1Homotopic mapping solving method for gain fluency of a laser pulse amplifier显示文摘The model for gain fluency of a laser pulse amplifier is studied. Using the homotopic mapping method, firstly, the system of the original model is packed up standardization; secondly, introducing a homotopic mapping, taking the property of the mapping, inducing a contrived parameter, the solving of a nonlinear problem translates into the solving of a linear problem. Then the approximate expressions of the solution for the corresponding model are obtained. And the precision for the approximate solution is compared. It illuminates that the obtained approximate solution using the homotopic mapping method possesses higher approximate degree. At one time, the expansion of solution through the homotopic mapping method can be kept in the analytic operation. 3Using lightning locating system based on time-of-arrival technique to study three-dimensional lightning discharge processes显示文摘A time-of-arrival(TOA) system based on GPS technology for locating VHF radiation sources from lightning has been developed and used in observation sites in the northern Shandong Province,China.The 3D images of the lightning progression have been obtained successfully for the first time in China.The 3D-channel evolutions of typical negative CG,positive CG and IC lightning flashes have been discussed together with the data of fast electric field change.It was found that significant differences existed between the negative and positive CG lightning flashes in terms of the initiation and propagation of the radiation sources.The preliminary breakdown of a negative CG lightning flash propagated at a speed about 5.2×104 m/s.The stepped leader of negative CG lightning flashes was trigged by negative initial breakdown.Thereafter,it propagated downward at a speed of 1.3×105 m/s.The initial process of the positive CG lightning flashes was also a propagation process of negative streamer.These streamers propagated dominantly horizontally in the positive charge region and accumulated positive charges at the origin of the lightning,and as a consequence,initiated downward positive streamers.A new type of lightning discharge that was triggered by a narrow bipolar pulse(NBP) is discussed in this study.The NBP was originated at altitude of about 10.5 km in the upper positive charge region.As a distinct difference from normal IC flash,its channels extended horizontally all around and produced a lot of radiation sources.The source power of the NBP could approach 16.7 kW,which is much greater than that of normal lightning discharge ranging between 100 mW and 500 W.The 3D propagation of this new type of lightning discharge was observed and obtained for the first time in China.The possible initiation mechanism of this new type of light-ning is discussed here.ZHANG GuangShu1,WANG YanHui1,QIE XiuShu2,ZHANG Tong1,ZHAO YuXiang1,3,LI YaJun1 & CAO DongJie11 Laboratory for Climate Environment and Disasters of Western China,Cold and Arid Regions Environmental and Engineering Research Institute,Chinese Academy of Sciences,Lanzhou 730000,China 2 Laboratory for Middle Atmosphere and Global Environment Observation(LAGEO),Institute of Atmospheric Physics,Chinese Academy of Sciences,Beijing 100029,China 3 Department of Physics,Tianshui Normal College,Tianshui 741001,China 2010Science China Earth Sciences2010,53,4:21

6Management hepatolithiasis with operative choledochoscopic FREDDY laser lithotripsy combined with or without hepatectomy显示文摘BACKGROUND: Hepatolithiasis is very common in East Asia. It is benign in nature, but has a high recurrence rate. It is likely to lead to biliary cirrhosis and increase the risk of cholangiocarcinoma. Hence, the treatment of hepatolithiasis is difficult but vital. In this report, we present a novel approach to manage hepatolithiasis using the choledochoscopic Frequency-Doubled Double pulse Nd:YAG (FREDDY) laser lithotripsy combined with or without hepatectomy. METHODS: Between July 2009 and October 2012, 45 patients underwent choledochoscopic FREDDY laser lithotripsy combined with or without hepatectomy (laser lithotripsy group). Fortyeight patients underwent a traditional operation (traditional method group) from January 2009 to June 2009. Comparative analysis was made of demographic and clinical characteristics of the two groups. RESULTS: The final stone clearance rate of the laser lithotripsy group was 93.3%, whereas that of the traditional method group was 85.4% (P=0.22). In the laser lithotripsy group, 2 patients experienced hemobilia and 3 patients had acute cholangitis. In the traditional method group, 3 patients had intraoperative hemorrhage, 1 patient had bile leakage, 6 patients had acute cholangitis, and 1 patient died of liver failure. Moreover, the operative time in the traditional method group was significantly longer than that in the laser lithotripsy group (P=0.01). The mean hospital stay of the patients in the traditional method group was longer than that in the laser lithotripsy group (9.8 vs8.2 days, P=0.17). Recurrent intrahepatic bile duct stones were not found during the follow-up period in the two groups. 7Inflammatory pseudotumor of the liver and spleen diagnosed by percutaneous needle biopsy显示文摘An inflammatory pseudotumor (IPT) is a relatively rare lesion characterized by chronic inf iltration of inflammatory cells and areas of f ibrosis. IPTs are diff icult to diagnose because of the absence of specif ic symptoms or of characteristic hematological or radiological f indings. In this study, a case of a woman aged over 70 years was reported, who presented with a general malaise lasting more than two months. A computed tomography scan demonstrated a diffusely spread lesion of the liver with a portal vein occlusion and a splenic lesion surrounded by a soft density layer. Since the percutaneous liver biopsy showed f indings that suggested an IPT, although the radiological f indings did not exclude the possibility of a malignancy, we performed a percutaneous spleen biopsy to enable a more defi nitive diagnosis. The microscopic f indings from the spleen specimen lead us to a diagnosis of IPT involving the liver and spleen. 8Sorting radar signal from symmetry clustering perspective显示文摘The main function of electronic support measure system is to detect threating signals in order to take countermeasures against them. To accomplish this objective, a process of associating each interleaved pulse with its emitter must be done. This process is termed sorting or de-interleaving. A novel point symmetry based radar sorting(PSBRS) algorithm is addressed. In order to deal with all kinds of radar signals, the symmetry measure distance is used to cluster pulses instead of the conventional Euclidean distance. The reference points of the symmetrical clusters are initialized by the alternative fuzzy c-means(AFCM) algorithm to ameliorate the effects of noise and the false sorting. Besides,the density filtering(DF) algorithm is proposed to discard the noise pulses or clutter. The performance of the algorithm is evaluated under the effects of noise and missing pulses. 9Effect of Shen-Fu Injection(参附注射液)on Hemodynamics in Early Volume Resuscitation Treated Septic Shock Patients显示文摘Objective: To investigate the hemodynamic effect of Shen-Fu Injection(参附注射液, SFI) in early volume resuscitation treated septic shock patients by monitoring pulse indicator continuous cardiac output(PICCO). Methods: All septic shock patients admitted in the Intensive Care Unit of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 1 st, 2014 to December 31 th, 2015, were reviewed, and totally 65 were enrolled in this study. They were assigned to SFI group(33 cases) and control group(32 cases). All 65 patients underwent conventional treatment mainly including volume resuscitation, antibiotics and vasoactive drugs therapy. The patients of the SFI group received additional 100 mL of SFI intravenously every 12 h. In all 65 patients, the PICCO arterial catheter and vein catheter were implanted within 1 h after the diagnosis of septic shock. In the course of early volume resuscitation, hemodynamic data of patients were recorded by PICCO monitor at 0, 12, and 24 h after the catheter implantation. Results: The hemodynamic indices of the two groups showed no significant differences at the beginning of 0 h(P>0.05). At 12 and 24 h, the hemodynamic indices of SFI group were significantly improved in comparison with the control group(P<0.05), including cardiac index(CI), global end diastolic volume index(GEDI), mean arterial pressure(MAP) and heart rate(HR). In addition, there was no significant change of extra-vascular lung water index between the two groups(P>0.05). 13Experimental characteristics of a two-electrode plasma synthetic jet actuator array in serial显示文摘Plasma Synthetic Jet(PSJ) actuators have shown wide and promising application prospects in high-speed flow control, due to their advantages including high exhaust speed, wide frequency band, rapid response, and non-moving components. Although previous studies on PSJ actuators are abundant, most of them have focused on the performance of a single actuator. However, in practice, an actuator array is very necessary for large-scale aerodynamic actuation on account of the small affected area of a single PSJ. In this paper, the characteristics of a twoelectrode plasma synthetic jet actuator array in serial are investigated experimentally. Compared to a parallel actuator array, the serial actuator array requires simpler power supply design and is much easier to realize. High-speed photography of the discharge evolution, voltage-current measurement, and shadowgraphy visualization are used in the investigation. Experimental results show that, for the serial actuator array, weak discharges happen firstly between energized and suspending electrodes, and then a strong pulse arc discharge is triggered. The breakdown voltage in serial is irrelevant to such factors as the number of actuators, the maximum or minimum gap in serial,the connection sequence, etc. It is mainly determined by the sum of gaps. For serial actuators with the same anode-to-cathode spacing, the energy deposition is the same, and the jet is synchronous and similar. Because of the entrainment and merging of adjacent jet vortices, the jet front speed of an aligned synchronous jet array increases as the orifice distance decreases. 16Application of pulse index continuous cardiac output system in elderly patients with acute myocardial infarction complicated by cardiogenic shock: A prospective randomized study显示文摘BACKGROUND Cardiogenic shock (CS) secondary to acute myocardial infarction (AMI) complicates management of the condition, and often leads to poor prognosis. Prompt and accurate monitoring of cardiovascular and accompanying hemodynamic changes is crucial in achieving adequate management of the condition. Advances in technology has availed procedures such as pulse index continuous cardiac output (PiCCO), which can offer precise monitoring of cardiovascular functions and hemodynamic parameters. In this study, PiCCO is evaluated for its potential utility in improving management and clinical outcomes among elderly patients with AMI complicated by CS. AIM To assess whether use of the PiCCO system can improve clinical outcomes in elderly patients with AMI complicated by CS.METHODS Patients from emergency intensive care units (EICU) or coronary care units (CCU) were randomized to receive PiCCO monitoring or not. The APACHE II score, SOFA score, hs-TnI, NT-proBNP, PaO2/FiO2 ratio and lactate levels on day 1, 3 and 7 after treatment were compared. The infusion and urine volume at 0-24 h, 24-48 h and 48-72 h were recorded, as were the cardiac index (CI), extravascular lung water index (EVLWI), intrathoracic blood volume index (ITBVI) and global end diastolic volume index (GEDVI) at similar time intervals. RESULTS Sixty patients with AMI complicated by CS were included in the study. The PiCCO group had a significantly lower APACHE II score, SOFA score, hs-TnI and NT-proBNP levels on day 1, 3 and 7 after treatment. The infusion and urine volume during 0-24 h in the PiCCO group were significantly greater, and this group also showed significantly higher ADL scores. Furthermore, the PiCCO group spent lesser days on vasoactive agents, mechanical ventilation, and had a reduced length of stay in EICU/CCU. Additionally, the CI was significantly higher at 48 h and 72 h in the PiCCO group compared with that at 24 h, and the EVLWI, ITBVI and GEDVI were significantly decreased at 48 h and 72 h. 17Analysis of Pulse Signals Based on Array Pulse Volume显示文摘Objective: To collect and analyze multi-dimensional pulse diagram features with the array sensor of a pressure profile system(PPS) and study the characteristic parameters of the new multi-dimensional pulse diagram by pulse diagram analysis technology. Methods: The pulse signals at the Guan position of left wrist were acquired from 105 volunteers at the Shanghai University of Traditional Chinese Medicine. We obtained the pulse data using an array sensor with 3×4 channels. Three dimensional pulse diagrams were constructed for the validated pulse data, and the array pulse volume(APV) parameter was computed by a linear interpolation algorithm. The APV differences among normal pulse(NP), wiry pulse(WP) and slippery pulse(SP) were analyzed using one-way analysis of variance. The coefficients of variation(CV) were calculated for WP, SP and NP. Results: The APV difference between WP and NP in the 105 volunteers was statistically significant(6.26±0.28 vs. 6.04±0.36, P=0.048), as well as the difference between WP and SP(6.26±0.28 vs. 6.07±0.46, P=0.049). However, no statistically significant difference was found between NP and SP(P=0.75). WP showed a similar CV(4.47%) to those of NP(5.96%) and SP(7.58%). Conclusion: The new parameter APV could differentiate between NP or SP and WP. 18Comparison of perceived pain and patients’ satisfaction with traditional local anesthesia and single tooth anesthesia: A randomized clinical trial显示文摘BACKGROUND Contemporary innovations in the area of local anesthesia have attempted to provide an absolutely pain free experience for patients.Since the introduction of Computer-Controlled Local Anesthetic Delivery Systems to dentistry,many studies have compared its efficacy and safety to conventional anesthesia.However,very few studies have compared single tooth anesthesia(STA)and traditional local anesthesia.AIM To compare pain rating,changes in blood pressure,and heart rate during the local anesthetic injection.The secondary objectives were to measure the patients’level of satisfaction and the differences in anesthetic efficiency between the STA system and traditional local infiltration.METHODS A randomized controlled trial was conducted and a total of 80 patients with dental restorative needs were enrolled for the study.The patients were evaluated for their general physical status and oral clinical findings before enrollment.Information regarding perceived pain,changes in heart rate and blood pressure,and patients’satisfaction was collected using an electronic data form and was analyzed using paired and unpaired t-tests.RESULTS No significant difference was noted in perceived pain(P=0.59)and systolic blood pressure(P=0.09)during anesthetic injection using both traditional and STA techniques.STA patients had a significantly higher heart rate during anesthesia,although a statistically significant difference was noted among the traditional anesthesia and the STA groups even before anesthesia.During the restorative procedure,less pain was perceived by STA patients on the Wong-Baker FACES pain scale,which was statistically significant(P<0.001).Analyses of post-procedure patient responses showed that STA patients had a significantly better treatment experience and preferred to have the same method of injection in the future(P=0.04).CONCLUSION STA system can provide less painful and more comfortable restorative treatment procedures in comparison to the traditional infiltration technique.Mohammad I Al-Obaida Mehdiya Haider Rawan Hashim Wafa AlGheriri Sree Lalita Celur Samar A Al-Saleh Ebtissam M Al-Madi 2019World Journal of Clinical Cases2019,7,19:8
