| 1 | Breast cancer-related lymphedema: Symptoms, diagnosis, risk reduction, and management显示文摘The global burden of breast cancer continues to increase largely because of the aging and growth of the world population. More than 1.38 million women worldwide were estimated to be diagnosed with breast cancer in 2008, accounting for 23% of all diagnosed cancers in women. Given that the 5-year survival rate for breast cancer is now 90%, experiencing breast cancer is ultimately about quality of life. Women treated for breast cancer are facing a life-time risk of developing lymphedema, a chronic condition that occurs in up to 40% of this population and negatively affects breast cancer survivors' quality of life. | 2 | Prevention of hepatocellular carcinoma in patients with chronic hepatitis B显示文摘Patients with chronic hepatitis B are at significant risk for hepatocellular carcinoma(HCC). Globally,over half a million people each year are diagnosed with HCC,with marked geographical variations. Despite overwhelming evidence for a causal role of hepatitis B virus(HBV) infection in the development of HCC and a well-established relationship between high baseline hepatitis B viral load and cumulative risk of HCC,the molecular basis for this association has not been fully elucidated. In addition,a beneficial role for antiviral therapy in preventing the development of HCC has been difficult to establish. This review examines the biological and molecular mechanisms of HBV-related hepatocarcinogenesis,recent results on the effect of modern nucleos(t)ides on the rate of HCC development in high risk HBV cohorts and the potential mechanisms by which long-term antiviral therapy with potent inhibitors of HBV replication might reduce the risk of HCC in patients with chronic hepatitis B. | 3 | An Analysis of Social Vulnerability to Natural Hazards in Nepal Using a Modified Social Vulnerability Index显示文摘Social vulnerability influences the ability to prepare for, respond to, and recover from disasters. The identification of vulnerable populations and factors that contribute to their vulnerability are crucial for effective disaster risk reduction. Nepal exhibits multihazard risk and has experienced socioeconomic and political upheaval in recent decades, further increasing susceptibility to hazards.However, we still know little regarding social vulnerability in Nepal. Here, we investigate social vulnerability in Nepal by adapting Social Vulnerability Index(SoVI) methods to the Nepali context. Variables such as caste, and populations who cannot speak/understand Nepali were added to reflect the essence of the Nepali context. Using principal component analysis, 39 variables were reduced to seven factors that explained 63.02% of variance in the data.Factor scores were summarized to calculate final SoVI scores. The highest levels of social vulnerability are concentrated in the central and western Mountain, western Hill, and central and eastern Tarai regions of Nepal, while the least vulnerable areas are in the central and eastern Hill regions. | 4 | Towards Quantitatively Understanding the Complexity of Social-Ecological Systems——From Connection to Consilience显示文摘The complexity of social-ecological systems(SES) is rooted in the outcomes of node activities connected by network topology. Thus far, in network dynamics research, the connectivity degree(CND), indicating how many nodes are connected to a given node, has been the dominant concept. However, connectivity focuses only on network topology, neglecting the crucial relation to node activities, and thereby leaving system outcomes largely unexplained. Inspired by the phenomenon of ‘‘consensus of wills and coordination of activities' ' often observed in disaster risk management, we propose a new concept of network characteristic, the consilience degree(CSD),aiming to measure the way in which network topology and node activities together contribute to system outcomes. The CSD captures the fact that nodes may assume different states that make their activities more or less compatible.Connecting two nodes with in/compatible states will lead to outcomes that are un/desirable from the perspective of the SES in question. We mathematically prove that the CSD is a generalized CND, and the CND is a special case of CSD. As a general, fundamental concept, the CSD can facilitate the development of a new framework of network properties, models, and theories that allows us to understand patterns of network behavior that cannot be explained in terms of connectivity alone. We further demonstrate that a co-evolutionary mechanism can naturally improve the CSD. | 5 | The 4th World Landslide Forum: Landslide Research and Risk Reduction for Advancing the Culture of Living with Natural Hazards显示文摘The World Landslide Forum is a triennial mainstream conference that gathers together the scientific and technological community, policymakers, industry actors, public officials, and other stakeholders, who deal with the understanding and management of landslide disaster risk. The establishment of the ISDR-ICL Sendai Partnerships 2015–2025 for Global Promotion of Understanding and Reducing Landslide Disaster Risk in Sendai during the 2015 World Conference on Disaster Risk Reduction(WCDRR) enabled the landslide science and technology community support the implementation of the Sendai Framework for Disaster Risk Reduction 2015–2030 in order to prevent new and reduce existing disaster risk.The 4th World Landslide Forum(WLF4) was held in Ljubljana, Slovenia, from 29 May to 2 June 2017 and discussed the progress of landslide research and risk reduction for advancing the culture of living with natural hazards. A high-level panel composed of United Nations officials, international stakeholders, and national organizations sought to identify the best mechanisms to be developed by the community of the International Consortium on Landslides(ICL). The objective was to advance the implementation of the ISDR-ICL Sendai Partnerships,to achieve a better commitment among partners, and to provide substantive services to developing countries. | 6 | Disaster Risk Reduction Including Climate Change Adaptation Over South Asia: Challenges and Ways Forward显示文摘South Asia is vulnerable to a variety of hydrometeorological hazards, which are often crossboundary in nature. Climate change is expected to influence many of these hazards. Thus, climate-related risks over South Asia make disaster risk reduction(DRR) and climate change adaptation(CCA) key policy goals.Recently there is an increasing consensus that DRR including CCA should be embedded in development planning. Disaster risk reduction including CCA has progressively gained importance in global governance. Across South Asia, however, such integration is only in a preliminary stage. This review was to assess the existing status and scope of DRR including CCA in development projects across South Asia, so that an effective and achievable deliberation may be made to regional policymakers. | 7 | Participatory Early Warning Systems: Youth, Citizen Science,and Intergenerational Dialogues on Disaster Risk Reduction in Brazil显示文摘Building national people-centered early warning systems(EWS) is strongly recommended by the United Nations International Strategy for Disaster Reduction(UNISDR). Most of the scientific literature is critical of the conventional view of EWS as a linear model with a topdown approach, in which technological features are given more attention than human factors. It is argued that EWS should be people-centered, and used for risk prevention,with an emphasis on resilience, rather than only being triggered when a hazard occurs. However, both the UNISDR and the literature fail to say how a people-centered EWS should be built, and what steps are needed to put EWS into effect. This article examines the obstacles and measures required to promote people-centered EWS,with a focus on the situation in Brazil. This article presents a study with 20 Pacific Island community leaders and connectors about their communities' perspectives and anticipated responses to natural hazards in Auckland, New Zealand. Home to the largest population of Pacific people in the world, Auckland provides the basis for understanding the complexities of delivering disaster information across numerous community groups. The rich cultural and linguistic backgrounds of multiple Pacific communities living in this city highlight the need to consider the complexities of disaster messaging related to natural hazards. In particular, the article forwards the importance of incorporating the guiding concepts of reach, relevance, receptiveness, and relationships into a DRR approach with culturally and linguistically diverse groups. With this aim, the #italiasicura web platform was developed by Fondazione Politecnico di Milano and released in 2015 to show country to local level hazard maps and risk reduction projects in Italy. Any stage of the user experience with the platform can be shared via social media. Using this tool, an awareness-oriented web analytics process was structured to develop a set of indicators for the increase of knowledge linked to flood and landslide hazards. The study shows that the formation and development of the down-faulted basin zone in Shanxi Province provides an available condition for preparation and occurrence of these hazards, so that the basin zone becomes an area with frequent occurrence of the hazards, such as earthquakes, floods and waterlogging, meteorological and geologic hazards and agrobiohazards in Shanxi and with their most serious interaction and mutual intensification. Moreover, the basin zone is an area with dense population and most concentrated industrial and agricultural productions and social-economic property in Shanxi. | 14 | Risk Perception and Knowledge in Fire Risk Reduction in a Dong Minority Rural Village in China: A Health-EDRM Education Intervention Study显示文摘Fire is one of the major disasters in rural communities but evidence of the effectiveness of education interventions against fire risks is limited. This was a 2-year study assessed the effectiveness of face-to-face Health Emergency and Disaster Risk Management(HealthEDRM) education interventions for raising fire risk reduction knowledge in a fire-prone rural ethnic minority community. The study was conducted in various pre-set time points of an intervention-based project in a Dongbased community in Nanjiang Village, Guizhou Province in 2015 and 2016 to increase knowledge among the villagers about how to reduce general-and electrical-fire risks. Pre-and post-intervention questionnaires were used to evaluate the effectiveness of increasing fire risk-related knowledge through these interventions, immediately after the 2015 and 2016 interventions, and 17 months after the 2015 intervention. The knowledge of using fire blanket,recalling the correct emergency telephone number,unplugging unused electrical appliances, and not using water to extinguish electrical fires had immediately improved after the interventions. Subjects demonstrated a better understanding that fire blankets can fight a blaze if used appropriately, and that knowledge was sustained for 17 months. The interventions were effective in improving fire prevention and response knowledge. | 15 | Long-Term and Immediate Impacts of Health Emergency and Disaster Risk Management(Health-EDRM) Education Interventions in a Rural Chinese Earthquake-Prone Transitional Village显示文摘Ma'an Qiao Village, a Dai and Yi ethnic minority-based community in Sichuan Province, China sustained complete infrastructure devastation during the 2008 Panzhihua earthquake. Health emergency and disaster risk management(Health-EDRM) education intervention programs were implemented in 2010 and 2011. This serial cross-sectional survey study aimed to examine the immediate and long-term impacts of the Health-EDRM interventions in this remote rural community. The findings demonstrate knowledge improvement in areas of water and sanitation, food and nutrition, and disaster preparedness immediately after the Health-EDRM education interventions. Temporal stability of knowledge retention was observed in household hygiene and waste management and smoking beliefs in 2018, 7 years after the interventions.Other important findings include knowledge uptake pattern differences of oral rehydration solution(ORS) between earthquake-prone and flood-prone communities. Usage of Internet and mobile technology for accessing disaster-related information was found to be independent of gender and income. Overall, this study demonstrated the knowledge improvement through Health-EDRM education interventions in a remote rural community. Promoting behavioral changes through interventions to raise awareness has the potential to reduce health risks in transitional post-disaster settings. | 17 | Advancing the Disaster and Development Paradigm显示文摘Consolidation of disaster and development studies as an integrated field of action research that influences policy has proved to be fundamental to global disaster risk reduction, sustainable development, climate change, and humanitarian agreements. However, challenges in achieving targets, such as those of the Sendai Framework for Disaster Risk Reduction 2015–2030 and the Sustainable Development Goals, requires further advances of the disaster and development paradigm underpinning these aspirations. This article presents perspectives that grew primarily from local action research, particularly research carried out with marginalized and highly at-risk groups of people in Southern Africa and South Asia.Analytical fronts from these findings emphasize disaster and development risk assessment opportunities that consolidate earlier ideas and extend understanding of disaster and development-related risk intervention options. These acknowledge severe shortcomings in disaster risk reduction progress while including greater use of hope as an active ingredient. This process of paradigm exploration remains fundamental to achieving disaster risk reduction, sustainable development, and associated policy objectives. The analysis presented here reiterates earlier groundings in people-centric perspectives, emphasizing social relations and systems of meaning as essential active ingredients for challenging power structures, technology, education, and human behavior. The analysis proposes some consequent thematic fronts for increased investment. These includeinvesting in early buildup of well-being before a disaster,better living with uncertainty, and overcoming the barriers to desired disaster and development outcomes.
