| 1 | Mechanism of diarrhea in microscopic colitis显示文摘AIM: To search the pathophysiological mechanism of diarrhea based on daily stool weights, fecal electrolytes,osmotic gap and pH.METHODS: Seventy-six patients were included: 51 with microscopic colitis (MC) [40 with lymphocytic colitis (LC);11 with collagenous colitis (CC)]; 7 with MC without diarrhea and 18 as a control group (CG). They collected stool for 3 d. Sodium and potassium concentration were determined by flame photometry and chloride concentration by titration method of Schales. Fecal osmotic gap was calculated from the difference of osmolarity of fecal fluid and double sum of sodium and potassium concentration.RESULTS: Fecal fluid sodium concentration was significantly increased in LC 58.11±5.38 mmol/L (P<0.01)and CC 54.14±8.42 mmol/L (P<0.05) than in CG 34.28±2.98 mmol/L. Potassium concentration in LC 74.65±5.29 mmol/L (P<0.01) and CC 75.53±8.78 mmol/L (P<0.05) was significantly less compared to CG 92.67±2.99 mmol/L.Chloride concentration in CC 36.07±7.29 mmol/L was significantly higher than in CG 24.11±2.05 mmol/L (P<0.05).Forty-four (86.7%) patients had a secretory diarrhea compared to fecal osmotic gap. Seven (13.3%) patients had osmotic diarrhea.CONCLUSION: Diarrhea in MC mostly belongs to the secretory type. The major pathophysiological mechanism in LC could be explained by a decrease of active sodium absorption. | 3 | Burden of celiac disease in the Mediterranean area显示文摘AIM: To estimate the burden of undiagnosed celiac disease (CD) in the Mediterranean area in terms of morbidity, mortality and health cost. METHODS: For statistics regarding the population of each country in the Mediterranean area, we accessed authoritative international sources (World Bank, World Health Organization and United Nations). The prevalence of CD was obtained for most countries from published reports. An overall prevalence rate of 1% cases/total population was finally estimated to represent the frequency of the disease in the area, since none of the available conf idence intervals of the reported rates significantly excluded this rate. The distribution of symptoms and complications was obtained from reliable reports in the same cohort. A standardized mortality rate of 1.8 was obtained from recent reports. Crude health cost was estimated for the years between symptoms and diagnosis for adults and children, and was standardized for purchasing power parity to account for the different economic prof iles amongst Mediterranean countries. RESULTS: In the next 10 years, the Mediterranean area will have about half a billion inhabitants, of which 120 million will be children. The projected number of CD diagnoses in 2020 is 5 million cases (1 million celiac children), with a relative increase of 11% compared to 2010. Based on the 2010 rate, there will be about 550 000 symptomatic adults and about 240 000 sick children: 85% of the symptomatic patients will suffer from gastrointestinal complaints, 40% are likely to have anemia, 30% will likely have osteopenia, 20% of children will have short stature, and 10% will have abnormal liver enzymes. The estimated standardized medical costs for symptomatic celiac patients during the delay between symptom onset and diagnosis (mean 6 years for adults, 2 years for children) will be about €4 billion (€387 million for children) over the next 10 years. A delay in diagnosis is expected to increase mortality: about 600 000 celiac patients will die in the next 10 years, with an excess of 44.4% vs age-and sexmatched controls. CONCLUSION: In the near future, the burden of CD will increase tremendously. | 17 | Influence of Tooth Loss and Unmet Treatment Needs on the Social Aspects of Quality of Life and Self-esteem in Adolescents显示文摘Adolescence represents specific period of life and development,when young person is going through physical,emotional and mental changes,risking poor general and oral health outcome that might affect quality of life.Negative effects of poor oral health are various:difficulties with speaking and eating,pain and malfunction.This might cause emotional and mental problems,leading to avoidance of speaking,smiling and socializing with friends.Cross-sectional study included 234 participants(15-16 years old).The research instruments used in the study were questionnaire and clinical examination.Questionnaire referred to socio-demographic characteristics of participants,their oral and general health related habits,self-evaluation of confidence and successfulness and effect of oral impacts on daily performances.Clinical examination was used to determine dental status in terms of tooth loss,need for prosthodontic rehabilitation and presence of dental malocclusion.Results:24.6%(58)of adolescents had at least one tooth missing.Significant difference between the genders was obtained,as 32.5%girls had at least one tooth extracted compared to 8.1%of boys.Normative need for prosthodontic rehabilitation was present in 22.0%of subjects.Dental problems negatively affected their sleep and emotions(14.4%and 10.2%few times in period of one month).Data analyses pointed one correlation between extracted teeth and psychosocial aspect of participants'life(difficulties and embarrassment when smiling,talking and spending time with friends,emotional problems).Unmet normative need for prosthodonic or orthodontic treatment affected participants'ability to sleep and relax.Percentage of adolescents with extracted teeth was high,as well as normative treatment need for prosthodontic rehabilitation.Poor dental status affected quality of life of adolescents,especially emotional and mental well-being.Dental service should provide sufficient information to adolescents about prevention of tooth loss and consequences of teeth extraction,not only within masticatory function but also concerning oral health related quality of life. | Pavlovic Maja Matijevic Dusanka Vukovic Branislava Mileusnic Ivan Gajic Milica Jevremovic Ana | 2019 | Journal of Pharmacy and Pharmacology2019，7，1： | 0 |
