| 2 | Colonoscopy quality with Entonox? vs intravenous conscious sedation:18608 colonoscopy retrospective study显示文摘AIM To compare colonoscopy quality with nitrous oxide gas(Entonox?) against intravenous conscious sedation using midazolam plus opioid.METHODS A retrospective analysis was performed on a prospectively held database of 18608 colonoscopies carried out in Lothian health board hospitals between July 2013 and January 2016.The quality of colonoscopies performed with Entonox was compared to intravenous conscious sedation(abbreviated in this article as IVM).Furthermore,the quality of colonoscopies performed with an unmedicated group was compared to IVM.The study used the following key markers of colonoscopy quality:(1) patient comfort scores;(2) caecal intubation rates(CIRs); and (3) polyp detection rates (PDRs).We used binary logistic regression to model the data.RESULTS There was no difference in the rate of moderate-toextreme discomfort between the Entonox and IVM groups (17.9% vs 18.8%; OR = 1.06,95%CI: 0.95-1.18,P = 0.27).Patients in the unmedicated group were less likely to experience moderate-to-extreme discomfort than those in the IVM group(11.4% vs 18.8%; OR = 0.71,95%CI: 0.60-0.83,P < 0.001).There was no difference in caecal intubation between the Entonox and IVM groups(94.4% vs 93.7%; OR = 1.08,95%CI: 0.92-1.28,P = 0.34).There was no difference in caecal intubation between the unmedicated and IVM groups (94.2% vs 93.7%; OR = 0.98,95%CI: 0.79-1.22,P = 0.87).Polyp detection in the Entonox group was not different from IVM group (35.0% vs 33.1%; OR = 1.01,95%CI: 0.93-1.10,P = 0.79).Polyp detection in the unmedicated group was not significantly different from the IVM group (37.4% vs 33.1%; OR = 0.97,95%CI: 0.87-1.08,P = 0.60).CONCLUSION The use of Entonox was not associated with lower colonoscopy quality when compared to intravenous conscious sedation using midazolam plus opioid. | Alexander R Robertson Nicholas A Kennedy James A Robertson Nicholas I Church Colin L Noble | 2017 | World Journal of Gastrointestinal Endoscopy2017,9,9: | 3 |
| 3 | Comparison of Two Dose-response Relationship of Noise Exposure Evaluation Results with High Frequency Hearing Loss显示文摘Background: Complex noise and its relation to hearing loss are difficult to measure and evaluate. In complex noise measurement, individual exposure results may not accurately represent lifetime noise exposure. Thus, the mean LAcq.8h values of individuals in the same workgroup were also used to represent LAcq.8h in our study. Our study aimed to explore whether the mean exposure levels of workers in the same workgroup represented real noise exposure better than individual exposure levels did. Methods: A cross-sectional study was conducted to establish a model for cumulative noise exposure (CNE) and hearing loss in 205 occupational noise-exposed workers who were recruited from two large automobile manufacturers in China. We used a personal noise dosimeter and a questionnaire to determine the workers' occupational noise exposure levels and exposure times, respectively. A qualified audiologist used standardized audiometric procedures to assess hearing acuity after at least 16 h of noise avoidance. Results: We observed that 88.3% of workers were exposed to more than 85 dB(A) of occupational noise (mean: 89.3 ± 4.2 dB(A)). The personal CNE (CNEp) and workgroup CNE (CNEg) were 100.5 ± 4.7 dB(A) and 100.5 ± 2.9 dB(A), respectively. In the binary logistic regression analysis, we established a regression model with high-frequency hearing loss as the dependent variable and CNE as the independent variable. The Wald value was 5.014 with CNEp as the independent variable and 8.653 with CNEg as the independent variable. Furthermore, we found that the figure for CNEg was more similar to the stationary noise reference than CNEp was. The CNEg model was better than the CNEp model. In this circumstance, we can measure some subjects instead of the whole workgroup and save manpower. | 4 | Metasurface-based multi-harmonic freeelectron light source显示文摘Metasurfaces are subwavelength spatial variations in geometry and material where the structures are of negligible thickness compared to the wavelength of light and are optimized for far-field applications,such as controlling the wavefronts of electromagnetic waves.Here,we investigate the potential of the metasurface near-field profile,generated by an incident few-cycle pulse laser,to facilitate the generation of high-frequency light from free electrons.In particular,the metasurface near-field contains higher-order spatial harmonics that can be leveraged to generate multiple higher-harmonic X-ray frequency peaks.We show that the X-ray spectral profile can be arbitrarily shaped by controlling the metasurface geometry,the electron energy,and the incidence angle of the laser input.Using ab initio simulations,we predict bright and monoenergetic X-rays,achieving energies of 30 keV(with harmonics spaced by 3 keV)from 5-MeV electrons using 3.4-eV plasmon polaritons on a metasurface with a period of 85 nm.As an example,we present the design of a four-color X-ray source,a potential candidate for tabletop multicolor hard X-ray spectroscopy.Our developments could help pave the way for compact multi-harmonic sources of high-energy photons,which have potential applications in industry,medicine,and the fundamental sciences. | Gilles Rosolen Liang Jie Wong Nicholas Rivera Bjorn Maes Marin Soljačić Ido Kaminer | 2018 | Light(Science & Applications)2018,7,1: | 2 |
