1Selection of patients with hepatocellular carcinoma for livertransplantation:Past and future显示文摘The aim of liver transplantation(LT) for hepatocellular carcinoma(HCC) is to ensure a rate of disease-free survival similar to that of patients transplanted due to benign disease. Therefore, we are forced to adopt strict criteria when selecting candidates for LT and prioritizing patients on the waiting list(WL), to have clarified indications for bridging therapy for groups at risk for progression or recurrence, and to establish certain limits for downstaging therapies. Although the Milan criteria(MC) remain the standard and most employed criteria for indication of HCC patients for LT by far, in the coming years, criteria will be consolidated that take into account not only data regarding the size/volume and number of tumors but also their biology. This criteria will mainly include the alpha fetoprotein(AFP) values and, in view of their wide variability, any of the published logarithmic models for the selection of candidates for LT. Bridging therapy is necessary for HCC patients on the WL who meet the MC and have the possibility of experiencing a delay for LT greater than 6 mo or any of the known risk factors for recurrence. It is difficult to define single AFP values that would indicate bridging therapy(200, 300 or 400 ng/m L); therefore, it is preferable to rely on the criteria of a French AFP model score > 2. Other single indications for bridging therapy include a tumor diameter greater than 3 cm, more than one tumor, and having an AFP slope greater than 15 ng/m L per month or > 50 ng/m L for three months during strict monitoring while on the WL. When considering the inclusion of patients on the WL who do not meet the MC, it is mandatory to determine their eligibility for downstaging therapy prior to inclusion. The upper limit for this therapy could be one lesion up to 8 cm, 2-3 lesions with a total tumor diameter up to 8 cm, or a total tumor volume of 115 cm^3. Lastly, liver allocation and the prioritization of patients with HCC onthe WL should take into account the recently described HCC model for end-stage liver disease, which considers hepatic function, HCC size and the number and the log of AFP values. 2Piwi like RNA-mediated gene silencing 1 gene as a possible major player in gastric cancer显示文摘AIM To establish a permanent piwi like RNA-mediated genesilencing 1(PIWIL1) gene knockout in AGP01 gastric cancer cell line using CRISPR-Cas9 system and analyze phenotypic modifications as well as gene expression alterations.METHODS CRISPR-Cas9 system used was purchased from Dharmacon GE Life Sciences(Lafayette, CO, United States) and permanent knockout was performed according to manufacturer's recommendations. Woundhealing assay was performed to investigate the effect of PIWIL1 knockout on migration capability of cells and Boyden chamber invasion assay was performed to investigate the effect on invasion capability. For the gene expression analysis, a one-color microarray-based gene expression analysis kit(Agilent Technologies, Santa Clara, CA, United States) was used according to the protocol provided by the manufacturer. RESULTS PIWIL1 gene knockout caused a significant decrease in AGP01 migration capacity as well as a significant decrease in cell invasiveness. Moreover, functional analysis based on grouping of all differentially expressed m RNAs identified a total of 35 genes(5 up-regulated and 30 down-regulated) encoding proteins involved in cellular invasion and migration. According to current literature, 9 of these 35 genes(DOCK2, ZNF503, PDE4 D, ABL1, ABL2, LPAR1, SMAD2, WASF3 and DACH1) are possibly related to the mechanisms used by PIWIL1 to promote carcinogenic effects related to migration and invasion, since their functions are consistent with the changes observed(being up-or down-regulated after knockout). 3Impacts of COVID-19 on children and adolescents:A systematic review analyzing its psychiatric effects显示文摘OBJECTIVE To summarize the most relevant data from a systematic review on the impact of COVID-19 on children and adolescents,particularly analyzing its psychiatric effects.METHODS This review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses(PRISMA)guidelines and included experimental studies(randomized-individually or pooled-and non-randomized controlled trials),observational studies with a group for internal comparison(cohort studies-prospective and retrospective-and case-control)and qualitative studies in the period from 2021 to 2022.RESULTS The search identified 325 articles;we removed 125 duplicates.We selected 200 manuscripts,chosen by title and selected abstracts.We excluded 50 records after screening titles and abstracts,as they did not meet the inclusion criteria.We retrieved 150 records selected for a full reading.We excluded 90 text articles and we selected 25 records for the(n)final.Limitations:Due to the short period of data collection,from 2021 to 2022,there is a possibility of lack of relevant studies related to the mental health care of children and adolescents.In addition,there is the possibility of publication bias,such as only significant findings being published.CONCLUSION The impact of COVID-19 on the mental health of children and adolescents is of great concern to child and youth psychiatry.Situations such as fear,anxiety,panic,depression,sleep and appetite disorders,as well as impairment in social interactions caused by psychic stress,are punctual markers of pain and psychic suffering,which have increasing impacts on the mental health panorama of children and adolescents globally,particularly in vulnerable and socially at-risk populations.Italo Wanderson de Moura Gabriel Danielly Gonçalves Sombra Lima Jeully Pereira Pires Nélio Barreto Vieira Aloisio Antonio Gomes de Matos Brasil Yara Talita Gomes Pereira Erika Galvao de Oliveira HildsonLeandro de Menezes Nadia Nara Rolim Lima Alberto Olavo Advíncula Reis Ruan Neto Pereira Alves Uanderson Pereira da Silva Jucier Gonçalves Junior Modesto Leite Rolim-Neto 2022World Journal of Psychiatry2022,12,11:0
