| 1 | Osteoarticular manifestations of human brucellosis:A review显示文摘Brucellosis is a common global zoonotic disease, which is responsible for a range of clinical manifestations. Fever, sweating and musculoskeletal pains are observed in most patients. The most frequent complication of brucellosis is osteoarticular involvement, with 10% to 85% of patients affected. The sacroiliac(up to 80%) and spinal joints(up to 54%) are the most common affected sites.Spondylitis and spondylodiscitis are the most frequent complications of brucellar spinal involvement. Peripheral arthritis, osteomyelitis, discitis, bursitis and tenosynovitis are other osteoarticular manifestations, but with a lower prevalence. Spinal brucellosis has two forms: focal and diffuse. Epidural abscess is a rare complication of spinal brucellosis but can lead to permanent neurological deficits or even death if not treated promptly. Spondylodiscitis is the most severe form of osteoarticular involvement by brucellosis, and can have single-or multifocal involvement. Early and appropriate diagnosis and treatment of the disease is important in order to have a successful management of the patients with osteoarticular brucellosis. Brucellosis should be considered as a differential diagnosis for sciatic and back pain, especially in endemic regions. Patients with septic arthritis living in endemic areas also need to be evaluated in terms of brucellosis. Physical examination, laboratory tests and imaging techniques are needed to diagnose the disease. Radiography, computed tomography, magnetic resonance imaging(MRI) and bone scintigraphy are imaging techniques for the diagnosis of osteoarticular brucellosis. MRI is helpful to differentiate between pyogenic spondylitis and brucellar spondylitis. | 2 | Anterior muscle sparing approach for total hip arthroplasty显示文摘The purpose of this review is to examine the validity of positive claims regarding the direct anterior approach(DAA) with a fracture table for total hip arthroplasty. Recent literature regarding the DAA was searched and specific claims investigated including improved early outcomes, speed of recovery, component placement, dislocation rates, and complication rates. Recent literature is positive regarding the effects of total hip arthroplasty with the anterior approach. While the data is not definitive at present, patients receiving the anterior approach for total hip arthroplasty tend to recover more quickly and have improved early outcomes. Component placement with the anterior approach is more often in the 'safe zone' than with other approaches. Dislocation rates tend to be less than 1% with the anterior approach. Complication rates vary widely in the published literature. A possible explanation is that the varianceis due to surgeon and institutional experience with the anterior approach procedure. Concerns remain regarding the 'learning curve' for both surgeons and institutions. Mobile Edge Computing (MEC), which enables mobile terminals to offload computation tasks to servers located at the edge of the cellular networks, has been considered as an efficient approach to relieve the heavy computational burdens and realize an efficient computation offloading. Driven by the consequent requirement for proper resource allocations for computation offloading via MEC, in this paper, we propose a Deep-Q Network (DQN) based task offloading and resource allocation algorithm for the MEC. Specifically, we consider a MEC system in which every mobile terminal has multiple tasks offloaded to the edge server and design a joint task offloading decision and bandwidth allocation optimization to minimize the overall offloading cost in terms of energy cost, computation cost, and delay cost. Although the proposed optimization problem is a mixed integer nonlinear programming in nature, we exploit an emerging DQN technique to solve it. In this study, the process, weld combinations, laser-CMT hybrid welding and applications of CMT welding are critically reviewed. Microstructure and other weld characteristics have been discussed at length for various base metal combinations. Particularly, the welding of aluminium and steel with better results has been possible with CMT Welding. The results reviewed in this article indicate that the CMT-Laser hybrid welding is more preferable to Laser or Laser hybrid welding. | 8 | Deformation and failure mechanism of slope in three dimensions显示文摘Understanding three-dimensional(3D)slope deformation and failure mechanism and corresponding stability analyses are crucially important issues in geotechnical engineering.In this paper,the mechanisms of progressive failure with thrust-type and pull-type landslides are described in detail.It is considered that the post-failure stress state and the pre-peak stress state may occur at different regions of a landslide body with deformation development,and a critical stress state element(or the soil slice block)exists between the post-failure stress state and the pre-peak stress state regions.In this regard,two sorts of failure modes are suggested for the thrust-type and three sorts for pull-type landslides,based on the characteristics of shear stress and strain(or tensile stress and strain).Accordingly,a new joint constitutive model(JCM)is proposed based on the current stability analytical theories,and it can be used to describe the mechanical behaviors of geo-materials with softening properties.Five methods,i.e.CSRM(comprehensive sliding resistance method),MTM(main thrust method),CDM(comprehensive displacement method),SDM(surplus displacement method),and MPM(main pull method),for slope stability calculation are proposed.The S-shaped curve of monitored displacement vs.time is presented for different points on the sliding surface during progressive failure process of landslide,and the relationship between the displacement of different points on the sliding surface and height of landslide body is regarded as the parabolic curve.The comparisons between the predicted and observed load e displacement and displacement e time relations of the points on the sliding surface are conducted.The classi fi cation of stable/unstable displacement e time curves is proposed.The de fi nition of the main sliding direction of a landslide is also suggested in such a way that the failure body of landslide(simpli fi ed as'collapse body')is only involved in the main sliding direction,and the strike and the dip are the same as the collapse body.The rake angle is taken as the direction of the sum of sliding forces or the sum of displacements in collapse body,in which the main slip direction is dependent on progressive deformation.The reason of non-convergence with fi nite element method(FEM)in calculating the stability of slope is also numerically analyzed,in which a new method considering the slip surface associated with the boundary condition is proposed.It is known that the boundary condition of sliding surface can be described by perfect elasto-plastic model(PEPM)and JCM,and that the stress and strain of a landslide can be described properly with the JCM. | Yingfa Lu | 2015 | Journal of Rock Mechanics and Geotechnical Engineering2015,7,2: | 17 |

| 12 | Experimental study and stress analysis of rock bolt anchorage performance显示文摘A new method was developed to apply pull-and-shear loads to the bolt specimen in order to evaluate the anchorage performance of the rebar bolt and the D-Bolt.In the tests,five displacing angles(0,20,40,60,and 90),two joint gaps(0 mm and 30 mm),and three kinds of host rock materials(weak concrete,strong concrete,and concrete-granite) were considered,and stressestrain measurements were conducted.Results show that the ultimate loads of both the D-Bolt and the rebar bolt remained constant with any displacing angles.The ultimate displacement of the D-Bolt changed from 140 mm at the0 displacing angle(pure pull) to approximately 70 mm at a displacing angle greater than 40.The displacement capacity of the D-Bolt is approximately 3.5 times that of the rebar bolt under pure pull and50% higher than that of the rebar bolt under pure shear.The compressive stress exists at 50 mm from the bolt head,and the maximum bending moment value rises with the increasing displacing angle.The rebar bolt mobilises greater applied load than the D-Bolt when subjected to the maximum bending.The yielding length(at 0) of the D-Bolt is longer than that of the rebar bolt.The displacement capacity of the bolts increased with the joint gap.The bolt subjected to joint gap effect yields more quickly with greater bending moment and smaller applied load.The displacement capacities of the D-Bolt and the rebar bolt are greater in the weak host rock than that in the hard host rock.In pure shear condition,the ultimate load of the bolts slightly decreases in the hard rock.The yielding speed in the hard rock is higher than that in the weak rock. | Yu Chen | 2014 | Journal of Rock Mechanics and Geotechnical Engineering2014,6,5: | 13 |

In order to improve the spectrum efficiency with low complexity, we propose a joint hybrid precoding algorithm for single-user mmWave MIMO systems in this paper. By using the concept of equivalent channel, the proposed algorithm skillfully utilizes the idea of alternating optimization to complete the design of RF precoder and combiner. Then, the baseband precoder and combiner are computed by calculating the singular value decomposition of the equivalent channel. Simulation results demonstrate that the proposed algorithm can achieve satisfactory performance with quite low complexity. | 15 | Treatment of acute periprosthetic infections with prosthesis retention: Review of current concepts显示文摘Periprosthetic joint infection(PJI) is a devastating complication after total joint arthroplasty, occurring in approximately 1%-2% of all cases. With growing populations and increasing age, PJI will have a growing effect on health care costs. Many risk factors have been identified that increase the risk of developing PJI, including obesity, immune system deficiencies, malignancy, previous surgery of the same joint and longer operating time. Acute PJI occurs either postoperatively(4 wk to 3 mo after initial arthroplasty, depending on the classification system), or via hematogenous spreading after a period in which the prosthesis had functioned properly. Diagnosis and the choice of treatment are the cornerstones to success. Although different definitions for PJI have been used in the past, most are more or less similar and include the presence of a sinus tract, blood infection values, synovial white blood cell count, signs of infection on histopathological analysis and one ormore positive culture results. Debridement, antibiotics and implant retention(DAIR) is the primary treatment for acute PJI, and should be performed as soon as possible after the development of symptoms. Success rates differ, but most studies report success rates of around 60%-80%. Whether single or multiple debridement procedures are more successful remains unclear. The use of local antibiotics in addition to the administration of systemic antibiotic agents is also subject to debate, and its pro's and con's should be carefully considered. Systemic treatment, based on culture results, is of importance for all PJI treatments. Additionally, rifampin should be given in Staphylococcal PJIs, unless all foreign material is removed. The most important factors contributing to treatment failure are longer duration of symptoms, a longer time after initial arthroplasty, the need for more debridement procedures, the retention of exchangeable components, and PJI caused by Staphylococcus(aureus or coagulase negative). If DAIR treatment is unsuccessful, the following treatment option should be based on the patient health status and his or her expectations. For the best functional outcome, one- or two-stage revision should be performed after DAIR failure. Because of increasing participation of people from any age group of the population in sports activities, health care professionals dealing with the care of trauma patients must have a thorough understanding of the anatomy, patho-physiology, risk factors, and management of anterior shoulder instability. The risk factors for recurrent shoulder dislocation are young age, participation in high demand contact sports activities, presence of Hill-Sachs or osseous Bankart lesion, previous history of ipsilateral traumatic dislocation, ipsilateral rotator cuff or deltoid muscle insufficiency, and underlying ligamentous laxity. Achieving the best result for any particular patient depends onthe procedure that allows observation of the joint surfaces, provides the anatomical repair, maintains range of motion, and also can be applied with low rates of complications and recurrence.
