1Risk of transmission of carbapenem-resistant Enterobacteriaceae and related “superbugs” during gastrointestinal endoscopy显示文摘To evaluate the risk of transmission of carbapenem-resistant Enterobacteriaceae(CRE) and their related superbugs during gastrointestinal(GI) endoscopy. Reports of outbreaks linked to GI endoscopes contami-nated with different types of infectious agents, includ-ing CRE and their related superbugs, were reviewed. Published during the past 30 years, both prior to and since CRE's emergence, these reports were obtained by searching the peer-reviewed medical literature(via the United States National Library of Medicine's 'MEDLINE' database); the Food and Drug Administration's Manu-facturer and User Facility Device Experience database, or 'MAUDE'; and the Internet(via Google's search engine). This review focused on an outbreak of CRE in 2013 following the GI endoscopic procedure known as endoscopic retrograde cholangiopancreatography, or ERCP, performed at 'Hospital X' located in the sub-urbs of Chicago(IL; United States). Part of the largest outbreak of CRE in United States history, the infection and colonization of 10 and 28 of this hospital's patients, respectively, received considerable media attention and was also investigated by the Centers for Disease Con-trol and Prevention(CDC), which published a report about this outbreak in Morbidity and Mortality WeeklyReport(MMWR), in 2014. This report, along with the results of an independent inspection of Hospital X's in-fection control practices following this CRE outbreak, were also reviewed. While this article focuses primar-ily on the prevention of transmissions of CRE and their related superbugs in the GI endoscopic setting, some of its discussion and recommendations may also apply to other healthcare settings, to other types of flexible endoscopes, and to other types of transmissible infec-tious agents. This review found that GI endoscopy is an important risk factor for the transmission of CRE and their related superbugs, having been recently as-sociated with patient morbidity and mortality following ERCP. The CDC reported in MMWR that the type of GI endoscope, known as an ERCP endoscope, that Hospi-tal X used to perform ERCP in 2013 on the 38 patients who became infected or colonized with CRE might be particularly challenging to clean and disinfect, because of the complexity of its physical design. If performed in strict accordance with the endoscope manufacturer's labeling, supplemented as needed with professional organizations' published guidelines, however, current practices for reprocessing GI endoscopes, which include high-level disinfection, are reportedly adequate for the prevention of transmission of CRE and their related superbugs. Several recommendations are provided to prevent CRE transmissions in the healthcare setting. CRE transmissions are not limited to contaminated GI endoscopes and also have been linked to other reusable flexible endoscopic instrumentation, including broncho-scopes and cystoscopes. In conclusion, contaminated GI endoscopes, particularly those used during ERCP, have been causally linked to outbreaks of CRE and their related superbugs, with associated patient morbidity and mortality. Thorough reprocessing of these complex reusable instruments is necessary to prevent disease transmission and ensure patient safety during GI endos-copy. Enhanced training and monitoring of reprocessing staffers to verify the proper cleaning and brushing of GI endoscopes, especially the area around, behind andnear the forceps elevator located at the distal end othe ERCP endoscope, are recommended. 2H2 strain attenuated live hepatitis A vaccines:protective efficacy in a hepatitis A outbreak显示文摘AIM To investigate the protective efficacy ofH2 strain attenuated live hepatitis A vaccines(H2-strain vaccines)in hepatitis A(HA)outbreaks.METHODS With the permission of theirparents,5551 pre-school and grade 1-3 primaryschool children were inoculated with 1 dose(106.5TCID50)of H2-strain vaccines in anonrandomized,controlled trial conducted inFucheng County,Hebei Province in May 1997.Another 6485 children in the same grades andcompatible in gender and age were enrolled ascontrols.Epidemiological and serological surveywas conducted to evaluate the protectiveefficacy of the vaccines.ELISA was used todetect serum IgM anti-HAV.RESULTS HA outbreak started in early May1998,peaked in the middle of the same month,and lasted about 80 days.Overall 302 HA caseswere found,192(53.58%)were 5-9 years old.One vaccinee and 25 control cases were found tohave hepatitis A,which account for 0.28%(1/356)and 5.92%(25/422)of all vaccinees andcontrols in the 14 villages,respectively.The protective efficacy of vaccines was 95.27%(95% Cl:85.83%-104.72%).In subjects testedfor anti-HAV IgM from 13 villages,1(0.40%)overt and 11(4.06%)asymptomatic HAV caseswere found in 271 vaccinees,but 21(6.69%)ofovert and asymptomatic ones were found in 314controls.CONCLUSION H2 strain vaccines were excellent in preventing overt hepatitis A, but not so effective in preventing asymptomatic hepatitis A virus infection. 3Relationship between El Nino /South Oscillation(ENSO) and population outbreaks of some lemmings and voles in Europe显示文摘Ecologists have been puzzled by population cycles of lemmings and voles for the over 70 years. At present, our understanding and explanation to this phenomenon remain controversial. Recently, El Nino/South Oscillation has attracted attention of ecologists on its links with population outbreaks of terrestrial animals. This paper aims to investigate the statistical relationship between outbreaks of microtine rodents and ENSO events by scanning available literature. During 1862-1894, outbreaks of Norway lemmings in Norway tended to occur in the Southern Oscillation Index (SOI) peak years or 1 year after the SOI peak years with an approximate significance level (p = 0.057). During 1885-1931, outbreaks of voles in France tended to occur 1 year before the SOI peak years (p = 0.01). 4Present Situation and Control of Outbreaks of Algal Blooms in the Three Gorges Reservoir显示文摘After water storage in the Three Gorges Reservoir Region,there are no outbreaks of algal blooms in the main stream of the reservoir region,but the density of algae increases obviously. Outbreaks of algal blooms mainly appeared in the tributaries of the reservoir region such as the Xiangxi River,Daning River,Shennong River and Xiaojiang River,but they did not occur every year. The reasons for outbreaks of algal blooms in the tributaries are shown as follows: the existence of sources of algae(blue-green algae) in the Three Gorges Reservoir is the root cause,and the sources include sources existing and being produced in the reservoir and sources from upstream main stream and its tributaries and other related lakes and reservoirs,of which the sources are mainly from the Dianchi Lake; slight or moderate eutrophication of water is the basic condition;hydrologic and hydrodynamic conditions and suitable temperature are conducive to proliferation and aggregation of algae(blue-green algae) after the operation of the Three Gorges Reservoir until outbreaks of algal blooms appear. Outbreaks of algal blooms in the tributaries of the Three Gorges Reservoir Region mainly appear in backwater reaches; they mainly occur in the tributaries in the north of the reservoir region and near to the dam;they mainly appear from March to July; the dominant species of algae( blue-green algae) in the Three Gorges Reservoir are Pyrrophyta,Bacillariophyta and Chlorophyta,but they tend to change into blue-green algae and other algae. To control outbreaks of algal blooms in the tributaries of the Three Gorges Reservoir Region,it is needed to prevent water containing blue-green algae collected from the Dianchi Lake and other lakes and reservoirs from being input into the lower reaches,reduce pollution load flowing into the Three Gorges Reservoir,use enclosures to change hydrodynamic conditions of backwater reaches of the tributaries appropriately,and adopt biological measures such as culturing fish and planting plants to improve ecosystem of the tributaries and other measures to inhibit and eliminate algae and decrease eutrophication level.Ma Jianhua Hu Mingming Zhu Xi 2016Meteorological and Environmental Research2016,7,6:3
5Modeling the role of public health education in Ebola virus disease outbreaks in Sudan显示文摘Public involvement in Ebola Virus Disease(EVD)prevention efforts is key to reducing disease outbreaks.Targeted education through practical health information to particular groups and sub-populations is crucial to controlling the disease.In this paper,we study the dynamics of Ebola virus disease in the presence of public health education with the aim of assessing the role of behavior change induced by health education to the dynamics of an outbreak.The power of behavior change is evident in two outbreaks of EVD that took place in Sudan only 3 years apart.The first occurrence was the first documented outbreak of EVD and produced a significant number of infections.The second outbreak produced far fewer cases,presumably because the population in the region learned from the first outbreak.We derive a system of ordinary differential equations to model these two contrasting behaviors.Since the population in Sudan learned from the first outbreak of EVD and changed their behavior prior to the second outbreak,we use data from these two instances of EVD to estimate parameters relevant to two contrasting behaviors.We then simulate a future outbreak of EVD in Sudan using our model that contains two susceptible populations,one being more informed about EVD.Our finding show how a more educated population results in fewer cases of EVD and highlights the importance of ongoing public health education.Benjamin Levy Christina Edholm Orou Gaoue Roselyn Kaondera-Shava Moatlhodi Kgosimore Suzanne Lenhart Benjamin Lephodisa Edward Lungu Theresia Marijani Farai Nyabadza 2017Infectious Disease Modelling2017,2,3:2
6The SARS-CoV-2 pandemic course in Saudi Arabia:A dynamic epidemiological model显示文摘Objective:Saudi Arabia ranks second in the number of coronavirus disease 2019(COVID-19)cases in the Eastern Mediterranean region.It houses the two most sacred religious places for Muslims:Mecca and Medina.It is important to know what the trend in case numbers will be in the next 4e6 months,especially during the Hajj pilgrimage season.Methods:Epidemiological data on COVID-19 were obtained from the Saudi Arabian Ministry of Health.A susceptible-exposed-infectious-recovered(SEIR)prediction model was constructed to predict the trend in COVID-19 in Saudi Arabia in the next 6 months.Findings:The model predicts that the number of active cases will peak by 22 May 2020.The cumulative infected cases are predicted to reach 70,321 at that time.The total number of infected individuals is estimated reach to 114,580 by the end of the pandemic.Conclusion:Our estimates show that by the time the Hajj season commences in Saudi Arabia,the pandemic will be in the midst of its deceleration phase(phase 3).This information will likely be useful to policymakers in their management of the outbreak.Abdullah Murhaf Al-Khani Mohamed Abdelghafour Khalifa Abdulrahman Almazrou Nazmus Saquib 2020Infectious Disease Modelling2020,5,1:2
7Modelling epidemic growth models for lumpy skin disease cases in Thailand using nationwide outbreak data,2021e2022显示文摘Lumpy skin disease(LSD)is a transboundary disease affecting cattle and has a detrimental effect on the cattle industries in numerous countries in Africa,Europe and Asia.In 2021,LSD outbreaks have been reported in almost all of Thailand's provinces.Indeed,fitting LSD occurrences using mathematical models provide important knowledge in the realm of animal disease modeling.Thus,the objective of this study is to fit the pattern of daily new LSD cases and daily cumulative LSD cases in Thailand using mathematical models.The first-and second-order models in the forms of Lorentzian,Gaussian and Pearson-type VII models are used to fit daily new LSD cases whereas Richard's growth,Boltzmann sigmoidal and Power-law growth models are utilized to fit the curve of cumulative LSD cases.Based on the root-mean-squared error(RMSE)and Akaike information criterion(AIC),results showed that both first and second orders of Pearson-type VII models and Richard's growth model(RGM)were fit to the data better than other models used in the present study.The obtained models and their parameters can be utilized to describe the LSD outbreak in Thailand.For disease preparedness purposes,we can use the first order of the Pearson-type VII model to estimate the time of maximum infected cases occurring when the growth rate of infected cases starts to slow down.Furthermore,the period when the growth rate changes at a slower rate,known as the inflection time,obtained from RGM allows us to anticipate when the pandemic has peaked and the situation has stabilized.This is the first study that utilizes mathematical methods to fit the LSD epidemics in Thailand.This study offers decision-makers and authorities with valuable information for establishing an effective disease control strategy.Sompop Moonchai Adsadang Himakalasa Thaned Rojsiraphisal Orapun Arjkumpa Pawares Panyasomboonying Noppasorn Kuatako Noppawan Buamithup Veerasak Punyapornwithaya 2023Infectious Disease Modelling2023,8,1:0

9Feasibility, acceptability, and effectiveness of non-pharmaceutical interventions against infectious diseases among crisis-affected populations: a scoping review显示文摘Background:Non-pharmaceutical interventions(NPIs)are a crucial suite of measures to prevent and control infectious disease outbreaks.Despite being particularly important for crisis-affected populations and those living in informal settlements,who typically reside in overcrowded and resource limited settings with inadequate access to healthcare,guidance on NPI implementation rarely takes the specific needs of such populations into account.We therefore conducted a systematic scoping review of the published evidence to describe the landscape of research and identify evidence gaps concerning the acceptability,feasibility,and effectiveness of NPIs among crisis-affected populations and informal settlements.Methods:We systematically reviewed peer-reviewed articles published between 1970 and 2020 to collate available evidence on the feasibility,acceptability,and effectiveness of NPIs in crisis-affected populations and informal settlements.We performed quality assessments of each study using a standardised questionnaire.We analysed the data to produce descriptive summaries according to a number of categories:date of publication;geographical region of intervention;typology of crisis,shelter,modes of transmission,NPI,research design;study design;and study quality.Results:Our review included 158 studies published in 85 peer-reviewed articles.Most research used low quality study designs.The acceptability,feasibility,and effectiveness of NPIs was highly context dependent.In general,simple and cost-effective interventions such as community-level environmental cleaning and provision of water,sanitation and hygiene services,and distribution of items for personal protection such as insecticide-treated nets,were both highly feasible and acceptable.Logistical,financial,and human resource constraints affected both the implementation and sustainability of measures.Community engagement emerged as a strong factor contributing to the effectiveness of NPIs.Conversely,measures that involve potential restriction on personal liberty such as case isolation and patient care and burial restrictions were found to be less acceptable,despite apparent effectiveness.Conclusions:Overall,the evidence base was variable,with substantial knowledge gaps which varied between settings and pathogens.Based on the current landscape,robust evidence-based guidance is not possible,and a research agenda is urgently required that focusses on these specific vulnerable populations.Although implementation of NPIs presents unique practical challenges in these settings,it is critical that such an agenda is put in place,and that the lessons learned from historical and present experiences are documented to build a firm evidence base.Jonathan A.Polonsky Sangeeta Bhatia Keith Fraser Arran Hamlet Janetta Skarp Isaac J.Stopard Stéphane Hugonnet Laurent Kaiser Christian Lengeler Karl Blanchet Paul Spiegel 2022Infectious Diseases of Poverty2022,11,1:0
10Modelling the population fluctuation of winter moth and mottled umber moth in central and northern Germany显示文摘Background: Winter moth(Operophtera brumata) and mottled umber moth(Erannis defoliaria) are forest Lepidoptera species characterized by periodic high abundance in a 7–11 year cycle. During outbreak years they cause severe defoliation in many forest stands in Europe. In order to better understand the spatio-temporal dynamics and elucidate possible influences of weather, stand and site conditions, a generalized additive mixed model was developed. The investigated data base was derived from glue band catch monitoring stands of both species in Central and North Germany. From the glue bands only female moth individuals are counted and a hazard code is calculated. The model can be employed to predict the exceedance of a warning threshold of this hazard code which indicates a potential severe defoliation of oak stands by winter moth and mottled umber in the coming spring.Results: The developed model accounts for specific temporal structured effects for three large ecoregions and random effects at stand level. During variable selection the negative model effect of pest control and the positive model effects of mean daily minimum temperature in adult stage and precipitation in early pupal stage were identified.Conclusion: The developed model can be used for short-term predictions of potential defoliation risk in Central and North Germany. These predictions are sensitive to weather conditions and the population dynamics. 11Clostridium difficile infection in the community:Are proton pump inhibitors to blame?显示文摘Once a nosocomial disease,Clostridium difficile infection(CDI)now appears frequently in the community in the absence of exposure to antibiotics.Prior studies have shown that patients with community-acquired CDI are younger,more likely to be female,and have fewer comorbidities compared to patients with hospital-associated CDI.Because most studies of CDI are hospitalbased,comparatively little is known about communityacquired CDI.The recent study by Chitnis has received widespread attention because it used active surveillance to capture all cases of community-acquired CDI within a large population and assessed key risk factors.The authors found that low-level healthcare exposure and proton pump inhibitor use were common among those with non-antibiotics associated,community-acquired CDI.In this commentary,we discuss the changing epidemiology of community-acquired CDI and the evidence basis for the controversial association between proton pump inhibitors and community-acquired CDI.Daniel E Freedberg Julian A Abrams 2013World Journal of Gastroenterology2013,19,40:0

13Establishment of an early warning system for cutaneous leishmaniasis in Fars province, Iran显示文摘Objective:To establish an early warning system for cutaneous leishmaniasis in Fars province,Iran in 2016.Methods:Time-series data were recorded from 29 201 cutaneous leishmaniasis cases in 25 cities of Fars province from 2010 to 2015 and were used to fit and predict the cases using time-series models.Different models were compared via Akaike information criterion/Bayesian information criterion statistics,residual analysis,autocorrelation function,and partial autocorrelation function sample/model.To decide on an outbreak,four endemic scores were evaluated including mean,median,mean+ 2 standard deviations,and median+ interquartile range of the past five years.Patients whose symptoms of cutaneous leishmaniasis began from 1 January 2010 to 31 December 2015 were included,and there were no exclusion criteria.Results:Regarding four statistically significant endemic values,four different cutaneous leishmaniasis space-time outbreaks were detected in 2016.The accuracy of all four endemic values was statistically significant(P<0.05).Conclusions:This study presents a protocol to set early warning systems regarding time and space features of cutaneous leishmaniasis in four steps:(i)to define endemic values based on which we could verify if there is an outbreak,(ii)to set different time-series models to forecast cutaneous leishmaniasis in future,(iii)to compare the forecasts with endemic values and decide on space-time outbreaks,and(iv)to set an alarm to health managers.Marjan Zare Abbas Rezaianzadeh Hamidreza Tabatabaee Hossain Faramarzi Mohsen Aliakbarpour MostafaEbrahimi 2019Asian Pacific Journal of Tropical Biomedicine2019,9,6:0
