| 1 | Barrett's esophagus:Prevalence and risk factors in patients with chronic GERD in Upper Egypt显示文摘AIM:To determine the prevalence and possible risk factors of Barrett's esophagus(BE) in patients with chronic gastroesophageal reflux disease(GERD) in El Minya and Assuit, Upper Egypt.METHODS:One thousand consecutive patients with chronic GERD symptoms were included in the study over 2 years.They were subjected to history taking including a questionnaire for GERD symptoms, clinical examination and upper digestive tract endoscopy.Endoscopic signs suggestive of columnar-lined esophagus(CLE) were defined as mucosal tongues or an upward shift of the squamocolumnar junction.BE was diagnosed by pathological examination when specialized intestinal metaplasia was detected histologically in suspected CLE.pH was monitored in 40 patients.RESULTS:BE was present in 7.3% of patients with chronic GERD symptoms, with a mean age of 48.3 ± 8.2 years, which was signif icantly higher than patients with GERD without BE(37.4 ± 13.6 years).Adenocarcinomawas detected in eight cases(0.8%), six of them in BE patients.There was no signiflcant difference between patients with BE and GERD regarding sex, smoking, alcohol consumption or symptoms of GERD.Patients with BE had significantly longer esophageal acid exposure time in the supine position, measured by pH monitoring.CONCLUSION:The prevalence of BE in patients with GERD who were referred for endoscopy was 7.3%.BE seems to be associated with older age and more in patients with nocturnal gastroesophageal reflux. | Yasser M Fouad Madiha M Makhlouf Heba M Tawfik Hussein El Amin Wael Abdel Ghany Hisham R El-khayat | 2009 | World Journal of Gastroenterology2009,15,28: | 4 |
| 2 | Helicobacter pylori and gastric cancer in the Middle East: A new enigma?显示文摘The Middle East is the home of ethnic groups from three main backgrounds: Semitic (Arabs and Jews), Indo-European (Persians and Kurdish) and Turkic (Turkish and Turkmens). Its geographic location, which has been under continuous influences from Asia, Europe and Africa, has made it an ideal site for epidemiological studies on Helicobacter pylori (H. pylori) infection and genotyping. The gastric cancer rate differs in this region from very high in Iran (26.1/105) to low in Israel (12.5/105) and very low in Egypt (3.4/105). Epidemiological studies showed that the prevalence of H. pylori is almost similar in those countries with a high level of infection in childhood. Importantly, the frequency of vacA s1 and m1 regions and cagA+ genotypes were higher in non Semitic populations who inhabit the North than Semitic populations, the inhabitants of Southern parts of the Middle East. H. pylori infection prevalence, distribution pattern of virulence factors, diet and smoking could not have explained the difference in cancer rate. This reflects the multifactorial aetiology of gastric cancer and suggests that H. pylori infection does not always directly correlate with the risk for gastrointestinal disease, such as gastric cancer. | 3 | Posterior lingual lidocaine: A novel method to improve tolerance in upper gastrointestinal endoscopy显示文摘AIM: To evaluate the effect of posterior lingual lidocaine swab on patient tolerance to esophagogastro-duodenoscopy, the ease of performance of the procedure, and to determine if such use will reduce the need for intravenous sedation. METHODS: Eighty patients undergoing diagnostic esophagogastroduodenoscopy in a tertiary care medical center were randomized to either lidocaine swab or spray. Intravenous meperidine and midazolam were given as needed during the procedure. RESULTS: Patients in the lidocaine swab group (SWG) tolerated the procedure better than those in the spray group (SPG) with a median tolerability score of 2 (1, 4) compared to 4 (2, 5) (P < 0.01). The endoscopists encountered less diffi culty performing the proceduresin the SWG with lower median difficulty scores of 1 (1, 5) compared to 4 (1, 5) in the SPG (P < 0.01). In addition, the need for intravenous sedation was also lower in the SWG compared to the SPG with fewer patients requiring intravenous sedation (13/40 patients vs 38/40 patients, respectively, P < 0.01). The patients in the SWG were more satisfi ed with the mode of local anesthesia they received as compared to the SPG. In addition, the endoscopists were happier with the use of lidocaine swab.
