| 3 | Does autologous blood transfusion during liver transplantation for hepatocellular carcinoma increase risk of recurrence?显示文摘AIM: To analyze outcomes in patients who underwent liver transplantation(LT) for hepatocellular carcinoma(HCC) and received autologous intraoperative blood salvage(IBS). METHODS: Consecutive HCC patients who underwent LT were studied retrospectively and analyzed according to the use of IBS or not. Demographic and surgical data were collected from a departmental prospective maintained database. Statistical analyses were performed using the Fisher's exact test and the Wilcoxon rank sum test to examine covariate differences between patients who underwent IBS and those who did not. Univariate and multivariate Cox regression models were developed to evaluate recurrence and death,and survival probabilities were estimated using the Kaplan-Meier method and compared by the log-rank test.RESULTS: Between 2002 and 2012,158 consecutive patients who underwent LT in the same medical center and by the same surgical team were identified. Among these patients,122(77.2%) were in the IBS group and 36(22.8%) in the non-IBS group. The overall survival(OS) and recurrence free survival(RFS) at 5 years were 59.7% and 83.3%,respectively. No differences in OS(P=0.51) or RFS(P=0.953) were detected between the IBS and non-IBS groups. On multivariate analysis for OS,degree of tumor differentiation remained as the only independent predictor. Regarding patients who received IBS,no differences were detected in OS or RFS(P=0.055 and P=0.512,respectively) according to the volume infused,even when outcomes at 90 d or longer were analyzed separately(P=0.518 for both outcomes).CONCLUSION: No differences in RFS or OS were detected according to IBS use. | 4 | Robotic surgery for rectal cancer: Current immediate clinical and oncological outcomes显示文摘Laparoscopic rectal surgery continues to be a challenging operation associated to a steep learning curve. Robotic surgical systems have dramatically changed minimally invasive surgery. Three-dimensional, magnified and stable view, articulated instruments, and reduction of physiologic tremors leading to superior dexterity and ergonomics. Therefore, robotic platforms could potentially address limitations of laparoscopic rectal surgery. It was aimed at reviewing current literature on short-term clinical and oncological(pathological) outcomes after robotic rectal cancer surgery in comparison with laparoscopic surgery. A systematic review was performed for the period 2002 to 2014. A total of 1776 patients with rectal cancer underwent minimally invasive robotic treatment in 32 studies. After robotic and laparoscopic approach to oncologic rectal surgery, respectively, mean operating time varied from 192-385 min, and from 158-297 min; mean estimated blood loss was between 33 and 283 mL, and between 127 and 300 mL; mean length of stay varied from 4-10 d; and from 6-15 d. Conversion after robotic rectal surgery varied from 0% to 9.4%, and from 0 to 22% after laparoscopy. There was no difference between robotic(0%-41.3%) and laparoscopic(5.5%-29.3%) surgery regarding morbidity and anastomotic complications(respectively, 0%-13.5%, and 0%-11.1%). Regarding immediate oncologic outcomes, respectively among robotic and laparoscopic cases, positive circumferential margins varied from 0% to 7.5%, and from 0% to 8.8%; the mean number of retrieved lymph nodes was between 10 and 20, and between 11 and 21; and the mean distal resection margin was from 0.8 to 4.7 cm, and from 1.9 to 4.5 cm. Robotic rectal cancer surgery is being undertaken by experienced surgeons. However, the quality of the assembled evidence does not support definite conclusions about most studies variables. Robotic rectal cancer surgery is associated to increased costs and operating time. It also seems to be associated to reduced conversion rates. Other short-term outcomes are comparable to conventional laparoscopy techniques, if not better. Ultimately, pathological data evaluation suggests that oncologic safety may be preserved after robotic total mesorectal excision. | 7 | Short-term complementary and alternative medicine on quality of life in women with fibromyalgia显示文摘BACKGROUND:Fibromyalgia(FMS)is a syndrome characterized by chronic widespread musculoskeletal pain,whose etiology is not completely understood.Different therapeutic approaches have been used with inconsistent results.This observation does not invalidate the continued search for alternative treatments aimed at improving quality of life(Qo L)in FMS.OBJECTIVE:This study compared three classical traditional Chinese medicine(TCM)therapies:acupuncture(AC),electroacupuncture(EAC)and moxibustion(MX)in the management of pain and promotion of Qo L in FMS patients.DESIGN,SETTING,PARTICIPANTS AND INTERVENTIONS:A preliminary,group-assigned,comparative study enrolled 30 women,mean age(46.90±9.24)years(range 20–60 years),who met the 1990 American College of Rheumatology criteria for FMS diagnosis and a pain-pressure threshold(PPT)< 4 kg/cm^2.The study was conducted in a teaching tertiary-care medical institution from May 2010 through April 2012.AC,EAC and MX were delivered for 30 min,once a week,for 8 weeks,bilaterally at Neiguan(PC6),Hegu(LI4),Yanglingquan(GB34),Sanyinjiao(SP6)and Taichong(LR3)acupoints.MAIN OUTCOME MEASURES:Each week,immediately before treatment and after treatment,subjects were tested for PPTs,Wong-Baker Faces Pain Scale(WBFPS;for pain intensity)and Medical Outcomes Study 36-item Short Form Health Survey(SF-36:for Qo L).RESULTS:There was no signifi cant improvement in pain or reduction of tender points in any of the groups studied,at the end of the 8th session.Signifi cant improvement of Qo L was perceived in vitality(after AC treatment)and in mental health(after EAC and MX treatments).CONCLUSION:TCM therapies(AC,EAC and MX)promoted an improvement in the Qo L in two areas(vitality and mental health)in FMS women.Further large-scale clinical trials are required to confi rm this effect. | Paulo Araujo Dias Andre Brito Bastos Guimaraes Andrea de Oliveira Albuquerque Karoline Lucas de Oliveira Maria Luzete Costa Cavalcante Sergio Botelho Guimaraes | 2016 | Journal of Integrative Medicine2016,14,1: | 7 |

| 14 | Synthesis of highly fluorescent carbon dots from lemon and onion juices for determination of riboflavin in multivitamin/mineral supplements显示文摘In this work, lemon and onion biomasses commonly found in street markets are for the first time used to develop a facile, fast and low-cost one-step microwave-assisted carbonization method for synthesis of highly fluorescent carbon dots (CDs). The structure and optical properties of CDs were investigated by TEM, XRD, XRF, UV-Vis, FTIR, and fluorescence spectroscopy. CDs displayed satisfactory optical pro-prieties, a high quantum yield of 23.6%, and excellent water solubility, and the particle size was 4.23-8.22 nm with an average diameter of 6.15 nm. An efficient fluorescent resonance energy transfer (FRET) between the CDs and riboflavin was achieved with CDs acting as donor and riboflavin as acceptor. A linear relationship between FRET and the riboflavin concentration from 0.10 to 3.0 μg/mL was observed, allowing the development of an accurate and fast analytical method to determine this vitamin in multivitamin/mineral supplements. Despite the potential interferences in these supplements, CDs were selective for riboflavin under optimized conditions. A paired t-test at a 95% confidence level indicated no statistically significant difference between the proposed and the reference methods. Recovery test presented values ranged from 96.0% to 101.4%. The limit of detection and relative standard deviation were estimated at 1.0 ng/mL and <2.6% (n = 3), respectively. CDs were successfully synthesized in a domestic microwave oven (1450 W, 6 min), presenting satisfactory parameters when compared with results of other studies reported in the literature, suggesting that the proposed method is a potentially useful method for the synthesis of CDs and determination of riboflavin. | Severino S. | 15 | Corneal stem cells and tissue engineering: Current advances and future perspectives显示文摘Major advances are currently being made in regenerative medicine for cornea. Stem cell-based therapies represent a novel strategy that may substitute conventional corneal transplantation, albeit there aremany challenges ahead given the singularities of each cellular layer of the cornea. This review recapitulates the current data on corneal epithelial stem cells, corneal stromal stem cells and corneal endothelial cell progenitors. Corneal limbal autografts containing epithelial stem cells have been transplanted in humans for more than 20 years with great successful rates, and researchers now focus on ex vivo cultures and other cell lineages to transplant to the ocular surface. A small population of cells in the corneal endothelium was recently reported to have self-renewal capacity, although they do not proliferate in vivo. Two main obstacles have hindered endothelial cell transplantation to date: culture protocols and cell delivery methods to the posterior cornea in vivo. Human corneal stromal stem cells have been identified shortly after the recognition of precursors of endothelial cells. Stromal stem cells may have the potential to provide a direct cell-based therapeutic approach when injected to corneal scars. Furthermore, they exhibit the ability to deposit organized connective tissue in vitro and may be useful in corneal stroma engineering in the future. | 20 | CAPTURING COMPLEX,NON-LINEAR TEAM BEHAVIOURS DURING COMPETITIVE FOOTBALL PERFORMANCE显示文摘This study investigated changes in the complexity(magnitude and structure of variability)of the collective behaviours of association football teams during competitive performance.Raw positional data from an entire competitive match between two professional teams were obtained with the ProZone^(?) tracking system.Five compound positional variables were used to investigate the collective patterns of performance of each team including:surface area,stretch index,team length,team width,and geometrical centre.Analyses involve the coefficient of variation(%CV) and approximate entropy(ApEn),as well as the linear association between both parameters.Collective measures successfully captured the idiosyncratic behaviours of each team and their variations across the six time periods of the match.Key events such as goals scored and game breaks(such as half time and full time) seemed to influence the collective patterns of performance.While ApEn values significantly decreased during each half,the%CV increased.Teams seem to become more regular and predictable,but with increased magnitudes of variation in their organisational shape over the natural course of a match. | DUARTE Ricardo ARAUJO Duarte FOLGADO Hugo ESTEVES Pedro MARQUES Pedro DAVIDS Keith | 2013 | Journal of Systems Science & Complexity2013,26,1: | 3 |
