1Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases显示文摘AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary tumors.METHODS This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the periods.CONCLUSION Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. 2Enhancing mechanical behaviors of collapsible soil using two biopolymers显示文摘This study aims to investigate the possibility of using biopolymer(environmental friendly material) to enhance the mechanical behaviors of collapsible soil.Two types of biopolymers were(xanthan gum and guar gum) used in this study due to their stable behaviors under severe conditions and their availability with reasonable prices.The experimental program focused on three major soil properties,i.e.compaction characterizations,collapsible potential and shear parameters.These three properties are essential in process of soil improvement.Different biopolymer concentrations were used in this study and the experimental program was performed at two curing periods(soon after mixing the soil with the biopolymer and after one week curing time).Shear parameters were measured for the treated specimens under both soaked and unsoaked conditions,while a collapsible potential test was performed under different mixing conditions(wet mix and dry mix).A numerical model was built to predict the behavior of the treated collapsible soil after and before water immersing.The results indicated that the ability of both xanthan gum and guar gum can be used as improvement materials for collapsible soil treatment.The collapsible potential has been reduced from 9%to 1%after mixing the soil with 2%biopolymer concentration in the wet case.After one week curing,the cohesion has been increased from 8.5 kPa to105 kPa by increasing the xanthan gum concentration from zero to 2%,leading to an overall improvement in soil shear strength.It also proves that the guar gum is superior to the xanthan gum.The shear strength of soil can be increased by about 30%when using the guar gum in comparison with the xanthan gum at the same conditions;however,the collapsible potential of soil material will be reduced by about 20%.Mohamed Ayeldeen Abdelazim Negm Mostafa El-Sawwaf Masaki Kitazume 2017Journal of Rock Mechanics and Geotechnical Engineering2017,9,2:14
3Transient elastography compared to liver biopsy and morphometry for predicting fibrosis in pediatric chronic liver disease: Does etiology matter?显示文摘AIM: To evaluate transient elastography(TE) as a noninvasive tool in staging liver fibrosis compared with liver biopsy and morphometry in children with different chronic liver diseases.METHODS: A total of 90 children [50 with chronic hepatitis C virus(HCV), 20 with autoimmune hepatitis(AIH) and 20 with Wilson disease] were included in the study and underwent liver stiffness measurement(LSM) using TE. Liver biopsies were evaluated for fibrosis, qualitatively, by Ishak score and quantitatively by fibrosis area fraction(FAF) using digital image analysis(morphometry). LSM was correlated with fibrosis and other studied variables using spearman correlation. A stepwise multiple regression analysis was alsoperformed to examine independent factors associated with LSM. Different cut-off values of LSM were calculated for predicting individual fibrosis stages using receiveroperating characteristic curve. Cut-off values with optimal clinical performance(optimal sensitivity and specificity simultaneously) were selected.RESULTS: The majority of HCV group had minimal activity(80%) and no/mild fibrosis(72%). On the other hand, the majority of AIH group had mild to moderate activity(70%) and moderate to severe fibrosis(95%) and all Wilson disease group had mild to moderate activity(100%) and moderate to severe fibrosis(100%). LSM correlated significantly with both FAF and Ishak scores and the correlation appeared better with the latter(r = 0.839 vs 0.879, P < 0.0001 for both). LSM discriminated individual stages of fibrosis with high performance. Sensitivity ranged from 81.4% to 100% and specificity ranged from 75.0% to 97.2%. When we compared LSM values for the same stage of fibrosis, they varied according to the different etiologies. Higher values were in AIH(16.15 ± 7.23 k Pa) compared to Wilson disease(8.30 ± 0.84 k Pa) and HCV groups(7.43 ± 1.73 k Pa). Multiple regression analysis revealed that Ishak fibrosis stage was the only independent variable associated with higher LSM(P < 0.0001).CONCLUSION: TE appears reliable in distinguishing different stages of liver fibrosis in children. However, its values vary according to the disease type. 4Human leukocyte antigen typing and crossmatch:A comprehensive review显示文摘Renal transplantation remains the best option for patients suffering from end stage renal disease(ESRD).Given the worldwide shortage of organs and growing population of patients with ESRD,those waitlisted for a transplant is ever expanding.Contemporary crossmatch methods and human leukocyte antigen(HLA) typing play a pivotal role in improving organ allocation and afford better matches to recipients.Understanding crossmatch as well as HLA typing for renal transplantation and applying it in clinical practice is the key step to achieve a successful outcome.Interpretation of crossmatch results can be quite challenging where clinicians have not had formal training in applied transplant immunology.This review aims to provide a worked example using a clinical vignette.Furthermore,each technique is discussed in detail with its pros and cons.The index case is that of a young male with ESRD secondary to Lupus nephritis.He is offered a deceased donor kidney with a 1-0-0 mismatch.His complement dependent cytotoxicity(CDC) crossmatch reported positive for B lymphocyte,but flow cytometry crossmatch(FCXM) was reported negative for both B and T lymphocytes.Luminex-SAB(single antigen bead) did not identify any donor specific antibodies(DSA).He never had a blood transfusion.The positive CDCcrossmatch result is not concordant with DSA status.These implausible results are due to underlying lupus erythematosus,leading to false-positive B-lymphocyte crossmatch as a result of binding immune complexes to Fc-receptors.False positive report of CDC crossmatch can be caused by the underlying autoimmune diseases such as lupus erythematosus,that may lead to inadvertent refusal of adequate kidney grafts.Detailed study of DSA by molecular technique would prevent wrong exclusion of such donors.Based on these investigations this patient is deemed to have 'standard immunological risk' for renal transplantation.Mohammed Mahdi Althaf Mohsen El Kossi Jon Kim Jin Ajay Sharma Ahmed Mostafa Halawa 2017World Journal of Transplantation2017,7,6:5
5Hepatocellular carcinoma in Egypt: A single center study over a decade显示文摘AIM: To identify the trend, possible risk factors and any pattern change of hepatocellular carcinoma (HCC) in Egyptover a decade.METHODS: All HCC patients attending Cairo Liver Center between January 1993 and December 2002, were enrolledin the study. Diagnosis of HCC was based on histopath ological examination and/or detection of hepatic focal lesions by two imaging techniques plus α-fetoprotein level above 200 ng/mL. The duration of the study was divided into two periods of 5 years each; period Ⅰ (1993-1997) and period Ⅱ (1998-2002). Trend, demographic features of patients (age, gender, and residence), risk factors (HBsAg,HCV-Ab, schistosomiasis and others) and pattern of the focal lesions were compared between the two periods.Logistic regression model was fitted to calculate the adjusted odds ratios for the potential risk factors. The population attributable risk percentage was calculated to estimate the proportion of HCC attributed to hepatitis B and C viral infections.RESULTS: Over a decade, 1 328 HCC patients out of 22 450 chronic liver disease (CLD) patients were diagnosed with an overall proportion of 5.9%. The annual proportion of HCC showed a significant rising trend from 4.0% in 1993 to 7.2% in 2002 (P = 0.000). A significant increase in male proportion from 82.5% to 87.6% (P = 0.009); M/F from 5:1 to 7:1 and a slight increase of the predominant age group (40-59 years) from 62.6% to 66.8% (P = 0.387)in periods Ⅰ and Ⅱ respectively, reflecting a shift to younger age group. In the bivariate analysis, HCC was significantly higher in rural residents, patients with history of schistosomiasis and/or blood transfusion. Yet, after adjustment, these variables did not have a significant risk for development of HCC. There was a significant decline of HBsAg from 38.6% to 20.5% (P = 0.000), and a slight increase of HCV-Ab from 85.6% to 87.9% in periods Ⅰ and Ⅱ respectively. HBV conferred a higher risk to develop HCC more than HCV in period Ⅰ (OR 1.9 vs 1.6) and period Ⅱ (OR 2.7 vs 2.0), but the relative contribution of HBV for development of HCC declined in period Ⅱ compared to period Ⅰ (PAR% 4.2%, 21.32%). At presentation,diagnostic α-fetoprotein level (≥200 ng/mL) was demonstrated in 15.6% vs28.9% and small HCC (≤3 cm)represented 14.9% vs 22.7% (P = 0.0002) in periods Ⅰ and Ⅱ respectively.CONCLUSION: Over a decade, there was nearly a twofold increase of the proportion of HCC among CLD patients in Egypt with a significant decline of HBV and slight increase of HCV as risk factors. α-Fetoprotein played a limited role in diagnosis of HCC, compared to imaging techniques.Increased detection of small lesions at presentation reflects increased awareness of the condition.Adbel-Rahman El-Zayadi Hanaa M Badran Eman MF Barakat Mohy El-Deen Attia Sherine Shawky Mostafa K Mohamed Osaima Selim Ahmed Saeid 2005World Journal of Gastroenterology2005,11,33:5
6Clinical outcomes of transepithelial photorefractive keratectomy versus femtosecond laser assisted keratomileusis for correction of high myopia in South Egyptian population显示文摘AIM: To evaluate the safety and efficacy of transepithelial photorefractive keratectomy(t-PRK) with adjuvant mitomycin C(MMC) versus femtosecond laser assisted keratomileusis(Femto-LASIK) in correction of high myopia.METHODS: Prospective randomized comparative study including 156 eyes of 156 patients with high myopia and a spherical equivalent refraction(SER) <-6.00 D. They were divided randomly into two groups: Group A included 72 eyes treated with t-PRK with adjuvant MMC and Group B included 84 eyes treated with Femto-LASIK. Visual acuity, SER, corneal topography, pachymetry and keratometry were assessed for 12 mo postoperatively.RESULTS: The preoperative mean SER was-8.86±1.81 and-9.25±1.70 D in t-PRK MMC group and Femto-LASIK respectively(P=0.99) which improved to-0.65±0.43 D and-0.69±0.50 D at 12 mo follow up. Mean SER remained stable during the 12 mo of follow-up, with no statistically significant difference between the two groups(P=0.64). In t-PRK MMC group, only six eyes needed retreatment after six months of follow up. 7利用测井曲线评价碎屑岩岩石物理参数——以埃及尼罗河三角洲El-Wastani气田上中新统为例显示文摘利用埃及尼罗河三角洲El-Wastani气田10口气井的测井数据,对该气田主要储集层上中新统Qawasim组和Abu Madi组进行岩石物理分析,利用测井参数交会图进行岩性分析,并评估泥质含量、孔隙度、含水饱和度、含油气孔隙体积等参数。分析各井中子孔隙度-密度交会图、中子孔隙度-无铀伽马交会图和岩性-饱和度交会图发现:下Abu Madi组主要为砂岩,偶夹页岩;Qawasim组主要为页岩,偶夹砂岩。利用参数交会图及无铀伽马测井结果,得到有效产层各参数的临界值分别为:泥质含量50%,孔隙度10%,含水饱和度70%。分别绘制了产层有效厚度、平均孔隙度、泥质含量、含水饱和度等参数的等值线图,综合分析发现,研究区Qawasim组和Abu Madi组碎屑岩为优质储集层,天然气有利聚集区呈北西—南东向展布,中部尤为有利。Ezz S El-Din Maher A Mesbah Mohamed A Kassab Ibtehal F Mohamed Burns A Cheadle Mostafa A Teama 2013石油勘探与开发2013,40,4:3
8Interaction of insulin with prokinetic drugs in STZ-induced diabetic mice显示文摘AIM:To study the possible interactions of metoclopramide,domperidone and erythromycin in streptozotocin-induced diabetic mice treated with insulin by various parameters.METHODS:Effects of the individual as well as combined drugs were studied in diabetic mice via estimation of the blood glucose and serum insulin levels,small intestinal transit(SIT),gastric emptying(GE),xylose absorption and glucose tolerance tests.Groups were given insulin 2 IU/kg s.c.,metoclopramide 20 mg/kg p.o.,domperidone 20 mg/kg p.o.and erythromycin 6 mg/kg p.o.individually and in combination.There were also normal and diabetic control groups.The first set of experiments was carried out to investigate the subchronic effect on blood glucose and serum insulin levels in diabetic mice of one week of daily dose administration of the tested drugs individually as well as the combination of insulin with each prokinetic drug.The other five sets of experiments were carried out to investigate the acute effect of a single dose of each drug individually and in combination on blood glucose and serum insulin levels,SIT,GE,oral xylose absorption and glucose tolerance tests.RESULTS:The study included the prokinetic drugs metoclopramide(20 mg/kg),domperidone(20 mg/kg) and erythromycin(6 mg/kg),as well as insulin(2 IU/kg),which was individually effective in decreasing SIT,enhancing GE and increasing xylose absorption significantly in diabetic mice.Erythromycin tended to decrease blood glucose level and increase serum insulin level after 1 wk of daily administration in diabetic mice.Erythromycin potentiated the effect of insulin on blood glucose level and serum insulin level whereas other prokinetic agents failed to do so after repeated dose administration in diabetic mice.Metoclopramide or erythromycin in combination with insulin significantly decreased SIT,in diabetic mice,to lower levels than with insulin alone.Administration of prokinetic drugs along with insulin antagonized the action of insulin on xylose absorption.These combinations also increased the rate of glucose absorption from the gut.CONCLUSION:The present study suggests that prokinetic drugs could potentially improve glycemic control in diabetic gastroparesis by allowing a more predictable absorption of nutrients,matched to the action of exogenous insulin.The use of prokinetics,such as erythromycin,may be interesting in the clinic in decreasing the need for insulin in diabetic patients.The dose of insulin may be safely decreased with erythromycin in chronic treatments.Mohamed A Fouad Shalaby Hekma A Abd El Latif Mostafa E El Sayed 2013World Journal of Gastrointestinal Pharmacology and Therapeutics2013,4,2:3
9Impact of cirrhosis on surgical outcome after pancreaticoduodenectomy显示文摘AIM:To elucidate surgical outcomes of pancreaticoduodenectomy(PD)in patients with liver cirrhosis.METHODS:We studied retrospectively all patients who underwent PD in our centre between January 2002and December 2011.Group A comprised patients with cirrhotic livers,and Group B comprised patients with non-cirrhotic livers.The cirrhotic patients had ChildPugh classes A and B(patient’s score less than 8).Preoperative demographic data,intra-operative data and postoperative details were collected.The primary outcome measure was hospital mortality rate.Secondary outcomes analysed included duration of the operation,postoperative hospital stay,postoperative morbidity and survival rate.RESULTS:Only 67/442 patients(15.2%)had cirrhotic livers.Intraoperative blood loss and blood transfusion were significantly higher in group A(P=0.0001).The mean surgical time in group A was significantly longer than that in group B(P=0.0001).Wound complications(P=0.02),internal haemorrhage(P=0.05),pancreatic fistula(P=0.02)and hospital mortality(P=0.0001)were significantly higher in the cirrhotic patients.Postoperative stay was significantly longer in group A(P=0.03).The median survival was 19 mo in group A and 24 mo in group B.Portal hypertension(PHT)was present in 16/67 cases of cirrhosis(23.9%).The intraoperative blood loss and blood transfusion were significantly higher in patients with PHT(P=0.001).Postoperative morbidity(0.07)and hospital mortality(P=0.007)were higher in cirrhotic patients with PHT.CONCLUSION:Patients with periampullary tumours and well-compensated chronic liver disease should be routinely considered for PD at high volume centres with available expertise to manage liver cirrhosis.PD is associated with an increased risk of postoperative morbidity in patients with liver cirrhosis;therefore,it is only recommended in patients with Child A cirrhosis without portal hypertension.Ayman El Nakeeb Ahmad M Sultan Tarek Salah Mohamed El Hemaly Emad Hamdy Ali Salem Ahmed Moneer Rami Said Ahmed AbuEleneen Mostafa Abu Zeid Talaat Abdallah Mohamed Abdel Wahab 2013World Journal of Gastroenterology2013,19,41:3

12Survival and outcomes for co-infection of chronic hepatitis C with and without cirrhosis and COVID-19: A multicenter retrospective study显示文摘BACKGROUND Chronic liver disease,particularly cirrhosis,is associated with worse outcomes in patients infected with coronavirus disease 2019(COVID-19).AIM To assess outcomes of COVID-19 infection among patients with pre-existing hepatitis C with or without liver cirrhosis.METHODS This multicenter,retrospective cohort study included all cases of confirmed coinfection of severe acute respiratory syndrome coronavirus 2 and chronic hepatitis C with or without liver cirrhosis who were admitted to six hospitals(Al-Sahel Hospital,Al-Matareya Hospital,Al-Ahrar Hospital,Ahmed Maher Teaching Hospital,Al-Gomhoreya Hospital,and the National Hepatology and Tropical Medicine Research Institute)affiliated with the General Organization for Teaching Hospitals and Institutes in Egypt.Patients were recruited from May 1,2020,to July 31,2020.Demographic,laboratory,imaging features,and outcomes were collected.Multivariate regression analysis was performed to detect factors affecting mortality.RESULTS This retrospective cohort study included 125 patients with chronic hepatitis C and COVID-19 co-infection,of which 64(51.20%)had liver cirrhosis and 40(32.00%)died.Fever,cough,dyspnea,and fatigue were the most frequent symptoms in patients with liver cirrhosis.Cough,sore throat,fatigue,myalgia,and diarrhea were significantly more common in patients with liver cirrhosis than in noncirrhotic patients.There was no difference between patients with and without cirrhosis regarding comorbidities.Fifteen patients(23.40%)with liver cirrhosis presented with hepatic encephalopathy.Patients with liver cirrhosis were more likely than non-cirrhotic patients to have combined ground-glass opacities and consolidations in CT chest scans:28(43.75%)vs 4(6.55%),respectively(P value<0.001).These patients also were more likely to have severe COVID-19 infection,compared to patients without liver cirrhosis:29(45.31%)vs 11(18.04%),respectively(P value<0.003).Mortality was higher in patients with liver cirrhosis,compared to those with no cirrhosis:33(51.56%)vs 9(14.75%),respectively(P value<0.001).All patients in Child-Pugh class A recovered and were discharged.Cirrhotic mortality occurred among decompensated patients only.A multivariate regression analysis revealed the following independent factors affecting mortality:Male gender(OR 7.17,95%CI:2.19–23.51;P value=0.001),diabetes mellitus(OR 4.03,95%CI:1.49–10.91;P value=0.006),and liver cirrhosis(OR 1.103,95%CI:1.037–1.282;P value<0.0001).We found no differences in liver function,COVID-19 disease severity,or outcomes between patients who previously received direct-acting antiviral therapy(and achieved sustained virological response)and patients who did not receive this therapy.CONCLUSION Patients with liver cirrhosis are susceptible to higher severity and mortality if infected with COVID-19.Male gender,diabetes mellitus,and liver cirrhosis are independent factors associated with increased mortality risk.Shimaa Afify Basem Eysa Fatma Abdel Hamid Omnia M Abo-Elazm Mohamed A Edris Rabab Maher Ahmed Abdelhalim Muhammad Mostafa Abdel Ghaffar Dalia A Omran Hend Ibrahim Shousha 2021World Journal of Gastroenterology2021,27,42:2

16Induction of sub-G0 arrest and apoptosis by seed extract of Moringa peregrina(Forssk.) Fiori in cervical and prostate cancer cell lines显示文摘Objective: This study investigated cytotoxicity and induction of apoptosis in human cervical cancer cells(HELA) and prostate cancer cells(PC-3) using the most active fraction of Moringa peregrina seed extract.Methods: Dried and powdered seeds were extracted using 95% ethanol. The total ethanolic extract was further dissolved in distilled water and separated into petroleum ether, chloroform, ethyl acetate and aqueous extracts. Based on the results of in vitro anticancer studies of all extracts, the most highly active extract was selected for evaluation of apoptosis induction and cell cycle analysis on HELA and PC-3 cells at its half maximal inhibitory concentration using flow cytometry;DNA fragmentation by agarose gel electrophoresis and the expression of protein were measured by Western blot.Results: The chloroform fraction from the ethanolic extract of M. peregrina(CFEE) was the most active antitumor fraction. The selectivity index, determined using the normal Vero cell line, indicated that CFEE had a high degree of selectivity against HELA and PC-3 cells. CFEE induced apoptosis, confirmed by cell cycle arrest at sub-G0 phase and DNA fragmentation. CFEE induced an increase in mRNA expression of caspase-3, a decrease in Bcl-2 mRNA expression, and decreased ATP levels. CFEE increased protein expression of caspase-3 and decreased protein expression of poly-ADP-ribose polymerase-1(PARP-1).Flow cytometric analysis showed an appreciable increase in the number of cells in the early apoptotic stage in CFEE-treated HELA and PC-3 cells. 17Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model显示文摘AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma(HCC) with a panel of serum proteins in combination with α-fetoprotein(AFP).METHODS: Serum levels of interleukin(IL)-8, soluble intercellular adhesion molecule-1(s ICAM-1), soluble tumor necrosis factor receptor Ⅱ(s TNF-RⅡ), proteasome, and β-catenin were measured in 479 subjects categorized into four groups:(1) HCC concurrent with hepatitis C virus(HCV) infection(n = 192);(2) HCV related liver cirrhosis(LC)(n = 96);(3) Chronic hepatitis C(CHC)(n = 96); and(4) Healthy controls(n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different groups. RESULTS: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model(-11.3 + 7.38 × Prot + 0.00108 × s ICAM + 0.2574 ×β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × s ICAM-1 +(4.251 × 10-4) × s TNF + 0.02567 ×β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Additionally, we derived an algorithm, based on a binary classifier, for resolving the multi-class classification problem by using three successive mathematical model predictions of liver disease status. 18Safety and efficacy of Hansenula-derived PEGylated-interferon alpha-2a and ribavirin combination in chronic hepatitis C Egyptian children显示文摘AIM:To investigate the safety and efficacy of a Hansenula-derived PEGylated(polyethylene glycol)interferon(IFN)-alpha-2a(Reiferon Retard)plus ribavirin customized regimen in treatment-na?ve and previously treated(non-responders and relapsers)Egyptian children with chronic hepatitis C infection.METHODS:Forty-six children with chronic hepatitis C virus(HCV)infection were selected from three tertiary pediatric hepatology centers.Clinical and laboratory evaluations were undertaken.Quantitative polymerase chain reaction(PCR)for HCV-RNA was performed before starting treatment,and again at 4,12,24,48,72wk during treatment and 6 mo after treatment cessation.All patients were assigned to receive a weekly subcutaneous injection of PEG-IFN-alpha-2a plus daily oral ribavirin for 12 wk.Thirty-four patients were treatment-na?ve and 12 had a previous treatment trial.Patients were then divided according to PCR results into two groups.GroupⅠincluded patients who continued treatment on a weekly basis(7-d schedule),while groupⅡincluded patients who continued treatment on a 5-d schedule.Patients from either group who were PCR-negative at week 48,but had at least one PCRpositive test during therapy,were assigned to have an extended treatment course up to 72 wk.The occurrence of adverse effects was assessed during treatment and follow up.The study was registered at www.ClinicalTrials.gov(NCT02027493).RESULTS:Only 11 out of 46(23.9%)patients showed a sustained virological response(SVR),two patients were responders at the end of treatment;however,they were lost to follow up at 6 mo post treatment.Breakthrough was seen in 18(39.1%)patients,one patient(2.17%)showed relapse and 14(30.4%)were non-responders.Male gender,short duration of infection,low viral load,mild activity,and mild fibrosis were the factors related to a better response.On the other hand,patients with high viral load and absence of fibrosis failed to respond to treatment.Before treatment,liver transaminases were elevated.After commencing treatment,they were normalized in all patients at week 4 and were maintained normal in responders till the end of treatment,while they increased again significantly in non-responders(P=0.007 and 0.003 at week 24 and 72 respectively).The 5-d schedule did not affect the response rate(1/17 had SVR).Treatment duration(whether 48 wk or extended course to 72 wk)gave similar response rates(9/36 vs 2/8 respectively;P=0.49).Type of previous treatment(short acting IFN vs PEG-IFN)did not affect the response to retreatment.On the other hand,SVR was significantly higher in previous relapsers than in previous non-responders(P=0.039).Only mild reversible adverse effects were observed and children tolerated the treatment well.CONCLUSION:Reiferon Retard plus ribavirin combined therapy was safe.Our customized regimen did not influence SVR rates.Further trials on larger numbers of patients are warranted.Suzan El Naghi Tawhida Y Abdel-Ghaffar Hanaa El-Karaksy Elham F Abdel-Aty Mona S El-Raziky Aleef A Allam Heba Helmy Hanaa A El-Araby Behairy E Behairy Mohamed A El-Guindi Hatem El-Sebaie Aisha Y Abdel-Ghaffar Nermin A Ehsan Ahmed M El-Hennawy Mostafa M Sira 2014World Journal of Gastroenterology2014,20,16:2
