1Metabolic syndrome does not affect sustained virologic response of direct-acting antivirals while hepatitis C clearance improves hemoglobin A1c显示文摘AIM To determine whether successful treatment with direc-tacting antivirals(DAA) is associated with improvements in hemoglobin A1 c(HbA1 c) and if type 2 diabetes mellitus(T2 DM) or metabolic syndrome affects sustained virologic response(SVR).METHODS We performed a retrospective analysis of all hepatitis C virus(HCV) patients at the VA Greater Los Angeles Healthcare System treated with varying DAA therapy between 2014-2016. Separate multivariable logistic regression was performed to determine predictors of HbA1 c decrease ≥ 0.5 after DAA treatment and predictors of SVR 12-wk post treatment(SVR12).RESULTS A total of 1068 patients were treated with DAA therapy between 2014-2016. The presence of T2 DM or metabolic syndrome did not adversely affect SVR12. 106 patients had both HCV and T2 DM. Within that cohort,patients who achieved SVR12 had lower mean HbA1 c pre treatment(7.35 vs 8.60,P = 0.02),and lower mean HbA1 c post-treatment compared to non-responders(6.55 vs 8.61,P = 0.01). The mean reduction in HbA1 c after treatment was greater for those who achieved SVR12 than for non-responders(0.79 vs 0.01,P = 0.03). In adjusted models,patients that achieved SVR12 were more likely to have a HbA1 c decrease of ≥ 0.5 than those that did not achieve SVR12(adjusted OR = 7.24,95%CI: 1.22-42.94). CONCLUSION In HCV patients with T2 DM,successful treatment with DAA was associated with a significant reduction in HbA1 c suggesting that DAA may have a role in improving insulin sensitivity. 2Perception and Practice of Emergency Contraception among Female Undergraduates of the University of Ibadan, Nigeria显示文摘Objective To assess perception and level of proper utilization of emergency contraception (EC) among female undergraduates. Methods Cross-sectional questionnaire study was performed on 383 female under- graduates in Nigeria in June 2006. Data analysis was with χ-square test and logistic regressions (P<0.05). Results One hundred and five (48.2%) had been sexually exposed. Only 32 (30.5%) used regular contraception. Seventy-three (24.3%) female undergraduates were aware of EC. Only 29 (7.6%) had used EC before. Most would not use emergency contraceptive drugs in future due to lack of awareness (64.8%), and fear for future fertility and of drugs being injurious to health. Use of EC was associated with awareness of correct interval for use (OR=9.1; 95%CI: 2.1-39.9). Conclusion There is poor knowledge about EC and poor use, while significant need remains. Most knowledge was acquired from peers and inaccurate. 3Therapeutic efficacy and effects of artemisinin-based combination treatments on uncomplicated Plasmodium falciparum malaria-associated anaemia in Nigerian children during seven years of adoption as first-line treatments显示文摘Background:Artemisinin-based combination treatments(ACTs)are the first-line treatments of uncomplicated Plasmodium falciparum malaria in many endemic areas but there are few evaluation of their efficacy in anaemic malarious children.Methods:Therapeutic efficacy of 3-day regimens of artesunate-amodiaquine and artemether-lumefantrine was evaluated in 437 anaemic and 909 non-anaemic malarious children following treatment during a seven-year period(2008-2014).Patterns of temporal changes in haematocrit were classified based on haematocrit values<30%and≥30%.Kinetics of the disposition of the deficit in haematocrit from 30%following treatment were evaluated using a non-compartment model.Results:PCR-corrected parasitological efficacy 28 days after start of treatment was significantly higher in artesunateamodiaquine-compared to artemether-lumefantrine-treated children[97%(95%CI:92.8-100)versus 96.4%(95%CI:91.3-99.4),P=0.02],but it was similar in non-anaemic and anaemic children.Fall in haematocrit/1000 asexual parasites cleared from peripheral blood was significantly greater at lower compared to higher parasitaemias(P<0.0001),and in non-anaemic compared to anaemic children(P=0.007).In anaemic children at presentation,mean anaemia recovery time(AnRT)was 15.4 days(95%CI:13.3-17.4)and it did not change over the years.Declines in haematocrit deficits from 30%were monoexponential with mean estimated half-time of 1.4 days(95%CI:1.2-1.6).Anaemia half-time(t_(1/2anaemia))correlated positively with AnRT in the same patients(r=0.69,P<0.0001).Bland-Altman analysis of 10 multiples of t_(1/2anaemia) and AnRT showed narrow limit of agreement with insignificant bias(P=0.07)suggesting both can be used interchangeably in the same patients.Conclusions:Artesunate-amodiaquine and artemether-lumefantrine remain efficacious treatments of uncomplicated P.falciparum infections in non-anaemic and anaemic Nigerian children in the last 7 years of adoption as first-line treatments.These ACTs may also conserve haematocrit at high parasitaemias and in anaemic children.Trials registration:Pan African Clinical Trial Registry PACTR201508001188143,3 July 2015;PACTR201510001189370,3 July 2015;PACTR201508001191898,7 July 2015 and PACTR201508001193368,8 July 2015.Akintunde Sowunmi Kazeem Akano Godwin Ntadom Adejumoke I.Ayede Folasade O.Ibironke Temitope Aderoyeje Elsie O.Adewoye Bayo Fatunmbi Stephen Oguche Henrietta U.Okafor Ismaila Watila Martin Meremikwu Philip Agomo William Ogala Chimere Agomo Onikepe A.Folarin Grace O.Gbotosho Christian T.Happi 2017Infectious Diseases of Poverty2017,6,1:2

5Declining responsiveness of childhood Plasmodium falciparum infections to artemisinin-based combination treatments ten years following deployment as first-line antimalarials in Nigeria显示文摘Background:The development and spread of artemisinin-resistant Plasmodium falciparum malaria in Greater Mekong Subregion has created impetus for continuing global monitoring of efficacy of artemisinin-based combination therapies(ACTs). This post analyses is aimed to evaluate changes in early treatment response markers 10 years after the adoption of ACTs as first-line treatments of uncomplicated falciparum malaria in Nigeria.Methods: At 14 sentinel sites in six geographical areas of Nigeria, we evaluated treatment responses in 1341 children under 5 years and in additional 360 children under 16 years with uncomplicated malaria enrolled in randomized trials of artemether-lumefantrine versus artesunate-amodiaquine at 5-year interval in 2009-2010 and 2014-2015 and at 2-year interval in 2009-2010 and 2012-2015, respectively after deployment in 2005.Results: Asexual parasite positivity 1 day after treatment initiation (APPD1) rose from 54 to 62% and 2 days after treatment initiation from 5 to 26% in 2009-2010 to 2014-2015(P=0.002 and P<0.0001, respectively).Parasite clearance time increased significantly from 1.6 days (95% confidence interval [CI]: 1.55-1.64) to 1.9 days (95% C,1.9-2.0) and geometric mean parasite reduction ratio 2 days after treatment initiation decreased significantly from 11 000 to 4700 within the same time period (P< 00001 for each). Enrolment parasitaemia > 75 000 μl^-1, haematocrit > 27% 1 day post-treatment initiatiortreatment with artemether-lumefantrine and enrolment in 2014-2015 independently predicted APPD1. In paralle , Kaplan-Meier estimated risk of recurrent infections by day 28 rose from 8 to 14% (P=0005) and from 9 to 15%(P=0.02) with artemether-lumefantrine and artesunate-amodiaquine, respectively. 6Response of Soybean(Glycine max)to Different Sources and Levels of Phosphours Fertilization显示文摘Phosphorus is crucial to crop growth and yield because of its multiple effects on plant nutrition.Most tropical soils have low plant available phosphorus and there is the need for external input of P to achieve optimum yield.The response of soybean to different sources and levels of phosphorus fertilization were therefore investigated between August,2013 and January,2014.The treatments consisted of four fertilizer sources[Sokoto Rock Phosphate(SRP),single super phosphate,Di-Ammonium Phosphate(DAP)and control]and fi ve levels of fertilizers(0,15,30,60 and 75 kg P·hm^(-2))fi tted into Randomized Complete Block Design(RCBD)with three replications.Soil samples were collected,processed and analyzed before planting and after harvest.Data on growth(plants height,numbers of leaves,numbers of branches and numbers of flowers)and yield(pod and grain weights)were analyzed using ANOVA and significant means were separated using LSD at 5%probability level.Application of DAP at15 kg P·hm^(-2) and 30 kg P·hm^(-2) produced the highest grain yield and showed signifi cant differences in plant height,numbers of leaves and numbers of branches.SRP should not be recommended for the study area,because there was no signifi cant difference across all the parameters and it was characterized with low grain yield,due to its level of acidity and the mildly acidic nature of the soil.Owoade Folasade Mary Adegboyega Adewumi Oluwaponmile 2017Journal of Northeast Agricultural University(English Edition)2017,24,1:0
7Prevalence and pattern of soft‐tissue rheumatism in a rural community in South‐South Nigeria: A cross‐sectional survey显示文摘Background:Soft tissue rheumatism arises from the pathology of periarticular structures.They are marked by pain and disability.Epidemiological studies in sub‐Saharan Africa are sparse.This study aimed to determine the epidemiology of soft‐tissue rheumatism in a rural community in South‐South Nigeria.Methods:This was a community‐based,cross‐sectional study conducted from February to July 2017 in South‐South Nigeria among 319 participants using the Community Oriented Program for the Control of Rheumatic Diseases(COPCORD)methodology.Participants aged 18 years and older were included in the study.Subjects reporting positive musculoskeletal pain were examined for specific soft tissue rheumatism.The diagnosis was based on established criteria.Results:The mean age of the study population was 48.87±19.58 years.The prevalence of soft‐tissue rheumatism was 11.0%(35/319).Soft tissue rheumatism was more prevalent in females(24,68.6%)than in males(11,31.4%).The most common form of soft‐tissue rheumatism diagnosed was adhesive capsulitis(20.0%),closely followed by rotator cuff tendinitis(14.3%),carpal tunnel syndrome(14.3%),and plantar fasciitis(14.3%).Age(p=0.06),body mass index(p=0.034),and dyslipidemia(p=0.028)were significantly associated with the prevalence of soft‐tissue rheumatism.A significant disability index(HAQ‐DI≥1)was recorded in 40.0%of participants with soft tissue rheumatism.Twelve(34.3%)participants with soft‐tissue rheumatism attributed the disorder to their work,and 74.3%used hospital‐prescribed medications for treatment.Conclusion:Soft‐tissue rheumatism is associated with a significant disability affecting activities of daily living.Factors associated with its prevalence include increasing age and obesity.Folasade Agbebaku Airenakho Emorinken Solomon Agbebaku 2022Rheumatology & Autoimmunity2022,2,4:0
8Jatonik polyherbal mixture induced rat liver MMPT pore opening in normal Wistar rat:In vitro and in vivo studies显示文摘Objective:To assess acute toxicity,the in vitro and in vivo effects of methanol and ethyl acetate extracts(JME and JEE)of Jatonik polyherbal mixture on some mitochondria-related parameters and their effect on the activity of some liver enzymes.Methods:Acute toxicity of JME and JEE was determined using Lorke’s method.In vitro and in vivo opening of the mitochondrial membrane permeability transition pore(MMPT pore)was spectrophotometrically assayed.Production of malondialdehyde(MDA)as an index of lipid peroxidation and the activity of mitochondrial ATPase was evaluated in vitro and in vivo and the effect of JME and JEE on the activity of liver enzymes such as alkaline phosphatase(ALP),aspartate and alanine aminotransferase(AST and ALT)and gamma-glutamyl transferase(GGT)was also investigated.Results:JME had an LD_(50) of 3808 mg/kg b.w whereas JEE had an LD_(50) greater than 5000 mg/kg b.w.of rats.After the rats have been fed with both extracts,a photomicrograph of a piece of liver tissue showed no apparent symptoms of toxicity.From the in vitro and in vivo studies,both extracts prompted intact mitochondria to open their MMPT pores.When compared to the control,lipid peroxide product release and ATPase activity were significantly increased(P<0.05)in vitro and in vivo.The activities of AST,ALT,and GGT were all reduced at 50 mg/kg when treated with JME,but the activity of AST was considerably enhanced when treated with JEE(P<0.05).The results revealed that both JME and JEE of the Jatonik polyherbal mixture had low toxicity,profound MMPTpore induction,and enhanced ATPase activity,but an increased MDA production.Conclusion:Jatonik extracts may be a promising target for drug development in diseases where there is dysregulation of apoptosis,however,further studies are needed to better clarify the molecular mechanism involved in these phenomena.Olabinri P.Folashade Ibrahim Damilare Boyenle Tolulope A.Oyedeji Fiyinfoluwa Demilade Ojeniyi Adisa Ayobami Damilare Leonard O.Ehigie Adeola Folasade Ehigie 2024Chinese Herbal Medicines2024,16,1:0
