| 1 | Antiviral therapy in hepatitis C virus cirrhotic patients in compensated and decompensated condition显示文摘The main goals of treating cirrhotic patients with antiviral therapy are to attain sustained viral clearance(SVR),halt disease progression,and prevent re-infection of the liver graft.However,while the medical need is great,the use of interferon and ribavirin might expose these patients to severe treated-related side effects as a large proportion of them have pre-existing hematological cytopenias.We have reviewed potential benefits and risks associated with antiviral drugs in patients with liver cirrhosis,due to hepatitis C virus(HCV) infection.In cases presenting with bridging fibrosis or cirrhosis,current regimens of antiviral therapy have attained a 44%-48% rate of SVR.In cirrhotic patients with portal hypertension,the SVR rate was 22% overall,12.5% in patients with genotype 1,and 66.7% in those with genotypes 2 and 3 following therapy with low doses of either Peg-IFN alpha-2b and of ribavirin.In patients with decompensated cirrhosis,full dosages of Peg-IFN alpha-2b and of ribavirin produced a SVR rate of 35% overall,16% in patients with genotype 1 and 4,and 59% in those with genotype 2 and 3.Use of hematological cytokines will either ensure full course of treatment to be accomplished with and prevent development of treatment-associated side effects.Major benefits after HCV eradication were partial recovery of liver metabolic activity,prevention of hepatitis C recurrence after transplantation,and removal of some patients from the waiting list for liver transplant.Several observations highlighted that therapy is inadvisable for individuals with poor hepatic reserve(Child-Pugh-Turcotte score ≥ 10).Although SVR rates are low indecompensated cirrhotics due to hepatitis C,these patients have the most to gain as successful antiviral therapy is potentially lifesaving. | Angelo Iacobellis Antonio Ippolito Angelo Andriulli | 2008 | World Journal of Gastroenterology2008,14,42: | 7 |
| 2 | Magnetic resonance imaging:A new tool for diagnosis of acute ischemic colitis?显示文摘AIM:To define the evolution of ischemic lesions with 7T magnetic resonance imaging(7T-MRI)in an animal model of acute colonic ischemia.METHODS:Adult Sprague-Dawley rats were divided into two groups.GroupⅠ underwent inferior mesenteric artery(IMA)ligation followed by macroscopic observations and histological analysis.In groupⅡ,7T-MRI was performed before and after IMA ligation and followed by histological analysis.RESULTS:Morphological alterations started to develop 1 h after IMA ligation,when pale areas became evident in the splenic flexure mesentery and progressively worsened up to 8 h thereafter,when the mesentery was less pale,and the splenic flexure loop appeared very dark.The 7T-MRI results reflected these alterations,showing a hyperintense signal in both the intraperitoneal space and the colonic loop wall 1 h after IMA ligation;the latter progressively increased to demonstrate a reduction in the colonic loop lumen at 6 h.Eight hours after IMA ligation,MRI showed a persistent colonic mural hyperintensity associated with a reduction in peritoneal free fluid.The 7T-MRI findings were correlated with histological alterations,varying from an attenuated epithelium with glandular apex lesions at 1 h to coagulative necrosis and loss of the surface epithelium detected 8 h after IMA ligation.CONCLUSION:MRI may be used as a substitute for invasive procedures in diagnosing and grading acute ischemic colitis,allowing for the early identification of pathological findings. | Francesca Iacobellis Daniela Berritto Francesco Somma Carlo Cavaliere Marco Corona Santolo Cozzolino Franco Fulciniti Salvatore Cappabianca Antonio Rotondo Roberto Grassi | 2012 | World Journal of Gastroenterology2012,18,13: | 7 |
| 3 | Acute arterial mesenteric ischemia and reperfusion:Macroscopic and MRI findings, preliminary report显示文摘AIM:To explore the physiopathology and magnetic resonance imaging(MRI)findings in an animal model of acute arterial mesenteric ischemia(AAMI)with and without reperfusion.METHODS:In this study,8 adult Sprague-Dawley rats underwent superior mesenteric artery(SMA)ligation and were then randomly divided in two groups of 4.In groupⅠ,the ischemia was maintained for 8 h.In groupⅡ,1-h after SMA occlusion,the ligation was removed by cutting the thread fixed on the back of the animal,and reperfusion was monitored for 8 h.MRI was performed using a 7-T system.RESULTS:We found that,in the case of AAMI without reperfusion,spastic reflex ileus,hypotonic reflex ileus,free abdominal fluid and bowel wall thinning are present from the second hour,and bowel wall hyperintensity in T2-W sequences are present from the fourth hour.The reperfusion model shows the presence of early bowel wall hyperintensity in T2-W sequences after 1 h and bowel wall thickening from the second hour.CONCLUSION:Our study has shown that MRI can assess pathological changes that occur in the small bowel and distinguish between the presence and absence of reperfusion after induced acute arterial ischemia. | Luca Saba Daniela Berritto Francesca Iacobellis Mariano Scaglione Sigismondo Castaldo Santolo Cozzolino Maria Antonietta Mazzei Veronica Di Mizio Roberto Grassi | 2013 | World Journal of Gastroenterology2013,19,40: | 5 |
| 4 | Non invasive evaluation of liver fibrosis in paediatric patients with nonalcoholic steatohepatitis显示文摘AIM: To identify the independent predictors of hepatic fibrosis in 69 children with nonalcoholic steatohepatitis (NASH) due to nonalcoholic fatty liver disease (NAFLD). METHODS: All patients with clinically suspected NASH underwent liver biopsy as a confirmatory test. The following clinical and biochemical variables at baseline were examined as likely predictors of fibrosis at histology: age, body mass index (BMI), systolic blood pressure (SBP), dyastolic blood pressure (DBP), fasting glucose, fasting insulin, homeostatic model assessment for insulin resistence (HOMA-IR), cholesterol, tryglicerides, alanine aminotransferase (ALT), aspartate aminotransferase (AST), AST/ALT ratio, gamma glutamil transferase (GT), platelet count, prothrombin time (PT). RESULTS: At histology 28 (40.6%) patients had no fibrosis and 41 (59.4%) had mild to bridging fibrosis. At multivariate analysis, BMI > 26.3 was the only independent predictor of fibrosis (OR = 5.85, 95% CI = 1.6-21). | 5 | Investigation of compensatory postures with videofluoromanometry in dysphagia patients显示文摘AIM: To investigate the effectiveness of head compensatory postures to ensure safe oropharyngeal transit. METHODS: A total of 321 dysphagia patients were enrolled and assessed with videofluoromanometry (VFM). The dysphagia patients were classified as follows: safe transit; penetration without aspiration; aspiration before, during or after swallowing; multiple aspirations and no transit. The patients with aspiration or no transit were tested with VFM to determine whether compensatory postures could correct their swallowing disorder. RESULTS: VFM revealed penetration without aspiration in 71 patients (22.1%); aspiration before swallowing in 17 patients (5.3%); aspiration during swallowing in 32 patients (10%); aspiration after swallowing in 21 patients (6.5%); multiple aspirations in six patients (1.9%); no transit in five patients (1.6%); and safe transit in 169 patients (52.6%). Compensatory postures guaranteed a safe transit in 66/75 (88%) patients with aspiration or no transit. A chin-down posture achieved a safe swallow in 42/75 (56%) patients, a head-turned posture in 19/75 (25.3%) and a hyperextended head posture in 5/75 (6.7%). The compensatory postures were not effective in 9/75 (12%) cases.
