1Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases显示文摘AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary tumors.METHODS This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the periods.CONCLUSION Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. 2Clinicopathological features and surgical outcome of patients with fibrolamellar hepatocellular carcinoma(experience with 22 patients over a 15-year period)显示文摘AIM To evaluate the clinicopathological features and the surgical outcomes of patients with fibrolamellar hepato-cellular carcinoma(FL-HCC)over a 15-year period. METHODS This is a retrospective study including 22 patients with a pathologic diagnosis of FL-HCC who underwent hepatectomy over a 15-year period. Tumor characteristics,survival and recurrence were evaluated. RESULTS There were 11 male and 11 female with a median age of 29 years(range from 21 to 58 years). Two(9%)patients had hepatitis C viral infection and only 2(9%)patients had alpha-fetoprotein level > 200 ng/m L. The median size of the tumors was 12 cm(range from 5-20 cm). Vascular invasion was detected in 5(23%)patients. Four(18%)patients had lymph node metastases. The median follow up period was 42 mo and the 5-year survival was 65%. Five(23%)patients had a recurrent disease,4 of them had a second surgery with 36 mo median time interval. Vascular invasion is the only significant negative prognostic factor CONCLUSION FL-HCC has a favorable prognosis than common HCC and should be suspected in young patients with non cirrhotic liver. Aggressive surgical resection should be done for all patients. 3Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients显示文摘AIM To evaluate the reversibility of minimal hepatic encephalopathy(MHE) following liver transplantation(LT) in Egyptian cirrhotic patients. METHODS This prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty ageand sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A(TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score(PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTS Before LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls(P < 0.001). There was a statistically significant improvement in test values in the patient group after LT; however, their values were still significantly worse than those of the controls(P < 0.001). The PHES detected MHE in 16 patients(80%) before LT with a median value of -7 ± 3.5. The median PHES value was significantly improved following LT, reaching-4.5 ± 5(P < 0.001), and the number of patients with MHE decreased to 11(55%). The pre-transplant model for end-stage liver disease(MELD) score ≥ 15 was significantly related to the presence of post-transplant MHE(P = 0.005). 4Predictors of long term survival after hepatic resection for hilar cholangiocarcinoma:A retrospective study of 5-year survivors显示文摘AIM:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived < 5 years.METHODS:This is a retrospective study of patients with pathologically proven HC who underwent surgical resection at the Gastroenterology Surgical Center,Mansoura University,Egypt between January 2002 and April 2013.All data of the patients were collected from the medical records.patients were divided into two groups according to their survival:patients surviving less than 5 years and those who survived > 5 years.RESULTS:There were 34(14%) long term survivors(5 year survivors) among the 243 patients.Fiveyear survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two groups.The level of CA 19-9 was significantly higher in patients surviving < 5 years(395.71 ± 31.43 vs 254.06 ± 42.19,p = 0.0001).Univariate analysis demonstrated nine variables to be significantly associated with survival > 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.Mohamed Abd El Wahab Ayman El Nakeeb Ehab El Hanafy Ahmad M Sultan Ahmed Elghawalby Waleed Askr Mahmoud Ali Mohamed Abd El Gawad Tarek Salah 2016World Journal of Gastrointestinal Surgery2016,8,6:10
5Post-endoscopic retrograde cholangiopancreatography pancreatitis: Risk factors and predictors of severity显示文摘AIM To detect risk factors for post-endoscopic retrograde cholangiopancreatography(ERCP) pancreatitis(PEP) and investigate the predictors of its severity.METHODS This is a prospective cohort study of all patients who underwent ERCP.Pre-ERCP data,intraoperative data,and post-ERCP data were collected.RESULTS The study population consisted of 996 patients.Their mean age at presentation was 58.42(± 14.72) years,and there were 454 male and 442 female patients.Overall,PEP occurred in 102(10.2%) patients of the study population; eighty(78.4%) cases were of mild to moderate degree,while severe pancreatitis occurred in 22(21.6%) patients.No hospital mortality was reported for any of PEP patients during the study duration.Age less than 35 years(P = 0.001,OR = 0.035),narrower common bile duct(CBD) diameter(P = 0.0001) and increased number of pancreatic cannulations(P = 0.0001) were independent risk factors for the occurrence of PEP.CONCLUSION PEP is the most frequent and devastating complication after ERCP.Age less than 35 years,narrower median CBD diameter and increased number of pancreaticcannulations are independent risk factors for the occurrence of PEP.Patients with these risk factors are candidates for prophylactic and preventive measures against PEP.Ayman El Nakeeb Ehab El Hanafy Tarek Salah Ehab Atef Hosam Hamed Ahmad M Sultan Emad Hamdy Mohamed Said Ahmed A El Geidie Tharwat Kandil Mohamed El Shobari Gamal El Ebidy 2016World Journal of Gastrointestinal Endoscopy2016,8,19:10
6Beamforming techniques for massive MIMO systems in 5G:overview,classification,and trends for future research显示文摘Massive multiple-input multiple-output(MIMO) systems combined with beamforming antenna array technologies are expected to play a key role in next-generation wireless communication systems(5G), which will be deployed in 2020 and beyond. The main objective of this review paper is to discuss the state-of-the-art research on the most favourable types of beamforming techniques that can be deployed in massive MIMO systems and to clarify the importance of beamforming techniques in massive MIMO systems for eliminating and resolving the many technical hitches that massive MIMO system implementation faces. Classifications of optimal beamforming techniques that are used in wireless communication systems are reviewed in detail to determine which techniques are more suitable for deployment in massive MIMO systems to improve system throughput and reduce intra-and inter-cell interference. 7Perioperative visual loss after spine surgery显示文摘Perioperative visual loss(POVL) is an uncommon, but devastating complication that remains primarily associated with spine and cardiac surgery. The incidence and mechanisms of visual loss after surgery remain difficult to determine. According to the American Society of Anesthesiologists Postoperative Visual Loss Registry, the most common causes of POVL in spine procedures are the two different forms of ischemic optic neuropathy: anterior ischemic optic neuropathy and posterior ischemic optic neuropathy, accounting for 89% of the cases. Retinal ischemia, cortical blindness, and posterior reversible encephalopathy are also observed, but in a small minority of cases. A recent multicenter case control study has identified risk factors associated with ischemic optic neuropathy for patients undergoing prone spinal fusion surgery. These include obesity, male sex, Wilson frame use, longer anesthetic duration, greater estimated blood loss, and decreased percent colloid administration. These risk factors are thought to contribute to the elevation of venous pressure and interstitial edema, resulting in damage to the optic nerve by compression of the vessels that feed the optic nerve, venous infarction or direct mechanical compression. This review will expand on these findings as well as the recently updated American Society of Anesthesiologists practice advisory on POVL. There are no effectivetreatment options for POVL and the diagnosis is often irreversible, so efforts must focus on prevention and risk factor modification. 8Adsorptive removal of iron and manganese ions from aqueous solutions with microporous chitosan/polyethylene glycol blend membrane显示文摘Microporous chitosan(CS) membranes were directly prepared by extraction of poly(ethylene glycol)(PEG) from CS/PEG blend membrane and were examined for iron and manganese ions removal from aqueous solutions.The different variables affecting the adsorption capacity of the membranes such as contact time,pH of the sorption medium,and initial metal ion concentration in the feed solution were investigated on a batch adsorption basis.The affinity of CS/PEG blend membrane to adsorb Fe(II) ions is higher than that of Mn(II) ions,with adsorption equilibrium achieved after 60 min for Fe(II) and Mn(II) ions.By increasing CS/PEG ratio in the blend membrane the adsorption capacity of metal ions increased.Among all parameters,pH has the most significant effect on the adsorption capacity,particularly in the range of 2.9-5.9.The increase in CS/PEG ratio was found to enhance the adsorption capacity of the membranes.The effects of initial concentration of metal ions on the extent of metal ions removal were investigated in detail.The experimental data were better fitted to Freundlich equation than Langmuir.In addition,it was found that the iron and manganese ions adsorbed on the membranes can be effectively desorbed in 0.1 mol/L HCl solution(up to 98% desorption efficiency) and the blend membranes can be reused almost without loss of the adsorption capacity for iron and manganese ions.Neama A.Reiad Omar E.Abdel Salam Ehab F.Abadir Farid A.Harraz 2012Journal of Environmental Sciences2012,24,8:7
9Molecular study on Y chromosome microdeletions in Egyptian males with idiopathic infertility显示文摘Aim:To determine the frequency of genetic deletions within the azoospermia factors in Egyptian infertile males.Methods:The Yq microdeletions in 33 infertile males with undetectable chromosomal anomalies were examined by mutiplex polymerase chain reaction (PCR).Deletions were confirmed using single PCR amplifications.Results:Four out of the total 33 (12%) men had Yqll microdeletions,thus supporting the average reported figures in other populations.Three of those 4 cases had single short tandem sequence deletions with discrete histological findings of their testes.Single sY272 deletion within AZFc was associated with Sertoli cell only syndrome,whereas a patient with isolated sY84 deletion within AZFa had immature testicular structure.The remaining case had a large deletion in AZFa-c and short stature.Conclusion:The present study supports the hypothesis that the Yq11 encompasses genetic determinants of stature besides genes controlling spermatogenesis.(Asian J Androl 2004 Mar;6:53-57)Mostafa K. 10Predictors of long-term survival after pancreaticoduodenectomy for peri-ampullary adenocarcinoma: A retrospective study of 5-year survivors显示文摘Background: Pancreaticoduodenectomy(PD) is the standard curative treatment for periampullary tumors. The aim of this study is to report the incidence and predictors of long-term survival( ≥ 5 years) after PD. Methods: This study included patients who underwent PD for pathologically proven periampullary adenocarcinomas. Patients were divided into 2 groups: group(I) patients who survived less than 5 years and group(II) patients who survived ≥ 5 years. Results: There were 47(20.6%) long-term survivors( ≥ 5 years) among 228 patients underwent PD for periampullary adenocarcinoma. Patients with ampullary adenocarcinoma represented 31(66.0%) of the long-term survivors. Primary analysis showed that favourable factors for long-term survival include age < 60 years old, serum CEA < 5 ng/mL, serum CA 19-9 < 37 U/mL, non-cirrhotic liver, tumor size < 2 cm, site of primary tumor, postoperative pancreatic fistula, R0 resection, postoperative chemotherapy, and no recurrence. Multivariate analysis demonstrated that CA 19-9 < 37 U/mL [OR(95% CI) = 1.712(1.24 8–2.34 8), P = 0.001], smaller tumor size [OR(95% CI) = 1.335(1.032–1.726), P = 0.028] and R0 resection [OR(95% CI) = 3.098(2.095–4.582), P < 0.001] were independent factors for survival ≥ 5 years. The prognosis was best for ampullary adenocarcinoma, for which the median survival was 54 months and 5-year survival rate was 39.0%, and the poorest was pancreatic head adenocarcinoma, for which the median survival was 27 months and 5-year survival rate was 7%. Conclusions: The majority of long-term survivors after PD for periampullary adenocarcinoma are patients with ampullary tumor. 11Intraoperative endoscopic retrograde cholangiopancreatography: A useful tool in the hands of the hepatobiliary surgeon显示文摘AIM: To evaluate the efficacy of intraoperative endoscopic retrograde cholangio-pancreatography(ERCP) combined with laparoscopic cholecystectomy(LC) forpatients with gall bladder stones(GS) and common bile duct stones(CBDS).METHODS: Patients treated for GS with CBDS were included. LC and intraoperative transcystic cholangiogram(TCC) were performed in most of the cases. Intraoperative ERCP was done for cases with proven CBDS.RESULTS: Eighty patients who had GS with CBDS were included. LC was successful in all cases. Intraoperative TCC revealed passed CBD stones in 4 cases so intraoperative ERCP was performed only in 76 patients. Intraoperative ERCP showed dilated CBD with stones in 64 cases(84.2%) where removal of stones were successful; passed stones in 6 cases(7.9%); short lower end stricture with small stones present in two cases(2.6%) which were treated by removal of stones with stent insertion; long stricture lower 1/3 CBD in one case(1.3%) which was treated by open hepaticojejunostomy; and one case(1.3%) was proved to be ampullary carcinoma and whipple's operation was scheduled. Also, in this paper the mean time to loss lock is determined in order to analyse the mentioned interferences effect on the GPS receiver. 14Detection of schistosomiasis antibodies in urine patients as a promising diagnostic maker显示文摘Objective:To determine secreted antischistosoma antibodies in urine and to discern the epidemiological situation of schistosomiasis in the agricultural field labourers’camps city in the Gezira State-central Sudan.Methods:Total of 66 urine and 66 serum paired samples were collected from those who confirmed parasitologically positive and negative with schistosomiasis from the two camps.Samples were tested using ELBA technique to measure and compare the immunoglobulin G(IgG) levels in serum and urine samples of schistosomiasis patients.Results: The overall prevalence of S.mansoni and S.haematobium was 53.8%and 15.4%,while the intensity were(2.04 GMEC) and(0.9 GMEC) respectively.The relative percentage of positive IgG individulas in urine was 92.40%where as 96.97%in serum.Statistically no significant difference between the IgG levels in serum and urine samples was observed.Conclusions:This study shows that the detection of secreted IgG antibodies in urine can substitute serum for diagnosis of schistosomiasis.Salma Medani Elhag Eshtiyag Abdalla Abdelkareem Ashraf Siddig Yousif Ehab Ahmed Frah Ahmed Babiker Mohamed 2011Asian Pacific Journal of Tropical Medicine2011,4,10:2
15Molecular mechanisms of the anti-obesity potential effect of Moringa oleifera in the experimental model显示文摘Objective: To elucidate the molecular mechanisms of the potent anti-obesity effect of Moringa oleifera Lam.(M. oleifera) ethanolic extract and to clarify the link between these mechanisms and the associated metabolic and vascular risks in the experimental model of visceral obesity.Methods: M. oleifera ethanolic extract was orally administered at 600 mg/kg body weight in obese female rats daily for 12 weeks. At the end of treatment, body weight was determined, and the atherogenic index, coronary artery index, glucose level, insulin resistance status, liver and kidney functions were assessed. Also, the m RNA of leptin, adiponectin and resistin in visceral adipose tissue was determined by quantitative real time-PCR.Results: The results showed that M. oleifera extract down-regulated m RNA expression of leptin and resistin, while it up-regulated adiponectin gene expression in obese rats relative to untreated obese control counterparts. This amelioration of genes expression was paralleled by a reduction in body weight and improvement of the atherogenic index and coronary artery index, as well as glucose level and insulin resistance value without adverse effects on liver or kidney functions, versus the untreated obese control ones.Conclusions: It is reasonable to assume that the anti-obesity, anti-atherogenic and antidiabetic properties of M. oleifera are mechanistically achieved via working directly on the adipokines of the visceral adipose tissue. 17Outcomes of pancreaticoduodenectomy in elderly patients显示文摘BACKGROUND: Although the mortality and morbidity of pancreaticoduodenectomy(PD) have improved significantly over the past years, the concerns for elderly patients undergoing PD are still present. Furthermore, the frequency of PD is increasing because of the increasing proportion of elderly patients and the increasing incidence of periampullary tumors. This study aimed to analyze the outcomes of PD in elderly patients.METHODS: We studied all patients who had undergone PD in our center between January 1995 and February 2015. The patients were divided into three groups based on age: group I(patients aged <60 years), group II(those aged 60 to 69 years) and group III(those aged ≥70 years). The primary outcome was the rate of total postoperative complications. Secondary endpoint included total operative time, hospital mortality, length of postoperative hospital stay, delayed gastric emptying, re-exploration, and survival rate.RESULTS: A total of 828 patients who had undergone PD for resection of periampullary tumor were included in this study. There were 579(69.9%) patients in group I, 201(24.3%) in group II, and 48(5.8%) in group III. The overall incidence of complications was higher in elderly patients(25.9% in group I, 36.8% in group II, and 37.5% in group III; P=0.006). There were more patients complicated with delayed gastric emptying in group II compared with the other two groups. There was no significant difference in the incidence of postoperative pancreatic fistula, biliary leakage, pancreatitis, pulmonary complications and hospital mortality.CONCLUSIONS: PD can be performed safely in selected elderly patients. Advanced age alone should not be a contraindication for PD. 18Efficient Channel Estimation Techniques for MIMO Systems with 1-Bit ADC显示文摘With a low resolution 1-bit ADC on its receiver(RX) side, MIMO with 1-bit ADC took a considerable step in the fulfillment of the hardware complexity constrains of the internet of things(IoT) PHY layer design. However, applying 1-bit ADC at MIMO RX results in severe nonlinear quantization error. By which, almost all received signal amplitude information is completely distorted. Thus, MIMO channel estimation is considered as a major barrier towards practical realization of 1-bit ADC MIMO system. In this paper, two efficient sparsity-based channel estimation techniques are proposed for 1-bit ADC MIMO systems, namely the low complexity sparsity-based channel estimation(LCSCE), and the iterative adaptive sparsity channel estimation(IASCE). In these techniques, the sparsity of the 1-bit ADC MIMO channel is exploited to propose a new adaptive and iterative compressive sensing(CS) recovery algorithm to handle the 1-bit ADC quantization effect. The proposed algorithms are tested with the state-of-the-art 1-bit ADC MIMO constant envelope modulation(MIMO-CEM). The 1-bit ADC MIMO-CEM system is chosen as it fulfills both energy and hardware complexity constraints of the IoT PHY layer. Simulation results reveal the high effectiveness of the proposed algorithms in terms of spectral efficiency(SE) and computational complexity. The proposed LCSCE reduces the computational complexity of the 1-bit ADC MIMO-CEM channel estimation by 86%, while the IASCE reduces it by 96% compared to the recent techniques of MIMO-CEM channel estimation. 19Plasma D-dimer level in early and late-onset neonatal sepsis显示文摘BACKGROUND Neonatal sepsis is a life-threatening disease.Early diagnosis is essential,but no single marker of infection has been identified.Sepsis activates a coagulation cascade with simultaneous production of the D-dimers due to lysis of fibrin.Ddimer test reflects the activation of the coagulation system.AIM To assess the D-dimer plasma level,elaborating its clinicopathological value in neonates with early-onset and late-onset neonatal sepsis.METHODS The study was a prospective cross-sectional study that included ninety neonates;divided into three groups:Group I:Early-onset sepsis(EOS);Group II:Late-onset sepsis(LOS);and GroupⅢ:Control group.We diagnosed neonatal sepsis according to our protocol.C-reactive protein(CRP)and D-dimer assays were compared between EOS and LOS and correlated to the causative microbiological agents.RESULTS D-dimer was significantly higher in septic groups with a considerably higher number of cases with positive D-dimer.Neonates with LOS had substantially higher levels of D-dimer than EOS,with no significant differences in CRP.Neonates with LOS had a significantly longer hospitalization duration and higher gram-negative bacteriemia and mortality rates than EOS(P<0.01).Gramnegative bacteria have the highest D-dimer levels(Acinetobacter,Klebsiella,and Pseudomonas)and CRP(Serratia,Klebsiella,and Pseudomonas);while gram-positive sepsis was associated with relatively lower levels.D-dimer had a significant negative correlation with hemoglobin level and platelet count;and a significant positive correlation with CRP,hospitalization duration,and mortality rates.The best-suggested cut-off point for D-dimer in neonatal sepsis was 0.75 mg/L,giving a sensitivity of 72.7%and specificity of 86.7%.The D-dimer assay has specificity and sensitivity comparable to CRP in the current study.CONCLUSION The current study revealed a significant diagnostic value for D-dimer in neonatal sepsis.D-dimer can be used as an adjunct to other sepsis markers to increase the sensitivity and specificity of diagnosing neonatal sepsis.Mohammed Al-Biltagi Ehab M Hantash Mohammed Ramadan El-Shanshory Enayat Aly Badr Mohamed Zahra Manar Hany Anwar 2022World Journal of Critical Care Medicine2022,11,3:2
