| 1 | Intentional ingestions of foreign objects among prisoners: A review显示文摘The intentional ingestion of foreign objects(IIFO) is described more commonly in prison populations than in the general population, with an estimated annual incidence of 1 in 1900 inmates in our state correctional facilities. Incidents often involve ingestion of small metal objects(e.g., paperclips, razor blades) or other commonly available items like pens or eating utensils. Despite ingestion of relatively sharp objects, most episodes can be clinically managed with either observation or endoscopy. Surgery should be reserved for those with signs or symptoms of gastrointestinal perforation or obstruction. For those with a history of IIFO, efforts should focus on prevention of recurrenceas subsequent episodes are associated with higher morbidity, significant healthcare and security costs. The pattern of IIFO is often repetitive, with escalation both in frequency of ingestions and in number of items ingested. Little is known about successful prevention strategies, but efforts to monitor patients and provide psychiatric care are potential best-practice strategies. | 2 | Superior growth performance in broiler chicks fed chelated compared to inorganic zinc in presence of elevated dietary copper显示文摘Background: The goal of this study was to compare the antagonism of elevated dietary Cu(250 mg/kg) from Cu SO4 on three different Zn sources(Zn SO4· H2O; [Zn bis(-2-hydroxy-4-(methylthio)butanoic acid)], Zn(HMTBa)2,a chelated Zn methionine hydroxy analogue; and Zn-Methionine), as measured using multiple indices of animal performance in ROSS 308 broilers.Methods: Three experiments were conducted in broiler chicks fed a semi-purified diet. Al birds were fed a Zn-deficient diet(8.5 mg/kg diet) for 1 wk, and then provided with the experimental diets for 2 wks.Results: Experiment 1 was a 2 × 2 factorial design with two levels of Cu(8 vs. 250 mg/kg diet from Cu SO4) and two Zn sources at 30 mg/kg [Zn SO4· H2 O vs. Zn(HMTBa)2]. Elevated Cu impaired growth performance only in birds fed Zn SO4.Compared to Zn SO4· H2 O, Zn(HMTBa)2improved feed intake(12 %; P < 0.001) and weight gain(12 %, P < 0.001) and the benefits were more pronounced in the presence of 250 mg/kg diet Cu. Experiment 2 was a dose titration of Zn SO4· H2 O and Zn(HMTBa)2at 30, 45, 60, and 75 mg/kg diet in the presence of 250 mg/kg Cu SO4. Feed:gain was decreased and tibia Zn was increased with increasing Zn levels from 30 to 75 mg/kg. Birds fed Zn(HMTBa)2consumed more food and gained more weight compared to birds fed Zn SO4, especially at lower supplementation levels(30 and45 mg/kg; interaction P < 0,05). Experiment 3 compared two organic Zn sources(Zn(HMTBa)2vs. Zn-Methionine)at 30 mg/kg with or without 250 mg/kg Cu SO4. No interactions were observed between Zn sources and Cu levels on performance or tissue mineral concentrations. High dietary Cu decreased weight gain(P < 0.01). Tibia Cu and liver Cu were significantly increased with 250 mg/kg dietary Cu supplementation(P < 0.01). No difference was observed between the two Zn sources.Conclusions: Dietary 250 mg/kg Cu significantly impaired feed intake and weight gain in birds fed Zn SO4· H2 O,but had less impact in birds fed Zn(HMTBa)2. No difference was observed between the two organic zinc sources.These results are consistent with the hypothesis that chelated organic Zn is better utilized than inorganic zinc in the presence of elevated Cu. | Junmei Zhao Robert B. | 3 | Pioneering function of Isl1 in the epigenetic control of cardiomyocyte cell fate显示文摘Generation of widely differing and specialized cell types from a single totipotent zygote involves large-scale transcriptional changes and chromatin reorganization. Pioneer transcription factors play key roles in programming the epigenome and facilitating recruitment of additional regulatory factors during successive cell lineage specification and differentiation steps. Here we show that Isl1 acts as a pioneer factor driving cardiomyocyte lineage commitment by shaping the chromatin landscape of cardiac progenitor cells. Using an Isl1 hypomorphic mouse line which shows congenital heart defects, genome-wide profiling of Isl1 binding together with RNA- and ATAC-sequencing of cardiac progenitor cells and their derivatives, we uncover a regulatory network downstream of Isl1 that orchestrates cardiogenesis. Mechanistically, we show that Isl1 binds to compacted chromatin and works in concert with the Brg1-Baf60c-based SWI/SNF complex to promote permissive cardiac lineage-specific alterations in the chromatin landscape not only of genes with critical functions in cardiac progenitor cells, but also of cardiomyocyte structural genes that are highly expressed when Isl1 itself is no longer present. | 4 | Inducible resistance to Fas-mediated apoptosis in B cells显示文摘Apoptosis produced in B cells through Fas (APO-1, CD95) triggering is regulated by signals derived from other surface receptors: CD40 engagement produces upregulation of Fas expres. sion and marked susceptibility to Fas-induced cell death, whereas antigen receptor engagement, or IL-4R engagement, inhibits Fas killing and in so doing induces a state of Fas-resistance, even in otherwise sensitive, CD40-stimulated targets. Surfaceim. munoglobulin and IL-4R utilize at least partially distinct pathways to produce Fas-resistance that differentially depend on PKC and STAT6, respectively. Further, surface immunoglob- 'lin signaling for inducible Fas-resistance bypasses Bib, requires NF-GB, and entails new macromolecular synthesis. Terminal effectors of B cell Fas-resistance include the known anti-apoptotic gene products, Bcl-xL and FLIP, and a novel anti-apoptotic gene that encodes FAIM (Fas Apoptosis Inhibitory Molecule). faim was identified by differential display and exists in two alternatively spliced forms; maim-S is broadly expressed, but faim-L expression is tissue-specific. The FAIM sequence is highly evolu tionarily conserved, suggesting an important role for this molecule throughout phylogeny. Inducible resistance to Fas killing is hypothesized to protect foreign antigen-specific B cells during potentially hazardous interactions with FasL-bearing T cells, whereas autoreactive B cells fail to become Fas-resistant and are deleted via Fas-dependent cytotoxicity. | 5 | Feasibility of confocal endomicroscopy in the diagnosis of pediatric gastrointestinal disorders显示文摘AIM:To evaluate the feasibility and utility of confocal laser endomicroscopy(CLE) in the description of normal gastrointestinal(GI) mucosa and in the diagnosis of GI disorders in children,in comparison to histology.METHODS:Forty-four patients(19 female) median age 10.9 years(range 0.7-16.6 years) with suspected or known GI pathology underwent esophago-gastro-duodenoscopy(OGD)(n = 36) and/or ileocolonoscopy(IC)(n = 31) with CLE using sodium fluorescein and acriflavine as contrast agents.Histological sections were compared with same site confocal images by two experienced pediatric and GI histopathologists and endoscopists,respectively.RESULTS:Duodenum and ileum were intubated in all but one patient undergoing OGD and IC.The median procedure time was 16.4 min(range 7-25 min) for OGD and 27.9 min(range 15-45 min) for IC.A total of 4798 confocal images were compared with 153 biopsies from the upper GI tract from 36 procedures,and 4661 confocal images were compared with 188 biopsies from the ileocolon from 31 procedures.Confocal images were comparable to conventional histology both in normal and in pathological conditions such as esophagitis,Helicobacter pylori gastritis,celiac disease,inflammatory bowel disease,colonic heterotopia,and graft versus host disease.CONCLUSION:CLE offers the prospect of targeting biopsies to abnormal mucosa,thereby increasing diagnostic yield,reducing the number of biopsies,decreasing the burden on the histopathological services,and reducing costs. | Krishnappa Venkatesh Marta Cohen Clair Evans Peter Delaney Steven Thomas Christopher Taylor Ashraf Abou-Taleb Ralf Kiesslich Mike Thomson | 2009 | World Journal of Gastroenterology2009,15,18: | 2 |
