1Development of an Arabic version of the National Eye Institute Visual Function Questionnaire as a tool to study eye diseases patients in Egypt显示文摘AIM:To develop and test an Arabic version of the National Eye Institute Visual Function Questionnaire-25(NEI-VFQ-25).METHODS:NEI-VFQ-25 was translated into Arabic according to WHO translation guidelines. We enrolled adult consenting patients with bilateral chronic eye diseases who presented to 14 hospitals across Egypt from October to December 2012, and documented their clinical findings. Psychometric properties were then tested using STATA.RESULTS:We recruited 379 patients, whose mean age was(54.5±15)y. Of 46.2% were males, 227 had cataract,31 had glaucoma, 23 had retinal detachment, 37 had diabetic retinopathy, and 61 had miscellaneous visual defects. Non-response rate and the floor and ceiling numbers of the Arabic version(ARB-VFQ-25) were calculated. Internal consistency was high in all subscales(except general health), with Cronbach-α ranging from0.702-0.911. Test-retest reliability was high(intraclass correlation coefficient 0.79).CONCLUSION:RB-VFQ-25 isareliableandvalidtool for assessing visual functions of Arabic speaking patients. However, some questions had high non-response rates and should be substituted by available alternatives. 2Safety and efficacy of Hansenula-derived PEGylated-interferon alpha-2a and ribavirin combination in chronic hepatitis C Egyptian children显示文摘AIM:To investigate the safety and efficacy of a Hansenula-derived PEGylated(polyethylene glycol)interferon(IFN)-alpha-2a(Reiferon Retard)plus ribavirin customized regimen in treatment-na?ve and previously treated(non-responders and relapsers)Egyptian children with chronic hepatitis C infection.METHODS:Forty-six children with chronic hepatitis C virus(HCV)infection were selected from three tertiary pediatric hepatology centers.Clinical and laboratory evaluations were undertaken.Quantitative polymerase chain reaction(PCR)for HCV-RNA was performed before starting treatment,and again at 4,12,24,48,72wk during treatment and 6 mo after treatment cessation.All patients were assigned to receive a weekly subcutaneous injection of PEG-IFN-alpha-2a plus daily oral ribavirin for 12 wk.Thirty-four patients were treatment-na?ve and 12 had a previous treatment trial.Patients were then divided according to PCR results into two groups.GroupⅠincluded patients who continued treatment on a weekly basis(7-d schedule),while groupⅡincluded patients who continued treatment on a 5-d schedule.Patients from either group who were PCR-negative at week 48,but had at least one PCRpositive test during therapy,were assigned to have an extended treatment course up to 72 wk.The occurrence of adverse effects was assessed during treatment and follow up.The study was registered at www.ClinicalTrials.gov(NCT02027493).RESULTS:Only 11 out of 46(23.9%)patients showed a sustained virological response(SVR),two patients were responders at the end of treatment;however,they were lost to follow up at 6 mo post treatment.Breakthrough was seen in 18(39.1%)patients,one patient(2.17%)showed relapse and 14(30.4%)were non-responders.Male gender,short duration of infection,low viral load,mild activity,and mild fibrosis were the factors related to a better response.On the other hand,patients with high viral load and absence of fibrosis failed to respond to treatment.Before treatment,liver transaminases were elevated.After commencing treatment,they were normalized in all patients at week 4 and were maintained normal in responders till the end of treatment,while they increased again significantly in non-responders(P=0.007 and 0.003 at week 24 and 72 respectively).The 5-d schedule did not affect the response rate(1/17 had SVR).Treatment duration(whether 48 wk or extended course to 72 wk)gave similar response rates(9/36 vs 2/8 respectively;P=0.49).Type of previous treatment(short acting IFN vs PEG-IFN)did not affect the response to retreatment.On the other hand,SVR was significantly higher in previous relapsers than in previous non-responders(P=0.039).Only mild reversible adverse effects were observed and children tolerated the treatment well.CONCLUSION:Reiferon Retard plus ribavirin combined therapy was safe.Our customized regimen did not influence SVR rates.Further trials on larger numbers of patients are warranted.Suzan El Naghi Tawhida Y Abdel-Ghaffar Hanaa El-Karaksy Elham F Abdel-Aty Mona S El-Raziky Aleef A Allam Heba Helmy Hanaa A El-Araby Behairy E Behairy Mohamed A El-Guindi Hatem El-Sebaie Aisha Y Abdel-Ghaffar Nermin A Ehsan Ahmed M El-Hennawy Mostafa M Sira 2014World Journal of Gastroenterology2014,20,16:2

11Role of endoscopic ultrasound and cyst fluid tumor markers in diagnosis of pancreatic cystic lesions显示文摘BACKGROUND Pancreatic cystic lesions(PCLs) are common in clinical practice. The accurate classification and diagnosis of these lesions are crucial to avoid unnecessary treatment of benign lesions and missed opportunities for early treatment of potentially malignant lesions.AIM To evaluate the role of cyst fluid analysis of different tumor markers such as cancer antigens [e.g., cancer antigen(CA)19-9, CA72-4], carcinoembryonic antigen(CEA), serine protease inhibitor Kazal-type 1(SPINK1), interleukin 1 beta(IL1-β), vascular endothelial growth factor A(VEGF-A), and prostaglandin E2(PGE2)], amylase, and mucin stain in diagnosing pancreatic cysts and differentiating malignant from benign lesions.METHODS This study included 76 patients diagnosed with PCLs using different imaging modalities. All patients underwent endoscopic ultrasound(EUS) and EUS-fine needle aspiration(EUS-FNA) for characterization and sampling of different PCLs.RESULTS The mean age of studied patients was 47.4 ± 11.4 years, with a slight female predominance(59.2%). Mucin stain showed high statistical significance in predicting malignancy with a sensitivity of 87.1% and specificity of 95.56%. It also showed a positive predictive value and negative predictive value of 93.1% and 91.49%, respectively(P < 0.001). We found that positive mucin stain, cyst fluid glucose, SPINK1, amylase, and CEA levels had high statistical significance(P < 0.0001). In contrast, IL-1β, CA 72-4, VEGF-A, VEGFR2, and PGE2 did not show any statistical significance. Univariate regression analysis for prediction of malignancy in PCLs showed a statistically significant positive correlation with mural nodules, lymph nodes, cyst diameter, mucin stain, and cyst fluid CEA. Meanwhile, logistic multivariable regression analysis proved that mural nodules, mucin stain, and SPINK1 were independent predictors of malignancy in cystic pancreatic lesions.CONCLUSION EUS examination of cyst morphology with cytopathological analysis and cyst fluid analysis could improve the differentiation between malignant and benign pancreatic cysts. 12STW 5 is effective against nonsteroidal anti-inflammatory drugs induced gastro-duodenal lesions in rats显示文摘BACKGROUND Proton pump inhibitors are often used to prevent gastro-intestinal lesions induced by nonsteroidal anti-inflammatory drugs.However,they are not always effective against both gastric and duodenal lesions and their use is not devoid of side effects.AIM To explore the mechanisms mediating the clinical efficacy of STW 5 in gastroduodenal lesions induced by nonsteroidal anti-inflammatory drugs(NSAIDs),exemplified here by diclofenac,in a comparison to omeprazole.METHODS Gastro-duodenal lesions were induced in rats by oral administration of diclofenac(5 mg/kg)for 6 successive days.One group was given concurrently STW 5(5 mL/kg)while another was given omeprazole(20 mg/kg).A day later,animals were sacrificed,stomach and duodenum excised and divided into 2 segments:One for histological examination and one for measuring inflammatory mediators(tumor necrosis factorα,interleukins-1βand 10),oxidative stress enzyme(heme oxygenase-1)and apoptosis regulator(B-cell lymphoma 2).RESULTS Diclofenac caused overt histological damage in both tissues,associated with parallel changes in all parameters measured.STW 5 and omeprazole effectively prevented these changes,but STW 5 superseded omeprazole in protecting against histological damage,particularly in the duodenum.CONCLUSION The findings support the therapeutic usefulness of STW 5 and its superiority over omeprazole as adjuvant therapy to NSAIDs to protect against their possible gastro-duodenal side effects.Mohamed T Khayyal Walaa Wadie Enas A Abd El-Haleim Kawkab A Ahmed Olaf Kelber Ramy M Ammar Heba Abdel-Aziz 2019World Journal of Gastroenterology2019,25,39:1

16Intensive care unit readmission in adult Egyptian patients undergoing living donor liver transplant:A single-centre retrospective cohort study显示文摘BACKGROUND Patients who undergo living donor liver transplantation(LDLT)may suffer complications that require intensive care unit(ICU)readmission.AIM To identify the incidence,causes,and outcomes of ICU readmission after LDLT.METHODS A retrospective cohort study was conducted on patients who underwent LDLT.The collected data included patient demographics,preoperative characteristics,intraoperative details;postoperative stay,complications,causes of ICU readmission,and outcomes.Patients were divided into two groups according to ICU readmission after hospital discharge.Risk factors for ICU readmission were identified in univariate and multivariate analyses.RESULTS The present study included 299 patients.Thirty-one(10.4%)patients were readmitted to the ICU after discharge.Patients who were readmitted to the ICU were older in age(53.0±5.1 vs 49.4±8.8,P=0.001)and had a significantly higher percentage of women(29%vs 13.4%,P=0.032),diabetics(41.9%vs 24.6%,P=0.039),hypertensives(22.6%vs 6.3%,P=0.006),and renal(6.5%vs 0%,P=0.010)patients as well as a significantly longer initial ICU stay(6 vs 4 d,respectively,P<0.001).Logistic regression analysis revealed that significant independent risk factors for ICU readmission included recipient age(OR=1.048,95%CI=1.005-1.094,P=0.030)and length of initial hospital stay(OR=0.836,95%CI=0.789-0.885,P<0.001).CONCLUSION The identification of high-risk patients(older age and shorter initial hospital stay)before ICU discharge may help provide optimal care and tailor follow-up to reduce the rate of ICU readmission.Manar Salah Iman Fawzy Montasser Hanaa A El Gendy Alaa A Korraa Gamal M Elewa Hany Dabbous Hossam R Mahfouz Mostafa Abdelrahman Mohammed Hisham Goda Mohamed Mohamed Bahaa El-Din Mahmoud El-Meteini Heba A Labib 2022World Journal of Hepatology2022,14,6:0
17Adherence to Advisory Committee on Immunization Practices in diabetes mellitus patients in Saudi Arabia:A multicenter retrospective study显示文摘BACKGROUND Patients with diabetes mellitus(DM)are predisposed to an increased risk of infection signifying the importance of vaccination to protect against its potentially severe complications.The Centers for Disease Control and Prevention/Advisory Committee on Immunization Practices(CDC/ACIP)issued immunization recommendations to protect this patient population.AIM To assess the adherence of patients with DM to the CDC/ACIP immunization recommendations in Saudi Arabia and to identify the factors associated with the vaccine adherence rate.METHODS An observational retrospective study conducted in 2023 was used to collect data on the vaccination records from 13 diabetes care centers in Saudi Arabia with 1000 eligible patients in phase I with data collected through chart review and 709 patients in phase II through online survey.RESULTS Among participants,10.01%(n=71)had never received any vaccine,while 85.89%(n=609)received at least one dose of the coronavirus disease 2019(COVID-19)vaccine,and 34.83%(n=247)had received the annual influenza vaccine.Only 2.96%(n=21),2.11%(n=15),and 1.12%(n=8)received herpes zoster,tetanus,diphtheria,and pertussis(Tdap),and human papillomavirus(HPV)vaccines,respectively.For patients with DM in Saudi Arabia,the rate of vaccination for annual influenza and COVID-19 vaccines was higher compared to other vaccinations such as herpes zoster,Tdap,pneumococcal,and HPV.Factors such as vaccine recommendations provided by family physicians or specialists,site of care,income level,DM-related hospitalization history,residency site,hemoglobin A1c(HbA1c)level,and health sector type can significantly influence the vaccination rate in patients with DM.Among non-vaccinated patients with DM,the most reported barriers were lack of knowledge and fear of side effects.This signifies the need for large-scale research in this area to identify additional factors that might facilitate adherence to CDC/ACIP vaccine recommendations in patients with DM.CONCLUSION In Saudi Arabia,patients with DM showed higher vaccination rates for annual influenza and COVID-19 vaccines compared to other vaccinations such as herpes zoster,Tdap,pneumococcal,and HPV.Factors such as vaccine recommendations provided by family physicians or specialists,the site of care,income level,DM-related hospitalization history,residency site,HbA1c level,and health sector type can significantly influence the vaccination rate in patients with DM.Saleh Fahad Alqifari Aya K Esmail Dalal M Alarifi Ghalya Y Alsuliman Maram M Alhati May R Mutlaq Mohammed Aldhaeefi Shaden A Alshuaibi Palanisamy Amirthalingam Abrar Abdallah Afaf S Wasel Heba R Hamad Shoroq Alamin Tasneem H Atia Tariq Alqahtani 2024World Journal of Diabetes2024,15,3:0

19Cardiac stem cells: Current knowledge and future prospects显示文摘Regenerative medicine is the field concerned with the repair and restoration of the integrity of damaged human tissues as well as whole organs.Since the inception of the field several decades ago,regenerative medicine therapies,namely stem cells,have received significant attention in preclinical studies and clinical trials.Apart from their known potential for differentiation into the various body cells,stem cells enhance the organ's intrinsic regenerative capacity by altering its environment,whether by exogenous injection or introducing their products that modulate endogenous stem cell function and fate for the sake of regeneration.Recently,research in cardiology has highlighted the evidence for the existence of cardiac stem and progenitor cells(CSCs/CPCs).The global burden of cardiovascular diseases’morbidity and mortality has demanded an in-depth understanding of the biology of CSCs/CPCs aiming at improving the outcome for an innovative therapeutic strategy.This review will discuss the nature of each of the CSCs/CPCs,their environment,their interplay with other cells,and their metabolism.In addition,important issues are tackled concerning the potency of CSCs/CPCs in relation to their secretome for mediating the ability to influence other cells.Moreover,the review will throw the light on the clinical trials and the preclinical studies using CSCs/CPCs and combined therapy for cardiac regeneration.Finally,the novel role of nanotechnology in cardiac regeneration will be explored.Radwa A Mehanna Marwa M Essawy Mona A Barkat Ashraf K Awaad Eman H Thabet Heba A Hamed Hagar Elkafrawy Nehal A Khalil Abeer Sallam Marwa A Kholief Samar S Ibrahim Ghada M Mourad 2022World Journal of Stem Cells2022,14,1:0
