| 1 | Relationship between intestinal microbiota and ulcerative colitis:Mechanisms and clinical application of probiotics and fecal microbiota transplantation显示文摘Ulcerative colitis(UC) is an inflammatory disease that mainly affects the colon and rectum. It is believed that genetic factors, host immune system disorders, intestinal microbiota dysbiosis, and environmental factors contribute to the pathogenesis of UC. however, studies on the role of intestinal microbiota in the pathogenesis of UC have been inconclusive. Studies have shown that probiotics improve intestinal mucosa barrier function and immune system function and promote secretion of anti-inflammatory factors, thereby inhibiting the growth of harmful bacteria in the intestine. Fecal microbiota transplantation(FMT) can reduce bowel permeability and thus the severity of disease by increasing the production of short-chain fatty acids, especially butyrate, which help maintain the integrity of the epithelial barrier. FMT can also restore immune dysbiosis by inhibiting Th1 differentiation, activity of T cells, leukocyte adhesion, and production of inflammatory factors. Probiotics and FMT are being increasingly used to treat UC, but their use is controversial because of uncertain efficacy. | 2 | Notch signaling mediated by TGF-β/Smad pathway in concanavalin A-induced liver fibrosis in rats显示文摘AIM To explore the exact interaction between Notch and transforming growth factor(TGF)-β signaling in liver fibrosis. METHODS We established a rat model of liver fibrosis induced by concanavalin A. Peripheral blood mononuclear cells(PBMCs) were isolated from the modeled rats, and cultured with γ-secretase inhibitor DAPT and TGF-β inhibitor for 24 h. The m RNA levels of Notch and TGF-β signaling were detected by quantitative real-time polymerase chain reaction. Expression of Notch and TGF-β proteins was analyzed by western blotting.RESULTS Compared to control rats, Notch and TGF-β signaling was activated in PBMCs of model rats. Administration of DAPT and TGF-β inhibitor suppressed Notch and TGF-β signal transducer in PBMCs of model rats. DAPT reduced m RNA and protein expression of TGF-β signaling, such as TGF-β1 and Smad3. TGF-β inhibitor also downregulated Notch1, Hes1 and Hes5, and m RNA and protein expression of the Notch signaling pathway.CONCLUSION Notch and TGF-β signaling play a role in liver fibrosis. | 3 | Performance and comparison of assessment models to predict 30-day mortality in patients with hospital-acquired pneumonia显示文摘Background:Hospital-acquired pneumonia(HAP)is the most common hospital-acquired infection in China with substantial morbidity and mortality.But no specific risk assessment model has been well validated in patients with HAP.The aim of this study was to investigate the published risk assessment models that could potentially be used to predict 30-day mortality in HAP patients in non-surgical departments.Methods:This study was a single-center,retrospective study.In total,223 patients diagnosed with HAP from 2012 to 2017 were included in this study.Clinical and laboratory data during the initial 24 hours after HAP diagnosis were collected to calculate the pneumonia severity index(PSI);consciousness,urea nitrogen,respiratory rate,blood pressure,and age≥65 years(CURB-65);Acute Physiology and Chronic Health Evaluation II(APACHE II);Sequential Organ Failure Assessment(SOFA);and Quick Sequential Organ Failure Assessment(qSOFA)scores.The discriminatory power was tested by constructing receiver operating characteristic(ROC)curves,and the areas under the curve(AUCs)were calculated.Results:The all-cause 30-day mortality rate was 18.4%(41/223).The PSI,CURB-65,SOFA,APACHE II,and qSOFA scores were significantly higher in non-survivors than in survivors(all P<0.001).The discriminatory abilities of the APACHE II and SOFA scores were better than those of the CURB-65 and qSOFA scores(ROC AUC:APACHE II vs.CURB-65,0.863 vs.0.744,Z=3.055,P=0.002;APACHE II vs.qSOFA,0.863 vs.0.767,Z=3.017,P=0.003;SOFA vs.CURB-65,0.856 vs.0.744,Z=2.589,P=0.010;SOFA vs.qSOFA,0.856 vs.0.767,Z=2.170,P=0.030).The cut-off values we defined for the SOFA,APACHE II,and qSOFA scores were 4,14,and 1.Conclusions:These results suggest that the APACHE II and SOFA scores determined during the initial 24 h after HAP diagnosis may be useful for the prediction of 30-day mortality in HAP patients in non-surgical departments.The qSOFA score may be a simple tool that can be used to quickly identify severe infections. | Jia-Ning Wen Nan Li Chen-Xia Guo Ning Shen Bei He | 2020 | Chinese Medical Journal2020,,24: | 7 |
| 4 | Tissue microarrays in pathological examination of apoptotic acinar cells induced by dexamethasone in the pancreas of rats with severe acute pancreatitis显示文摘BACKGROUND:The good therapeutic effects of large dose of dexamethasone on severe acute pancreatitis(SAP) patients have been proved.This study was designed to investigate the influence of dexamethasone on apoptosis of acinar cells in the pancreas of rats with SAP and the protein expression of the apoptosis-regulating genes Bax and Bcl-2. METHODS:Ninety Sprague-Dawley rats with SAP were randomly divided into a model group and a dexamethasone treated group(45 rats in each group),and another 45 rats formed the sham operation group.Survival rates were calculated and gross pathological changes in the pancreas of each group were observed under a light microscope 3,6 and 12 hours after operation.Tissue microarray technology was applied to prepare pancreatic tissue sections.The changes in Bax and Bcl-2 protein expression levels of pancreatic tissues from each group were assessed by immunohistochemical staining,and TUNEL staining was used to evaluate changes in apoptosis index. RESULTS:The model and treated groups did not differ in mortality at each time point.The pathological score for the pancreas in the treated group was significantly lower than that in the model group at 3 and 6 hours.The positive rates of Bax protein expression in the head and tail of the pancreas in the treated group at all time points were all markedly higher than those of the model group. The positive rate of Bcl-2 protein expression in the head of the pancreas in the treated group was significantly higher than that of the model group at 3 hours.TUNEL staining showed that the pancreas head and tail apoptosis indices of the treated group were markedly higher than those of the model group after 6 hours. CONCLUSIONS:Apoptosis may be a protective response to pancreatic cell injury.The mechanism of action of dexamethasone in treating SAP may be related to the apoptosis of acinar cells in the pancreas induced by apoptosis-regulating genes such as Bax and Bcl-2. | 5 | Easily manageable prognostic factors in 152 Chinese elderly acute myeloid leukemia patients:a single-center retrospective study显示文摘We retrospectively investigated the prognostic factors of acute myeloid leukemia(AML) in 152 Chinese patients with de novo AML who were older than 60 years of age and who received treatment at our hospital.Log-rank test showed that 6 parameters including older age,higher white blood cell(WBC) counts,lactate dehydrogenase(LDH)and bone marrow(BM) blasts at diagnosis,unfavorable risk cytogenetics,and non-mutated CEBPα were significant adverse prognostic factors of overall survival(OS) for elderly AML patients(P = 0.0013,0.0358,0.0132,0.0242,0.0236 and 0.0130,respectively).Moreover,older age and higher LDH were significant adverse predictors for relapse-free survival(RFS)(P = 0.0447 and 0.0470,respectively).Univariate analysis revealed similar results for OS to those of the log-rank test and only higher LDH at diagnosis was a significant adverse predictor for RFS(P = 0.028,HR:1.979,95%CI:1.075-3.644).In multivariate analysis,we identified 2 trends towards independent prognostic factors for OS,including BM blasts at diagnosis(P = 0.057,HR:1.676,95%CI:0.984-2.854)and mutation status of CEBPα(P = 0.064,HR:4.173,95%CI:0.918-18.966).Our data indicated that older age,gender and a previous history of hematologic diseases resulted in lower complete remission rate(P = 0.012,0.051 and 0.086,respectively).We further developed an easy scoring system for predicting prognosis and response to induction therapy in older AML patients.Patients who had lower scores showed significantly longer OS and RFS(P = 0.0006 and 0.1001,respectively) and higher CR rate(P = 0.014).Our research is limited by its retrospective nature and the results from our study need to be further validated by prospective randomized clinical trials. | Jiadai Xu Tingmei Chen Yun Liu Huayuan Zhu Wei Wu WenYi Shen Bei Xu Sixuan Qian Jianyong Li Peng Liu | 2014 | The Journal of Biomedical Research2014,28,5: | 7 |

| 15 | Solvent polarity-induced photoluminescence enhancement (SPIPE): A method enables several-fold increase in quantum yield of silicon nanoparticles显示文摘Fluorescent silicon nanoparticles (SiNPs) bring exciting opportunities for long-awaited silicon-based optical application, while intrinsic indirect band gap of silicon severely limits photoluminescent quantum yield (PLQY) of SiNPs. To address this critical issue, we herein demonstrate a facile and general method, Le., solvent polarity-induced photoluminescence enhancement (SPIPE)1 yielding several-fold increase in quantum yield (QY) of SiNPs. Typically, different kinds of 4-substituented-1,8-naphthalic anhydride molecules, Le., 4-Br-1,8-naphthalic anhydride (BNA)1 4-triphenylamino-1,8-naphthalic anhydride (TPNA)1 and 4-dimethylamino-1,8-naphthalic anhydride (DMNA)1 are rationally designed and synthesized, which serve as surface ligands for the production of BNA-, TPNA-, and DMNA-capped small-sized (diameter:- 3.8-5.8 nm) SiNPs with QY of^8%,~15%,~16%, respectively. Of particular significance, QY of the resultant SiNPs could be greatly enhanced from^10% to^50% through the SPlPE strategy. Taken together with the theoretical calculation and the results of time-correlated single photon counting, we reveal that actived excited-state charge transfer interactions between surface-covered ligand and silicon oxide coating would be responsible for the observed QY enhancement.
