14WONCA研究论文摘要汇编(六)——老年人甲状腺功能不全5年发病率和病情进展显示文摘背景很少有几个研究单纯基于老年人这个群组患甲状腺功能不全的发病和病情进展做评估。本研究的目标是评估澳大利亚老年人甲状腺功能不全5年发病率、进展和患甲状腺功能不全的风险因素。方法蓝岭眼科研究是纵向cohort定群研究。1997-9,有1768名参试者(≥55岁)做了甲状腺功能评估。排除了有任何因甲状腺情况做了治疗的参试者后,951名参试者(91.4%)无甲状腺功能不全,54(5.4%)5年后检查出甲状腺功能不全。甲状腺功能不全的确诊,采用了血清促甲状腺素(TSH)的筛查,接着评估血清游离甲状腺素T4(FT4)。结果甲状腺功能不全5年的总发病率为4.7%[95%CI(3.4,6.1)]。肥胖症(BMI≥30kg/m2)和基线血清TSH>2mIU/L可明确预示着会发生甲状腺功能减退,分别为[OR=4.05,CI(1.74,9.41)]和[OR=5.46,CI(1.16,25.67)]。患亚临床型甲状腺功能减退症的女性明显高于男性,2.5%vs0.7%(P=0.03)。观察发现,患临床型甲状腺机能减退症的受试者中有17.9%经5年会进展为甲状腺机能减退。结论这一老年人群甲状腺机能不全发病率相对低,与肥胖症和血清TSH基线水平>2mIU/L相关。患亚临床型甲状腺机能减退症6人中有1人5年进展为甲状腺功能不全。我们的发现亮化了对患亚临床型甲状腺机能减退症的老年病人需采取恰当的管理。Bamini Gopinath Jie Jin Wang Annette Kifley 周淑新 2010中国全科医学2010,13,7:0
