| 3 | Persistence and clearance of viral RNA in 2019 novel coronavirus disease rehabilitation patients显示文摘Background:A patient’s infectivity is determined by the presence of the virus in different body fluids,secretions,and excreta.The persistence and clearance of viral RNA from different specimens of patients with 2019 novel coronavirus disease(COVID-19)remain unclear.This study analyzed the clearance time and factors influencing 2019 novel coronavirus(2019-nCoV)RNA in different samples from patients with COVID-19,providing further evidence to improve the management of patients during convalescence.Methods:The clinical data and laboratory test results of convalescent patients with COVID-19 who were admitted to from January 20,2020 to February 10,2020 were collected retrospectively.The reverse transcription polymerase chain reaction(RT-PCR)results for patients’oropharyngeal swab,stool,urine,and serum samples were collected and analyzed.Convalescent patients refer to recovered non-febrile patients without respiratory symptoms who had two successive(minimum 24 h sampling interval)negative RT-PCR results for viral RNA from oropharyngeal swabs.The effects of cluster of differentiation 4(CD4)+T lymphocytes,inflammatory indicators,and glucocorticoid treatment on viral nucleic acid clearance were analyzed.Results:In the 292 confirmed cases,66 patients recovered after treatment and were included in our study.In total,28(42.4%)women and 38 men(57.6%)with a median age of 44.0(34.0-62.0)years were analyzed.After in-hospital treatment,patients’inflammatory indicators decreased with improved clinical condition.The median time from the onset of symptoms to first negative RT-PCR results for oropharyngeal swabs in convalescent patients was 9.5(6.0-11.0)days.By February 10,2020,11 convalescent patients(16.7%)still tested positive for viral RNA from stool specimens and the other 55 patients’stool specimens were negative for 2019-nCoV following a median duration of 11.0(9.0-16.0)days after symptom onset.Among these 55 patients,43 had a longer duration until stool specimens were negative for viral RNA than for throat swabs,with a median delay of 2.0(1.0-4.0)days.Results for only four(6.9%)urine samples were positive for viral nucleic acid out of 58 cases;viral RNA was still present in three patients’urine specimens after throat swabs were negative.Using a multiple linear regression model(F=2.669,P=0.044,and adjusted R2=0.122),the analysis showed that the CD4+T lymphocyte count may help predict the duration of viral RNA detection in patients’stools(t=-2.699,P=0.010).The duration of viral RNA detection from oropharyngeal swabs and fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(15 days vs.8.0 days,respectively;t=2.550,P=0.013)and the duration of viral RNA detection in fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(20 days vs.11 days,respectively;t=4.631,P<0.001).There was no statistically significant difference in inflammatory indicators between patients with positive fecal viral RNA test results and those with negative results(P>0.05).Conclusions:In brief,as the clearance of viral RNA in patients’stools was delayed compared to that in oropharyngeal swabs,it is important to identify viral RNA in feces during convalescence.Because of the delayed clearance of viral RNA in the glucocorticoid treatment group,glucocorticoids are not recommended in the treatment of COVID-19,especially for mild disease.The duration of RNA detection may relate to host cell immunity. | Yun Ling Shui-Bao Xu Yi-Xiao Lin Di Tian Zhao-Qin Zhu Fa-Hui Dai Fan Wu Zhi-Gang Song Wei Huang Jun Chen Bi-Jie Hu Sheng Wang En-Qiang Mao Lei Zhu Wen-Hong Zhang Hong-Zhou Lu | 2020 | Chinese Medical Journal2020,,9: | 58 |

| 8 | Electroacupuncture regulates glucose-inhibited neurons in treatment of simple obesity显示文摘The glucose-inhibited neurons present in the lateral hypothalamic area are regarded as glucose detectors. This structure is involved in the regulation of food intake through extracellular blood glucose concentrations, and plays a crucial role in obesity onset. In the present study, obesity models established with high fat feeding were treated with electroacupuncture at Zusanli (ST36)/ Inner Court (ST44) on the left side and Tianshu (ST25) bilaterally. We found that electroacupuncture could effectively reduce body weight and the fat-weight ratio, and decrease serum leptin, resistin, tumor necrosis factor alpha, and neuropeptide Y levels, while increase serum adiponectin and cholecystokinin-8 levels. This treatment altered the electrical activity of glucose-inhibited neurons in the lateral hypothalamic area, with electroacupuncture at Zusanli/ Inner Court exerting an inhibitory effect, while electroacupuncture at bilateral Tianshu exerting an excitatory effect. | 10 | Large-scale real-world radio signal recognition with deep learning显示文摘In the past ten years, many high-quality datasets have been released to support the rapid development of deep learning in the fields of computer vision, voice, and natural language processing. Nowadays, deep learning has become a key research component of the Sixth-Generation wireless systems(6G) with numerous regulatory and defense applications. In order to facilitate the application of deep learning in radio signal recognition, in this work, a large-scale real-world radio signal dataset is created based on a special aeronautical monitoring system-Automatic Dependent Surveillance-Broadcast(ADS-B). This paper makes two main contributions. First, an automatic data collection and labeling system is designed to capture over-the-air ADS-B signals in the open and real-world scenario without human participation. Through data cleaning and sorting, a high-quality dataset of ADS-B signals is created for radio signal recognition. | 11 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. | 12 | Comparison of persistent submacular fluid in different preoperative macular status after vitrectomy for rhegmatogenous retinal detachment显示文摘AIM: To compare the incidence of persistent submacular fluid(SMF) and visual outcome after pars plana vitrectomy(PPV) for rhegmatogenous retinal detachment(RRD) in different preoperative macular status according to optical coherence tomography(OCT).METHODS: A non-randomized, retrospective review was performed for patients who underwent successful PPV for RRD. OCT exams were taken preoperatively and 1 mo after surgery, until SMF disappeared. According to the preoperative macular status on OCT, patients were divided into two groups: macula-off RRD(Group A) and maculaon RRD(Group B). In Group A, there were two subgroups: macula partly detached(Group A1) and macula totally detached(Group A2). The main outcome measures were the presence of SMF on OCT 1 mo after surgery, and the preoperative and postoperative best corrected visual acuities(BCVA), among the different groups and depending on the presence or absence of persistent SMF.RESULTS: A total of 139 eyes of 139 patients were included in the study. Persistent SMF at 1 mo after surgery was 15.8%(22/139), all occurring in Group A(22/101); Group B had no SMF at 1 mo after surgery(0/38, P=0.002). The incidence of persistent SMF at 1 mo after surgery in Group A1 was 50%(14/28), and in Group A2 was 11.0%(8/73, P<0.001). Significant differences were shown between the presence and absence of persistent SMF on foveolaoff RRD, the preoperative BCVA, the 1 mo postoperative BCVA, and the degree of the BCVA improvement from 1 mo postoperatively to the final follow-up(P<0.05). However, there were no significant differences in the final BCVA(P>0.05).CONCLUSION: Persistent SMF after PPV for retinal detachment is associated with preoperative macular status. Macula-uninvolving RRD shows no persistent SMF after PPV. Macular partly detached RRD has a higher incidence of SMF than macula totally detached RRD after PPV. The experiments show that when stirred for 2 minutes under the test condition, the semi-solid slurry with 50%-60% fraction solid phase and spherical primary austenite in the size of 100-300μm can be obtained and is easy to be discharged from the little bottom hole of the stirring chamber. The more homogeneous temperature field and solute distribution of the 60Si2Mn spring steel melt appear because of the electromagnetic stirring, which restrains the formation of large primary austenitic dendrites and create a base to form spheroidized structure. The stronger temperature fluctuation in the melt with many rosette primary austenites caused by stirring and the remelting of the secondary arm roots in the same time are the most important reasons for deposition of spheroidized primary austenites. | W.M. The experimental results show that when stirred for 2-3 min under the given test condition, the semi-solid slurry with about 50%-60% (volume fraction) solid and the spherical primary austenitic grains in the size of 100-200 μm can be obtained, and it is easy to be discharged from the bottom little hole of the stirring chamber. The semi-solid slurry of 1Crl8Ni9Ti stainless steel can be rolled into the given plate successfully.However, the solid phase and liquid phase are easily separated in the rolling process, so that the solid primary austenitic grains are concentrated in the center and the liquid phase is near the edge of the rolled plate. | 17 | Effect of intravitreal conbercept treatment before vitrectomy in proliferative diabetic retinopathy显示文摘● AIM: To evaluate the safety and efficacy of intravitreal conbercept(IVC) injections as pretreatment for pars plana vitrectomy(PPV) in severe proliferative diabetic retinopathy(PDR).● METHODS: This was a retrospective chart review of all patients who underwent PPV for PDR from January 2014 to October 2016. Patients who underwent IVC injection before PPV were assigned to the IVC group; the others were assigned to the control group. The IVC was performed 3-7 d before surgery in the IVC group. All the eyes in the two groups were operated by the same doctor to complete the vitrectomy. Intraoperative complications and the changes in best-corrected visual acuity(BCVA) before and after surgery were compared between the two groups.● RESULTS: A total of 68 eyes of 63 patients(22 eyes in the IVC group and 46 eyes in the control group) were examined. The risk of intraoperative bleeding was lower in the IVC group(2/22) than in the control group(25/46, P=0.000). Furthermore, the use of endodiathermy was significantly lower in the IVC group(1/22) than in the control group(12/46, P=0.047). The surgical time in the IVC group(112.64±34.52 min) was significantly shorter than in the control group(132.85±40.04 min, P<0.05). Compared to the BCVA before surgery, the mean BCVA was significantly improved after surgery for both groups(P<0.05).● CONCLUSION: PPV is an effective treatment and can improve vision in patients with PDR. | 20 | Genetic and Antibiotic Resistance Characteristics ofCampylobacterjejuni Isolated from Diarrheal Patients,Poultry and Cattle in Shenzhen显示文摘Objective To investigate genetic and antibiotic resistance characteristics of Campylobacter jejuni(C. jejuni) isolated from Shenzhen. Methods Multilocs sequence typing and agar dilution methods were used to define the genotype and antibiotic resistance of C. jejuni, respectively. Results In total, 126 C. jejuni strains were isolated. The prevalence of C. jejuni was 5.3% in diarrheal patients. The prevalence in poultry meat(36.5%) was higher than that in cattle meat(1.1%). However, the prevalence in poultry cloacal swabs(27.0%) was lower than that in cattle stool(57.3%). Sixty-two sequence types were obtained, among which 27 of the STs and 10 alleles were previously unreported. The most frequently observed clonal complexes were ST-21(11.9%), ST-22(10.3%), and ST-403(7.1%). ST-21, ST-45, ST-354, ST-403, and ST-443 complexes overlapped between isolates from patients and cattle, whereas ST-45 and ST-574 complexes overlapped between isolates from patients and poultry. All C. jejuni were resistant to at least one antibiotic. The highest resistance rate was toward ciprofloxacin(89.7%), followed by tetracycline(74.6%), and nalidixic acid(69.0%). Conclusion This is the first report of the genotypes and antibiotic resistance of C. jejuni in Shenzhen.
