1Poor performance of the modified early warning score for predicting mortality in critically ill patients presenting to an emergency department显示文摘BACKGROUND:This study was undertaken to validate the use of the modified early warning score(MEWS) as a predictor of patient mortality and intensive care unit(ICU)/ high dependency(HD)admission in an Asian population.METHODS:The MEWS was applied to a retrospective cohort of 1 024 critically ill patients presenting to a large Asian tertiary emergency department(ED) between November 2006 and December2007.Individual MEWS was calculated based on vital signs parameters on arrival at ED.Outcomes of mortality and ICU/HD admission were obtained from hospital records.The ability of the composite MEWS and its individual components to predict mortality within 30 days from ED visit was assessed.Sensitivity,specificity,positive and negative predictive values were derived and compared with values from other cohorts.A MEWS of ≥4 was chosen as the cut-off value for poor prognosis based on previous studies.RESULTS:A total of 311(30.4%) critically ill patients were presented with a MEWS ≥4.Their mean age was 61.4 years(SD 18.1) with a male to female ratio of 1.10.Of the 311 patients,53(17%)died within 30 days,64(20.6%) were admitted to ICU and 86(27.7%) were admitted to HD.The area under the receiver operating characteristic curve was 0.71 with a sensitivity of 53.0%and a specificity of 72.1%in addition to a positive predictive value(PPV) of 17.0%and a negative predictive value(NPV)of 93.4%(MEWS cut-off of ≥4) for predicting mortality.CONCLUSION:The composite MEWS did not perform well in predicting poor patient outcomes for critically ill patients presenting to an ED.Le Onn Ho Huihua Li Nur Shahidah Zhi Xiong Koh Papia Sultana Marcus Eng Hock Ong 2013World Journal of Emergency Medicine2013,4,4:12
2高效液相色谱-紫外检测法同时测定药物制剂及人体血清中的奥美沙坦酯和厄贝沙坦及氢氯噻嗪(英文)显示文摘A simple,selective,sensitive,precise,simultaneous high performance liquid chromatographic analysis of serum samples and commercial tablet formulation containing hydrochlorothiazide,olmesartan medoxomil and irbesartan are reported.Good chromatographic separation was achieved using a μ-Bondapak,C18 column(15 cm×4.6 mm,5 μm),and a mobile phase consisting of acetonitrile-0.2% acetic acid aqueous solution(50∶50,v/v)at a flow rate of 1.0 mL/min.The ultraviolet detector was set at a wavelength of 260 nm.Hydrochlorothiazide,olmesartan medoxomil,and irbesartan were eluted at 1.2,3.8,and 4.4 min,respectively.No extraneous materials were found to interfere.The method uses protein precipitation with acetonitrile for the preparation of serum sample.The linear ranges for hydrochlorothiazide,olmesartan medoxomil,and irbesartan were 6.25-18.75,20-60,and 75-225 ng/mL,respectively.The recoveries of hydrochlorothiazide,olmesartan medoxomil,and irbesartan in spiked samples were all greater than 98%,and their relative standard deviations were less than 2.0%.The limits of detection were 1,2,and 2 ng/mL for hydrochlorothiazide,olmesartan medoxomil,and irbesartan,respectively,and the limits of quantification were 3 ng/mL,which allow their determination at the expected serum concentration levels.SULTANA Najma ARAYNE M Saeed ALI S Shahid SAJID Shahnawaz 2008色谱2008,26,5:6
3Predicting herbicides concentrations in paddy water and runoff to the river basin显示文摘This study was conducted to investigate the dissipation pattern and runoff of herbicides to the river basin from the paddy fields. Pesticide paddy field model(PADDY) was applied to predict herbicide concentration in paddy fields. A field study was conducted in a paddy farm of Higashi Hiroshima City, Hiroshima Prefecture, Japan in the year of 2003 paddy season. The herbicides were mefenacet, thiobencarb, and bensulfuron methyl. The sample water was analyzed by using gas chromatography and HPLC after solid phase extraction. Predicted dissipation rate of thiobencarb in paddy water was higher(DT_ 50 = 4 36) than that measured, with a lower k value(-0 069). Two weeks after application no thiobencarb was detected in the drainage channel and down stream. In the down stream, thiobencarb was detected until 3 d after application, with a range of 0 02% to 0 08% of applied herbicide. The predicted dissipation rate(k) and half-life(DT_ 50 ) of mefenacet was not significantly different from that of measured. In the drainage channel, upstream and downstream mefenacet was found during the whole study period. In downstream, the maximum concentration of mefenacet was present 0 61% of applied in the paddy field on 3DAH. The dissipation rate(k) of BSM varied from -0 0860 to -0 1059 to with half-life(DT_ 50 ) 3 5 and 2 84 d. In upstream water, no BSM was detected except trace amounts(0 01 μg/L) at 3 d after application. However, in the drainage channel 8%, 6% and 1 58% of applied BSM was present at 0, 1 and 3 d after application respectively. 4Mitochondrial DNA mutations-candidate biomarkers for breast cancer diagnosis in Bangladesh显示文摘Breast cancer is a major health problem that affects more than 24% of women in Bangladesh. Furthermore, among low-income countries including Bangladesh, individuals have a high risk for developing breast cancer. This study aimed to identify candidate mitochondrial DNA (mtDNA) biomarkers for breast cancer diagnosis in Bangladeshi women to be used as a preventive approach. We screened the blood samples from 24 breast cancer patients and 20 healthy controls to detect polymorphisms in the D-loop and the ND3-and ND4-coding regions of mtDNA by direct sequencing. Among 14 distinct mutations, 10 polymorphisms were found in the D-loop, 3 were found in the ND3-coding region, and 1 was found in the ND4-coding region. The frequency of two novel polymorphisms in the D-loop, one at position 16290 (Tins) and the other at position 16293 (A-del), was higher in breast cancer patients than in control subjects (position 16290: odds ratio = 6.011, 95% confidence interval = 1.2482 to 28.8411, P = 0.002; position 16293: odds ratio = 5.6028, 95% confidence interval = 1.4357 to 21.8925, P = 0.010). We also observed one novel mutation in the ND3-coding region at position 10316 (A > G) in 69% of breast cancer patients but not in control subjects. 7Effect of Low Level Subchronic Microwave Radiation on Rat Brain显示文摘Objective The present study was designed to investigate the effects of subchronic low level microwave radiation(MWR) on cognitive function,heat shock protein 70(HSP70) level and DNA damage in brain of Fischer rats.Methods Experiments were performed on male Fischer rats exposed to microwave radiation for 90 days at three different frequencies: 900,1800,and 2450 MHz.Animals were divided into 4 groups: Group I: Sham exposed,Group II: animals exposed to microwave radiation at 900 MHz and specific absorption rate(SAR) 5.953 × 10-4 W/kg,Group III: animals exposed to 1800 MHz at SAR 5.835 × 10-4 W/kg and Group IV: animals exposed to 2450 MHz at SAR 6.672 × 10-4 W/kg.All the animals were tested for cognitive function using elevated plus maze and Morris water maze at the end of the exposure period and subsequently sacrificed to collect brain tissues.HSP70 levels were estimated by ELISA and DNA damage was assessed using alkaline comet assay.Results Microwave exposure at 900-2450 MHz with SAR values as mentioned above lead to decline in cognitive function,increase in HSP70 level and DNA damage in brain.Conclusion The results of the present study suggest that low level microwave exposure at frequencies 900,1800,and 2450 MHz may lead to hazardous effects on brain.Pravin Suryakantrao Deshmukh Kanu Megha Namita Nasare Basu Dev Banerjee Rafat Sultana Ahmed Mahesh Pandurang Abegaonkar Ashok Kumar Tripathi Pramod Kumari Mediratta 2016Biomedical and Environmental Sciences2016,29,12:4
8Prevalence and Factors Associated with Concomitant Chinese Medicine Use by Rheumatoid Arthritis Patients in A Multi-Ethnic Asian Population显示文摘Objective:To determine the prevalence,factors associated with and patterns of concomitant Chinese medicine(CM)with Western treatment use among patients with rheumatoid arthritis(RA)in a tertiary referral centre(Singapore General Hospital)in Singapore.Methods:We conducted a cross-sectional intervieweradministered survey of a consecutive sample of patients with RA in Singapore General Hospital centre regarding their CM use including data on patient demographics,disease characteristics,concomitant use of CM and reasons,concerns and disclosure patterns from March to August 2015.Univariate and multivariate logistic regression analyses were performed to determine the associations of CM use.Results:Prevalence of CM use among the 258 patients surveyed(male:female 42:216;Chinese:Malay:Indian 191:29:34;mean age:61 years;mean duration of RA:10 years)was 46.1%(119/258).On muutivariate analysis,Chinese ethnicity(OR,95%CI:4.11,1.49-11.36),Chinese speakers(OR,95%CI:2.35,1.03-5.54),middle-income group(OR,95%CI:2.53,1.01-6.31)and greater learned helplessness(OR,95%CI:1.13,1.04-1.22)were significantly associated with CM use.More CM users disclosed their CM use to CM physicians(87.3%,96/110),sought advice from them on treatment interactions(59.4%,57/96)and how best to combine treatments(49.0%,47/96)than did so with rheumatologists(42.0%,50/119;40.0%,20/50;and 42.0%,21/50,respectively).Forty-two percentage(29/69)of patients who concealed CM use from rheumatologists because their rheumatologists did not specifically enquire about CM use.Conclusions:Concomitant CM use among patients with RA treated in a tertiary referral centre in Singapore is high but voluntary disclosure is low.The associations identified can help doctors identify and enquire about CM use,minimizing potential adverse interactions.HUANG You-yi XIN Xiao-hui Rehena Sultana Julian Thumboo FONG Kok Yong 2022Chinese Journal of Integrative Medicine2022,28,3:3

10Synthesis and antibacterial studies of ciprofloxacin-metal complexes显示文摘Metal complexes of ciprofloxacin were synthesized to study the effect of the drug on the metals present in the body or metals co-administered as multivitamin therapy. These complexes were then characterized by IR, UV, 1 H NMR, CHN and AA analyses. It was found that the metals coordinated through the β-keto carboxylic group, forming four coordinated complexes having square planar geometry. The effect of the antibacterial activity of these complexes with respect to parent drug was also studied against 14 different Gram + ve and Gram – ve organisms by the disk susceptibility technique. 11Wheat leaf senescence and its regulatory gene network显示文摘Wheat leaf senescence is a developmental process that involves expressional changes in thousands of genes that ultimately impact grain protein content(GPC), grain yield(GY), and nitrogen use efficiency.The onset and rate of senescence are strongly influenced by plant hormones and environmental factors e.g. nitrogen availability. At maturity, decrease in nitrogen uptake could enhance N remobilization from leaves and stem to grain, eventually leading to leaf senescence. Early senescence is related to high GPC and somewhat low yield whereas late senescence is often related to high yield and somewhat low GPC. Early or late senescence is principally regulated by up and down-regulation of senescence associated genes. Integration of external and internal factors together with genotypic variation influence senescence associated genes in a developmental age dependent manner. Although regulation of genes involved in senescence has been studied in rice, Arabidopsis, maize, and currently in wheat, there are genotypespecific variations yet to explore. A major effort is needed to understand the interaction of positive and negative senescence regulators in determining the onset of senescence. In wheat, increasing attention has been paid to understand the role of positive senescence regulator, e.g. GPC-1, regulated gene network during early senescence time course. Recently, gene regulatory network involved early to late senescence time course revealed important senescence regulators. However, the known negative senescence regulator Ta NAC-S gene has not been extensively studied in wheat and little is known about its value in breeding. 14Roles of different fertilizer management practices on mulberry leaf yield and quality显示文摘Effects of fertilizer management practices on leaf yield and quality of mulberry plant are important for sustainable mulberry plant production.This study was undertaken to improve the growth,yield and biochemical contents in an instant available form in mulberry plant through different fertilizer management practices.There were four fertilizer management practices like control(CT),basal(BS),urea+magic growth(UM)and basal+urea+magic growth(BUM)were applied on mulberry plant(Morus SPP.).Results of this investigation revealed that among the various fertilizer treatments on the foliar application of(3 g urea+3 mL magic growth)per liter water with BSRTI recommended BS of NPK performed well in respect of growth parameters and biochemical constituents.The highest 10 leaf area(638.64 cm^(2)),leaf number per branch(50),total leaf weight per plant(785.64 g),node per meter(28.67),specific leaf weight(0.005817 g/cm^(2)),length of longest shoot(173.25 cm)and total leaf yield per hectare per year(41610 kg)were noticed in BUM treatment followed by UM,BS and CT treatments.Further the highest moisture(74.38%),moisture retention capacity(33.31%),total mineral(12.10%),total sugar(3.99%),reducing sugar(4.05%),crude protein(21.01%),starch(9.72%)and soluble carbohydrate(11.41%)were also recorded in BUM treatment as compare to UM,BS and CT treatments.The outcome of research implies that among the four fertilizer management practices foliar spray of(3 g urea+3 mL magic growth)per liter water with BSRTI recommended BS dose of NPK performed well and successfully augments the productivity and biochemical constituents of mulberry plant.Faruque Ahmed Rafia Sultana Oli Ahmed Md.Akhtaruzzaman Md.Toufiq Iqbal 2017International Journal of Agricultural and Biological Engineering2017,10,5:2
15Status and ethno-medicinal usage of invasive plants in traditional health care practices: a case study from northeastern Bangladesh显示文摘Species diversity and ethno-medicinal usage of 39 species of invasive plants were explored in and around two protected areas, Khadimnagar National Park (KNP) and Rema-Kalenga Wildlife Sanctu- ary (RKWS), of northeastern Bangladesh,. Status of invasive plants were investigated in 60 sample plots with 5 different habitat types, including forest, roadside, homestead, fallow land and others (ponds, canals, water logged areas, agricultural land, etc.). Data about the usage of invasive plants in traditional health care were collected through interviewing 110 households. Among the five habitat types, fallow land (28 species) pos- sessed the highest number of species, followed by roadside (25 species), forest (23 species), homestead (22 species) and others (13 species). Based on the survey, invasive plants of study areas were also categorized into five degrees of invasiveness e.g., naturalized, introduced, possibly invasive, moderately invasive and highly invasive. Additionally, there is the linear trend between degrees of invasiveness and use percentage of invasive plants. Total 39 species of invasive plants belonging to 29 fami- lies were recorded, which are generally used to treat 37 diseases, ranging from simple headache to highly complicated eye and heart diseases. The majority of the species used by the local inhabitants were herbs (16 spe- cies), followed by some shrubs (11 species), climbers (5 species), trees (5 species) and grasses (2 species). 16Multisystem inflammatory syndrome in pediatric COVID-19 patients:a meta-analysis显示文摘Background We aimed to systematically review the clinical and laboratory features of patients with the multisystem inflam-matory syndrome in pediatrics diagnosed during the COVID-19 pandemic.Data sources A literature search in Web of Science,PubMed,Scopus,and Science Direct was made up to June 29,2020.Results Analysis of 15 articles(318 COVID-19 patients)revealed that although many patients presented with the typical multisystem inflammatory syndrome in pediatrics,Kawasaki-like features as fever(82.4%),polymorphous maculopapular exanthema(63.7%),oral mucosal changes(58.1%),conjunctival injections(56.0%),edematous extremities(40.7%),and cervical lymphadenopathy(28.5%),atypical gastrointestinal(79.4%)and neurocognitive symptoms(31.8%)were also com-mon.They had elevated serum lactic acid dehydrogenase,D-dimer,C-reactive protein,procalcitonin,interleukin-6,troponin I levels,and lymphopenia.Nearly 77.0%developed hypotension,and 68.1%went into shock,while 41.1%had acute kidney injury.Intensive care was needed in 73.7%of cases;13.2%were intubated,and 37.9%required mechanical ventilation.Intravenous immunoglobulins and steroids were given in 87.7%and 56.9%of the patients,respectively,and anticoagulants were utilized in 67.0%.Pediatric patients were discharged after a hospital stay of 6.77 days on average(95%CI 4.93-8.6).Conclusions Recognizing the typical and atypical presentation of the multisystem inflammatory syndrome in pediatric COVID-19 patients has important implications in identifying children at risk.Monitoring cardiac and renal decompensation and early interventions in patients with multisystem inflammatory syndrome is critical to prevent further morbidity.Eman A.Toraih Mohammad H.Hussein Rami M.Elshazli Adam Kline Ruhul Munshi Nasrin Sultana Sharven Taghavi Mary Killackey Juan Duchesne Manal S.Fawzy Emad Kandil 2021World Journal of Pediatrics2021,17,2:2
