| 1 | Endoscopic transluminal pancreatic necrosectomy using a self-expanding metal stent and high-flow water-jet system显示文摘Walled-off pancreatic necrosis and a pancreatic abscess are the most severe complications of acute pancreatitis. Surgery in such critically ill patients is often associated with significant morbidity and mortality within the first few weeks after the onset of symptoms. Minimal invasive approaches with high success and low mortality rates are therefore of considerable interest. Endoscopic therapy has the potential to offer safe and effective alternative treatment. We report here on 3 consecutive patients with infected walled-off pancreatic necrosis and 1 patient with a pancreatic abscess who underwent direct endoscopic necrosectomy 19-21 d after the onset of acute pancreatitis. The infected pancreatic necrosis or abscess was punctured transluminally with a cystostome and, after balloon dilatation, a non-covered self-expanding biliary metal stent was placed into the necrotic cavity. Following stent deployment, a nasobiliary pigtail catheter was placed into the cavity to ensure continuous irrigation. After 5-7 d, the metal stent was removed endoscopically and the necrotic cavity was entered with a therapeutic gastroscope. Endoscopic debridement was performed via the simultaneous application of a high-flow water-jet system; using a flush knife, a Dormia basket, and hot biopsy forceps. The transluminal endotherapy was repeated 2-5 times daily during the next 10 d. Supportive care included parenteral antibiotics and jejunal feeding. All patients improved dramatically and with resolution of their septic conditions; 3 patients were completely cured without any further complications or the need for surgery. One patient died from a complication of prolonged ventilation severe bilateral pneumonia, not related to the endoscopic procedure. No procedure related complications were observed. | 3 | Preparation and Characterization of Flexible, Transparent and Thermally Stable Aromatic Co-polyamides显示文摘Two aromatic co-polyamides were synthesized combining two diacid monomers containing bulky pendant groups, 5-(9,10-dihydro-9,10-ethanoanthracene-11,12-dicarboximido)isophthalic acid(DEAIA) and 5-tert-butylisophthalic acid(TERT), with 4,4′-(hexafluoroisopropylidene)dianiline(HFA) or 2,3,5,6-tetramethyl-1,4-phenylenediamine(Durene) by direct polycondensation. The structures of the obtained aromatic co-polyamides were confirmed by FTIR, Raman and 1 H-NMR. The co-copolyamide films, DHTH and DDTD, exhibited rms-roughness values between 0.94 and 1.60 nm, respectively. Moreover, they presented good thermal stability up to300 °C. Young's moduli of the co-polyamide films were between 4.1 and 4.3 GPa. X-ray diffraction results showed that the co-polyamide films were amorphous due to the incorporation of both bulky pendant groups, tert-butyl and dibenzobarrelene. | 5 | Following New Philanthropy by Network Ethnography:How Did the Varkey Foundation Land and Expand in Latin America?显示文摘Purpose:New philanthropy develops new forms of policy“problem-solving”through the influx of private actors’money and ideas.It adopts singular configurations across different policy spaces,with implications for education governance.We address this phenomenon by analyzing the Varkey Foundation’s(VF)“landing”and“expansion”in Latin America.VF is a global philanthropic organization registered in the United Kingdom in 2011 with a main hub in Argentina,where it landed in 2017.Design/Approach/Methods:We use network ethnography to examine the influence of new philanthropy on education governance as this phenomenon demands new methods to grasp the“realities”of governing beyond conventional ways and understanding policy mobilities.Findings:Results indicate how VF“landed”in a little pro-market space via productive uses of policy networks.The second phase of data collection and analysis presents further consolidation of VF in Argentina,its expansion into Latin America,and its establishment as a legitimate education policy actor.Originality/Value:This study provides new evidence and insights into how policy networks enable new philanthropy to land and consolidate within national boundaries and,later,across the region as a legitimate actor in education. | Iván Matovich Tomás Esper | 2023 | ECNU Review of Education2023,6,4: | 2 |

| 15 | The short form of the SUR1 and its functional implications in the damaged brain显示文摘Sulfonylurea receptor(SUR) belongs to the adenosine 5′-triphosphate(ATP)-binding cassette(ABC) transporter family;however,SUR is associated with ion channels and acts as a regulatory subunit determining the opening or closing of the pore.Abcc8 and Abcc9 genes code for the proteins SUR1 and SUR2,respectively.The SUR1 transcript encodes a protein of 1582 amino acids with a mass around 140–177 k Da expressed in the pancreas,brain,heart,and other tissues.It is well known that SUR1 assembles with Kir6.2 and TRPM4 to establish K_(ATP) channels and non-selective cation channels,respectively.Abbc8 and 9 are alternatively spliced,and the resulting transcripts encode different isoforms of SUR1 and SUR2,which have been detected by different experimental strategies.Interestingly,the use of binding assays to sulfonylureas and Western blotting has allowed the detection of shorter forms of SUR(-65 k Da).Identity of the SUR1 variants has not been clarified,and some authors have suggested that the shorter forms are unspecific.However,immunoprecipitation assays have shown that SUR2 short forms are part of a functional channel even coexisting with the typical forms of the receptor in the heart.This evidence confirms that the structure of the short forms of the SURs is fully functional and does not lose the ability to interact with the channels.Since structural changes in short forms of SUR modify its affinity to ATP,regulation of its expression might represent an advantage in pathologies where ATP concentrations decrease and a therapeutic target to induce neuroprotection.Remarkably,the expression of SUR1 variants might be induced by conditions associated to the decrease of energetic substrates in the brain(e.g.during stroke and epilepsy).In this review,we want to contribute to the knowledge of SUR1 complexity by analyzing evidence that shows the existence of short SUR1 variants and its possible implications in brain function. | Iván Alquisiras-Burgos Javier Franco-Pérez Moisés Rubio-Osornio Penélope Aguilera | 2022 | Neural Regeneration Research2022,17,3: | 1 |
