| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 4 | The characteristics of atmospheric CO_2 concentration variation of four national background stations in China显示文摘Atmospheric carbon dioxide is an important kind of greenhouse gas which influences global temperature. Its concentration variation could indicate the distribution of human and natural activities in various regions. Through the non-dispersive infrared method, flask sampling of atmospheric CO2 concen- tration was measured weekly at four national background stations including Waliguan, Shangdianzi, Lin’an, and Longfengshan. Based on the data collected from September 2006 to August 2007, along with the Waliguan station’s experience on in situ observational data processing, the selection methods for sampling data through the atmospheric background CO2 concentration analysis were preliminarily discussed. On the basis of this result, the variation features of the four typical regions’ atmospheric background CO2 concentration was analyzed for the first time. The results show that the atmospheric CO2 concentration at Waliguan, Shangdianzi, Lin’an, and Longfengshan is 383.5, 385.9, 387.8, and 384.3 ppm, respectively. During the research period, CO2 concentration at the Waliguan station changed slightly. However, the CO2 concentration changed sharply at the Shangdianzi and the Lin’an stations due to the great influence of human activities in the Jingjinji and the Changjiang Delta economic zones, and changed regularly with seasons at Longfengshan station under dual influences of human activities and plant photosynthesis. | 8 | Predictive value of D-dimer for portal vein thrombosis after portal hypertension surgery in hepatitis B virus-related cirrhosis显示文摘AIM: To evaluate the predictive value of D-dimer as a predictive indicator of portal vein thrombosis (PVT) after portal hypertension surgery in hepatitis B virus-related cirrhosis. METHODS: A prospective study was carried out in 52 patients who had undergone surgery for portal hypertension in hepatitis B virus-related cirrhosis. Changes in perioperative dynamic D-dimer were observed. The sensitivity, specifi city, positive predictive values and negative predictive values of D-dimer were calculated, and ROC curves were analyzed. RESULTS: The D-dimer levels in the group developing postoperative PVT was signifi cantly higher than those in the group not developing PVT (P = 0.001), and the ROC semi-quantitative and qualitative analysis of D-dimer showed a moderate predictive value in PVT (semi- quantitative value Az = 0.794, P = 0.000; qualitative analysis: Az = 0.739, P = 0.001). | 10 | The first year follow-up after colorectal adenoma polypectomyis important: A multiple-center study in symptomatichospital-based individuals in China显示文摘The recurrence of colorectal adenoma(CRA)is high.Although there are guidelines for colonoscopy surveillance after polypectomy in other countries,little is known about its recurrence rate and recurrence peak,especially in China.The aim of the present research is to investigate how long after polypectomy follow-up should take and to analyze risk factors of recurrence.1208 patients who received polypectomies fromfive clinical research centers in four regions of China(Shanghai,Guangzhou,Nanjing and Beijing)were included.They were divided into 4 groups:group A(follow-up≤1 year after poly-pectomy),group B(follow-up 2–3 years after polypect-omy),group C(follow-up 4–5 years after polypectomy),and group D(follow-up>5 years after polypectomy).The sex,age,adenoma location,size,number,and pathological characteristics were compared.On the whole,the recur-rence rate was 59.46%in group A,61.09%in group B,78.07%in group C,and 87.12%in group D,which indicated an increased tendency with a prolonged follow-up duration.There was a significant difference between group A and C or D,and between group B and C or D(P<0.01),but there was no statistical difference between group A and B.Additionally,the recurrent patients in thefirst year had a recurrence rate of 97.33%in thefirst three years(59.46/61.09),which means that the peak of recurrence was almost entirely concentrated in thefirst year.The recurrence rate was higher in males and the elder.The risk factors included multiple numbers,villous feature,high-grade dysplasia of medium or smaller size and location in the distal colon.In conclusion,the peak of recurrence was almost totally concentrated in thefirst year;meanwhile,thefirst year follow-up is of critical importance in China.It may not be necessary to do the follow-up examination during the second and third years,but after three years,another colonoscopy should be undertaken. | Qin-Yan GAO Hui-Min CHEN Jian-Qiu SHENG Ping ZHENG Cheng-Gong YU Bo JIANG Jing-Yuan FANG | 2010 | Frontiers of Medicine2010,4,4: | 17 |

| 15 | Study of atmospheric CO2 and CH4 at Longfengshan WMO/GAW regional station: The variations, trends, influence of local sources/sinks, and transport显示文摘Atmospheric CO_2 and CH_4 have been continuously measured since 2009 at Longfengshan WMO/GAW station(LFS) in China. Variations of the mole fractions, influence of long-distance transport, effects of local sources/sinks and the characteristics of synoptic scale variations have been studied based on the records from 2009 to 2013. Both the CO_2 and CH_4 mole fractions display increasing trends in the last five years, with growth rates of 3.1±0.02 ppm yr.1 for CO_2 and 8±0.04 ppb yr.1(standard error, 1-σ)for CH_4. In summer, the regional CO_2 mole fractions are apparently lower than the Marine Boundary Layer reference, with the lowest value of.13.6±0.7 ppm in July, while the CH_4 values are higher than the MBL reference, with the maximum of 139±6 ppb.From 9 to 17(Local time, LT) in summer, the atmospheric CO_2 mole fractions at 10 m a.g.l. are always lower than at 80 m, with a mean difference of.1.1±0.2 ppm, indicating that the flask sampling approach deployed may underestimate the background mole fractions in summer. In winter, anthropogenic emissions dominate the regional CO_2 and CH_4 mole fractions. Cluster analysis of backward trajectories shows that atmospheric CO_2 and CH_4 at LFS are influenced by anthropogenic emissions from the southwest(Changchun and Jilin city) all year. | 17 | Melittin induces human gastric cancer cell apoptosis via activation of mitochondrial pathway显示文摘AIM: To investigate the apoptotic effects of melittin on SGC-7901 cells via activation of the mitochondrial signaling pathway in vitro.METHODS: SGC-7901 cells were stimulated by melittin, and its effect on proliferation and apoptosis of was investigated by methyl thiazolyl tetrazolium assay, morphologic structure with transmission electron microscopy, annexin-V/propidium iodide double-staining assay, measuring mitochondrial membrane potential(MMP) levels, and analyzing reactive oxygen species(ROS) concentrations were analyzed by flow cytometry. Cytochrome C(Cyt C), apoptosis-inducing factor(AIF), endonuclease G(Endo G), second mitochondria-derived activator of caspases(Smac)/direct IAP binding protein with low isoelectric point(Diablo), and FAS were analyzed by western blot. The expression of caspase-3 and caspase-8 was measured using activity assay kits.RESULTS: Melittin was incubated at 1.0, 2.0, 4.0, or 6.0 μg/m L for 1, 2, 4, 6, or 8 h and showed a timeand concentration-dependent inhibition of SGC-7901 cell growth. Melittin induced SGC-7901 cell apoptosis, which was confirmed by typical morphological changes. Treatment with 4 μg/m L melittin induced early apoptosis of SGC-7901 cells, and the early apoptosis rates were 39.97% ± 3.19%, 59.27% ± 3.94%, and 71.50% ± 2.87% vs 32.63% ± 2.75% for 1, 2, and 4 h vs 0 h(n = 3, P < 0.05); the ROS levels were 616.53% ± 79.78%, 974.81% ± 102.40%, and 1330.94% ± 93.09% vs 603.74% ± 71.99%(n = 3, P < 0.05); the MMP values were 2.07 ± 0.05, 1.78 ± 0.29, and 1.16 ± 0.25 vs 2.55 ± 0.42(n = 3, P < 0.05); caspase-3 activity was significantly higher compared to the control(5492.3 ± 321.1, 6562.0 ± 381.3, and 8695.7 ± 449.1 vs 2330.0 ± 121.9), but the caspase activity of the non-tumor cell line L-O2 was not different from that of the control. With the addition of the caspase-3 inhibitor(Ac-DEVD-CHO), caspase-3 activity was significantly decreased compared to the control group(1067.0 ± 132.5 U/g vs 8695.7 ± 449.1 U/g). The expression of the Cyt C, Endo G, and AIF proteins in SGC-7901 cells was significantly higher than those in the control(P < 0.05), while the expression of the Smac/Diablo protein was significantly lower than the control group after melittin exposure(P < 0.01). | 18 | Epidemiological studies of esophageal cancer in the era of genome-wide association studies显示文摘Esophageal cancer(EC) caused about 395000 deaths in 2010. China has the most cases of EC and EC is the fourth leading cause of cancer death in China. Esophageal squamous cell carcinoma(ESCC) is the predominant histologic type(90%-95%), while the incidence of esophageal adenocarcinoma(EAC) remains extremely low in China. Traditional epidemiological studies have revealed that environmental carcinogens are risk factors for EC. Molecular epidemiological studies revealed that susceptibility to EC is influenced by both environmental and genetic risk factors. Of all the risk factors for EC, some are associated with the risk of ESCC and others with the risk of EAC. However, the details and mechanisms of risk factors involved in the process for EC are unclear. The advanced methods and techniques used in human genome studies bring a great opportunity for researchers to explore and identify the details of those risk factors or susceptibility genes involved inthe process of EC. Human genome epidemiology is a new branch of epidemiology, which leads the epidemiology study from the molecular epidemiology era to the era of genome wide association studies(GWAS). | 19 | Prediction of Primary Climate Variability Modes at the Beijing Climate Center显示文摘Climate variability modes, usually known as primary climate phenomena, are well recognized as the most important predictability sources in subseasonal–interannual climate prediction. This paper begins by reviewing the research and development carried out, and the recent progress made, at the Beijing Climate Center(BCC) in predicting some primary climate variability modes. These include the El Ni?o–Southern Oscillation(ENSO), Madden–Julian Oscillation(MJO), and Arctic Oscillation(AO), on global scales, as well as the sea surface temperature(SST) modes in the Indian Ocean and North Atlantic, western Pacific subtropical high(WPSH), and the East Asian winter and summer monsoons(EAWM and EASM, respectively), on regional scales. Based on its latest climate and statistical models, the BCC has established a climate phenomenon prediction system(CPPS) and completed a hindcast experiment for the period 1991–2014. The performance of the CPPS in predicting such climate variability modes is systematically evaluated. The results show that skillful predictions have been made for ENSO, MJO, the Indian Ocean basin mode, the WPSH, and partly for the EASM, whereas less skillful predictions were made for the Indian Ocean Dipole(IOD) and North Atlantic SST Tripole, and no clear skill at all for the AO, subtropical IOD, and EAWM. | 20 | Evaluation of prognostic markers in severe drug-induced liver disease显示文摘AIM: To analyze the outcome of patients with severe drug-induced liver disease (DILD) associated with jaundice classified as hepatocellular, cholestatic or mixed liver injury and to evaluate the validity of Hy’s rule and the most important predictors for outcome. METHODS: The Adverse Drug Reaction Advisory Committee was set up in 1997 in our hospital to identify all suspicions of DILD following a structured prospective report form. Liver damage was divided into hepatocellular, cholestatic, and mixed types according to laboratory and histologic criteria when available. Further evaluation of causality assessment was performed. RESULTS: From January 1997 to December 2004, 265 patients were diagnosed with DILD,and 140 (52.8%) of them were female. hepatocellular damage was the most common (72.1%), the incidence of death was 9.9% in patients with hepatocellular damage and 9.5% in patients with cholestatic/mixed damage (P < 0.05). There was no difference in age of dead and recovered patients. The proportion of females and males was similar in recovered and dead patients, no difference was observed in duration of treatment between the two groups. The serum total bilirubin (P < 0.001), direct bilirubin (P < 0.001) and aspartate transaminase (AST) (P = 0.013) values were higher in dead patients than in recovered patients. Chinese herbal medicine was the most frequently prescribed, accounting for 24.2% of the whole series. However, antitubercular drugs (3.4%) were found to be the primary etiological factor for fetal DILD. Factors associated with the development of fulminanthepatic failure were hepatic encephalopathy (OR = 43.66, 95% CI = 8.47-224.95, P < 0.0001), ascite (OR = 28.48, 95% CI = 9.26-87.58, P < 0.0001), jaundice (OR = 11.43, 95% CI = 1.52-85.96, P = 0.003), alcohol abuse (OR = 3.83, 95% CI = 1.26-11.67, P = 0.035) and direct bilirubin (OR = 1.93, 95% CI = 1.25-2.58, P = 0.012). CONCLUSION: Death occurs in 9.8% of patients with DILD. Chinese herbal medicine stands out as the most common drug for DILD.
