| 1 | 持续性姿势-感知性头晕(PPPD)诊断标准:Bárány学会前庭疾病分类委员会共识显示文摘本文提出被纳入前庭疾病国际分类(International Classification of Vestibular Disorders,ICVD)的持续性姿势-感知性头晕(PPPD)的诊断标准。PPPD虽然是一个新的医学术语,但该病的存在由来已久。前庭疾病分类委员会深入汇总、分析了近30年来恐惧性姿势性眩晕(Phobic Postural Vertigo,PPV)、空间运动不适(Space℃Motion Discomfort,SMD)、视觉性眩晕(Visual Vertigo,VV)和慢性主观性头晕(Chronic Subjective Dizziness,CSD)相关的研究,在此基础上提出了PPPD的诊断标准。临床上,头晕、不稳、非旋转性眩晕为PPPD的3个核心症状。PPPD患者常表现为这3个核心症状中的一个或多个,症状持续时间超过3个月,并且在直立姿势、主动/被动运动、暴露于移动视觉刺激或复杂视觉环境中时症状出现加重。PPPD可能与影响平衡系统或引起眩晕、不稳及头晕的疾病所触发有关,包括周围或中枢前庭病变、其他内科疾病或心理疾病。临床上,PPPD既可单独存在,也可与其他临床疾病共病。PPPD可能的亚型分类尚需进一步识别和验证。迄今为止,PPPD确切的病理生理机制尚不明确。新近研究表明,PPPD可能与躯体姿势控制、多感觉信息处理、威胁评估系统与空间定向系统皮质整合的功能异常相关。因此,PPPD被归类为慢性功能性前庭疾病,并不是结构性前庭疾病或精神心理性前庭疾病。 | 李康之(译) 刘博(译) 顾平(审校) 杨旭(审校) Jeffrey PStaab Annegret Eckhardt-Henn Arata Horii Rolf Jacob Michael Strupp Thomas Brandt Adolfo Bronstein | 2020 | 神经损伤与功能重建2020,15,2: | 20 |

| 3 | Genome engineering using the CRISPR/Cas system显示文摘Recently, an epoch-making genome engineering technology using clustered regularly at interspaced short palindromic repeats(CRISPR) and CRISPR associated(Cas) nucleases, was developed. Previous technologies for genome manipulation require the time-consuming design and construction of genome-engineered nucleases for each target and have, therefore, not been widely used in mouse research where standard techniques based on homologous recombination are commonly used. The CRISPR/Cas system only requires the design of sequences complementary to a target locus, making this technology fast and straightforward. In addition, CRISPR/Cas can be used to generate mice carrying mutations in multiple genes in a single step, an achievement not possible using other methods. | 4 | Elucidation of the relationship of BNIP3 expression to gemcitabine chemosensitivity and prognosis显示文摘AIM: To evaluate the significance of BNIP3 in the pathogenesis of pancreatic cancer, we analyzed the relationship between the expression of BNIP3 and survival rate of the patients with pancreatic cancer, or chemosensitivities in pancreatic cancer cell lines, particularly for gemcitabine, the first-line anti-tumor drug for pancreatic cancer. METHODS: To compare the expression level of BNIP3 with the resistance to gemcitabine, eight pancreatic cancer cell lines were subjected to gemcitabine treatment and the quantitative real-time RT-PCR method was used to evaluate BNIP3 expression. Immunohistochemical analysis was also performed using 22 pancreatic cancer specimens to study relationship between BNIP3 expression and survival rate. RESULTS: Although no significantly positive association between BNIP3 mRNA level and gemcitabine chemosensitivity was observed, pancreatic cancer cell lines that were sensitive to gemcitabine treatment tended to show high levels of BNIP3 expression. The converse, an absence of BNIP3 expression, was not correlated with gemcitabine resistance. We further compared the BNIP3 expression profiles of resected primary pancreaticcancer specimens with the prognosis of the patients, and found a tendency of favorable prognosis and low BNIP3 expression. CONCLUSION: High levels of BNIP3 expression cannot be used as one of the predicting factors for gemcitabine chemosensitivity, and some yet to be known factors will have to fill the gap for the accurate prediction of pancreatic cancer chemosensitivity to gemcitabine. | 6 | CO_2 insufflation for potentially diffi cult colonoscopies: Effi cacy when used by less experienced colonoscopists显示文摘AIM: To clarify the effectiveness of CO2 insufflation in potentially difficult colonoscopy cases, particularly in relation to the experience level of colonoscopists.METHODS: One hundred twenty potentially difficult cases were included in this study, which involved females with a low body mass index and patients with earlier abdominal and/or pelvic open surgery or previously diagnosed left-side colon diverticulosis. Patients receiving colonoscopy examinations without sedation using a pediatric variable-stiffness colonoscope were divided into two groups based on either CO2 or standard air insufflation. Both insufflation procedures were also evaluated according to the experience level of the respective colonoscopists who were divided into an experienced colonoscopist (EC) group and a less experienced colonoscopist (LEC) group. Study measurements included a 100-mm visual analogue scale (VAS) for patient pain during and after colonoscopyexaminations, in addition to insertion to the cecum and withdrawal times.RESULTS: Examination times did not differ, however, VAS scores in the CO2 group were signifi cantly better than in the air group (P < 0.001, two-way ANOVA) from immediately after the procedure and up to 2 h later. | 9 | Stronger inhibition of gastric acid secretion by lafutidine, a novel H_2 receptor antagonist, than by the proton pump inhibitor lansoprazole显示文摘AIM: To compare the antisecretory activity and plasma drug concentrations of a single oral dose of 10 mg lafutidine, a novel H2 receptor antagonist, with those of the proton pump inhibitor lansoprazole (LPZ) 30 mg. METHODS: Ten volunteers without H pylori infection participated in this crossover study comparing lafutidine 10 mg with LPZ 30 mg. Intragastric pH was monitored for 6 h in all participants, and blood samples were collected from four randomly selected individuals after single-dose administration of each drug. RESULTS: The median intragastric pH was significantly higher in individuals who received lafutidine 10 mg than in those who received LPZ 30 mg 2, 3, 4, 5, and 6 h after administration. Maximal plasma drug concentration was reached more promptly with lafutidine 10 mg than with LPZ 30 mg. | 10 | Early effects of Lansoprazole orally disintegrating tablets on intragastric pH in CYP2C19 extensive metabolizers显示文摘AIM: To compare rabeprazole (RPZ; 10 mg) with Lansoprazole orally disintegrating tablets (LPZ; 30 mg OD) in terms of antisecretory activity and blood drug concentration after a single dose. METHODS: Eight H pylori-negative cytochrome P450 (CYP) 2C19 extensive metabolizers were assigned to receive a single oral dose of RPZ 10 mg or LPZ 30 mg OD. Twelve hour intragastric pH monitoring was perform- ed on the day of treatment. Blood samples were also collected after the administration of each drug. RESULTS: LPZ 30 mg OD induced a significantly earlier rise in blood drug concentration than RPZ 10 mg; consequently, LPZ 30 mg OD induced a significantly earlier rise in median pH in the third and fourth hours of the study.
