1Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases显示文摘AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary tumors.METHODS This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the periods.CONCLUSION Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. 2Clinicopathological features and surgical outcome of patients with fibrolamellar hepatocellular carcinoma(experience with 22 patients over a 15-year period)显示文摘AIM To evaluate the clinicopathological features and the surgical outcomes of patients with fibrolamellar hepato-cellular carcinoma(FL-HCC)over a 15-year period. METHODS This is a retrospective study including 22 patients with a pathologic diagnosis of FL-HCC who underwent hepatectomy over a 15-year period. Tumor characteristics,survival and recurrence were evaluated. RESULTS There were 11 male and 11 female with a median age of 29 years(range from 21 to 58 years). Two(9%)patients had hepatitis C viral infection and only 2(9%)patients had alpha-fetoprotein level > 200 ng/m L. The median size of the tumors was 12 cm(range from 5-20 cm). Vascular invasion was detected in 5(23%)patients. Four(18%)patients had lymph node metastases. The median follow up period was 42 mo and the 5-year survival was 65%. Five(23%)patients had a recurrent disease,4 of them had a second surgery with 36 mo median time interval. Vascular invasion is the only significant negative prognostic factor CONCLUSION FL-HCC has a favorable prognosis than common HCC and should be suspected in young patients with non cirrhotic liver. Aggressive surgical resection should be done for all patients. 3Predictors of long term survival after hepatic resection for hilar cholangiocarcinoma:A retrospective study of 5-year survivors显示文摘AIM:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived < 5 years.METHODS:This is a retrospective study of patients with pathologically proven HC who underwent surgical resection at the Gastroenterology Surgical Center,Mansoura University,Egypt between January 2002 and April 2013.All data of the patients were collected from the medical records.patients were divided into two groups according to their survival:patients surviving less than 5 years and those who survived > 5 years.RESULTS:There were 34(14%) long term survivors(5 year survivors) among the 243 patients.Fiveyear survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two groups.The level of CA 19-9 was significantly higher in patients surviving < 5 years(395.71 ± 31.43 vs 254.06 ± 42.19,p = 0.0001).Univariate analysis demonstrated nine variables to be significantly associated with survival > 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.Mohamed Abd El Wahab Ayman El Nakeeb Ehab El Hanafy Ahmad M Sultan Ahmed Elghawalby Waleed Askr Mahmoud Ali Mohamed Abd El Gawad Tarek Salah 2016World Journal of Gastrointestinal Surgery2016,8,6:10
4Assessment of KL-6 as a tumor marker in patients with hepatocellular carcinoma显示文摘AIM: To investigate the clinical significance of KL-6 as a tumor marker of HCC in two different ethnic groups with chronic liver disease consecutively encountered at outpatient clinics.METHODS: Serum KL-6 was measured by the sandwich enzyme immunoassay method using the KL-6 antibody (Ab) as both the capture and tracerAb according to the manufacturer's instructions (Eisai, Tokyo, Japan).Assessment of alpha fetoprotein (AFP) and protein induced vitamin K deficiency or absence (PIVKA-Ⅱ) was performed in both groups using commercially available kits.RESULTS: A significantly higher mean serum KL-6(556±467 U/L) was found in HCC in comparison with non-HCC groups either with (391±176 U/L; P<0.001)or without (361±161 U/L; P<0.001) liver cirrhosis (LC).Serum KL-6 level did not correlate with either AFP or PIVKA-Ⅱ serU/Levels. Using receiver operating curve analysis for KL-6 as a predictor for HCC showed that the area under the curve was 0.574 (95%CI = 0.50-0.64)and the KL-6 level that gave the best sensitivity (61%) was found to be 334 U/L but according to the manufacturer's instructions; a cut-off point of 500 U/Lwas used that showed the highest specificity (80%)in comparison with AFP and PIVKA-Ⅱ (78% vs 72%respectively). Combining the values of the three markers improved specificity of AFP for HCC diagnosis from 78%for AFP alone; 93% for AFP plus PIVKA-Ⅱ to 99% for both plus KL-6 value (P<0.001). 5Bioefficacy of Mentha piperita essential oil against dengue fever mosquito Aedes aegypti L显示文摘Objective:To assess the larvicidal and repellent potential of the essential oil extracted from the leaves of peppermint plant,Mentha piperita(M.piperita)against the larval and adult stages of Aedes aegypli(Ae.Aegypti).Methods:The larvicidal potential of peppermint oil was evaluated against early fourth instar larvae of Ae.aegypli using WHO protocol.The mortality counts were made after 24 and 48 h,and LC_(50)and LC_(90)values were calculated.The efficacy of peppermint oil as mosquito repellent was assessed using the human-bait technique.The measured area of one arm of a human volunteer was applied with the oil and the other arm was applied with ethanol.The mosquito bites on both the arms were recorded for 3 rain after every 15 min.The experiment continued for 3 h and the percent protection was calculated.Results:The essential oil extracted from M.piperita possessed excellent larvicidal efficiency against dengue vector.The bioassays showed an LC_(50)and LC_(90)value of 111.9 and 295.18 ppm,respectively after 24 h of exposure.The toxicity of the oil increased 11.8%when the larvae were exposed to the oil for 48h.The remarkable repellent properties of M.piperita essential oil were established against adults Ae.aegypti.The application of oil resulted in 100%protection till 150 min.After next 30 min,only 1-2 bites were recorded as compared with 8-9 bites on the control arm.Conclusions:The peppermint essential oil is proved to be efficient larvicide and repellent against dengue vector.Further studies are needed to identify the possible role of oil as adulticide.oviposition deterrent and ovicidal agent.The isolation of active ingredient from the oil could help in formulating strategies for mosquito control.Sarita Kumar Naim Wahab Radhika Warikoo 2011Asian Pacific Journal of Tropical Biomedicine2011,1,2:5
6Numerical simulation of multi-phase combustion flow in solid rocket motors with metalized propellant显示文摘Multi-phase flowfield simulation has been performed on solid rocket motor and effect of multi-phases on the performance prediction of the solid rocket motor(SRM) is investigation.During the combustion of aluminized propellant,the aluminum particles in the propellant melt and formliquid aluminum at the burning propellant surface.So the flow within the rocket motor is multi phase or two phase because it contains droplets and smoke particles of Al2O3.Flowsi mulations have been performed on a large scale motor,to observe the effect of the flowfield onthe chamber and nozzle as well.Uniform particles diameters and Rosin-Rammler diameter distribution method that is based on the assumption that an exponential relationship exists betweenthe droplet diameter,dand mass fraction of droplets with diameter greater thandhave been used for the si mulation of different distribution of Al2O3 droplets present in SRM.Particles sizes in the range of 1-100μm are used,as being the most common droplets.In this approachthe complete range of particle sizes is dividedinto a set of discrete size ranges,eachto be defined by single streamthat is part of the group.Roe scheme-flux differencing splitting based on approxi mate Riemann problem has been used to si mulate the effects of the multi-phase flowfeild.This is second order upwind scheme in which flux differencing splitting method is employed.To cater for the turbulence effect,Spalart-All maras model has been used.The results obtained show the great sensitivity of this diameters distribution and particles concentrations to the SRMflowdynamics,primarily at the motor chamber and nozzle exit.The results are shown with various sizes of the particles concentrations and geometrical configurations including models for SRM and nozzle.The analysis also provides effect of multi-phase on performance prediction of solid rocket motor.SHAFQAT Wahab 2009航空动力学报2009,24,7:4

8Impact of cirrhosis on surgical outcome after pancreaticoduodenectomy显示文摘AIM:To elucidate surgical outcomes of pancreaticoduodenectomy(PD)in patients with liver cirrhosis.METHODS:We studied retrospectively all patients who underwent PD in our centre between January 2002and December 2011.Group A comprised patients with cirrhotic livers,and Group B comprised patients with non-cirrhotic livers.The cirrhotic patients had ChildPugh classes A and B(patient’s score less than 8).Preoperative demographic data,intra-operative data and postoperative details were collected.The primary outcome measure was hospital mortality rate.Secondary outcomes analysed included duration of the operation,postoperative hospital stay,postoperative morbidity and survival rate.RESULTS:Only 67/442 patients(15.2%)had cirrhotic livers.Intraoperative blood loss and blood transfusion were significantly higher in group A(P=0.0001).The mean surgical time in group A was significantly longer than that in group B(P=0.0001).Wound complications(P=0.02),internal haemorrhage(P=0.05),pancreatic fistula(P=0.02)and hospital mortality(P=0.0001)were significantly higher in the cirrhotic patients.Postoperative stay was significantly longer in group A(P=0.03).The median survival was 19 mo in group A and 24 mo in group B.Portal hypertension(PHT)was present in 16/67 cases of cirrhosis(23.9%).The intraoperative blood loss and blood transfusion were significantly higher in patients with PHT(P=0.001).Postoperative morbidity(0.07)and hospital mortality(P=0.007)were higher in cirrhotic patients with PHT.CONCLUSION:Patients with periampullary tumours and well-compensated chronic liver disease should be routinely considered for PD at high volume centres with available expertise to manage liver cirrhosis.PD is associated with an increased risk of postoperative morbidity in patients with liver cirrhosis;therefore,it is only recommended in patients with Child A cirrhosis without portal hypertension.Ayman El Nakeeb Ahmad M Sultan Tarek Salah Mohamed El Hemaly Emad Hamdy Ali Salem Ahmed Moneer Rami Said Ahmed AbuEleneen Mostafa Abu Zeid Talaat Abdallah Mohamed Abdel Wahab 2013World Journal of Gastroenterology2013,19,41:3

11Simultaneous removal of Congo red and cadmium(Ⅱ) from aqueous solutions using graphene oxide–silica composite as a multifunctional adsorbent显示文摘Graphene oxide is a very high capacity adsorbent due to its functional groups and π-π interactions with other compounds. Adsorption capacity of graphene oxide, however, can be further enhanced by having synergistic effects through the use of mixed-matrix composite. In this study, silica-decorated graphene oxide(SGO) was used as a high-efficiency adsorbent to remove Congo red(CR) and Cadmium(Ⅱ) from aqueous solutions. The effects of solution initial concentration(20 to 120 mg/l), solution pH(pH 2 to 7), adsorption duration(0 to 140 min) and temperature(298 to 323 K) were measured in order to optimize the adsorption conditions using the SGO adsorbent. Morphological analysis indicated that the silica nanoparticles could be dispersed uniformly on the graphene oxide surfaces. The maximum capacities of adsorbent for effective removal of Cd(Ⅱ) and CR were 43.45 and 333.33 mg/g based on Freundlich and Langmuir isotherms, respectively. Langmuir and Freundlich isotherms displayed the highest values of Q max for CR and Cd(Ⅱ) adsorption in this study, which indicated monolayer adsorption of CR and multilayer adsorption of Cd(II) onto the SGO, respectively. Thermodynamic study showed that the enthalpy( H) and Gibbs free energy( G) values of the adsorption process for both pollutants were negative, suggesting that the process was spontaneous and exothermic in nature. 13Martensitic transformation and magnetic properties of aged Ni-Fe-Ga-Ti shape memory alloy显示文摘This article reports the efect of ageing on the microstructure,martensitic transformation,magnetic properties,and mechanical properties of Ni51Fe18Ga27Ti4shape memory alloy.There are fve specimens of this alloy aged at 573 up to 973 K for 3 h per each.This range of ageing temperature greatly afects the microstructure of the alloy.As the ageing temperature increased from 573 up to 973 K,the microstructure of Ni51Fe18Ga27Ti4alloy gradually changed from the entirely martensitic matrix at 573 K to the fully austenitic microstructure at 973 K.The volume fraction of precipitated Ni3Ti particles increased with the ageing temperature increasing from 573 to 773 K.Further increasing the ageing temperature to 973 K decreased the content of Ni3Ti in the microstructure.The martensitic transformation temperature was decreased steadily by increasing the ageing temperature.The magnetization saturation,remnant magnetization,and coercivity increased with the ageing temperature increasing up to 773 K.A further increase in ageing temperature decreased these magnetic properties.Moreover,the hardness values were gradually increased at frst by increasing the ageing temperature to 773 K,and then dramatically decreased to the lowest value at 973 K.Nader El-Bagoury Mahmoud Mohamed Hessien Magdy Abdel Wahab Kaseem 2013International Journal of Minerals,Metallurgy and Materials2013,20,9:2

16Outcomes of pancreaticoduodenectomy in elderly patients显示文摘BACKGROUND: Although the mortality and morbidity of pancreaticoduodenectomy(PD) have improved significantly over the past years, the concerns for elderly patients undergoing PD are still present. Furthermore, the frequency of PD is increasing because of the increasing proportion of elderly patients and the increasing incidence of periampullary tumors. This study aimed to analyze the outcomes of PD in elderly patients.METHODS: We studied all patients who had undergone PD in our center between January 1995 and February 2015. The patients were divided into three groups based on age: group I(patients aged <60 years), group II(those aged 60 to 69 years) and group III(those aged ≥70 years). The primary outcome was the rate of total postoperative complications. Secondary endpoint included total operative time, hospital mortality, length of postoperative hospital stay, delayed gastric emptying, re-exploration, and survival rate.RESULTS: A total of 828 patients who had undergone PD for resection of periampullary tumor were included in this study. There were 579(69.9%) patients in group I, 201(24.3%) in group II, and 48(5.8%) in group III. The overall incidence of complications was higher in elderly patients(25.9% in group I, 36.8% in group II, and 37.5% in group III; P=0.006). There were more patients complicated with delayed gastric emptying in group II compared with the other two groups. There was no significant difference in the incidence of postoperative pancreatic fistula, biliary leakage, pancreatitis, pulmonary complications and hospital mortality.CONCLUSIONS: PD can be performed safely in selected elderly patients. Advanced age alone should not be a contraindication for PD. 17Larvicidal and irritant activities of hexane leaf extracts of Citrus sinensis against dengue vector Aedes aegypti L.显示文摘Objective:To assess the larvicidal and irritant activities of the hexane extracts of leaves of Citrus sinensis(C.sinensis)against the early fourth instars and female adults of Aedes aegypti(Ae.aegypti).Methods:The larvicidal potential of the prepared leaf extract was evaluated against early fourth instar larvae of Ae.aegypli using WHO protocol.The mortality counts were made after 24 h and LC_(50)and LG_(50)values were calculated.The efficacy of extract as mosquito irritant was assessed by contact irritancy assays.Extract-impregnated paper was placed on a glass plate over which a perspex funnel with a hole on the top was kept inverted.Single female adult,3-day old unfed/blood-fed,was released inside the funnel.After 3 min of acclimatization time,the time taken for the first take-off and total number of flights undertaken during 15 min were scored.Results:The citrus leaf extracts from hexane possessed moderate larvicidal efficiency against dengue vector.The bioassays resulted in an LC_(50)and LC_(90)value of 446.84 and 1370.96 ppm,respectively after 24 h of exposure.However,the extracts were proved to be remarkable irritant against adults Ae.aegypti,more pronounced effects being observed on blood-fed females than unfed females.The extract-impregnated paper was thus proved to be 7-11 times more irritable as compared with the control paper.Conclusions:The hexane extracts from C.sinensis leaves are proved to be reasonably larvicidal But remarkably irritant against dengue vector.Further studies are needed to identify the possible role of extract as adulticide,oviposition deterrent and ovicidal agent.The isolation of active ingredient from the extract could help in formulating strategies for mosquito control.Radhika Warikoo Ankita Ray Jasdeep Kaur Sandhu Roopa Sanial Naim Wahab Sarita Kumar 2012Asian Pacific Journal of Tropical Biomedicine2012,2,2:2
18MANAGING CONSTRUCTION RISKS OF AP1000 NUCLEAR POWER PLANTS IN CHINA显示文摘Large and complex construction projects face risk from various sources and the successful completion of such projects depends on effective risk management.This study investigates the risk faced by Chinese firms participating in constructing AP1000 nuclear power plants in China.AP1000 nuclear reactors are new,Generation III+ reactors designed by Westinghouse and to be built first in China.The semi-structured interview approach is used to elicit information from experts involved in the AP1000 projects in China.Based on the interviews,various sources of risk are identified.In addition to general risks that megaprojects normally face,there are unique risks that arise from various sources such as technological,political,organizational,and individual personnel risks.Risk management strategies are proposed to manage general and unique risks identified in the study.The findings of this study would be helpful for Chinese companies involved in the construction of AP1000 nuclear power plants to mitigate the risks associated with the projects.Shufeng WANG M.I.M. 19市售长叶松油和锡兰肉桂油对登革热媒蚊埃及伊蚊的灭幼效果(英文)显示文摘松油和桂皮油由于具有芳香性气味,因而成为良好的成虫驱避剂,但是关于它们对蚊虫的杀幼虫作用研究不多。为揭示市售的长叶松Pinus longifolia油和锡兰肉桂Cinnamomum zeylanicum油对来源于印度德里的埃及伊蚊Aedes aegypti4龄幼虫的毒杀潜力,我们进行了室内研究,以幼虫死亡率及行为改变和形态改变等指标评估其杀幼虫潜力。结果表明:两种油对埃及伊蚊4龄幼虫均具有毒杀作用,且松油的灭幼效果优于桂皮油。松油的LC50和LC70值分别为0.33093mg/L和0.54476mg/L,而桂皮油的LC50和LC70值分别为0.63159mg/L和0.77736。进一步观察发现,LC90剂量下桂皮油的杀幼虫潜力强于松油,其LC90为1.11879mg/L,而松油的LC90为1.04915mg/L。在处理的幼虫中观察到行为改变,如兴奋、坐立不安、颤抖、痉挛然后瘫痪,说明这两种油可能对其神经肌肉系统产生了影响。显微观察处理幼虫的形态改变发现,与对照相比,大多数器官的外观正常,只是肛鳃略为内收缩而引起结构畸形,提示肛腮可能是这两种油的作用位点,腮的功能异常引起了幼虫死亡。这两种油品可开发用作防治蚊虫的新型杀幼虫药剂。Radhika WARIKOO Naim WAHAB Sarita KUMAR 2011昆虫学报2011,54,7:2
20腊肠树叶绿体基因组序列特征及其系统发育分析显示文摘目的获取腊肠树(Cassia fistula L.)叶绿体基因组信息特征,对其在决明属(Cassia Linn.)中的系统发育位置进行研究,为腊肠树有效的开发、保护和利用提供理论依据。方法使用Illumina测序技术对腊肠树进行测序,并结合生物信息学方法对其进行叶绿体基因组组装、注释和系统发育分析。结果腊肠树叶绿体基因组长度为161,724 bp,具有典型的四分体结构,总GC含量为36.1%,包含128个基因,其中83个蛋白质编码基因,37个tRNA基因和8个rRNA基因。系统发育分析表明,7个决明属物种被聚为3支,美丽决明S.spectabilis、望江南S.occidentalis、铁刀木S.siamea和决明S.tora聚为一支,腊肠树C.fistula与布鲁斯特决明C.brewsteri聚为一支,山扁豆Chamaecrista mimosoides单独聚为一支,且腊肠树与布鲁斯特决明C.brewsteri的亲缘关系最近。结论从叶绿体基因组系统发育的角度来看,本研究支持2010版的Flora of China的决明亚族植物种类的划分结论,决明属物种中的美丽决明S.spectabilis、望江南S.occidentalis、铁刀木S.siamea和决明S.tora应归为番泻决明属(Senna),腊肠树C.fistula应归为决明属(Cassia),山扁豆Ch.mimosoide应归为山扁豆属(Chamaecrista)。向如双 段宝忠 孙伟 宋驰 王艳 Atia tul Wahab 孟祥霄 2022环球中医药2022,15,12:2
