1Relationship between intubation rate and continuous positive airway pressure therapy in the prehospital setting显示文摘BACKGROUND: To determine whether the prehospital use of continuous positive airway pressure(CPAP) therapy is associated with a reduced rate of endotracheal intubation in patients with an acute respiratory disorder brought to the emergency department(ED).METHODS: We reviewed medical records of patients with acute respiratory distress who had been treated with CPAP in the Mobile Intensive Care Unit(MICU) from January 2010 to December 2011. These records were compared with those of patients who received standardized care without CPAP in the MICU from January 2004 to December 2004. Categorical variables were summarized as frequencies and compared between groups using Fisher's exact test or the Chi-square test. Continuous variables were summarized as medians(interquartile range), and comparison between the groups was made using Wilcoxon's rank-sum test. The relationship between CPAP and intubation rate was determined using multivariable logistic regression analysis of propensity scores. The results were presented as odds ratio(OR), 95% confidence interval(CI), and P value for test effect. The adequacy of the model was calibrated using Hosmer and Lemeshow's goodness-of-fit test. P<0.05 was considered statistically significant.RESULTS: The records of 785 patients were reviewed. Of the 215 patients treated with CPAP in the MICU, 13% were intubated after admission. In contrast, of the 570 patients who did not receive CPAP, 28% were intubated after ED admission. Unadjusted logistic regression analysis showed that patients who had been treated with CPAP were less likely to be intubated than those without CPAP treatment(OR=0.37, 95% CI, 0.24–0.57, P<0.0001). With propensity scores adjusted, multivariate logistic regression analysis showed that CPAP treatment was associated with a 62% reduction of intubation(OR=0.384, 95%CI, 0.25–0.60, P≤0.0001).CONCLUSIONS: In patients with acute respiratory disorder, there was a relationship between CPAP therapy and the decreased intubation rate. 2A Mother-to-Child Transmission Study in Nigeria:The Impact of Maternal HIV Infection and HAART on Plasma Immunoglobulins,Cytokine Profiles and Infant Outcome显示文摘Prevention of mother-to-child transmission(PMTCT)of HIV with highly active antiretroviral therapy(HARRT)allows the HIV^+pregnant mothers to have vaginal delivery and breastfeed.Here we investigated the maternal plasma immunoglobulin,cytokine secretion and the outcome of the exposed infants among the HIV^+HAART treated pregnan women in Nigeria.In this study,different plasma immunoglobulins and cytokines were measured in the HIV^+HAART treated pregnant mothers.Pooled culture supernatants of B and T lymphocytes showed lower levels of IFN-γ,IL-10 and IL-4.There were lower IFN-γand IL-10 secretions at 1st trimester;however,IL-10 continued to be lower throughout 2nd and 3rd trimesters.TNF-αsecretion significantly decreased as pregnancy progressed to term.There were high plasma IgG and low IgM in the HIV^+HAART treated pregnant women.Plasma IgG was high during 1st and 3rd trimesters.After one year of follow up,all the exposed children were seronegative for HIV-1 and HIV-2.Vaginal delivery and breastfeeding among HIV^+HAART treated mothers have shown to be safe.The use of HAART by the infected mothers and the use of septrin and niverapin by the exposed infants prevented mother to-child transmission of HIV.Chinwe O.Ewenighi-Amankwah Charles Chinedum Onyenekwe Ogochukwu Udemba Patience Muogbo Lijun Rong 2020Virologica Sinica2020,35,4:3
3Fatal case of newborn Lassa fever virus infection mimicking late onset neonatal sepsis: a case report from northern Nigeria显示文摘Background:Lassa fever is a zoonotic viral infection endemic to the West Africa countries.It is highly fatal during pregnancy and as such reports of neonatal onset Lassa fever infections are rare in scientific literature.We report a fatal case of Lassa fever in a 26-day-old neonate mimicking the diagnosis of late-onset neonatal sepsis.Case presentation:The patient is a 26-day-old neonate who was admitted with a day history of fever,poor feeding,pre-auricular lymphadenopathy and sudden parental death.He was initially evaluated for late onset neonatal sepsis.He later developed abnormal bleeding and multiple convulsions while on admission,prompting the need to evaluate for Lassa fever using reverse transcription polymerase chain reaction(RT-PCR).He died 31 h into admission and RT-PCR result was positive for Lassa fever.Conclusions:Neonatal Lassa fever infection is highly fatal and can mimic neonatal sepsis.High index of suspicion is needed particularly for atypical presentations of neonatal sepsis in Lassa fever endemic areas.Taofik Oluwaseun Ogunkunle Surajudeen Oyeleke Bello Chinwe Immaculata Anderson Rashida Musa Rasaq Olaosebikan Abdulazeez Imam 2020Infectious Diseases of Poverty2020,9,4:2

8In vitro and in vivo cytochrome P450 3A enzyme inhibition by Aframomum melengueta and Dennettia tripetala extracts显示文摘Objective: To evaluate the in vitro and in vivo inhibitory effects of two commonly used herbs, Aframomum melengueta(A. melengueta) and Dennettia tripetala(D. tripetala) on CYP 3A enzymes. Methods: In vitro inhibition of the enzymes were assessed with microsomes extracted from female albino rats using erythromycin-N-demethylation assay(EMND) method while their in vivo effects were measured by estimating simvastatin plasma concentrations in rats. Pharmacokinetic parameters were determined using non-compartmental anaysis as implemented in Win Nonlin pharmacokinetic program. Results: EMND assay with intestinal microsomes indicated that aqueous extracts of D. tripetala and A. melengueta significantly(P < 0.05) inhibited intestinal CYP 3A activity at both 50 μg and 100 μg concentrations. Petroleum ether extract of D. tripetala and ethanol extracts of A. melengueta inhibited intestinal CYP3 A activity at 100 μg but not at 50 μg concentrations. All the extracts showed an in vitrodose dependent CYP 3A inhibition with liver microsomes. In vivo analysis showed that pretreatment with the extracts enhanced systemic absorption of simvastatin with reductions in metabolizing enzymes activity as indicated in significant increases in maximal concentration, area under curve, area under moment curve and mean resident time of simvastatin(P < 0.05). 16Evaluation of the antiulcer activity of Olax suhscorpioidea Oliv.roots in rats显示文摘Objective:To evaluated the antiulcer activity of the methanolic root extract of the plant Olax subscorpioidea in experimental rats.Methods:Phytochemical tests and acute toxicity tests were carried out on its methanolic root extract.Pre-treatments with three doses of the extract(200,400 and 600 mg/kg body weight orally) and Sucralfate at 100 mg/kg orally were used for the various groups of rats.The indomethacin and ethanol models for experimental induction of ulcers in rats were used.Mean ulcer indices were measured and percentage inhibition was derived.Results: Phytochemistry revealed presence of alkaloids,steroids,glycosides and terpenoids and the extract showed an LD50 of 2 154 mg/kg in mice.Ulcer was produced in all the rats in both models with the extract showing potent antiulcer activity of ethanol model.There was no significant ulcer inhibition by any of the treatments compared to control group in the indomethacin model but the extract’s antiulcer effect was dose-dependent(11.8%,19.2%,32.7%,P>0.05).The ulcer reduction in the ethanol model was significant ulcer reduction in the highest dose group and Sucralfate group compared to control group(79.3%and 82.9%,P <0.05).However,the extract at all dosage showed a dose-dependent ulcer inhibition in this model.Conclusion:The above results suggest that the roots of Olax subscorpioidea possess antiulcer activity in experimental rats as claimed by traditional users.Ukwe Chinwe Victoria Ubaka Chukwuemeka Michael Madusque Uche Johnny 2010Asian Pacific Journal of Tropical Medicine2010,3,1:0
17尼日利亚阿巴卡利基地区三级医院的先天性眼及附属器疾病(英文)显示文摘目的:确定在尼日利亚阿巴卡利基地区两个三级医院就诊的先天性眼及附属器疾病的类型和发病率。方法:回顾分析2001-05/2008-04就诊于两个三级医院眼科的1岁及1岁以下的先天性眼病患者的病历记录,用简单的统计方法分析年龄、性别、病种、发病率、眼部及全身的相关疾病等信息。结果:研究共记录有199例眼病患儿。其中先天性眼病的50例(25.1%)。包括男27例(54%),女23例(46%)。患儿<6个月龄的占2/3(64%)。最常见的疾病为鼻泪管阻塞10例(20%),先天性上睑下垂10例(14%)皮样囊肿5例(10%),牛眼4例(8%),以及先天性白内障3例(6%)。这些都可能导致可避免的儿童盲。少见的疾病包括白化病1例(2%),无眼畸形1例(2%),视网膜母细胞瘤2例(4%)和视神经萎缩2例(4%)。这些是不可逆的导致失明和视力低下的原因。结论:尽管先天性眼病不常见,但是这些疾病是潜在的儿童致盲原因。Chijioke G Onyekonwu Chinwe L Onyekonwu 2009国际眼科杂志2009,9,1:0

19Monitoring and Quantification of Carbon Dioxide Emissions and Impact of Sea Surface Temperature on Marine Ecosystems as Climate Change Indicators in the Niger Delta Using Geospatial Technology显示文摘The Niger Delta marine environment has experienced a series of environmental disasters since the inception of oil and gas exploration,which can be attributed to climate change.Carbon dioxide(CO_(2))emissions and sea surface temperature(T)ties associated with burning fossil fuels,such as gas flaring,vehicular traffic,and marine vessel movement along the sea,are increasing.Using data extracted from the NASA Giovanni satellite’s Atmospheric Infrared Sounder(AIRS)and Moderate Resolution Imaging Spectroradiometer(MODIS),this study mapped the carbon footprint and T along the coastline into the deep sea from 2003 to 2011,using ArcGIS software.The spatial distribution of CO_(2) and T concentrations determined by the inverse distance weighting(IDW)method reveals variations in the study area.The results show an increase in the quantity of the mean tropospheric CO_(2) from July 2003 to December 2011,from 374.5129 ppm to 390.7831 ppm annual CO_(2) emissions,which also reflects a continuous increase.The average Monthly sea surface temperature had a general increasing trend from 25.79℃ in July 2003 to 27.8°C in December,with the Pearson correlation coefficient between CO_(2) and T indicating 50%strongly positive,20%strongly negative,20%weakly positive,and 10%weakly negative.CO_(2) levels,like temperature,follow a seasonal cycle,with a decrease during the wet season due to precipitation dissolving and plant uptake during the growing season,and then a rise during the dry season.Carbon capture and storage technologies must be implemented to benefit the marine ecosystem and human well-being.Okechukwu Okpobiri Eteh Desmond Rowland Francis Emeka Egobueze Mogo Felicia Chinwe 2023Journal of Atmospheric Science Research2023,6,1:0
20Antimalarial activity of Ageratum conyzoides in combination with chloroquine and artesunate显示文摘Objective:To determine the suppressive and curative activity of aqueous leaf extract of Ageratum conyzoides(A.conyzoides) in combination with chloroquine and artesunate, respectively against Plasmodium berghei infection in mice.Methods:Using malaria(Plasmodium berghei) infected albino mice of both sexes,aqueous extracts of A.conyzoides in combination with chloroquine and artesunate were tested for antimalarial activity,respectively.Four-day suppressive test and Rane’s curative test were carried out.Results:Suppressive tests showed significant dose dependent reduction in parasitemia level produced by the extract-chloroquine and extract-artesunate combinations.Suppressive activities of both extract-drug combinations were greater than the individual drugs alone.Extract-chloroquine(100:5) produced the highest suppressive effect(98%suppression).Curative tests showed absolute survival in two extract-drug combinations.Two extract-drug combinations produced higher curative effects than the individual drugs alone.The highest dose combinations of extract-chloroquine(100:5) and extract-artesunate(100:5) produced absolute parasitemia clearance(cure) in the infected mice.
