1Maintenance therapy in advanced non-small cell lung cancer: a prime-time for change?显示文摘Lung cancer is the leading cancer in terms of incidence and mortality in China and its incidence in China is predicted to increase in the next 20 years.1 The majority of the lung cancer patients are diagnosed with advanced stage disease,for which chemotherapy or targeted therapies are the mainstay palliative treatments.Before the identification of single-driver mutations,like the epidermal growth factor receptor (EGFR) mutation,platinum-based doublet was the standard first line treatment for advanced stage nonsmall cell lung cancer (NSCLC) patients with a good performance status (PS).However,patient prognosis remains poor and the modest treatment efficacy seems to have reached a plateau despite newer generation of platinum doublets.2 The recommendation for the duration of first line platinum doublet treatment is 4-6 cycles.3 The paradigm was based on the fact that a protracted course of chemotherapy did not prolong survival but introduced cumulative toxicities.4,5 The use of maintenance therapy (MT),defined by Grossi et al6 as the prolongation of chemotherapy with the administration of additional drugs at the end of a defined number of initial chemotherapy cycles after achieving maximum tumor response,was not considered as a standard option in the past.Oscar Siu Hong Chan Rebecca Mei WanYeung Anne Wing Mui Lee 2013Chinese Medical Journal2013,,12:2

8A machine learning model for colorectal liver metastasis post-hepatectomy prognostications显示文摘Background:Currently,surgical resection is the mainstay for colorectal liver metastases(CRLM)management and the only potentially curative treatment modality.Prognostication tools can support patient selection for surgical resection to maximize therapeutic benefit.This study aimed to develop a survival prediction model using machine learning based on a multicenter patient sample in Hong Kong.Methods:Patients who underwent hepatectomy for CRLM between 1 January 2009 and 31 December 2018 in four hospitals in Hong Kong were included in the study.Survival analysis was performed using Cox proportional hazards(CPH).A stepwise selection on Cox multivariable models with Least Absolute Shrinkage and Selection Operator(LASSO)regression was applied to a multiply-imputed dataset to build a prediction model.The model was validated in the validation set,and its performance was compared with that of Fong Clinical Risk Score(CRS)using concordance index.Results:A total of 572 patients were included with a median follow-up of 3.6 years.The full models for overall survival(OS)and recurrence-free survival(RFS)consist of the same 8 established and novel variables,namely colorectal cancer nodal stage,CRLM neoadjuvant treatment,Charlson Comorbidity Score,pre-hepatectomy bilirubin and carcinoembryonic antigen(CEA)levels,CRLM largest tumor diameter,extrahepatic metastasis detected on positron emission-tomography(PET)-scan as well as KRAS status.Our CRLM Machine-learning Algorithm Prognostication model(CMAP)demonstrated better ability to predict OS(C-index=0.651),compared with the Fong CRS for 1-year(C-index=0.571)and 5-year OS(C-index=0.574).It also achieved a C-index of 0.651 for RFS.Conclusions:We present a promising machine learning algorithm to individualize prognostications for patients following resection of CRLM with good discriminative ability.Cynthia Sin Nga Lam Alina Ashok Bharwani Evelyn Hui Yi Chan Vernice Hui Yan Chan Howard Lai Ho Au Margaret Kay Ho Shireen Rashed Bernard Ming Hong Kwong Wentao Fang Ka Wing Ma Chung Mau Lo Tan To Cheung 2023Hepatobiliary Surgery and Nutrition2023,12,4:0

11A historical re-analysis of the calamitous midget typhoon passing through Hong Kong region on 18 September 1906 and its storm surge impact to Hong Kong region显示文摘A typhoon passed through Hong Kong region suddenly and unexpectedly on 18 September 1906(the“Typhoon 1906”)and had a disastrous impact on Victoria Harbour and its surroundings in just a couple of hours.Since the year 1906 was the“Bingwu”year in the Chinese calendar,the typhoon is also known historically as the“Bingwu Typhoon”.Tremendous loss of lives and property resulted,and the shipping and fishing communities were devastated.Two mysteries arising from this calamitous typhoon have existed to date:1)Why the Hong Kong region Observatory was not able to provide any forewarning?2)whether the storm surge reported in some contemporary records is entirely credible?This paper will focus on both of these.In this paper,we re-analyse historical weather observations recorded in various historical documents and estimate the possible storm size,intensity and track of Typhoon 1906 using tropical cyclone models.Based on the re-analyses,the storm surges,storm tides and wave heights in Hong Kong region are also estimated using storm surge and wave models.The results reveal that Typhoon 1906 was a midget typhoon,with a radius of maximum winds of 11 km or smaller,during its passage through Hong Kong region.This explains why it was technically impossible for a forewarning to be given at that time when real-time weather observations from ships,meteorological satellites and radars were non-existent.We also estimate that the maximum storm surges(storm tides)in Hong Kong region were not higher than 0.82 m(2.43 mCD)and 1.98 m(4.15 mCD)in Victoria Harbour and Tolo Harbour respectively.These figures are found to be limited by the intensity and the storm size of the typhoon.Therefore,we conclude that the previously documented storm surge figures are not supported by the present study.Hing Yim Mok Chi Ming Shun Stephen Davies Wing Hong Lui Dick Shum Lau Kai Chun Cheung Kwan Yin Kong Sai Tick Chan 2022Tropical Cyclone Research and Review2022,11,3:0
