2Antibiotic prophylaxis to prevent complications in endoscopic retrograde cholangiopancreatography:A systematic review and meta-analysis of randomized controlled trials显示文摘BACKGROUND The prophylactic use of antibiotics in endoscopic retrograde cholangiopancreatography(ERCP)is still controversial.AIM To assess whether antibiotic prophylaxis reduces the rates of complications in patients undergoing elective ERCP.METHODS This systematic review and meta-analysis were performed following the Preferred Reporting Items for Systematic Reviews and Meta-analysis guidelines.A comprehensive search of multiple electronic databases was performed.Only randomized controlled trials were included.The outcomes analyzed included bacteremia,cholangitis,sepsis,pancreatitis,and mortality.The risk of bias was assessed by the Cochrane revised Risk-of-Bias tool for randomized controlled trials.The quality of evidence was assessed by the Grading of Recommendation Assessment,Development,and Evaluation.Meta-analysis was performed using the Review Manager 5.4 software.RESULTS Ten randomized controlled trials with a total of 1757 patients that compared the use of antibiotic and non-antibiotic prophylaxis in patients undergoing elective ERCP were included.There was no significant difference between groups regarding incidence of cholangitis after ERCP[risk difference(RD)=-0.02,95%confidence interval(CI):-0.05,0.02,P=0.32,cholangitis in patients with suspected biliary obstruction(RD=0.02,95%CI:-0.08 to 0.13,P=0.66),cholangitis on intravenous antibiotic prophylaxis(RD=-0.02,95%CI:-0.05 to 0.01,P=0.25),septicemia(RD=-0.02,95%CI:-0.06 to 0.01,P=0.25),pancreatitis(RD=-0.02,95%CI:-0.06 to 0.01,P=0.19),and allcause mortality(RD=0.00,95%CI:-0.01 to 0.01,P=0.71).However,the antibiotic prophylaxis group presented a 7%risk reduction in the incidence of bacteremia(RD=-0.07,95%CI:-0.14 to-0.01,P=0.03).CONCLUSION The prophylactic use of antibiotics in patients undergoing elective ERCP reduces the risk of bacteremia but does not appear to have an impact on the rates of cholangitis,septicemia,pancreatitis,and mortality.Maria Fernanda Shinin Merchan Diogo Turiani Hourneaux de Moura Guilherme Henrique Peixoto de Oliveira Igor Mendonça Proença Epifanio Silvino do Monte Junior Edson Ide Caroline Moll Sergio A Sánchez-Luna Wanderley Marques Bernardo Eduardo Guimarães Hourneaux de Moura 2022World Journal of Gastrointestinal Endoscopy2022,14,11:1

6In vitro induced pluripotency from urine-derived cells in porcine显示文摘BACKGROUND The generation of induced pluripotent stem cells(iPSC)has been a game-changer in translational and regenerative medicine;however,their large-scale applicability is still hampered by the scarcity of accessible,safe,and reproducible protocols.The porcine model is a large biomedical model that enables translational applications,including gene editing,long term in vivo and offspring analysis;therefore,suitable for both medicine and animal production.AIM To reprogramme in vitro into pluripotency,and herein urine-derived cells(UDCs)were isolated from porcine urine.METHODS The UDCs were reprogrammed in vitro using human or murine octamer-binding transcription factor 4(OCT4),SRY-box2(SOX2),Kruppel-like factor 4(KLF4),and C-MYC,and cultured with basic fibroblast growth factor(bFGF)supplementation.To characterize the putative porcine iPSCs three clonal lineages were submitted to immunocytochemistry for alkaline phosphatase(AP),OCT4,SOX2,NANOG,TRA181 and SSEA 1 detection.Endogenous transcripts related to the pluripotency(OCT4,SOX2 and NANOG)were analyzed via reverse transcription quantitative realtime polymerase chain reaction in different time points during the culture,and all three lineages formed embryoid bodies(EBs)when cultured in suspension without bFGF supplementation.RESULTS The UDCs were isolated from swine urine samples and when at passage 2 submitted to in vitro reprogramming.Colonies of putative iPSCs were obtained only from UDCs transduced with the murine factors(mOSKM),but not from human factors(hOSKM).Three clonal lineages were isolated and further cultured for at least 28 passages,all the lineages were positive for AP detection,the OCT4,SOX2,NANOG markers,albeit the immunocytochemical analysis also revealed heterogeneous phenotypic profiles among lineages and passages for NANOG and SSEA1,similar results were observed in the abundance of the endogenous transcripts related to pluripotent state.All the clonal lineages when cultured in suspension without bFGF were able to form EBs expressing ectoderm and mesoderm layers transcripts.CONCLUSION For the first time UDCs were isolated in the swine model and reprogrammed into a pluripotentlike state,enabling new numerous applications in both human or veterinary regenerative medicine.Kaiana Recchia Lucas Simões Machado Ramon Cesar Botigelli Naira Caroline Godoy Pieri Gabriela Barbosa Raquel Vasconcelos Guimarães de Castro Mariana Groke Marques Laís Vicari de Figueiredo Pessôa Paulo Fantinato Neto Flávio Vieira Meirelles Aline Fernanda de Souza Simone Maria Massami Kitamura Martins Fabiana Fernandes Bressan 2022World Journal of Stem Cells2022,14,3:0
7Oral lichenoid lesion:A review of the literature显示文摘The oral lichenoid lesion(OLL) is response that occurs on the oral mucosa. The OLL include allergic responseto the dental materials, drugs, and on graft-vs-host disease(GVHD). OLL to dental material happen when restorative materials, most commonly amalgam, are in direct contact with the mucosa in sensitized individuals. Medications that produce OLL are oral hypoglycemic agents, angiotensin-converting enzyme inhibitors, and nonsteroidal anti-inflammatory agents. GVHD is a complication in bone marrow transplantation and OLL is a common lesion observed in this disease especially in chronic GVHD. The clinical and histological aspects of OLL are similar to oral lichen planus and turn it difficult to make a differential diagnosis. 8Urea breath test for Helicobacter pylori infection in adult dyspeptic patients: A meta-analysis of diagnostic test accuracy显示文摘BACKGROUND Helicobacter pylori(H.pylori)infection has been well-established as a significant risk factor for several gastrointestinal disorders.The urea breath test(UBT)has emerged as a leading non-invasive method for detecting H.pylori.Despite numerous studies confirming its substantial accuracy,the reliability of UBT results is often compromised by inherent limitations.These findings underscore the need for a rigorous statistical synthesis to clarify and reconcile the diagnostic accuracy of the UBT for the diagnosis of H.pylori infection.AIM To determine and compare the diagnostic accuracy of 13C-UBT and 14C-UBT for H.pylori infection in adult patients with dyspepsia.METHODS We conducted an independent search of the PubMed/MEDLINE,EMBASE,and Cochrane Central databases until April 2022.Our search included diagnostic accuracy studies that evaluated at least one of the index tests(^(13)C-UBT or ^(14)C-UBT)against a reference standard.We used the QUADAS-2 tool to assess the methodo-logical quality of the studies.We utilized the bivariate random-effects model to calculate sensitivity,specificity,positive and negative test likelihood ratios(LR+and LR-),as well as the diagnostic odds ratio(DOR),and their 95%confidence intervals.We conducted subgroup analyses based on urea dosing,time after urea administration,and assessment technique.To investigate a possible threshold effect,we conducted Spearman correlation analysis,and we generated summary receiver operating characteristic(SROC)curves to assess heterogeneity.Finally,we visually inspected a funnel plot and used Egger’s test to evaluate publication bias.endorsing both as reliable diagnostic tools in clinical practice.CONCLUSION In summary,our study has demonstrated that ^(13)C-UBT has been found to outperform the ^(14)C-UBT,making it the preferred diagnostic approach.Additionally,our results emphasize the significance of carefully considering urea dosage,assessment timing,and measurement techniques for both tests to enhance diagnostic precision.Nevertheless,it is crucial for researchers and clinicians to evaluate the strengths and limitations of our findings before implementing them in practice.Fabian Fellipe Bueno Lemos Caroline Tianeze de Castro Marcel Silva Luz Gabriel Reis Rocha Gabriel Lima Correa Santos Luís Guilherme de Oliveira Silva Mariana Santos Calmon Cláudio Lima Souza Ana Carla Zarpelon-Schutz Kádima Nayara Teixeira Dulciene Maria de Magalhães Queiroz Fabrício Freire de Melo 2024World Journal of Gastroenterology2024,30,6:0
9The roles of anurans in antagonistic networks are explained by life-habit and body-size显示文摘Interactions among living beings are the structuring basis of ecosystems,and studies of networks allow us to identify the patterns and consistency of such interactions.Antagonistic networks reflect the energy flow of communities,and identifying network structure and the biological aspects that influence its stability is crucial to understanding ecosystem functioning.We used antagonistic anuran interactions-predator-prey and host-parasite-to assess structural patterns and to identify the key anuran species structuring these networks.We tested whether anuran body-size and life-habit are related to their roles in these networks.We collected individuals of 9 species of anurans from an area of the Atlantic Forest in Brazil and identified their prey and helminth parasites.We used network(modularity,specialization,and nestedness)and centrality metrics(degree,closeness,and betweenness)to identify the role of anuran species in both networks.We then evaluated whether anuran body-size or life-habit were related to anuran centrality using generalized linear mixed models.The networks formed specialized interactions in compartments composed by key species from different habits.In our networks,anurans with rheophilic and cryptozoic habit are central in predator-prey networks,and those with larger body size and arboreal and cryptozoic habit in the host-parasite network.This study represents a step towards a better understanding of the influential factors that affect the structure of anuran antagonist networks,as well as to recognize the functioning roles of anuran species.Amanda Caroline DUDCZAK Gabriel Massaccesi DE LA TORRE Lorena EUCLYDES Karla Magalhães CAMPIÃO 2022Integrative Zoology2022,17,4:0
