| 1 | How to manage inflammatory bowel disease during the COVID-19 pandemic:A guide for the practicing clinician显示文摘Managing inflammatory bowel disease(IBD)during the coronavirus disease 2019(COVID-19)pandemic has been a challenge faced by clinicians and their patients,especially concerning whether to proceed with biologics and immunosuppressive agents in the background of a global outbreak of a highly contagious new coronavirus(severe acute respiratory syndrome coronavirus 2,SARS-CoV-2).The knowledge about the impact of this virus on patients with IBD,although it is still scarce,is rapidly evolving.In particular,concerns surrounding medications’impact for IBD on the risk of acquiring SARS-CoV-2 infection or developing COVID-19,and potentially exacerbate viral replication and the COVID-19 course,are a current thinking of both practicing clinicians and providers caring for patients with IBD.Managing patients with IBD infected with SARS-CoV-2 depends on both the clinical activity of the IBD and the occasional development and severity of COVID-19.In this review,we summarize the current data regarding gastrointestinal involvement by SARS-CoV-2 and pharmacologic and surgical management for IBD concerning this infection,and the COVID-19 impact on both the patient's psychological functioning and endoscopy services,and we concisely summarize the telemedicine roles during the COVID-19 pandemic. | Júlio Maria Fonseca Chebli Natália Sousa Freitas Queiroz Adérson Omar Mourão Cintra Damião Liliana Andrade Chebli Márcia Henriques de Magalhães Costa Rogério Serafim Parra | 2021 | World Journal of Gastroenterology2021,27,11: | 2 |
| 2 | Serum vitamin D content is associated with semen|parameters and serum testosterone levels in men显示文摘The present study aimed to evaluate the influence of serum vitamin D levels on semen quality and testosterone levels.This is a crosssectional study conducted at Androscience,Science and Innovation Center in Andrology and High-Complex Clinical and Andrology Laboratory in Sao Paulo,Brazil,with 508 male patients,aged 18-60 years,from 2007 to 2017.Seminal parameters and serum sexual hormones were correlated with serum vitamin D concentrations in 260 men selected by strict selection criteria.Patients were divided into normozoospermic group(NZG,n=124)and a group with seminal abnormalities(SAG,n=136).Evaluation included complete physical examination,past medical history,habits and lifestyle factors,two complete seminal analysis with sperm functional tests,serum levels of 25-hydroxy-vitamin D_(3)(25(OH)VD_(3)),total and free testosterone,luteinizing hormone(LH),follicle-stimulating hormone(FSH),sex hormone-binding globulin(SHBG),total cholesterol,homeostatic model assessment of insulin resistance(HOMA-IR)index,and karyotype.The mean concentration of 25(OH)VD_(3)was significantly lower in the SAG(P<0.001)and positively correlated with all baseline seminal parameters and total testosterone levels.In addition,serum vitamin D_(3)concentration was found to be positively correlated with sperm concentration(β=2.103;P<0.001),total number of spermatozoa with progressive motility(β=2.069;P=0.003),total number of motile spermatozoa(β=2.571;P=0.015),and strict morphology(β=0.056;P=0.006),regardless of other variables.This is the first comparative study to address the issue of serum vitamin D_(3)content between normozoospermic patients and those with sperm abnormalities.It clearly demonstrates a direct and positive relationship between serum vitamin D level and overall semen quality,male reproductive potential,and testosterone levels. | Inari M Ciccone Elaine MF Costa Juliana R Pariz Thiago A Teixeira Joel R Drevet Parviz Gharagozloo Robert J Aitken Jorge Hallak | 2021 | Asian Journal of Andrology2021,23,1: | 2 |
| 3 | Resveratrol attenuates metabolic,sperm,and testicular changes in adult Wistar rats fed a diet rich in lipids and simple carbohydrates显示文摘High-fat diets affect male reproduction and sexual function.Therefore,we evaluated the effects of prolonged resveratrol administration on the metabolic,sperm,and testicular parameters of rats fed a cafeteria diet.Male Wistar rats were divided at weaning into control(Cv n=20)and cafeteria(CAF,n=16)groups.At 3 months,half of them were given daily supplementations of resveratrol(C-R,n=10;CAF-R,n=8)at a dosage of 30 mg kg-1 body mass for 2 months.Animals were killed at 5 months of age,and blood,spermatozoa,and testes were collected for further analysis.Data were analyzed by one-way ANOVA,and P<0.05 was considered statistically significant.The CAF diet promoted hyperglycemia(P<0.0001),and treatment with resveratrol reversed this condition(P<0.0001).The CAF diet reduced sperm viability and motility,while resveratrol improved these parameters(F<0.05).Regarding testicular morphology,the height of the seminiferous epithelium was reduced in the CAF group compared with that of the C group(P=0.0007).Spermatogenic cell proliferation was also reduced in the CAF group compared with that of the C group.However,the CAF-R showed an increase in cell proliferation rate compared with that of the untreated CAF group(P=0.0024).Although it did not modify body mass,the consumption of a CAF diet promoted hyperglycemia,adverse testicular morphology remodeling,and abnormal sperm,which were attenuated by treatment with resveratrol,thus suggesting a protective effect of this antioxidant on spermatogenesis. | Fabiana A de Oliveira Waldemar S Costa Francisco J B Sampaio Bianca M Gregorio | 2019 | Asian Journal of Andrology2019,21,2: | 2 |
| 4 | Non-pharmacological management of pediatric functional abdominal pain disorders:Current evidence and future perspectives显示文摘Functional abdominal pain disorders(FAPDs) are an important and prevalent cause of functional gastrointestinal disorders among children, encompassing the diagnoses of functional dyspepsia, irritable bowel syndrome, abdominal migraine, and the one not previously present in Rome Ⅲ, functional abdominal pain not otherwise specified. In the absence of sufficiently effective and safe pharmacological treatments for this public problem, non-pharmacological therapies emerge as a viable means of treating these patients, avoiding not only possible side effects, but also unnecessary prescription, since many of the pharmacological treatments prescribed do not have good efficacy when compared to placebo. Thus, the present study provides a review of current and relevant evidence on non-pharmacological management of FAPDs, covering the most commonly indicated treatments, from cognitive behavioral therapy to meditation, acupuncture, yoga, massage, spinal manipulation, moxibustion, and physical activities. In addition, this article also analyzes the quality of publications in the area, assessing whether it is possible to state if non-pharmacological therapies are viable, safe, and sufficiently well-based for an appropriate and effective prescription of these treatments. Finally, it is possible to observe an increase not only in the number of publications on the non-pharmacological treatments for FAPDs in recent years, but also an increase in the quality of these publications. | 6 | Energetic-protein supplementation in the last 60 days of gestation improves performance of beef cows grazing tropical pastures显示文摘Background: Nutrition is one of the most important factors that affect animal performance, and it therefore also impacts on financial results in beef systems. In this way, finding the best strategy for feeding supplements is of paramount importance. Aiming to evaluate the effect of supplement feeding strategies for beef cows in the last third of gestation, two experiments were conducted. In Experiment 1, 35 pregnant Nellore cows were assigned to a completely randomized design with four treatments: control, which received no supplement; supplementation for the last 30 d of gestation(30-d; 3.0 kg/d); supplementation for the last 60 d of gestation(60-d; 1.5 kg/d); or supplementation for the last 90 d of gestation(90-d; 1.0 kg/d). All supplemented treatments received the same total amount of supplement throughout the experiment: 90 kg(20% of crude protein). A second experiment(Experiment 2) was delineated to evaluate the effects of the amounts offered in Experiment 1 on intake and metabolism. Four multiparous pregnant Nellore cows were assigned to a 4 × 4 Latin square design, with periods of15 d each.Results: There was a linear effect of the number of days of supplementation on calving body weight(BW; P < 0.05)and a quadratic effect on BW change from parturition to d 31 post-calving(P < 0.05), with cows on the 60-d strategy losing less BW post-calving. No difference was found in offspring birth BW(P > 0.10). A significant linear effect on interval from parturition to conception(P < 0.05) was observed, with the highest calving to conception interval being observed in the 90-d strategy. The level of supplementation did not affect forage intake or neutral detergent fiber digestibility(P > 0.10).
