| 8 | Vancomycin flushing syndrome in orthopaedic practice:A case report显示文摘BACKGROUND Vancomycin flushing syndrome(VFS),also known as red man syndrome,is an allergic reaction to vancomycin.It typically presents as a rash on the face,neck,and upper torso after intravenous administration of vancomycin.VFS is blamed on rapid intravenous infusion of vancomycin during management and rarely happens after local use.A review of the literature showed that in the last 23 years,4 such cases have been reported.Here,we add another case of VFS developed after slow local absorption of vancomycin in cement beads.CASE SUMMARY A 44-year-old male with a known case of hypertension,no history of allergies to medications,and a history of chronic osteomyelitis of the right tibia with discharging sinus over the anterolateral aspect of the leg.The pus culture grew Staphylococcus aureus,which was sensitive to clindamycin and vancomycin.The patient underwent irrigation and debridement with the placement of vancomycin cement beads made from 4 g of vancomycin powder and 40 g of polymethyl methacrylate.Three hours postoperatively,the patient developed a pruritic,erythematous,macular rash predominantly on his face,neck,chest,and lower extremities and to a lesser extent his upper extremities.A diagnosis of VFS was made and was successfully treated with cetirizine(10 mg,oral)and methylprednisolone sodium succinate(125 mg,intravenous).The patient continued to have itching with a facial rash for 12 h with gradual improvement.A decision was made to not remove the beads as the patient continued to improve.Gradually,the rash disappeared after 96 h with no further sequela.CONCLUSION VFS can occur not only after rapid intravenous injection of vancomycin but also with local release,as in our case.As orthopaedic surgeons routinely use vancomycin with polymethyl methacrylate in chronic osteomyelitis and revision arthroplasty,they should be aware of such a complication occurring. | Fawaz M Al-Anii Mir Sadat-Ali Khalid Waleed Al-Tabash Ahmad I AlMulhim Sulaiman A AlMousa Abdulaziz M AlHawas | 2023 | World Journal of Orthopedics2023,14,10: | 0 |
| 9 | Business Blockchain Suitability Determinants: Decision-Making through an Intuitionistic Fuzzy Method显示文摘Blockchain is one of the innovative and disruptive technologies that has a wide range of applications in multiple industries beyond cryptocurrency.The widespread adoption of blockchain technology in various industries has shown its potential to solve challenging business problems,as well as the possibility to create new business models which can increase a firm’s competitiveness.Due to the novelty of the technology,whereby many companies are still exploring potential use cases,and considering the complexity of blockchain technology,which may require huge changes to a company’s existing systems and processes,it is important for companies to carefully evaluate suitable use cases and determine if blockchain technology is the best solution for their specific needs.This research aims to provide an evaluation framework that determines the important dimensions of blockchain suitability assessment by identifying the key determinants of suitable use cases in a business context.In this paper,a novel approach that utilizes both qualitative(Delphi method)and quantitative(fuzzy set theory)methods has been proposed to objectively account for the uncertainty associated with data collection and the vagueness of subjective judgments.This work started by scanning available literature to identify major suitability dimensions and collected a range of criteria,indicators,and factors that had been previously identified for related purposes.Expert opinions were then gathered using a questionnaire to rank the importance and relevance of these elements to suitability decisions.Subsequently,the data were analyzed and we proceeded to integrate multi-criteria group decision-making(MCGDM)and intuitionistic fuzzy set(IFS)theory.The findings demonstrated a high level of agreement among experts,with the model being extremely sensitive to variances in expert assessments.Furthermore,the results helped to refine and select the most relevant suitability determinants under three important dimensions:functional suitability of the use case,organizational applicability,and ecosystem readiness. | Tomader Almeshal Jawad Berri Tarifa Almulhim Areej Alhogail Emam Ahmed | 2023 | Computer Systems Science & Engineering2023,47,11: | 0 |
| 10 | Unique Roberts syndrome with bilateral congenital glaucoma: A case report显示文摘BACKGROUND Congenital glaucoma associated with Roberts syndrome(RS)is an unusual and unique condition.No previous report describes this association.A multidisciplinary approach including molecular studies were conducted to reach the final diagnosis.CASE SUMMARY We present a rare case of a 1-wk-old male with RS associated with bilateral congenital glaucoma,left ectopic kidney,and left-hand rudimentary digits.A comprehensive approach was applied by which bilateral non-penetrating glaucoma surgery was performed with good control of intraocular pressure for more than 6 mo.Cytogenetic and molecular testing were conducted and revealed normal measurements.CONCLUSION This report described a case of a male baby with clinical features of RS but with a negative molecular analysis,presenting with left-hand rudimentary digits,bilateral congenital glaucoma,and left ectopic kidney.To the best of our knowledge,this is the first case reported with phocomelia,bilateral congenital glaucoma,and unilateral ectopic kidney. | Amar Almulhim Basamat Almoallem Ehab Alsirrhy Essam A Osman | 2023 | World Journal of Clinical Cases2023,11,19: | 0 |
| 11 | Burden of routine orthopedic implant removal a single center retrospective study显示文摘BACKGROUND Open reduction and internal fixation represent prevalent orthopedic procedures,sparking ongoing discourse over whether to retain or remove asymptomatic implants.Achieving consensus on this matter is paramount for orthopedic surgeons.This study aims to quantify the impact of routine implant removal on patients and healthcare facilities.A retrospective analysis of implant removal cases from 2016 to 2022 at King Fahad Hospital of the University(KFHU)was conducted and subjected to statistical scrutiny.Among these cases,44%necessitated hospitalization exceeding one day,while 56%required only a single day.Adults exhibited a 55%need for extended hospital stays,contrasting with 22.8%among the pediatric cohort.The complication rate was 6%,with all patients experiencing at least one complication.Notably,34.1%required sick leave and 4.8%exceeded 14 d.General anesthesia was predominant(88%).Routine implant removal introduces unwarranted complications,particularly in adults,potentially prolonging hospitalization.This procedure strains hospital resources,tying up the operating room that could otherwise accommodate critical surgeries.Clearly defined institutional guidelines are imperative to regulate this practice.AIM To measure the burden of routine implant removal on the patients and hospital.METHODS This is a retrospective analysis study of 167 routine implant removal cases treated at KFHU,a tertiary hospital in Saudi Arabia.Data were collected in the orthopedic department at KFHU from February 2016 to August 2022,which includes routine asymptomatic implant removal cases across all age categories.Nonroutine indications such as infection,pain,implant failure,malunion,nonunion,restricted range of motion,and prominent hardware were excluded.Patients who had external fixators removed or joints replaced were also excluded.RESULTS Between February 2016 and August 2022,360 implants were retrieved;however,only 167 of those who met the inclusion criteria were included in this study.The remaining implants were rejected due to exclusion criteria.Among the cases,44%required more than one day in the hospital,whereas 56%required only one day.55%of adults required more than one day of hospitalization,while 22.8%of pediatric patients required more than one day of inpatient care.The complication rate was 6%,with each patient experiencing at least one complication.Sick leave was required in 34.1%of cases,with 4.8%requiring more than 14 d.The most common type of anesthesia used in the surgeries was general anesthesia(88%),and the mean(SD)surgery duration was 77.1(54.7)min.CONCLUSION Routine implant removal causes unnecessary complications,prolongs hospital stays,depletes resources and monopolizing operating rooms that could serve more critical procedures. | Ammar K AlOmran Nader Alosaimi Ahmed A Alshaikhi Omar M Bakhurji Khalid J Alzahrani Basil Ziyad Salloot Tamim Omar Alabduladhem Ahmed I AlMulhim Arwa Alumran | 2024 | World Journal of Orthopedics2024,15,2: | 0 |
