| 1 | Pathological,immunohistochemical and microbiologicalal analysis of lacrimal sac biopsies in patients with chronic dacrocystitis显示文摘AIM:To analyze cases of obstruction of the nasolacrimal duct which creates a fertile environment for secondary bacterial Infection and can result in dacryocystitis,which is a constant threat to cornea and orbital soft tissue and a potential source of endophthalmitis following intraocular surgery.The majority of obstructions of the lacrimal excretory outflow system are acquired ones occurring in adulthood and involving the distal parts of the system.Acquired obstruction may be primary/idiopathic or secondary to a wide variety of infectious,inflammatory,traumatic,mechanical,toxic or neoplastic causes mimicking idiopathic inflammation.These cases are treated by dacryocystorhinostomy(DCR).METHODS:The present study was conducted to determine the histopathologic,immunohistochemical and current microbiologic characteristics of lacrimal sac specimens in patients undergoing external dacryocystorhinostomy.RESULTS:Non-specific lacrimal sac pathology was present in all 33 cases and 81.8%of the cases showed moderate chronic inflammation with a chronic inflammatory score(CIS)ranging between 4 and 6,whereas 12.12%showed severe inflammatory changes with a CIS of 7.Mild degree of inflammation was seen in6.06%with a CIS of 3.The total prevalence of grampositive,gram-negative,and culture-negative samples were 59.4%,37.5%,and 3%respectively.CONCLUSION:Non-specific chronic inflammation withfibrosis is indeed the most commonly reported histopathological finding in lacrimal sac wall biopsy specimens. | Rowayda Mahmoud Amin Faten Aly Hussein Hisham Farouk Idriss Nesrine Fathy Hanafy Dina Mohamed Abdallah | 2013 | International Journal of Ophthalmology(English edition)2013,6,6: | 7 |

| 3 | A Hybrid Particle Swarm Optimization and Genetic Algorithm for Model Updating of A Pier-Type Structure Using Experimental Modal Analysis显示文摘Conventional design of pier structures is based on the assumption of fully rigid joints. In practice, the real connections are semi-rigid that cause changes in dynamic characteristics. In this study, quality of the joints is investigated by considering changes in natural frequencies. For this purpose, numerical and experimental modal analyses are carried out on related physical model of a pier type structure. When numerical results are evaluated,natural frequencies generally do not match the expected experimental results. Uncertainties in different aspects of engineering problems are always a challenge for researchers. The numerical models which are constructed on the basis of highly idealized scheme may not be able to represent all of the physical aspects of the physical one. For this study, determination of percentage of semi-rigid joints is considered as an optimization problem based on the numerical and experimental frequencies. Probabilistic sensitivity analysis is also used to determine the search space.A new technique of optimization problem is solved by a combination of smart particle swarm optimization(PSO)and genetic algorithms, and a complicated and efficient system for model updating process is introduced. It is observed that the hybrid PSO-Genetic algorithm is applicable and appropriate in model updating process. | 4 | Novel non-invasive biological predictive index for liver fibrosis in hepatitis C virus genotype 4 patients显示文摘AIM To investigate the diagnostic ability of a non-invasive biological marker to predict liver fibrosis in hepatitis C genotype 4 patients with high accuracy.METHODS A cohort of 332 patients infected with hepatitis C genotype 4 was included in this cross-sectional study. Fasting plasma glucose, insulin, C-peptide, and angiotensinconverting enzyme serum levels were measured. Insulin resistance was mathematically calculated using the homeostasis model of insulin resistance(HOMA-IR).RESULTS Fibrosis stages were distributed based on Metavir score as follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 and F4 = 44. Statistical analysis relied upon reclassification of fibrosis stages into mild fibrosis(F0-F) = 179, moderate fibrosis(F2) = 64, and advanced fibrosis(F3-F4) = 89. Univariate analysis indicated that age, log aspartate amino transaminase, log HOMA-IR and log platelet count were independent predictors of liver fibrosis stage(P < 0.0001). A stepwise multivariate discriminant functional analysis was used to drive a discriminative model for liver fibrosis. Our index used cut-off values of ≥ 0.86 and ≤-0.31 to diagnose advanced and mild fibrosis, respectively, with receiving operating characteristics of 0.91 and 0.88, respectively.
