| 1 | Transjugular intrahepatic portosystemic shunt in cirrhosis: An exhaustive critical update显示文摘More than five decades after it was originally conceptualized as rescue therapy for patients with intractable variceal bleeding,the transjugular intrahepatic portosystemic shunt(TIPS)procedure continues to remain a focus of intense clinical and biomedical research.By the impressive reduction in portal pressure achieved by this intervention,coupled with its minimally invasive nature,TIPS has gained increasing acceptance in the treatment of complications of portal hypertension.The early years of TIPS were plagued by poor long-term patency of the stents and increased incidence of hepatic encephalopathy.Moreover,the diversion of portal flow after placement of TIPS often resulted in derangement of hepatic functions,which was occasionally severe.While the incidence of shunt dysfunction has markedly reduced with the advent of covered stents,hepatic encephalopathy and instances of early liver failure continue to remain a significant issue after TIPS.It has emerged over the years that careful selection of patients and diligent post-procedural care is of paramount importance to optimize the outcome after TIPS.The past twenty years have seen multiple studies redefining the role of TIPS in the management of variceal bleeding and refractory ascites while exploring its application in other complications of cirrhosis like hepatic hydrothorax,portal hypertensive gastropathy,ectopic varices,hepatorenal and hepatopulmonary syndromes,non-tumoral portal vein thrombosis and chylous ascites.It has also been utilized to good effect before extrahepatic abdominal surgery to reduce perioperative morbidity and mortality.The current article aims to review the updated literature on the status of TIPS in the management of patients with liver cirrhosis. | Sasidharan Rajesh Tom George Cyriac Abby Philips Rizwan Ahamed Sandeep Kumbar Narain Mohan Meera Mohanan Philip Augustine | 2020 | World Journal of Gastroenterology2020,26,37: | 26 |
| 2 | Potent antiviral therapy improves survival in acute on chronic liver failure due to hepatitis B virus reactivation显示文摘Acute on chronic liver failure(ACLF)is a disease entity with a high mortality rate.The acute event arises from drugs and toxins,viral infections,bacterial sepsis,interventions(both surgical and non-surgical)and vascular events on top of a known or occult chronic liver disease.ACLF secondary to reactivation of chronic hepatitis B virus is a distinct condition;the high mortality of which can be managed in the wake of new potent antiviral therapy.For example,lamivudine and entecavir use has shown definite short-term survival benefits,even though drug resistance is a concern in the former.The renoprotective effects of telbivudine have been shown in a few studies to be useful in the presence of renal dysfunction.Monotherapy with newer agents such as tenofovir and a combination of nucleos(t)ides is promising for improving survival in this special group of liver disease patients.This review describes the current status of potent antiviral therapy in patient with acute on chronic liver failure due to reactivation of chronic hepatitis B,thereby providing an algorithm in management of such patients. | Cyriac Abby Philips Shiv Kumar Sarin | 2014 | World Journal of Gastroenterology2014,20,43: | 20 |

| 5 | Influences of Annealing Temperature and Doping Concentration on Microstructural and Optical Properties of CeO_2:Sm^(3+) Nanocrystals显示文摘Nanocrystals of CeO2 with different doping concentrations of Sm3+were synthesized by a novel and costeffective method. The crystal structure, morphology and particle size were systematically investigated by X-ray diffraction,scanning electron microscopy and transmission electron microscopy. Effects of the annealing temperature and doping concentrations on the microstructural properties of the crystals were studied. X-ray diffraction analysis indicates that the cubic structure of the Ce O2 is not affected by the doping of Sm3+up to a doping concentration of 20%. Different structural parameters such as lattice constant, surface area, bulk density and porosity of the crystal were determined and discussed.Microscopic images of the CeO2:Sm3+suggest that the thermal decomposition of oxalate precursor is a suitable synthesis pathway to produce uniform-sized microparticles and nanoparticles. The influences of annealing temperature and doping concentration of Sm3+on the optical properties of the nanocrystals were also discussed. The photoluminescence excitation spectra reveal that the charge transfer band is redshifted with increasing annealing temperatures. | 19 | Blood and CSF biomarkers for post-stroke epilepsy:a systematic review显示文摘Post-stroke epilepsy is a common complication of ischemic stroke which adversely affects the prognosis of patients.Clinical and radiological parameters cannot adequately predict the risk.Therefore,the discovery of biomarkers is imperatively needed for predicting post-stroke epilepsy.We conducted a systematic review of diagnostic and prognostic biomarkers for post-stroke epilepsy through a comprehensive literature search in different databases.All articles that met our inclusion criteria were assessed for quality using the modified Quality Assessment of Diagnostic Accuracy Studies questionnaire.Eight eligible studies were included in this systematic review.Out of 22 assessed biomarkers,nine biomarkers showed significant association with post-stroke epilepsy.The T allele of CD40(cluster of differentiation 40)−1C/T polymorphism,the CC genotype of TRPM6(transient receptor potential cation channel subfamily M member 6)rs2274924,the allele polymorphism of MAD2(mitochondrial aldehyde dehydrogenase 2),the mRNA level of interleukin-6(IL-6),the plasma level of endostatin,and the mRNA expression of IL-1βshow a positive correlation with post-stroke epilepsy;while S100 calcium-binding protein B,heat shock 70 kDa protein-8 and neuropeptide Y are inversely associated with post-stroke epilepsy.As a small number of patients were recruited,further studies are needed to confirm their potential use for predicting post-stroke epilepsy. | Priya Dev Mareena Cyriac Kamalesh Chakravarty Abhishek Pathak | 2022 | Acta Epileptologica2022,5,3: | 1 |
