9WONCA研究论文摘要汇编——高血压患者的血压自我监测:针对英国全科医生的基于互联网的调查显示文摘背景已有研究表明,英国大多数的全科医生使用血压自我监测(SMBP)来控制而非诊断高血压。本文旨在评估SMBP的当前使用情况,以及使用中依据较多的指南建议的变化。目的调查2015年英国全科医生关于SMBP的观点和做法,并将其与2011年的调查结果进行比较。设计与场所选择有区域代表性的样本共计300例英国全科医生进行网络调查。方法全科医生完成了关于高血压管理中使用SMBP的在线调查问卷。进行了描述性分析、组间比较及多因素Logistic回归分析。结果结果来自300例全科医生。全科医生记录显示,使用SMBP的目的分别是诊断高血压〔169/291,58%,95%CI(52%,64%)〕和控制高血压〔245/291,84%,95%CI(80%,88%)〕,前者较2011年显著增加〔37%,95%CI(33%,41%),P<0.001〕,而后者没有明显变化。超过半数的全科医生使用高于指南中建议的收缩压阈值来诊断〔118/169,70%,95%CI(63%,77%)〕和治疗〔168/225,75%,95%CI(69%,80%)〕高血压,不到半数的全科医生〔120/289,42%,95%CI(36%,47%)〕调整了SMBP结果来指导治疗决策。结论自2011年新版英国国家指南推出以来,全科医生更倾向于使用SMBP来诊断高血压。然而,全科医生继续使用非标准诊断和监控阈值所占比例增多,如果使用次优的阈值采用诊室外方法来提高诊断准确性是无益处的。FLETCHER B R HINTON L BRAY E P 本刊编辑部整理 2016中国全科医学2016,19,30:1
