| 1 | Interleukin-17 levels in Helicobacter pylori-infected gastric mucosa and pathologic sequelae of colonization显示文摘AIM: To determine the role of interleukin (IL)-17 in gastric ulcerogenesis. METHODS: Thirty-six gastric ulcer (GU) patients and 29 non-ulcer (NU) patients were enrolled in this study. Mucosal biopsy samples were obtained from the gastric antrum and GU site during endoscopy. Samples were used in in situ stimulation for 48 h in the presence of 10 μg/mL phytohemagglutinin-P (PHA), histological examination, and Helicobacter pylori(H pylori) culture. IL-17 and IL-8 protein levels in culture supernatants were assayed by ELISA. IL-17 mRNA expression was analyzed by reverse transcriptase-polymerase chain reaction (RT-PCR). H pylori cagA and vacA status was assessed by reverse hybridization using a line probe assay (LiPA). IL-8 levels in culture supernatants were assayed after AGS cells were co-cultured with H pylori strain 26 695 or recombinant human (rh) IL-17. RESULTS: All 36 GU patients and 15 of 29 NU patients were found to be H pylori-positive, while 14 NU patients were H pylori-negative. All 51 H pylori strains from both GU and NU patients were cagA- and vacAs1/m1-positive. Antral mucosal tissues from H pylori-positive patients contained significantly (H pylori-positive NU patients: median 467 pg/mg/protein, range 53-2 499; H pylori-negative NU patients: median 104 pg/mg/protein, range 16-312, P<0.0005) higher levels of IL-17 than those from uninfected patients. IL-17 levels at the ulcer site were significantly (ulcer site: median 1356 pg/mg/protein, range 121-1 3730; antrum: median 761 pg/mg/protein, range 24-7 620, P<0.005) higher than those at distant sites in the antrum. Biopsies from H pylori-positive GU and NU patients showed IL-17 mRNA expression in all samples whereas those from the antrum of the H pylori-negative controls showed no detectable expression. A significant correlation was seen between IL-17 and IL-8 levels at each biopsy site (ulcer: r= 0.62, P<0.0001; antrum: r = 0.61, P<0.0001) in GU patients. RhIL-17 and H pylori strain 26 695 each stimulated IL-8 production from AGS cells. | 2 | A+C群脑膜炎球菌多糖疫苗安全性评估显示文摘目的 评估A+C群脑膜炎球菌多糖疫苗的安全性。方法 以A+C群脑膜炎球菌多糖疫苗为观察组,伤寒Vi多糖疫苗为对照组,按整群随机分层配对的原则,将研究现场分为108个组群,观察组和对照组各分配54个组。对两组同时建立接种反应监测系统,按统一表格和方法对两组的接种反应进行监测、记录,并进行流行病学调查。结果 两组共接种34543人,其中A+C群脑膜炎球菌多糖疫苗接种18167人,伤寒Vi疫苗接种16376人。A+C群脑膜炎球菌多糖疫苗速发接种反应率为0.44‰,一般接种反应率为0.38%o;伤寒Vi疫苗速发接种反应率为0.79%,一般接种反应率为0.73%o;跟踪随访接种反应对象共1239人,其中接种A+C群脑膜炎球菌多糖疫苗的771人,接种伤寒Vi疫苗的468人。接种后第1天,A+C群脑膜炎球菌多糖疫苗组的局部反应发生率要明显高于伤寒Vi疫苗(X^2。=13.98,P=0.0002);接种后第2—3天局部反应和全身反应的发生率,两组差异无统计学意义;两组各自接种人群和未接种人群发热发生率的差异无统计学意义,两组已接种人群的发热发生率差异也无统计学意义;两组均未发现严重反应。结论 A+C群脑膜炎球菌多糖疫苗与伤寒Vi疫苗均可在不同年龄组人群开展大规模接种,局部及全身接种反应轻微,接种反应率低,有良好的安全性。 | 董柏青 叶强 崔萱林 杨进 龚健 杨明 廖和壮 韦士良 张杰 吴兴华 司国爱 杨宏徽 Ataru Tsuzuki JinKyung Park Mohammad Ali R. | 4 | Epstein–Barr virus-associated hemophagocytic syndrome in a patient with ulcerative colitis during treatment with azathioprine: A case report and review of literature显示文摘A 19-year-old female was diagnosed with ulcerative colitis when she presented with persistent melena, and has been treated with 5-aminosalicylic acid for 4 years,with additional azathioprine for 2 years at our hospital.The patient experienced high-grade fevers, chills, and cough five d prior to presenting to the outpatient unit.At first, the patient was suspected to have developed neutropenic fever; however, she was diagnosed with Epstein-Barr virus-associated hemophagocytic syndrome(EB-VAHS) upon fulfilling the diagnostic criteria after bone marrow aspiration. | 5 | Laparoscopic-extracorporeal surgery performed with a fixation device for adnexal masses complicating pregnancy: Report of two cases显示文摘The potential complications associated with an adnexal mass discovered during early pregnancy call for surgical treatment. Ideally, surgery is performed after gestational week 12, but uterine expansion after the first trimester makes surgery difficult. We report two pregnancies complicated by adnexal masses for which we used an organ fixation device for safe performance of single-site umbilical laparoscopic surgery. Pelvic magnetic resonance imaging depicted a dichorionic, diamniotic twin pregnancy and 60-mm right adnexal mass in the first patient and bilateral adnexae in the second. All three masses were suspected mature cystic teratomas. Both patients underwent laparoscopic surgery during gestational week 14. With use of an organ fixation device, traction was applied until the mass reached the umbilicus; tumor resection was performed extracorporeally. In the second patient, the second mass was simply aspirated because adhesions were encountered.
