| 1 | Endoscopic dilation of complete oesophageal obstructionswith a combined antegrade-retrograde rendezvoustechnique显示文摘AIM: To investigate the combined antegraderetrograde endoscopic rendezvous technique for complete oesophageal obstruction and the swallowing outcome.METHODS: This single-centre case series includes consecutive patients who were unable to swallow due to complete oesophageal obstruction and underwent combined antegrade-retrograde endoscopic dilation(CARD) within the last 10 years. The patients' demographic characteristics, clinical parameters, endoscopic therapy, adverse events, and outcomes were obtained retrospectively. Technical success was defined as effective restoration of oesophageal patency. Swallowing success was defined as either percutaneous endoscopic gastrostomy(PEG)-tube independency and/or relevant improvement of oral food intake, as assessed by the functional oral intake scale(FOIS)(≥ level 3).RESULTS: The cohort consisted of six patients [five males; mean age 71 years(range, 54-74)]. All but one patient had undergone radiotherapy for head and neck or oesophageal cancer. Technical success was achieved in five out of six patients. After discharge, repeated dilations were performed in all five patients. During follow-up(median 27 mo, range, 2-115), three patients remained PEG-tube dependent. Three of four patients achieved relevant improvement of swallowing(two patients: FOIS 6, one patient: FOIS 7). One patient developed mediastinal emphysema following CARD, without a need for surgery.CONCLUSION: The CARD technique is safe and a viable alternative to high-risk blind antegrade dilation in patients with complete proximal oesophageal obstruction. | 2 | Liver stiffness reversibly increases during pregnancy and independently predicts preeclampsia显示文摘AIM To study liver stiffness(LS) during pregnancy and its association with complications during pregnancy.METHODS In this observational, diagnostic study, 537 pregnant women were prospectively enrolled at the Department of Obstetrics and Gynecology, University hospital Heidelberg and Salem Medical Center. LS was measured using the Fibroscan device(Echosens, Paris) in all women and in 41 cases 24 h after delivery. Clinical and morphological data were recorded and abdominal ultrasound and standard laboratory tests were performed. No complications were observed in 475 women(controls) while preeclampsia and intrahepatic cholestasis of pregnancy(ICP) developed in 22 and 40 women, respectively.RESULTS In controls, LS increased significantly from initially 4.5 ± 1.2 kPa in the second trimester to 6.0 ± 2.3 kPa(P < 0.001) in the third trimester. In the third trimester, 41% of women had a LS higher than 6 kPa. Elevated LS in controls was significantly correlated with alkaline phosphatase, leukocytes, gestational age and an increase in body weight and body mass index(BMI). In women with pregnancy complications, LS was significantly higher as compared to controls(P < 0.0001). Moreover, in multivariate analysis, LS was an independent predictor for preeclampsia with an odds ratio of 2.05(1.27-3.31) and a cut-off value of 7.6 kPa. In contrast, ICP could not be predicted by LS. Finally, LS rapidly decreased in all women within 24 h after delivery from 7.2 ± 3.3 kPa down to 4.9 ± 2.2 kPa(P < 0.001).CONCLUSION During pregnancy, LS significantly and reversibly increases in the final trimester of pregnant women without complications. | 3 | Development of an educational cartoon to prevent worm infections in Chinese schoolchildren显示文摘Background:With more than two billion people infected worldwide,soil-transmitted helminths(STH)are the most widespread infections.To date,STH control efforts rely predominantly on recurrent mass drug administration(MDA),which does not prevent reinfection.Additional public health measures including novel health educational tools are required for more sustained integrated control of STH.We describe the development of an educational cartoon video(The Magic Glasses)targeting STH infections in Chinese schoolchildren and its pilot testing in China.We applied an extensive community-based mixed methods approach involving input from the target group of 9–10 year old schoolchildren and key informants,such as teachers,doctors and parents,in order to identify potential STH infection risks in the study area and to formulate key messages for the cartoon.The development of the educational cartoon included three major steps:formative research,production,and pilot testing and revision.Results:We found that most adults and approximately 50%of the schoolchildren were aware of roundworm(Ascaris)infection,but knowledge of transmission,prevention and treatment of STH was poor.Observations in the study area showed that unhygienic food practices,such as eating raw and unwashed fruit or playing in vegetable gardens previously fertilised with human faeces,posed major STH infection risks.Conclusions:It was crucial to assess the intellectual,emotional,social and cultural background of the target population prior to video production in order to integrate the key messages of the cartoon into everyday situations.Overall,our strategy for the development of the cartoon and its incorporation into a health education package proved successful,and we provide a summary of recommendations for the development of future educational videos based on our experiences in China. | Franziska A Bieri Li-Ping Yuan Yue-Sheng Li Yong-Kang He Andrew Bedford Robert S Li Feng-Ying Guo Sheng-Ming Li Gail M Williams Donald P McManus Giovanna Raso Darren J Gray | 2013 | Infectious Diseases of Poverty2013，2，1： | 2 |
