Colonoscopy evaluation after short-term anti-tuberculosis treatment in nonspecific ulcers on the ileocecal area显示文摘AIM: To evaluate the eff icacy of colonoscopy follow-up after short-term anti-tuberculosis treatment in patients with nonspecific ulcers on ileocecal areas being suspicious of tuberculous colitis. METHODS: We prospectively analyzed the colonoscopic fi ndings before and after short term anti- tuberculosis treatment in 18 patients with nonspecifi c ulcers on the ileocecal area and compared them with 7 patients of confi rmed tuberculous colitis by acid-fast bacilli or caseating granuloma on colonic biopsy. RESULTS: Mean duration for short-term follow- up was 107.3 d with combined chemotherapy containing isoniazid, rifampicin, ethambutol and pyrazinamide. Seven patients with tuberculous colitis showed complete healing of active ulcers after short- term medication. After short-term anti-tuberculosis treatment, follow-up colonoscopy findings devided 18 patients with nonspecific ulcers into two groups by ulcer state. One is the 'suspicious tuberculous colitis group' showing healing of ulcers and erosions and another is the 'suspicious inflammatory bowel disease group' showing active ulcers with or without aggravation of the lesion. Finally, all 9 of the 'suspicious tuberculous colitis group' were diagnosed as tuberculous colitis showing no recurrence of ulcers after termination of 9 mo of anti-tuberculosis medication. Patients of the 'suspicious inflammatorybowel disease group' were f inally diagnosed as Crohn's disease or nonspecifi c colonic ulcers during long-term follow up. CONCLUSION: Follow-up colonoscopy shows a healing stage ulcer or scarring change without an active ulcer with just 2 mo to 3 mo of medication in patients with tuberculous colitis. Colonoscopy follow-up after short term anti-tuberculosis trial in patients with nonspecif ic ulcers on the ileocecal area is valuable in making early differential diagnosis of tuberculous colitis. | Young Sook Park Dae Won Jun Seong Hwan Kim Han Hyo Lee Yun Ju Jo Moon Hee Song Nam In Kim Jun Seok Lee | 2008 | World Journal of Gastroenterology2008,14,32: | 14
Safety and efficacy of tenofovir in chronic hepatitis B-related decompensated cirrhosis显示文摘AIM To evaluate the safety and efficacy of tenofovir disoproxil fumarate(TDF) as a first-line therapy in decompensated liver disease. METHODS We enrolled 174 chronic hepatitis B-related liver cirrhosis patients treated with 300 mg/d TDF at six Korean centers. Of the 174 cirrhosis patients, 57 were assigned to the decompensated cirrhosis group and 117 were assigned to the compensated cirrhosis group. We followed the patients for 12 mo and evaluated clinical outcomes, including biochemical, virological, and serological responses. We also evaluated changes in hepatic and renal function and compared the decompensated and compensated cirrhosis groups. RESULTS The 1-year complete virological response(CVR) and Hepatitis B e antigen(HBe Ag) seroconversion were seen in 70.2% and 14.2% in the decompensated cirrhosis group, respectively. The rates of HBe Ag seroconversion/loss and ALT normalization at month 12 were similar in both groups. TDF treatment was also effective for decreasing the level of hepatitis B virus(HBV) DNA in both groups, but CVR was higher in the compensated group(88.9% vs 70.2%, P = 0.005). Tenofovir treatment for 12 mo resulted in improved Child-Turcotte-Pugh(CTP) and model for end-stage liver disease(MELD) scores in decompensated group(P < 0.001). Of the 57 decompensated patients, 39(68.4%) achieved CTP class A and 27(49.1%) showed improvement in the CTP score of 2 points after 12 mo of TDF. The observed rate of confirmed 0.5 mg/d L increases in serum levels of creatinine in the decompensated and compensated cirrhosis group were 7.0% and 2.5%, respectively(P < 1.000).CONCLUSION TDF therapy in decompensated cirrhosis patients was effective for decreasing HBV DNA levels and improving hepatic function with relatively lower CVR than in compensated cirrhosis. Thus, physicians should carefully monitor not only renal function but also treatment responses when using TDF in decompensated cirrhosis patients. | Soon Kyu Lee Myeong Jun Song Seok Hyun Kim Byung Seok Lee Tae Hee Lee Young Woo Kang Suk Bae Kim Il Han Song Hee Bok Chae Soon Young Ko Jae Dong Lee | 2017 | World Journal of Gastroenterology2017,23,13: | 14
Piglet gut microbial shifts early in life:causes and effects显示文摘The gut microbiome has long been known to play fundamentally important roles in the animal health and the well-being of its host. As such, the establishment and maintenance of a beneficial gut microbiota early in life is crucial in pigs, since early gut colonizers are pivotal in the establishment of permanent microbial community structures affecting the health and growth performance of pigs later in life. Emphasizing this importance of early gut colonizers, it is critical to understand the factors impacting the establishment of the piglet gut microbiome at weaning. Factors include, among others, diet, in-feed antibiotics, probiotics and prebiotic administration. The impact of these factors on establishment of the gut microbiome of piglets at weaning includes effects on piglet gut microbial diversity, structure, and succession. In this review, we thoroughly reviewed the most recent findings on the piglet gut microbiome shifts as influenced by weaning, and how these microbiome changes brought about by various factors that have been shown to affect the development of microbiota in piglets. This review will provide a general overview of recent studies that can help to facilitate the design of new strategies to modulate the gut microbiome in order to enhance gastrointestinal health, growth performance and well-being of piglets. | Robin B.Guevarra Jun Hyung Lee Sun Hee Lee Min-Jae Seok Doo Wan Kim Bit Na Kang Timothy J.Johnson Richard E.Isaacson Hyeun Bum Kim | 2019 | Journal of Animal Science and Biotechnology2019,10,3: | 13
Short-term interval combined chemoembolization and radiofrequency ablation for hepatocellular carcinoma显示文摘AIM:To investigate hepatic function after combined transcatheter arterial chemoembolization(TACE) and radiofrequency ablation(RFA) with a short-term interval(0-2 d).METHODS:A total of 115 patients with compensated liver cirrhosis underwent RFA combined with TACE at a time-interval of 0-2 d for the treatment of hepatocellular carcinoma(HCC)<5.0 cm.There were 21 patients who received further hepatic directed treatment altering liver function within 12 mo after the combined therapy for HCC-recurrence,and were excluded. The remaining94 patients who survived without HCC-recurrence were included in this retrospective study.RESULTS:At 1 mo after treatment,Child-Pugh scores(CPs)remained unchanged in 89 of 94 patients(94.7%),and transiently increased by one-point in 5 patients(5.3%).However,the score returned to baseline score at 3 mo and was maintained until 6 mo in all patients. The baseline CPs of 8 or more was identified as a factor for transient rise of CPs after the treatment(CPs 8/9 vs5/6/7;21.4%vs 2.5%;P=0.022).At 12 mo followup,CPs was unchanged in 90 patients(95.7%),and increased by one-point in 4 patients(4.3%).The rise of CPs at 12 mo was not statistically associated with the initial transient rise of CPs.There were procedure-related complications in 3 patients(3.2%),but the complications were resolved by medical and interventional treatments without hepatic functional sequelae.CONCLUSION:The combined TACE and RFA with an interval of 0-2 d are safe for the management of HCC<5 cm in cirrhotic patients. | Won Hyeok Choe Young Jun Kim Hee Sun Park Sang Woo Park Jeong Han Kim So Young Kwon | 2014 | World Journal of Gastroenterology2014,20,35: | 13
Management of entecavir-resistant chronic hepatitis B with adefovir-based combination therapies显示文摘AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method, and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,respectively.RESULTS: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall, cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis, low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. During the same period, the cumulative incidence of virologic breakthrough was as low as 6% in patients with the both favorable predictors.CONCLUSION: If tenofovir is not available,ADV/ETV combination could be considered in ETV-resistant patients with low HBV DNA titers, and may becontinued if IVR-3 is achieved. | Hyoung Su Kim Hyung Joon Yim Myoung Kuk Jang Ji Won Park Sang Jun Suh Yeon Seok Seo Ji Hoon Kim Bo Hyun Kim Sang Jong Park Sae Hwan Lee Sang Gyune Kim Young Seok Kim Jung Il Lee Jin-Woo Lee In Hee Kim Tae Yeob Kim Jin-Wook Kim Sook-Hyang Jeong Young Kul Jung Hana Park Seong Gyu Hwang | 2015 | World Journal of Gastroenterology2015,21,38: | 13
Pretreated quercetin protects gerbil hippocampal CA1 pyramidal neurons from transient cerebral ischemic injury by increasing the expression of antioxidant enzymes显示文摘Quercetin(QE; 3,5,7,3′,4′-pentahydroxyflavone), a well-known flavonoid, has been shown to prevent against neurodegenerative disorders and ischemic insults. However, few studies are reported regarding the neuroprotective mechanisms of QE after ischemic insults. Therefore, in this study, we investigated the effects of QE on ischemic injury and the expression of antioxidant enzymes in the hippocampal CA1 region of gerbils subjected to 5 minutes of transient cerebral ischemia. QE was pre-treated once daily for 15 days before ischemia. Pretreatment with QE protected hippocampal CA1 pyramidal neurons from ischemic injury, which was confirmed by neuronal nuclear antigen immunohistochemistry and Fluoro-Jade B histofluorescence staining. In addition, pretreatment with QE significantly increased the expression levels of endogenous antioxidant enzymes Cu/Zn superoxide dismutase, Mn superoxide dismutase, catalase and glutathione peroxidase in the hippocampal CA1 pyramidal neurons of animals with ischemic injury. These findings demonstrate that pretreated QE displayed strong neuroprotective effects against transient cerebral ischemia by increasing the expression of antioxidant enzymes. | Bai Hui Chen Joon Ha Park Ji Hyeon Ahn Jeong Hwi Cho In Hye Kim Jae Chul Lee Moo-Ho Won Choong-Hyun Lee In Koo Hwang Jong-Dai Kim Il Jun Kang Jun Hwi Cho Bich Na Shin Yang Hee Kim Yun Lyul Lee Seung Min Park | 2017 | Neural Regeneration Research2017,12,2: | 9
Three-Tesla magnetic resonance elastography for hepatic fibrosis:Comparison with diffusion-weighted imaging and gadoxetic acid-enhanced magnetic resonance imaging显示文摘AIM:To evaluate the feasibility of 3-Tesla magnetic resonance elastography(MRE)for hepatic fibrosis and to compare that with diffusion-weighted imaging(DWI)and gadoxetic acid-enhanced magnetic resonance(MR)imaging.METHODS:Forty-two patients were included in the study. On MRE,mean stiffness values were measured on the elastograms in kilopascals. The apparent diffusion coefficient(ADC)of the liver was measured using DWI.On gadoxetic acid enhanced MR,the contrast enhancement index(CEI)was calculated as signal intensity(SI)post/SIpre,where SIpost is liver-to-muscle SI ratio on hepatobiliary phase images and SIpre is that on nonenhanced images.Correlation between aspartate aminotransferase to the platelet ratio index(APRI)and three MR parameters was assessed. Each MR parameter was compared between a hepatic fibrosis(HF)group and non-hepatic fibrosis(n HF)group.RESULTS:Liver stiffness showed strong positive correlation with APRI[Spearman correlation coeffiecient(r)=0.773,P<0.0001],while ADC and CEI showed weak or prominent negative correlation(r=-0.28 and-0.321,respectively).In the HF group, only liver stiffness showed strong correlation with APRI(r=0.731,P<0.0001).Liver stiffness,ADC,and APRI were significantly different between the HF group and n HF group.CONCLUSION:MRE at 3-Tesla could be a feasible method for the assessment of hepatic fibrosis. | Hee Sun Park Young Jun Kim Mi Hye Yu Won Hyeok Choe Sung Il Jung Hae Jeong Jeon | 2014 | World Journal of Gastroenterology2014,20,46: | 9
Peroxisome proliferator-activated receptor-delta agonist ameliorated inflammasome activation in nonalcoholic fatty liver disease显示文摘AIM: To evaluate the inflammasome activation and the effect of peroxisome proliferator-activated receptors(PPAR)-δ agonist treatment in nonalcoholic fatty liver disease(NAFLD) models.METHODS: Male C57BL/6J mice were classified according to control or high fat diet(HFD) with or without PPAR-δ agonist(GW) over period of 12 wk [control, HFD, HFD + lipopolysaccharide(LPS), HFD + LPS + GW group]. Hep G2 cells were exposed to palmitic acid(PA) and/or LPS in the absence or presence of GW.RESULTS: HFD caused glucose intolerance and hepatic steatosis. In mice fed an HFD with LPS, caspase-1 and interleukin(IL)-1β in the liver were significantly increased. Treatment with GW ameliorated the steatosis and inhibited overexpression of pro-inflammatory cytokines. In Hep G2 cells, PA and LPS treatment markedly increased m RNA of several nucleotide-binding andoligomerization domain-like receptor family members(NLRP3, NLRP6, and NLRP10), caspase-1 and IL-1β. PA and LPS also exaggerated reactive oxygen species production. All of the above effects of PA and LPS were reduced by GW. GW also enhanced the phosphorylation of AMPK-α.CONCLUSION: PPAR-δ agonist reduces fatty acidinduced inflammation and steatosis by suppressing inflammasome activation. Targeting the inflammasome by the PPAR-δ agonist may have therapeutic implication for NAFLD. | Hyun Jung Lee Jong Eun Yeon Eun Jung Ko Eileen L Yoon Sang Jun Suh Keunhee Kang Hae Rim Kim Seoung Hee Kang Yang Jae Yoo Jihye Je Beom Jae Lee Ji Hoon Kim Yeon Seok Seo Hyung Joon Yim Kwan Soo Byun | 2015 | World Journal of Gastroenterology2015,21,45: | 9
Benign gallbladder diseases:Imaging techniques and tips for differentiating with malignant gallbladder diseases显示文摘Benign gallbladder diseases usually present with intraluminal lesions and localized or diffuse wall thickening.Intraluminal lesions of the gallbladder include gallstones, cholesterol polyps, adenomas,or sludge and polypoid type of gallbladder cancer must subsequently be excluded.Polyp size, stalk width, and enhancement intensity on contrast-enhanced ultrasound and degree of diffusion restriction may help differentiate cholesterol polyps and adenomas from gallbladder cancer.Localized gallbladder wall thickening is largely due to segmental or focal gallbladder adenomyomatosis, although infiltrative cancer may present similarly.Identification of Rokitansky-Aschoff sinuses is pivotal in diagnosing adenomyomatosis. The layered pattern, degree of enhancement, and integrity of the wall are imaging clues that help discriminate innocuous thickening from gallbladder cancer.High-resolution ultrasound is especially useful for analyzing the layering of gallbladder wall.A diffusely thickened wall is frequently seen in inflammatory processes of the gallbladder.Nevertheless,it is important to check for coexistent cancer in instances of acute cholecystitis.Ultrasound used alone is limited in evaluating complicated cholecystitis and often requires complementary computed tomography. In chronic cholecystitis, preservation of a two-layered wall and weak wall enhancement are diagnostic clues for excluding malignancy.Magnetic resonance imaging in conjunction with diffusion-weighted imaging helps to differentiate xathogranulomatous cholecystitis from gallbladder cancer by identifying the presence of fat and degree of diffusion restriction.Such distinctions require a familiarity with typical imaging features of various gallbladder diseases and an understanding of the roles that assorted imaging modalities play in gallbladder evaluations. | Mi Hye Yu Young Jun Kim Hee Sun Park Sung Il Jung | 2020 | World Journal of Gastroenterology2020,26,22: | 9
Assessing significant fibrosis using imaging-based elastography in chronic hepatitis B patients: Pilot study显示文摘BACKGROUND Accurate detection of significant fibrosis(fibrosis stage 2 or higher on the METAVIR scale)is important especially for chronic hepatitis B(CHB)patients with high viral loads but with normal or mildly elevated alanine aminotransferase(ALT)levels because the presence of significant fibrosis is accepted as the indication for antiviral treatment.Liver biopsy is the reference standard for diagnosing significant fibrosis, but it is an invasive procedure.Consequently, noninvasive imaging-based measurements, such as magnetic resonance elastography(MRE)or two-dimensional shear-wave elastography(2DSWE),have been proposed for the quantitative assessment of liver fibrosis.AIM To explore MRE and 2D-SWE to identify fibrosis stage, and to compare their performance with that of serum-based indices.METHODS The study enrolled 63 treatment-na?ve CHB patients with high viral loads but with normal or mildly elevated ALT levels who underwent liver biopsy before a decision was made to initiate antiviral therapy.MRE and 2D-SWE were performed, and serum-based indices, such as FIB-4 and aspartate transaminase to platelet ratio index(APRI),were calculated. The diagnostic performances of MRE,2D-SWE,FIB-4,and APRI for assessing significant fibrosis(≥F2)and cirrhosis(F4)were evaluated with liver histology as the reference standard, using receiver operating characteristic analyses.RESULTS The liver fibrosis stage was F0/F1 in 19,F2 in 14,F3 in 14,and F4 in 16 patients, respectively.MRE significantly discriminated F2 from F0/1(P=0.022),whereas 2D-SWE showed a broad overlap in distinguishing those stages.MRE showed a higher correlation coefficient value with fibrosis stage than 2D-SWE with fibrosis stage(0.869 vs 0.649,Spearman test;P<0.001).Multivariate linear regression analyses showed that fibrosis stage was the only factor affecting the values of MRE(P<0.001),whereas body mass index(P=0.042)and fibrosis stage(P<0.001)were independent factors affecting 2D-SWE values.MRE performance for diagnosing significant fibrosis was better[area under the curve(AUC)=0.906,positive predictive value(PPV)97.3%,negative predictive value(NPV)69.2%]than that of FIB-4(AUC=0.697,P=0.002)and APRI(AUC=0.717,P=0.010),whereas the performance of 2D-SWE(AUC=0.843,PPV 86%,NPV 65%)was not significantly different from that of FIB-4 or APRI.CONCLUSION Compared to SWE,MRE might be more precise non-invasive assessment for depicting significant fibrosis and for making-decision to initiate antiviral-therapy in treatment-na?ve CHB patients with normal or mildly-elevated ALT levels. | Hee Sun Park Won Hyeok Choe Hye Seung Han Mi Hye Yu Young Jun Kim Sung Il Jung Jeong Han Kim So Young Kwon | 2019 | World Journal of Gastroenterology2019,25,25: | 7
Durability of viral response after off-treatment in HBeAg positive chronic hepatitis B显示文摘AIM:To evaluate the durability in hepatitis B e antigen (HBeAg) positive chronic hepatitis B patients who discontinued antiviral treatment. METHODS:A total of 48 HBeAg positive chronic hepatitis B patients who were administered nucleoside analogues and maintained virological response for ≥ 6 mo [hepatitis B virus (HBV) DNA < 300 copies/mL and HBeAg seroconversion] before cessation of treatment were enrolled between February 2007 and January 2010. The criteria for the cessation of the antiviral treatment were defined as follows:(1) achievement of virological response; and (2) duration of consolidation therapy (≥ 6 mo). After treatment cessation, the patients were followed up at 3-6 mo intervals. The primary endpoint was serologic and virologic recurrence rates after withdrawal of antiviral treatment. Serologic recurrence was defined as reappearance of HBeAg positivity after HBeAg seroconversion. Virologic recurrence was defined as an increase in HBV-DNA level > 104 copies/mL after HBeAg seroconversion with previously undetectable HBV-DNA level. RESULTS:During the median follow-up period of 18.2 mo (range:5.1-47.5 mo) after cessation of antiviral treatment, the cumulative serological recurrence rate was 15 % at 12 mo. The median duration between the cessation of antiviral treatment and serologic recurrence was 7.2 mo (range:1.2-10.9 mo). Of the 48 patients with HBeAg positive chronic hepatitis, 20 (41.6%) showed virological recurrence. The cumulative virologic recurrence rates at 12 mo after discontinuing the antiviral agent were 41%. The median duration between off-treatment and virologic recurrence was 7.6 mo (range:4.3-27.1 mo). The mean age of the virological recurrence group was older than that of the non-recurrence group (46.7 ± 12.1 years vs 38.8 ± 12.7 years, respectively; P = 0.022). Age (> 40 years) and the duration of consolidation treatment (≥ 15 mo) were significant predictive factors for offtreatment durability in the multivariate analysis [P = 0.049, relative risk (RR) 0.31, 95% CI (0.096-0.998) and P = 0.005, RR 11.29, 95% CI (2.054-65.12), respectively]. Patients with age (≤ 40 years) who received consolidation treatment (≥ 15 mo) significantly showed durability in HBeAg positive chronic hepatitis B patients (P = 0.014). These results suggest that additional treatment for more than 15 mo after HBeAg seroconversion in patients who are ≤ 40 years old may be beneficial in providing a sustained virological response. CONCLUSION:Our data suggest that HBeAg seroconversion is an imperfect end point in antiviral treatment. Long-term consolidation treatment (≥ 15 mo) in younger patients is important for producing better prognosis in HBeAg positive chronic hepatitis B. | Myeong Jun Song Do Seon Song Hee Yeon Kim Sun Hong Yoo Si Hyun Bae Jong Young Choi Seung Kew Yoon Yong-Han Paik June Sung Lee Hyun Woong Lee Hyung Joon Kim | 2012 | World Journal of Gastroenterology2012,18,43: | 7

Asymptomatic lymphangioma involving the spleen and retroperitoneum in adults显示文摘Lymphangioma,a benign neoplasm of the lymphatic system,is common in children but rare in adults. Its clinical manifestations include abdominal pain, nausea, vomiting and a palpable mass. However, abdominal sonography or abdominal computed tomography(CT) scan can also incidentally reveal lymphangioma.A larger or symptomatic lymphangioma is treated with total resection to prevent recurrence, infection, torsion and enlargement.Although lymphangioma rarely becomes malignant, its prognosis is generally good. We report a cystic lymphangioma of the spleen and retroperitoneum, which was incidentally found in a 56-year-old man who was hospitalized due to a colon mass.Physical examination showed no specific findings.Abdominal CT revealed a 5.7 cm,non-enhanced multilobulated cystic mass with multiple septa in the spleen and a 10 cm lobulated cystic mass in the paraaortic area.Splenectomy and retroperitoneal resection of the cystic mass were conducted. The endotheliumof splenic and retroperitoneal cyst was immunohisto- chemically stained with D2-40 antibody. The patient was finally diagnosed with splenic cystic and retroperitoneal cavernous lymphangioma. | Sook Hee Chung Young Sook Park Yun Ju Jo Seong Hwan Kim Dae Won Jun Byoung Kwan Son Jun Young Jung Dae Hyun Baek Dong Hee Kim Yoon Young Jung Won Mi Lee | 2009 | World Journal of Gastroenterology2009,15,44: | 7
Hepatic arterial infusion chemotherapy in hepatocellular carcinoma with portal vein tumor thrombosis显示文摘AIM: To evaluate the prognostic factors and efficacy of hepatic arterial infusion chemotherapy in hepatocellular carcinoma with portal vein tumor thrombosis. METHODS: Fifty hepatocellular carcinoma (HCC) patients with portal vein tumor thrombosis (PVTT) were treated using hepatic arterial infusion chemotherapy (HAIC) via a subcutaneously implanted port. The epirubicin-cisplatin-5-fluorouracil (ECF) chemotherapeutic regimen consisted of 35 mg/m 2 epirubicin on day 1, 60 mg/m 2 cisplatin for 2 h on day 2, and 500 mg/m 2 5-fluorouracil for 5 h on days 1-3. The treatments were repeated every 3 or 4 wk. RESULTS: Three (6%) of the 50 patients achieved a complete response (CR), 13 (26%) showed partial responses (PR), and 22 (44%) had stable disease (SD).The median survival and time to progression were 7 and 2 mo, respectively. After 2 cycles of HAIC, CR was achieved in 1 patient (2%), PR in 10 patients (20%) and SD in 26 patients (52%). Significant pre-treatment prognostic factors were a tumor volume of < 400 cm 3 (P = 0.01) and normal levels of protein induced by vitamin K absence or antagonist (PIVKA)-Ⅱ (P = 0.022). After 2 cycles of treatment, disease control (CR + PR + SD) (P = 0.001), PVTT response (P = 0.003) and α-fetoprotein reduction of over 50% (P = 0.02) were independent factors for survival. Objective response (CR + PR), disease control, PVTT response, and combination therapy during the HAIC were also significant prognostic factors. Adverse events were tolerable and successfully managed. CONCLUSION: HAIC may be an effective treatment modality for advanced HCC with PVTT in patients with tumors < 400 cm 3 and good prognostic factors. | Do Seon Song Si Hyun Bae Myeong Jun Song Sung Won Lee Hee Yeon Kim Young Joon Lee Jung Suk Oh Ho Jong Chun Hae Giu Lee Jong Young Choi Seung Kew Yoon | 2013 | World Journal of Gastroenterology2013,19,29: | 5
Gastric inverted hyperplastic polyp: A rare cause of iron deficiency anemia显示文摘Gastric inverted hyperplastic polyp(IHP) is a rare gastric polyp characterized by the downward growth of hyperplastic mucosal components into the submucosal layer. Macroscopically, a gastric IHP resembles a subepithelial tumor(SET); as a result, accurately diagnosing gastric IHP is difficult. This issue has clinical significance because gastric IHP can be misdiagnosed as SET or as malignant neoplasm In addition, adenocarcinoma can accompany benign gastric IHP. Although in most cases, gastric IHPs are asymptomatic and are found incidentally, these polyps may cause anemia secondary to chronic bleeding. Here, we report one case involving gastric IHP accompanied by chronic iron deficiency anemia that was successfully managed using endoscopic submucosal dissection. | Jin Tak Yun Seung Woo Lee Dong Pil Kim Seung Hwa Choi Seok-Hwan Kim Jun Kyu Park Sun Hee Jang Yun Jung Park Ye Gyu Sung Hae Jung Sul | 2016 | World Journal of Gastroenterology2016,22,15: | 5

New insights into the phylogeny and biogeography of subfamily Orontioideae (Araceae)显示文摘Proto-Araceae,the earliest diverged lineage within the family Araceae, includes two subfamilies,Gymnostachydoideae(one species)and Orontioideae(eight species).Based on an extensive sampling(a total of 198 accessions)of six chloroplast non-coding regions(5799 aligned sites),we assessed phylogenetic relationships among the genera and species within subfamily Orontioideae and estimated the timing of intercontinental disjunct events in the Northern Hemisphere.Overall phylogenetic relationships among the genera were consistent with results from previous studies, but several new important findings were discovered, primarily within Symplocarpus Salisb.ex W.P.C.Barton.First,two major lineages within Symplocarpus were identified:one lineage included S.foetidus(L.)Salisb.ex W.Barton,S.nabekuraensis Otsuka&K.Inoue, and S.renifolius Schott ex Tzvelev(Japan),whereas the other included S.nipponicus Makino,S.egorovi N.S.Pavlova&V.A.Nechaev,and S.renifolius(Korea).Symplocarpus renifolius in Japan was tetraploid and closely related to the tetraploid S.foetidus in eastern North America.Populations of S.renifolius in Korea were confirmed to be diploid(2n=30)and shared the most recent common ancestor with the other diploid species,S.nipponicus.Second,two recently described species,S.nabekuraensis and S.egorovi, were deeply embedded within S.renifolius in Japan and Korea, respectively, and their distinct taxonomic status requires further assessment. Finally, two intercontinental disjunction events in the subfamily, one in Lysichiton Schott between eastern Asia and western North America and the other in Symplocarpus between eastern Asia and eastern North America, were estimated to be between 4.5 and 1.4 million years ago(Pliocene and Pleistocene)and between 1.9 and 0.5 million years ago(Pleistocene),respectively. | Joon Seon Lee Seon Hee Kim Sangryong Lee Masayuki Maki Koichi Otsuka Andrey E.Kozhevnikov Zoya V.Kozhevnikova Jun Wen Seung-Chul Kim | 2019 | Journal of Systematics and Evolution2019,57,6: | 4
Enhanced efficiency in lead-free bismuth iodide with post treatment based on a hole-conductor-free perovskite solar cell显示文摘Despite the excellent merits of lead perovskite solar cells, their instability and toxicity still present a bottleneck for practical applications.Bismuth perovskite has emerged as a candidate for photovoltaic (PV) applications, because it not only has a low toxicity but is also stable in air. However, the power conversion efficiency (PCE) remains an unsolved problem. We performed band gap tuning experiments to improve the efficiency. The absorption of ABi3I10 structure films was extended within the visible region, and the optical band gap was decreased considerably compared to that for Cs3Bi2I9.Furthermore,we explained the correlation between the structure and the optical properties via a first-principles study.A device employing CsBi3I10 as a photoactive layer exhibits a PCE of 1.51% and an excellent ambient stability over 30 days. | Jongmoon Shin Maengsuk Kim Sujeong Jung Chang Su Kim Jucheol Park Aeran Song Kwun-Bum Chung Sung-Ho Jin Jun Hee Lee Myungkwan Song | 2018 | Nano Research2018,11,12: | 4
A pilot study of single-use endoscopy in screening acute gastrointestinal bleeding显示文摘AIM:To investigate the feasibility of a single-use endoscopy as an alternative procedure to nasogastric lavage in patients with acute gastrointestinal(GI) bleeding.METHODS:Patients who presented with hematemesis, melena or hematochezia were enrolled in this study.EG scan and conventional esophagogastroduodenoscopy(EGD) were subsequently performed.Active bleeding was defined as blood in the stomach, and inactive bleeding was defined as coffee ground clots and clear fluid in the stomach. The findings were recorded and compared.RESULTS:Between January and March,2011,13 patients that presented with hematemesis(n = 4),melena(n = 6),or bleeding from a previous nasogastric feeding tube(n = 3),were enrolled in this study. In 12 patients with upper GI bleeding, the EG scan device revealed that 7 patients had active bleeding and 5 patients had inactive bleeding, whereas conventional EGD revealed that 8 patients had active bleeding and 4 patients had inactive bleeding. The sensitivity and specificity of the EG scan device was 87.5% and 100% for active bleeding, with conventional EGD serving as a reference. No complication were reported during the EG scan procedures.CONCLUSION:The EG scan is a feasible device for screening acute upper GI bleeding. It may replace nasogastric lavage for the evaluation of acute upper GI bleeding. | Jae Hee Cho Hee Man Kim Sangheun Lee Yu Jin Kim Ki Jun Han Hyeon Geun Cho Si Young Song | 2013 | World Journal of Gastroenterology2013,19,1: | 3
