patient without any additional therapy on the basis of her request. Six months after ESD, local recurrence was detected, and the patient consented to wide local excision. | 3 | DIFFERENTIAL CYTOTOXIC SENSITIVITY AMONG MTT,NR AND ALPASE ACTIVITY ASSAYS IN HUMAN PULP CELLS EXPOSED TO DENTAL RESIN MONOMERS显示文摘A transformed human dental pulp cell(LSC) was made to assess thecytotoxicity of dental resin monomers with MTT assay,NR assay and ALPase activity assay. L- 929 cells were used as comparative cell line. The sensitivity amongMTT,NR and ALPase activity assays and between two types of cells were investigated. The relationship of ALPase activity to two colorimetric assays was also evaluated. According to IC50 and statistics analysis on cytotoxicity curve,there were significance differences between MTT and NR assay or both cell lines for TECDMAmonomer (P<0.005). MTT assay showed lower sensitivity than NR assay in twokinds of cell for both monomer,and LSC cells presented less sensitivity than L-929cells for TEGDMA monomer. The ALPase activity assay revealed the highest sensitivity among the three tested assays. There was a positive relationship between thecontent of ALPase activity and cytotoxic response of the other two assays. The results indicated that the sensitive degree of test assay or cell line may differ from thetest chemicals. | 4 | Rare histological subtype of invasive micropapillary carcinoma in the ampulla of Vater: A case report显示文摘BACKGROUND Carcinoma of the ampulla of Vater is an uncommon ampullo-pancreatobiliary neoplasm,and the most common histological type is adenocarcinoma with a tubular growth pattern.Invasive micropapillary carcinoma(IMPC)is an aggressive variant of adenocarcinoma in several organs that is associated with lymph node metastasis and poor prognosis.IMPC was first described as a histological subtype of breast cancer;however,IMPC of the ampulla of Vater is extremely rare,with only three articles reported in the English literature.CASE SUMMARY We have reported a case of IMPC of the ampulla of Vater in an 80-year-old man.Microscopically,the surface area of the carcinoma was composed of tubulopapillary structures mimicking intra-ampullary papillary-tubular neoplasm,and the deep invasive front area exhibited a pattern of IMPC.The carcinoma showed lymphatic invasion and extensive lymph node metastasis.The immunohistochemical study revealed mixed intestinal and gastric/pancreatobiliary phenotypes.CONCLUSION This rare subtype tumor in the ampulla of Vater showed a histologically mixed phenotype and exhibited aggressive behavior. | Hirotsugu Noguchi Michiyo Higashi Tetsuya Idichi Hiroshi Kurahara Yuko Mataki Takashi Tasaki Ikumi Kitazono Takao Ohtsuka Akihide Tanimoto | 2021 | World Journal of Clinical Cases2021，9，11： | 2 |
