2Evaluation of modifi ed Alvarado scoring system and RIPASA scoring system as diagnostic tools of acute appendicitis显示文摘BACKGROUND: Acute appendicitis is the most common surgical condition presented in emergency departments worldwide. Clinical scoring systems, such as the Alvarado and modified Alvarado scoring systems, were developed with the goal of reducing the negative appendectomy rate to 5%–10%. The Raja Isteri Pengiran Anak Saleha Appendicitis(RIPASA) scoring system was established in 2008 specif ically for Asian populations. The aim of this study was to compare the modif ied Alvarado with the RIPASA scoring system in Kuwait population.METHODS: This study included 180 patients who underwent appendectomies and were documented as having 'acute appendicitis' or 'abdominal pain' in the operating theatre logbook(unit B) from November 2014 to March 2016. The sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), diagnostic accuracy, predicted negative appendectomy and receiver operating characteristic(ROC) curve of the modified Alvarado and RIPASA scoring systems were derived using SPSS statistical software.RESULTS: A total of 136 patients were included in this study according to our criteria. The cut-off threshold point of the modif ied Alvarado score was set at 7.0, which yielded a sensitivity of 82.8% and a specif icity of 56%. The PPV was 89.3% and the NPV was 42.4%. The cut-off threshold point of the RIPASA score was set at 7.5, which yielded a 94.5% sensitivity and an 88% specif icity. The PPV was 97.2% and the NPV was 78.5%. The predicted negative appendectomy rates were 10.7% and 2.2% for the modif ied Alvarado and RIPASA scoring systems, respectively. The negative appendectomy rate decreased significantly, from 18.4% to 10.7% for the modified Alvarado, and to 2.2% for the RIPASA scoring system, which was a signif icant difference(P<0.001) for both scoring systems.CONCLUSION: Based on the results of this study, the RIPASA score is a simple scoring system with better sensitivity and specif icity than the modif ied Alvarado scoring system in Asian populations. It consists of 14 clinical parameters that can be obtained from a good patient history, clinical examination and laboratory investigations. 3IL28B polymorphism and cytomegalovirus predict response to treatment in Egyptian HCV type 4 patients显示文摘AIM:To test whether the status of positive cytomegalovirus(CMV) DNA detection adds to the predictive value of IL28B and to further categorize C/T allele carriers.METHODS:This study included 166 chronic hepatitis C(CHC) patients who received combined interferon and ribavirin therapy for 48 wk,84 spontaneous hepatitis C virus(HCV) resolvers who were positive for IgG anti-HCV antibody and negative for HCV RNA,and 100 healthy subjects who were negative for both HCV antibodies and RNA as controls.Genomic DNA from peripheral blood was used for IL28B rs.12979860 single nucleotide polymorphism(SNP) and CMV DNA detection.A 139 bp fragment containing IL28B SNP was amplified in all subjects by polymerase chain reaction using a specifically designed primer.Then the IL28B rs.12979860 SNP was detected by restriction fragment length polymorphism(RFLP) genotyping.The presence of CMV DNA was tested by amplification of the gB1 gene using nested polymerase chain reaction.The role of CMV and IL28B rs.12979860 SNP genotypes in determining the response rate to combined interferon therapy and clinical status of patients were statistically analyzed.RESULTS:Current data showed that 67% of patients carrying the IL28B 12979860 C/C allele had a sustained viral response(SVR) while the genotypes C/T and TT were associated with lower SVR rates,50% and 48%,respectively.SVR rates for the C/C allele were lower than other HCV genotypes and/or other populations.Genotype CC was associated with the response to interferon(P = 0.025).Genotype C/C was reduced from 48% in controls to 14% in CHC patients suggesting its protective role against progression to chronicity.The majority of spontaneously cleared subjects(86%) were C/C,confirming its protective role.The C/T allele was present in 71% of CHC patients compared with 38% of controls,so the use of IL28B SNP genotyping only in these patients may be of little value as a predictor of response.CMV reactivation occurred in 40% of CHC patients.Co-infection with CMV seriously diminished the response to interferon(IFN) therapy,with SVR rates in C/C genotypes 87.5% in CMV-negative patients and 12.5% in CMV-positive patients(P < 0.0001).SVR rates among C/T carriers were reduced to < 50% in patients with positive CMV DNA while the non-response rate doubled.These data indicate that a supplemental assay for CMV viremia adds to the prognostic value of IL28B genotyping.CONCLUSION:The results suggest that both genetic(i.e.,spontaneous) and therapeutic(IFN-based therapy) arms are complementary in the battle against HCV.CMV DNA testing may be of value to better predict the response to IFN,particularly in IL28B C/T carriers.Mostafa K El Awady Noha G Bader El Din Ashraf Tabll Yaser El Hosary Ashraf O Abdel Aziz Hesham El Khayat Mohsen Salama Tawfeek H Abdelhafez 2013World Journal of Gastroenterology2013,19,2:8
4Prolonged high-fat-diet feeding promotes non-alcoholic fatty liver disease and alters gut microbiota in mice显示文摘BACKGROUND Non-alcoholic fatty liver disease (NAFLD) has become an epidemic largely due to the worldwide increase in obesity. While lifestyle modifications and pharmacotherapies have been used to alleviate NAFLD, successful treatment options are limited. One of the main barriers to finding safe and effective drugs for long-term use in NAFLD is the fast initiation and progression of disease in the available preclinical models. Therefore, we are in need of preclinical models that (1) mimic the human manifestation of NAFLD and (2) have a longer progression time to allow for the design of superior treatments. AIM To characterize a model of prolonged high-fat diet (HFD) feeding for investigation of the long-term progression of NAFLD. METHODS In this study, we utilized prolonged HFD feeding to examine NAFLD features in C57BL/6 male mice. We fed mice with a HFD (60% fat, 20% protein, and 20% carbohydrate) for 80 wk to promote obesity (Old-HFD group, n = 18). A low-fat diet (LFD)(14% fat, 32% protein, and 54% carbohydrate) was administered for the same duration to age-matched mice (Old-LFD group, n = 15). An additional group of mice was maintained on the LFD (Young-LFD, n = 20) for a shorter duration (6 wk) to distinguish between age-dependent and age-independent effects. Liver, colon, adipose tissue, and feces were collected for histological and molecular assessments.RESULTS Prolonged HFD feeding led to obesity and insulin resistance. Histological analysis in the liver of HFD mice demonstrated steatosis, cell injury, portal and lobular inflammation and fibrosis. In addition, molecular analysis for markers of endoplasmic reticulum stress established that the liver tissue of HFD mice have increased phosphorylated Jnk and CHOP. Lastly, we evaluated the gut microbial composition of Old-LFD and Old-HFD. We observed that prolonged HFD feeding in mice increased the relative abundance of the Firmicutes phylum. At the genus level, we observed a significant increase in the abundance of Adercreutzia, Coprococcus, Dorea, and Ruminococcus and decreased relative abundance of Turicibacter and Anaeroplasma in HFD mice. 5Three-dimensional Localization of Impacted Canines and Root Resorption Assessment Using Cone Beam Computed Tomography显示文摘The purpose of this study was to develop a new way to localize the impacted canines from three dimensions and to investigate the root resorption of the adjacent teeth by using cone beam computed tomography(CBCT). Forty-six patients undergoing orthodontic treatments and having impacted canines in Tongji Hospital were examined. The images of CBCT scans were obtained from KaVo 3D exam vision. Angular and linear measurements of the cusp tip and root apex according to the three planes(mid-sagittal, occlusal and frontal) have been taken using the cephalometric tool of the InVivo Dental Anatomage Version 5.1.10. The measurements of the angular and linear coordinates of the maxillary and mandibular canines were obtained. Using this technique the operators could envision the location of the impacted canine according to the three clinical planes. Adjacent teeth root resorption of 28.26 % was in the upper lateral incisors while 17.39% in upper central incisors, but no lower root resorption was found in our samples. 7超基性岩中锆石年龄的特征和意义——以西南天山UHP蛇纹岩中锆石U-Pb年龄为例显示文摘超基性岩本身难以生长锆石的特性,使得研究其中的锆石需要特别谨慎。超基性岩中的锆石虽然具有多解性,但是锆石也携带了很多演化信息。产出不同地质背景的超基性岩,其中的锆石特征不同。本文总结现有的研究实例表明:(1)经历高温高压变质作用的石榴橄榄岩通常通过交代作用获得锆石,且锆石能够记录峰期变质时代,其中的继承锆石较少,可能在高温高压条件下,继承锆石发生分解重结晶;(2)大洋蛇绿岩型超基性岩和地幔岩捕掳体中通常具有年龄分布很广的锆石年龄特征,锆石年龄峰值通常与区域上构造事件相吻合,为捕掳晶锆石。接下来本文以西南天山超高压(UHP)蛇纹岩为例,对其锆石年龄进行解释。西南天山蛇纹岩为经历过超高压变质作用的大洋蛇绿岩型超基性岩,2个蛇纹岩样品中锆石的阴极发光图像分析和SIMS U-Pb定年分析结果显示,西南天山UHP蛇纹岩中的锆石包含捕掳晶锆石和变质锆石,捕掳晶锆石的年龄为2.1~1.0Ga,对应该区变泥质岩中碎屑锆石记录的年龄峰值。409~537Ma可能代表了蛇纹岩原岩结晶时代。区域上的变质压力峰期年龄(~320Ma)在蛇纹岩中没有记录,仅有1颗锆石记录了309±5Ma的近峰期时代。270~155Ma的退变质时代在西南天山蛇纹岩中出现较广,这与榴辉岩中出现的退变年龄相吻合,代表了折返过程中较为普遍的后期热液事件。基于对超基性岩中锆石特征的初步了解,结合西南天山蛇纹岩的研究实例,认为通过研究锆石的年代学,结合锆石矿物化学、包体矿物学、同位素地球化学等特征,不仅可以提供年代学信息,还可以对超基性岩的来源和演化过程进行解析。申婷婷 张立飞 杨经绥 张聪 邱添 BADER Thomas 2017岩石学报2017,33,12:4

9人免疫缺陷病毒感染者辅助性T淋巴细胞因子变化及机会性感染的临床观察显示文摘目的观察人免疫缺陷病毒(human immunodeficiency virus,HIV)感染者不同时期及其机会性感染者血清辅助性 T 淋巴细胞(helper T-Lymphoeytes,Th)细胞因子水平的变化规律。方法正常对照组17例。研究组 HIV 阳性85例(A 期17例,B 期29例,C 期39例),其中机会性感染31例。用流式细胞仪检测血 CD_4^+T 利 CD_8^+T 细胞,用酶联免疫吸附技术(enzyme linked immunosorbentassay,ELISA)检测血清白介素-2(interleukin-2,IL-2)、γ-干扰素(interferon-γ,IFN-γ)、白介素-6(interleukin-6,IL-6)、白介素-10(interleukin-10,IL-10),数据采用 SPSS11.0软件进行统计。结果研究组 CD_4^+细胞(361.85±230.61)10~6/L 低于对照组(772.41±161.56)10~6/L(t=6.992,P<0.01),IL-2(61.82±63.59)pg/ml 低于对照组(11 1.25±66.14)pg/ml(t=2.907,P<0.01),研究组 CD_8^+T细胞(713.36±317.59)10~6/L 高于对照组(583.24±96.28)10~6/L(t=3.127,P<0.01)、IL-10(1362.70±869.49)pg/ml 高于对照组(818.54±276.22)pg/ml(t=4.704,P<0.01)和 IL-6(1883.14±1058.61)pg/ml 高于对照组(1208.52±745.36)pg/ml(t=2.502,P<0.05)。随着病程进展,IL-2逐渐下降,C 期(51.72±62.28)pg/ml 和 B 期(69.02±62.77)pg/ml 分别低于对照组,而 IL-6、IL-10逐渐上升,C 期的 IL-6(2040.27±1078.95)pg/ml、IL-10(1472.10±982.03)pg/ml 均高于对照组;B期的 IL-10(1347.35±780.95)pg/ml 高于对照组(818.54±276.22)pg/ml。机会性感染组 IL-6(2236.24±1052.42)pg/ml 高于无机会性感染组(1680.43±1017.05)pg/ml(t=2.395,P<0.05)。结论 HIV 感染者应动态检测血清 IL-2、IL-6、IL-10的变化,同时可考虑上调 IL-2和下调 IL-6、IL-10,调整机体 TH1/TH2细胞的平衡,以延缓疾病进展。饶和平 冯磊 李丹 Bader A 吴南屏 2007中华预防医学杂志2007,41,1:4
10HIV感染对记忆型和处女型CD4^+T细胞上CD95的影响显示文摘目的 :了解 CD95在 HIV感染者记忆型和处女型 CD4 + T细胞表面的表达情况 ,探讨 Fas/ CD95在 HIV感染中的作用。方法 :采用流式细胞检测仪 (FACS)对 HIV感染者外周血 CD4 + T细胞表面 CD95、CD4 5 RA、CD4 5 RO的表达进行分析 ,用 EL ISA检测血清中 Fas水平。结果 :HIV感染后 ,血清 Fas水平随疾病的进展而逐渐升高 ,同时伴有处女型 T细胞表面 CD95表达增高以及记忆型 T细胞 CD95表达逐渐下降。结论 :Fas参与了 HIV的感染过程。研究 Fas和 CD95、CD4 5 RA、CD4 5 RO之间的关系 ,可以加深对 HIV致病机制的认识 ,并可了解疾病的进展情况和指导治疗。吴南屏 李丹 Armin Bader Hoxtermann Stefan Norbert Brockmeyer 2003浙江大学学报(医学版)2003,32,2:4
11Sustained virological response based on rapid virological response in genotype-3 chronic hepatitis C treated with standard interferon in the Pakistani population显示文摘AIM:To document the sustained virological response (SVR) in rapid virological responders (RVR) of genotype-3 chronic hepatitis C with standard interferon (SdIF). METHODS:Hepatitis C genotype-3 patients during the period July 2006 and June 2007 were included. Complete blood counts, prothrombin time, ALT, albumin, qualitative HCV RNA were done. SdIF and ribavirin were given for 4 wk and qualitative HCV RNA was repeated. Those testing negative were allocated to group-A while the rest were allocated to group-B. Treatment was continued a total of 16 and 24 wk for group A and B respectively. HCV RNA was repeated after 24 wk of treatment. End virological and sustained virological responses were compared by χ2 test. ROC of pretreatment age, ALT and albumin were plotted for failure to achieve SVR. RESULTS:Of 74 patients treated, RCV RNA after 16 wk of therapy became undetectable in 34 (45.9%) and was detectable in 40 (54.1%) and were allocated to groups A and B respectively. SVR was achieved in 58.8% and 27.8% in groups A and B respectively. SVR rates were significantly higher in patients who had RVR as compared to those who did not (P = 0.0;γ = 2). Both groups combined ETR and SVR were 70% and 33% respectively. 13Tumor necrosis factor-α-G308A polymorphism is associated with liver pathological changes in hepatitis C virus patients显示文摘AIM To investigate the association of tumor necrosis factor alpha(TNFα)-G308 A polymorphism with different liver pathological changes in treatment-na?ve Egyptian patients infected with hepatitis C virus(HCV) genotype 4.METHODS This study included 180 subjects,composed of 120 treatment-na?ve chronic HCV patients with different fibrosis grades(F0-F4) and 60 healthy controls. The TNFα-G308 A region was amplified by PCR and the different genotypes were detected by restriction fragment length polymorphism analysis. The TNFα protein was detected by enzyme-linked immunosorbent assay. The influence of different TNFα-G308 A genotypes on TNFα expression and liver disease progression were statistically analyzed. The OR and 95%CI were calculated to assess the relative risk confidence.RESULTS Current data showed that the TNFα-G308 A SNP frequency was significantly different between controls and HCV infected patients(P = 0.001). Both the AA genotype and A allele were significantly higher in late fibrosis patients(F2-F4,n = 60) than in early fibrosis patients(F0-F1,n = 60)(P = 0.05,0.04 respectively). Moreover,the GA or AA genotypes increased the TNFα serum level greater than the GG genotype(P = 0.002). The results showed a clear association between severe liver pathological conditions(inflammation,steatosis and fibrosis) and(GA + AA) genotypes(P = 0.035,0.03,0.04 respectively). The stepwise logistic regression analysis showed that the TNFα genotypes(GA + AA) were significantly associated with liver inflammation(OR = 3.776,95%CI: 1.399-10.194,P = 0.009),severe steatosis(OR = 4.49,95%CI: 1.441-14.0,P = 0.010) and fibrosis progression(OR = 2.84,95%CI: 1.080-7.472,P = 0.034). Also,the A allele was an independent risk factor for liver inflammation(P = 0.003),steatosis(P = 0.003) and fibrosis(P = 0.014). 15Cranial variation in allactagine jerboas(Allactaginae,Dipodidae, Rodentia): a geometric morphometric study显示文摘Allactaginae is a subfamily of dipodids consisting of four-and five-toed jerboas(Allactaga, Allactodipus,Orientallactaga, Pygeretmus, Scarturus) found in open habitats of Asia and North Africa. Recent molecular phylogenies have upended our understanding of this group's systematics across taxonomic scales. Here, I used cranial geometric morphometrics to examine variation across 219 specimens of 14 allactagine species(Allactaga major, A. severtzovi, Orientallactaga balikunica, O.bullata, O. sibirica, Pygeretmus platyurus, P. pumilio,P. shitkovi, Scarturus aralychensis, S. euphraticus,S. hotsoni, S. indicus, S. tetradactylus, S. williamsi)in light of their revised taxonomy. Results showed no significant sexual size or shape dimorphism. Species significantly differed in cranial size and shape both overall and as species pairs. Species identity had a strong effect on both cranial size and shape. Only a small part of cranial shape variation was allometric,with no evidence of unique species allometries, and most specimens fit closely to the common allometric regression vector. Allactaga was the largest,followed by Orientallactaga, Scarturus, and finally Pygeretmus. Principal component 1(PC1) separated O. bullata+O. balikunica+S. hotsoni(with inflated bullae along with reduced zygomatic arches and rostra) from A. major+A. severtzovi+O. sibirica(with converse patterns), while PC2 differentiated Orientallactaga(with enlarged cranial bases and rostra along with reduced zygomatic arches and foramina magna) from Scarturus+Pygeretmus(with the opposite patterns). Clustering based on the unweighted pair group method with arithmetic mean(UPGMA) contained the four genera, but S. hotsoni clustered with O. bullata+O. balikunica and O.sibirica clustered with A. major+A. severtzovi, likely due to convergence and allometry, respectively.Bader H.Alhajeri 2021Zoological Research2021,42,2:3
16艾滋病相关的卡波西肉瘤免疫激活状态研究显示文摘目的 :进一步探讨免疫功能改变在艾滋病相关的卡波西肉瘤 (AIDS- KS)发病中的作用。方法 :对 12例AIDS- KS,32例无卡波西肉瘤的 HIV感染者 (HIV- NKS)和 16例正常对照进行了研究 ,用 EL ISA对 8例 AIDS- KS,2 8例 HIV- NKS和 16例正常对照血清 (或血浆 )进行了 s Fas,β 2 -微球蛋白 (β 2 - MG) ,IL - 10 ,IL- 16 ,IL- 18,IL- 6和可溶性 IL - 4受体 (s IL- 4R)测定 ,对 12例 AIDS- KS,32例 HIV- NKS外周血淋巴细胞、淋巴细胞亚群及 CD38+CD8,HLA- DR+ CD8进行了分析。结果 :β 2 - MG,s IL- 4R水平在 HIV- NKS组明显高于正常对照组 ,IL- 16水平在HIV- NKS组明显低于正常对照组 ,IL - 18水平在 HIV- NKS组和 AIDS- KS组均明显高于正常对照组 ;CD3、CD4、CD8、NK、HLA- DR+ CD8在 AIDS- KS均低于 HIV- NKS,而 AIDS- KS组 CD19、CD38+ CD8高于 HIV- NKS组 ,但各组间差异无显著性。结论 :AIDS- KS和 HIV- NKS一样存在一定程度的免疫激活 ,但 AIDS- KS和 HIV- NKS间免疫状态无明显差异 ,提示免疫状态的差异可能不是艾滋病患者并发卡波西肉瘤的主要原因。朱彪 吴南屏 Hoxtermann Stefan Armin Bader Norbert Brockmeyer 2003浙江大学学报(医学版)2003,32,2:2

18HIV和HCV感染者重叠感染HGV对病毒复制的影响显示文摘目的 :了解人免疫缺陷病毒 (HIV)感染者和丙型肝炎病毒 (HCV)感染者重叠感染庚型肝炎病毒 (HGV)的情况 ,探讨重叠感染病毒在体内的相互作用机理。方法 :用定量 PCR测定 HIV和 HCV感染者血浆中病毒载量 ,同时用 RT- PCR检测这些患者 HGV的感染情况 ,并对部分 HGV阳性者进行序列测定。结果 :317例 HIV患者中检出 HGV阳性 12 3例 ,阳性率为 38.8% ;91例 HCV患者中 HGV阳性 19例 ,阳性率为 2 0 .9% ,统计学分析有显著差异。进一步研究显示 ,HIV和 HGV重叠感染中 HIV病毒载量明显低于单独 HIV感染者 [(1.8± 0 .6 )× 10copies/ ml vs(1.9± 1.1)× 10 2 copies/ ml];而 HCV和 HGV重叠感染者中 HCV的病毒载量与单独 HCV感染者比较没有显著差异 [(1.5± 0 .6 )× 10 4copies/ ml vs(5 .4± 1.8)× 10 4copies/ ml]。序例分析表明 ,与 HIV或 HCV重叠感染的 HGV来源于同一病毒株。结论 :HIV感染者有很高的 HGV重叠感染率 ,而且 HGV感染能明显抑制 HIV在体内的病毒复制。徐陈槐 黄晓燕 沃健尔 吴南屏 Armin Bader 2003浙江大学学报(医学版)2003,32,2:2
19Is the Songshugou Complex,Qinling Belt,China,an Eclogite Facies Neoproterozoic Ophiolite?显示文摘Ophiolites play a key role in understanding subduction-accretion-collision processes. Herein, we discuss origin and metamorphic evolution of an enigmatic, Neoproterozoic ophiolite candidate—the mafic-ultramafic Songshugou Complex, Qinling belt, China—summarizing published thermobarometr);U/Pb geochronology, and geochemistry and presenting new phase equilibrium modeling. Garnet, rarely preserved in amphibolites of the Songshugou Complex, has prograde zoning and low-pvrope cores |Alm54-71(Grs+And)25-30PrP1-6Sps5-12. It formed at quartz eclogite facies conditions of 1.93-2.54 GPa, 462-542℃. During exhumation, garnet first was mantled by plagioclase-rich coronas at about 0.7-1.2 GPa, 660-710℃. During an isothermal uplift to 0.5-0.8 GPa, these coronas evolved widely into n-shaped aggregates and eventually into whitish ribbons oriented with a steeply southwest dipping mineral stretching lineation. The exhumation into middle-upper crustal levels proceeded till the Late Devonian. The oceanic protoliths of the eclogites were emplaced into continental crust in the Neoproterozoic and dragged into a subduction zone in North Qinling in the Cambrian. The ultramafic rocks of the Songshugou Complex were not subducted with the mafic rocks in a coherent block given the absence of garnet but ubiquitous occurrence of spinel implies a P maximum of ?1.7 GPa. Rather, mafic and ultramafic rocks belonged to downgoing and overriding plate, respectively.
