2Helicobacter pylori infection:How does age influence the inflammatory pattern?显示文摘The inflammatory pattern during Helicobacter pylori(H.pylori)infection is changeable and complex.During childhood,it is possible to observe a predominantly regulatory response,evidenced by high concentrations of key cytokines for the maintenance of Treg responses such as TGF-β1 and IL-10,in addition to high expression of the transcription factor FOXP3.On the other hand,there is a predominance of cytokines associated with the Th1 and Th17 responses among H.pylori-positive adults.In the last few years,the participation of the Th17 response in the gastric inflammation against H.pylori infection has been highlighted due to the high levels of TGF-β1 and IL-17 found in this infectious scenario,and growing evidence has supported a close relationship between this immune response profile and unfavorable outcomes related to the infection.Moreover,this cytokine profile might play a pivotal role in the effectiveness of anti-H.pylori vaccines.It is evident that age is one of the main factors influencing the gastric inflammatory pattern during the infection with H.pylori,and understanding the immune response against the bacterium can assist in the development of alternative prophylactic and therapeutic strategies against the infection as well as in the comprehension of the pathogenesis of the outcomes related to that microorganism.Glauber Rocha Lima Araújo Hanna Santos Marques Maria Luísa Cordeiro Santos Filipe Antônio França daSilva Breno Bittencourt da Brito Gabriel Lima Correa Santos Fabrício Freire de Melo 2022World Journal of Gastroenterology2022,28,4:2

17Financial Analysis of Crop-Livestock-Forestry Systems in Goias,Brazil显示文摘Integration system is used to denote practices that combine systematic use of the land and technologies,in which forest species are used in conjunction with herbaceous plants and/or animals respecting a spatial or temporal arrangement.Knowing that this type of production seeks to balance ecological and economic factors,it is important to understand the financial benefits and risks involved in this production.Financial analysis,therefore,acts as an important analysis tool to foster this type of activity.The paper aimed to conduct analysis of investment risk of a crop-livestock-forestry system deployed in Brazil,comparing two different production scenarios,scenario I with 17 ha and scenario II with 25 ha.The risk analysis was performed using the Monte Carlo method and sensitivity analysis(by varying the factors:the discount rate,productivity and price).A cash flow was elaborated based on annual cost and revenues data of the agricultural crops(corn and soybeans),livestock and eucalyptus,using an interest rate of 6%per year.The results indicated that the optimal age for cutting the eucalyptus was at seven years on both scenarios;scenario I had better return on investment using deterministic and probabilistic methods;scenario I presents higher investments risks;there is a negative relation between discount rate and annualized net present value(ANPV);increased productivity of crops provides greater profitability to the system;there has been an increase in the economic viability of the system,as value has been added to the products.Monte Carlo method and the sensitivity analysis showed to be an appropriate tool to analyze the risk of crop-livestock-forestry systems,making it possible to foresee how the project will respond to possible scenarios.Juliana Baldan Costa Neves Araújo Alvaro Nogueira de Souza Maísa Santos Joaquim Ilvan Medeiros Lustosa Junior Maisa Isabela Rodrigues Lucas Francisco de Dominicis 2019Journal of Agricultural Science and Technology(B)2019,9,2:0
18Evolution of costs of cancer drugs in a Portuguese hospital显示文摘AIM:To analyze the costs of cancer drugs administered in a Portuguese Hospital compared with the Karolinska Institute study.METHODS:To evaluate spending on cancer drugs, we retrospectively analyzed data on the overall costs of cancer drugs, obtained at the Department of Medical Oncology of the Centro Hospitalar de Entre Douro e Vouga, between 2004 and 2010. In this comparative study we selected only drugs belonging to the following groups:chemotherapy, targeted therapy, immunotherapy and endocrine therapy. The selected drugs were further grouped according to their market placement year:≤ 1998, 1999 to 2002, 2003 to 2005, and 2006 to 2010. Drugs used as supportive therapy and bisphosphonates were excluded.RESULTS:The overall costs of cancer drugs increased gradually between 2004 and 2008(from €1911947 to €3666284), with an increase in the number of patients treated during this period. The expenditure decreased in 2009(€3438155) and increased again in 2010(€3673116), but the costs increment was not the same as in previous years. Chemotherapy and targeted therapy were responsible for most of the expenditure. Drugs placed on the national market before 1999 accounted for more than 50% of the expenditure up to 2007. From 2008, these drugs represented less than 50% of the total expenditure. Cancer drugs placed between 1999 and 2002 accounted for 25%-35% of the costs in all the years studied, while drugs placed between 2003 and 2005 accounted for less than 30%. 19Influence of the COVID-19 pandemic in the gastrointestinal oncology setting:An overview显示文摘Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection has been impacting healthcare in various ways worldwide and cancer patients are greatly affected by the coronavirus disease 2019(COVID-19)pandemic.The reorganization of the health facilities in order to supply the high demand resulting from the aforementioned infection as well as the social isolation measures led to impairments for the diagnosis and follow-up of patients with gastrointestinal cancers,which has had an impact on the prognosis of the oncologic patients.In that context,health authorities and organizations have elaborated new guidelines with specific recommendations for the management of individuals with gastrointestinal neoplasms during the pandemic.Of note,oncologic populations seem to be more susceptible to unfavorable outcomes when exposed to SARS-CoV-2 infection and some interactions involving virus,tumor,host immune system and anticancer therapies are probably related to the poorer prognosis observed in those COVID-19 patients.Moreover,vaccination stands out as the main prevention method against severe SARS-CoV-2 infection and some particularities have been observed regarding the seroconversion of vaccinated oncologic patients including those with gastrointestinal malignancies.In this minireview,we gather updated information regarding the influence of the pandemic in the diagnosis of gastrointestinal neoplasms,new recommendations for the management of gastrointestinal cancer patients,the occurrence of SARSCoV-2 infection in those individuals and the scenario of the vaccination against the virus in that population.Breno Bittencourt de Brito Hanna Santos Marques Filipe Antônio França da Silva Maria Luísa Cordeiro Santos Glauber Rocha Lima Araújo Lara de Araujo Valente Fabrício Freire de Melo 2022World Journal of Gastrointestinal Pathophysiology2022,13,5:0
