| 1 | Liver insulin-like growth factor 2 methylation in hepatitis C virus cirrhosis and further occurrence of hepatocellular carcinoma显示文摘AIM: To assess the predictive value of the insulinlike growth factor 2 (Igf2) methylation profile for the occurrence of Hepatocellular Carcinoma (HCC) in hepatitis C (HCV) cirrhosis. METHODS: Patients with: (1) biopsy-proven compensated HCV cirrhosis; (2) available baseline frozen liver sample; (3) absence of detectable HCC; (4) regular screening for HCC; (5) informed consent for genetic analysis were studied. After DNA extraction from liver samples and bisulfite treatment, unbiased PCR and DHPLC analysis were performed for methylation analysis at the Igf2 locus. The predictive value of the Igf2 methylation profile for HCC wasassessed by Kaplan-Meier and Cox methods. RESULTS: Among 94 included patients, 20 developed an HCC during follow-up (6.9 ± 3.2 years). The methylation profile was hypomethylated, intermediate and hypermethylated in 13, 64 and 17 cases, respectively. In univariate analysis, two baseline parameters were associated with the occurrence of HCC: age (P = 0.01) and prothrombin (P = 0.04). The test of linear tendency between the three ordered levels of Igf2 methylation and probability of HCC occurrence was significant (Log Rank, P = 0.043; Breslow, P = 0.037; Tarone-Ware, P = 0.039). | 2 | Estimation of intra-operator variability in perfusion parameter measurements using DCE-US显示文摘AIM:To investigate intra-operator variability of semiquantitative perfusion parameters using dynamic contrast-enhanced ultrasonography(DCE-US),following bolus injections of SonoVue.METHODS:The in vitro experiments were conducted using three in-house sets up based on pumping a fluid through a phantom placed in a water tank.In the in vivo experiments,B16F10 melanoma cells were xenografted to five nude mice.Both in vitro and in vivo,images were acquired following bolus injections of the ultrasound contrast agent SonoVue(Bracco,Milan,Italy) and using a Toshiba Aplio ultrasound scanner connected to a 2.9-5.8 MHz linear transducer(PZT,PLT 604AT probe)(Toshiba,Japan) allowing harmonic imaging('Vascular Recognition Imaging') involving linear raw data.A mathematical model based on the dye-dilution theory was developed by the Gustave Roussy Institute,Villejuif,France and used to evaluate seven perfusion parameters from time-intensity curves.Intra-operator variability analyses were based on determining perfusion parameter coefficients of variation(CV).RESULTS:In vitro,different volumes of SonoVue were tested with the three phantoms:intra-operator variability was found to range from 2.33% to 23.72%.In vivo,experiments were performed on tumor tissues and perfusion parameters exhibited values ranging from 1.48% to 29.97%.In addition,the area under the curve(AUC) and the area under the wash-out(AUWO) were two of the parameters of great interest since throughout in vitro and in vivo experiments their variability was lower than 15.79%.CONCLUSION:AUC and AUWO appear to be the most reliable parameters for assessing tumor perfusion using DCE-US as they exhibited the lowest CV values. | Marianne Gauthier Ingrid Leguerney Jessie Thalmensi Mohamed Chebil Sarah Parisot Pierre Peronneau Alain Roche Nathalie Lassau | 2011 | World Journal of Radiology2011，3，3： | 6 |

| 7 | Ribavirin:Past,present and future显示文摘Before the advent of direct acting antiviral agents(DAAs) ribavirin, associated to pegylated-interferon played a crucial role in the treatment of chronic hepatitis C, preventing relapses and breakthroughs. In the present era of new potent DAAs, a place is still devoted to the drug. Ribavirin associated with sofosbuvir alone is efficient in the treatment of most cases of G2 infected patients. All options currently available for the last difficult-to-treat cirrhotic G3 patients contain ribavirin. Reducing treatment duration to 12 wk in G1 or G4 cirrhotic compensated patients is feasible thanks to ribavirin. Retreating patients with acquired anti NS5 A resistance-associated variants using ribavirin-based strategies could be useful. The addition of ribavirin with DAAs combinations however, leads to more frequent but mild adverse events especially in cirrhotic patients.
