| 1 | Deep sedation during gastrointestinal endoscopy: Propofol-fentanyl and midazolam-fentanyl regimens显示文摘AIM: To compare deep sedation with propofol-fentanyl and midazolam-fentanyl regimens during upper gastrointestinal endoscopy. METHODS: After obtaining approval of the research ethics committee and informed consent, 200 patients were evaluated and referred for upper gastrointestinal endoscopy. Patients were randomized to receive propofol-fentanyl or midazolam-fentanyl (n = 100/group).We assessed the level of sedation using the observer's assessment of alertness/sedation (OAA/S) score and bispectral index (BIS). We evaluated patient and physician satisfaction, as well as the recovery time and complication rates. The statistical analysis was performed using SPSS statistical software and included the MannWhitney test, χ 2 test, measurement of analysis of variance, and the κ statistic. RESULTS: The times to induction of sedation, recovery, and discharge were shorter in the propofolfentanyl group than the midazolam-fentanyl group. According to the OAA/S score, deep sedation events occurred in 25% of the propofol-fentanyl group and 11% of the midazolam-fentanyl group (P = 0.014). Additionally, deep sedation events occurred in 19% of the propofol-fentanyl group and 7% of the midazolamfentanyl group according to the BIS scale (P = 0.039). There was good concordance between the OAA/S score and BIS for both groups (κ = 0.71 and κ = 0.63, respectively). Oxygen supplementation was required in 42% of the propofol-fentanyl group and 26% of the midazolam-fentanyl group (P = 0.025). The mean time to recovery was 28.82 and 44.13 min in the propofolfentanyl and midazolam-fentanyl groups, respectively (P < 0.001). There were no severe complications in either group. Although patients were equally satisfied with both drug combinations, physicians were more satisfied with the propofol-fentanyl combination. CONCLUSION: Deep sedation occurred with propofolfentanyl and midazolam-fentanyl, but was more frequent in the former. | 2 | Association of Helicobacter pylori infection and giardiasis: Results from a study of surrogate markers for fecal exposure among children显示文摘AIM: To investigate whether Helicobacter pylori (H pykori)infection is associated with hepatitis A virus (HAV) infection, presence of enteroparasites, and other surrogates of fecal exposure.METHODS: We conducted a cross-sectional study in 121 children consecutively admitted at a pediatric hospital inSalvador, Brazil.H pylori and HAV infection were identified by the presence of serum antibodies. Stool specimens were examined for the presence of ova and parasites. A structured questionnaire inquiring about sanitary conditions and life style was applied to each subject. RESULTS: Fifty-one of the 121 children (42.1%) werefound to be seropositive for H pylori, and 45 (37.2%) for HAV. The seroprevalence ofHpyloriand HAV both increased significantly with age. Cross-tabulation of data showed that 26 (21.5%) were seropositive and 51 (42.1%) were negative for bothHpyloriand HAV antibodies (χ2 = 7.18,OR = 2.8, CI 1.30-5.97). The age adjusted OR for an HAV-infected child being H pylori positive was 2.3 (CI1.02-5.03). The agreement between H pylori and HAVseropositivity was fair (κ = 0.24). After controlling for possible confounding, the variables remaining independently associated with seropositivity to H pyloriwere age,presence of Giardia lamblia in feces (OR = 3.2, 95%CI, 1.1-9.5) and poor garbage disposal quality (OR = 2.4,95%CI, 1.1-5.1).CONCLUSION: Our data suggest that H pylori infection is associated with surrogate markers of fecal exposure. Thus, we conclude that the fecal-oral route is relevant in the transmission of HP among children in an urban setting of a developing country. The association observed between G. lamblia and H pylori infection may have several explanations. | 3 | Thermal behaviour analysis in a porcelainhoused ZnO surge arrester by computer simulations and thermography显示文摘In this article,an analysis of heat transfer in a porcelain-housed ZnO surge arrester is presented.Due to the fact that ZnO-based surge arresters are key components for protection and reliability of electrical systems,it is necessary to seek techniques to assure correct assessment.In porcelain-housed arresters,the air gap leads to an increased thermal resistance between the ambient and the varistor column.Consequently,a thermal analysis must consider convection and radiation heat transfer mechanisms,besides heat conduction.This study proposes the use of computational simulations in combination with thermography as a tool for a temperature-based estimation of the varistor state.For the analysis,temperature rise tests were performed in a 69-kV ceramic-housed arrester to measure material emissivity and produce data to fit the simulated response.Furthermore,the thermal response of the surge arresters subject to an impulsive energy input was evaluated.The simulations allow to relate varistors temperature and housing temperature with regard to a specified amount of dissipated power.The obtained results show that the used technique can be an effective method for arrester monitoring and may allow to define assessing criteria for a porcelain-housed arrester based on housing temperature measurements. | Arthur F.Andrade Edson G.Costa Josué M.B.Fernandes Helem M.M.Alves Cícero R.C.Amorim Filho | 2019 | High Voltage2019，4，3： | 2 |
