| 1 | 基于Pareto熵的多目标粒子群优化算法显示文摘粒子群优化算法因形式简洁、收敛快速和参数调节机制灵活等优点,同时一次运行可得到多个解,且能逼近非凸或不连续的Pareto最优前端,因而被认为是求解多目标优化问题最具潜力的方法之一.但当粒子群优化算法从单目标问题扩展到多目标问题时,Pareto最优解集的存储与维护、全局和个体最优解的选择以及开发与开采的平衡等问题亦随之出现.通过目标空间变换方法,采用Pareto前端在被称为平行格坐标系统的新目标空间中的分布熵及差熵评估种群的多样性及进化状态,并以此为反馈信息来设计进化策略,使得算法能够兼顾近似Pareto前端的收敛性和多样性.同时,引入格占优和格距离密度的概念来评估Pareto最优解的个体环境适应度,以此建立外部档案更新方法和全局最优解选择机制,最终形成了基于Pareto熵的多目标粒子群优化算法.实验结果表明:在IGD性能指标上,与另外8种对等算法相比,该算法在由ZDT和DTLZ系列组成的12个多目标测试问题集中表现出了显著的性能优势. | 胡旺 Gary G. | 3 | Thalidomide-based multidisciplinary treatment for patients with advanced hepatocellular carcinoma:A retrospective analysis显示文摘AIM:To evaluate the efficacy of thalidomide in combination with other therapies to treat patients with advanced hepatocellular carcinoma (HCC).METHODS:We performed a retrospective analysis of all patients with HCC who were treated with thalidomide for at least two months.The medical records of patients with HCC who were treated at our institution between April 2003 and March 2008 were reviewed.Image studies performed before and after treatment,tumor response,overall survival,and the decrease in α-fetoprotein (AFP) levels were evaluated.RESULTS:A total of 53 patients with HCC received either 100 or 200 mg/d of thalidomide.The patient population consisted of 9 women and 44 men with a median age of 61 years.Thirty patients (56.6%) were classified as Child-Pugh A,and 12 patients (22.6%) were classified as Child-Pugh B.Twenty-six patients had portal vein thrombosis (49.1%),and 25 patients had extrahepatic metastasis (47.1%).The median duration of thalidomide treatment was 6.0 mo.Six of the 53 patients achieved a confirmed response (11.3%),one achieved a complete response (1.9%) and 5 achieved a partial response (9.4%).The disease control rate (CR + PR + SD) was 28.3% (95% CI:17.8-42.4),and the median overall survival rate was 10.5 mo.The 1-and 2-year survival rates were 45% and 20%,respectively.Only one complete response patient showed an improved overall survival rate of 66.8 mo.Sixteen patients (30.2%) showed more than a 50% decrease in their serum AFP levels from baseline,indicating a better response rate (31.3%),disease control rate (43.8%),and overall survival time (20.7 mo).The therapy was well tolerated,and no significant toxicities were observed.CONCLUSION:Thalidomide was found to be safe for advanced HCC patients,demonstrating anti-tumor activity including response,survival,and AFP decreases of greater than 50% from baseline. | Yang-Yuan Chen Hsu-Heng Yen Kun-Ching Chou Shun-Sheng Wu | 2012 | World Journal of Gastroenterology2012,18,5: | 25 |

| 7 | Insulin resistance is associated with hepatocellular carcinoma in chronic hepatitis C infection显示文摘AIM:To elucidate the role of insulin resistance(IR) and serum adiponectin level in hepatocellular carcinoma(HCC) associated with chronic hepatitis C.METHODS:Clinical and biochemical characteristics were collected from 165 consecutive patients with newly diagnosed HCC.Homeostasis model assessment of IR(HOMA-IR) and serum adiponectin level were investigated in 188 patients with different stages of hepatitis C virus(HCV) infection.RESULTS:Among HCC patients,type 2 diabetics(DM) was more prevalent in HCV subjects(35.6%,n = 59) compared to hepatitis B virus(HBV;12.7%,n = 63) or non-HBV,non-HCV cases(7.1%,n = 28).In patients with chronic hepatitis C,HCC subjects had higher blood sugar(P < 0.001),insulin level(P = 0.003) and HOMAIR(P = 0.018) than those with chronic hepatitis and advanced fibrosis.Age,male sex and body mass index were significantly associated with serum adiponectin level,whereas HOMA-IR was not.Based on stepwise logistic regression analysis,age(OR:1.124,P < 0.001),serum insulin level(OR:1.585,P < 0.001),HOMA-IR(OR:0.495,P = 0.001),DM(OR:11.601,P = 0.002) and male sex(OR:3.877,P = 0.016) were independently associated with HCC.This result was similar even if the diabetic subjects were excluded for analysis.CONCLUSION:Insulin resistance measured by HOMAIR,regardless of the presence of diabetes,is significantly associated with HCC development in patients with chronic HCV infection. | Chao-Hung Hung Jing-Houng Wang Tsung-Hui Hu Chien-Hung Chen Kuo-Chin Chang Yi-Hao Yen YuanHung Kuo Ming-Chao Tsai Chuan-Mo Lee | 2010 | World Journal of Gastroenterology2010,16,18: | 18 |

| 9 | Cenozoic stratigraphy of Taiwan:Window into rifting,stratigraphy and paleoceanography of South China Sea显示文摘Shallow marine sequences of the northern South China Sea (SCS) are uplifted and exposed by plate convergence in the Taiwan mountain belt. These deposits provide detailed geological information about the rifting event, stratigraphy, sedimentology, paleoclimate and paleoceanography of the shallow SCS to compare with what are recorded in the ODP 1148 deep-sea core. Seismic surveys and marine micropalentological studies show that Eocene sequences in the offshore Taiwan Strait and onland Taiwan mountain belt are all deposited in rifting basins and are covered unconformably by the Late Oligocene-Neogene post-rifting strata. Between syn-rifting and post-rifting sequences, there is a regional break-up unconformity throughout the island. Early Oligocene and Late Eocene strata are missing along the break-up unconformity equivalent to the T7 unconformity in the Pearl River Mouth Basin off south China. This may suggest that the SCS oceanic crust could have initiated between 33 and 39 Ma. Neither obvious stratigraphic gap nor slumping features are found in the Oligocene-Miocene transition interval of Taiwan. This observation highly contrasts with what has been documented from the ODP 1148 deep-sea core. This suggests that the stratigraphic gap and slumping features could only be recorded in the SCS deep sea region, but not in the shallow shelf near Taiwan. Compared to the Middle Miocene paleoceanographic re-organization events in the SCS deep sea, the geological history of the Taiwan shallow sequence shows changes of in sedimentation and faunal composition. Due to the Antarctic glacial expansion at^14 Ma, Middle to late Miocene strata of the Western Foothills show progressive regression sedimentations associated with a decrease of benthic foraminif-eral abundance and a sharp faunal turnover event. Many Early-Middle Miocene endemic benthic foraminifers were extinct in 14-13 Ma and new benthic foraminifers of the Kuroshio Current fauna appeared from 10.2 Ma, comparable with new occurrence of Modern benthic foraminifers at 9 Ma in the Java Sea area. This reveals that the Western Boundary Kuroshio Current in the North Pacific could initiate from 10-9 Ma due to closures of the Indo-Pacific seaways by convergent tectonics between the Australian Continent and the Indonesian Arc in 12-8 Ma. Subduction of the SCS oceanic lithosphere since the Middle Miocene resulted in formation of the Hengchun Ridge accretionary prism and the North Luzon Arc. Occurrence of these two bathymetric highs ( 2400 m) since the Middle Miocene and closures of the inter-arc passages in the North Luzon arc in the last 3.5 Ma would control the water exchanges between the West Pacific and the deep SCS. Accordingly, the tectonic evolution in the Central Range-Hengchun Peninsula accretionary prism and the arc-forearc Coastal Range not only control directly the route for water exchanges between the West Pacific and the SCS, but also indirectly shows a great influence on the geochemistry of deep SCS waters. | 15 | Cost-effectiveness analysis of population-based screening of hepatocellular carcinoma: Comparing ultrasonography with two-stage screening显示文摘AIM: To assess the cost-effectiveness of two populationbased hepatocellular carcinoma(HCC) screening programs, two-stage biomarker-ultrasound method and mass screening using abdominal ultrasonography(AUS).METHODS: In this study, we applied a Markov decision model with a societal perspective and a lifetime horizon for the general population-based cohorts in an area with high HCC incidence, such as Taiwan. The accuracy of biomarkers and ultrasonography was estimated from published meta-analyses. The costs of surveillance, diagnosis, and treatment were based on a combination of published literature, Medicare payments, and medical expenditure at the National Taiwan University Hospital. The main outcome measure was cost per lifeyear gained with a 3% annual discount rate. RESULTS: The results show that the mass screening using AUS was associated with an incremental costeffectiveness ratio of USD39825 per life-year gained, whereas two-stage screening was associated with an incremental cost-effectiveness ratio of USD49733 per life-year gained, as compared with no screening. Screening programs with an initial screening age of 50 years old and biennial screening interval were the most cost-effective. These findings were sensitive to the costs of screening tools and the specificity of biomarker screening.CONCLUSION: Mass screening using AUS is more cost effective than two-stage biomarker-ultrasound screening. | 17 | miR-106b promotes cancer progression in hepatitis B virusassociated hepatocellular carcinoma显示文摘AIM: To investigate the effect of mi R-106 b on tumor progression in hepatitis B virus(HBV)-associated hepatocellular carcinoma(HCC).METHODS: A total of 120 patients who underwent liver resection for HCC at National Cheng Kung University Hospital were enrolled in the present study. Micro RNA(mi RNA) array was first used to screen the mi RNA expression profiles in HCC patients. The clinical records were retrospectively analyzed, and correlations with the mi RNA expression profiles were evaluated. The m RNA expression levels of the mi R-106b-25 cluster(mi R-106 b, mi R-93 and mi R-25), and MCM7 in tumor and non-tumor samples were quantitated using quantitative real-time reverse transcription-polymerase chain reaction(q-RT-PCR) analysis, and correlations in the levels of mi R-106 b, mi R-93 and mi R-25 expression were calculated. Kaplan-Meier overall and diseasefree survival rates of HBV-associated HCC patients were analyzed using the log-rank test based on mi R-106 b expression. The comparison of the mi R-106 b expression levels in patients with different clinical outcomes was analyzed using Mann-Whitney U tests. Furthermore, a hepatitis B virus X protein(HBx) expression plasmid was transfected into Huh7 and Hep 3B cells. The expression levels of the mi R-106b-25 cluster and MCM7 in HBx-expressing Huh7 and Hep 3B cells were detected using q-RT-PCR. RESULTS: mi RNA array screening showed that mi R-106 b and its cluster, mi R-93 and mi R-25 were upregulated in HCC patients(P < 0.01). The value of mi R-106 b expression in HBV-associated HCC patients was significantly higher than that in HCV-(P < 0.05) or non-B/non-C-(P < 0.001) associated HCC patients. The expression of the mi R-106b-25 cluster was significantly higher in tumor tissue(P < 0.001) and associated with the host gene, MCM7, in clinical specimens from HBVassociated HCC patients. Furthermore, the expression levels of mi R-106 b, mi R-93 and mi R-25 were positively correlated in HBV-associated HCC tissues(mi R-106 vs mi R-93, r = 0.75; mi R-93 vs mi R-25, r = 0.69; mi R-106 b vs mi R-25, r = 0.33). The overall and diseasefree survival curves showed that high-mi R-106 b expression was correlated with the poor prognosis of HBV-associated HCC. HCC differentiation was significantly correlated with mi R-106 b expression(P < 0.05). Lower mi R-106 b expression levels resulted in the well differentiation of HCC. C.) beams using prestressed glass fi- ber-reinforced polymer (PGFRP). Carbon fiber-reinforced polymer (CFRP) has recently become popular for use as repair or rehabilitation material for deteriorated R. C. structures, but because CFRP material is very stiff, the difference in CFRP sheet and concrete material properties is not favorable for transferring the prestress from CFRP sheets to R. C. members. Glass fi- ber-reinforced polymer (GFRP) sheets with Modulus of Elasticity quite close to that of concrete was chosen in this study. The load-carrying capacities (ultimate loads) and the deflections of strengthened R. C. beams using GFRP and PGFRP sheets were tested and compared. T- and ⊥-shaped beams were used as the under-strengthened and over-strengthened beams. The GFRP sheets were prestressed to one-half their tensile capacities before being bonded to the T- and ⊥-shaped R. C. beams. The prestressed tension in the PGFRP sheets caused cambers in the R. C. beams without cracks on the tensile faces. The PGFRP sheets also enhanced the load-carrying capacity. The test results indicated that T-shaped beams with GFRP sheets increased in load-carrying capacity by 55% while the same beams with PGFRP sheets could increase load-carrying capacity by 100%. The ⊥-shaped beams with GFRP sheets could increase load-carrying capacity by 97% while the same beams with PGFRP sheets could increase the loading-carrying capacity by 117%. Under the same external loads, beams with GFRP sheets underwent larger deflections than beams with PGFRP sheets. While GFRP sheets strengthen R. | 20 | Does computed tomography permeability predict hemorrhagic transformation after ischemic stroke?显示文摘AIM: To use perfusion-derived permeability-surface area product maps to predict hemorrhagic transformation following thrombolytic treatment for acute ischemic stroke.METHODS: We retrospectively analyzed our prospectively kept acute stroke database over five consecutive months for patients with symptoms of acute ischemic stroke(AIS) who had computed tomography(CT) perfusion(CTP) done at arrival. Patients included in the analyses also had to have a follow-up CT. The permeability-surface area product maps(PS) was calculated for the side of the ischemia and/or infarction and for the contralateral unaffected side at the same level. The cerebral blood flow map was used to delineate the ischemic territory. Next, a region of interest was drawn at the centre of this territory on the PS parametric map. Finally, a mirror region of interest was created on the contralateral side at the same level. The relative permeability-surface area product maps(r PS) provided an internal control and was calculated as the ratio of the PS on the side of the AIS to the PS on the contralateral side. A student t-test was performed after log conversion of r PS between patients with and without hemorrhagic transformation. Log conversion was used to convert the data into normal distribution to use t-test. For the group of patients who experienced intracranial bleed, a student t-test was performed between those with only petechial hemorrhage and those with more severe parenchymal hematoma with subarachnoid haemorrhage.RESULTS: Of 84 patients with AIS and CTP at admission, only 42 patients had a follow-up CT. The r PSderived using the normal side as the internal control was significantly higher(P = 0.003) for the 15 cases of hemorrhagic transformation(1.71 + 1.64) compared to 27 cases that did not have any(1.07 + 1.30). Patients with values above the overall mean r PS of 1.3 had an increased likelihood of subsequent hemorrhagic transformation. The sensitivity of using this score to predict hemorrhagic transformation was 71.4, the specificity was 78.6, with a positive predictive value of 62.5 and negative predictive value of 84.6. The accuracy was 76.2. The odds ratio of an event occurring with such an r PS was 9.2. Of the 15 cases of hemorrhagic transformation, there was no difference(P = 0.35) in the r PS between the eight cases of petechial and the seven cases of more severe hemorrhagic events.CONCLUSION: Pretreatment PS can predict the occurrence of hemorrhagic transformation on follow-up of AIS patients with relatively high sensitivity, specificity, positive and negative predictive value. | Peggy Yen Allison Cobb Jai Jai Shiva Shankar | 2016 | World Journal of Radiology2016,8,6: | 10 |
